Treating disruptive behavior disorders with risperidone: a 1-year, open-label safety study in children and adolescents.

OBJECTIVE: The aim of this study was to determine the long-term safety of risperidone as maintenance therapy in children and adolescents with disruptive behavior disorders (DBDs) and normal intelligence. METHOD: An open-label, 1-year extension study was conducted from January, 2002, to July, 2004, in 232 subjects with DBDs (5-17 years) previously randomized to risperidone (RIS) (n = 115, RIS/RIS) or placebo (PLA) (n = 117, PLA/RIS) in a double-blind, 6-month withdrawal study. Adverse events (AEs) and clinical laboratory test results were recorded. Efficacy was assessed using Nisonger Child Be

Disentangling the effects of Tourette syndrome and attention deficit hyperactivity disorder on cognitive and behavioral phenotypes.

Eighty participants (62 males; 18 females; age range: 6-16 years) took part in the study, comprising four groups of 20 subjects each: TS-only, ADHD-only, TS+ADHD, controls. The age distributions, did not differ significantly among the four groups. The severity of symptoms, assessed by the TSGS, did not differ significantly between the two TS groups. Standardised measures were used throughout. The "cases" (i.e. TS-only, TS+ADHD, ADHD-only) were significantly different from controls on most measures of behavior. There were also differences amongst the various clinical subgroups, with, in general

Effect of Parental-Child Age Gaps and Skipped-Generation Families on Comorbidities Related to Attention Deficit Hyperactivity Disorder: A Population-Based Case-Control Study.

Background: While attention deficit/hyperactivity disorder (ADHD) is characterized by neurodevelopmental heterogeneity, the influence of familial structural factors-particularly parental age and skipped-generation caregiving-on comorbidity patterns remains insufficiently studied. This study examined the associations between parent-child age gaps, skipped-generation family structures, and psychiatric comorbidities in children with ADHD. Methods: Data came from Taiwan's NHIRD (2009-2013), including 79,163 ADHD cases and 395,815 matched controls. Key variables included maternal and paternal age a

Unique Behavior Profiles That Specify Mental Distress in Autism.

PURPOSE: Patients with autism spectrum disorder (ASD), particularly those with limited speech, may not communicate internal distress. Clinicians therefore often infer distress from observable aberrant behaviors, whose clinical significance may differ from that in typically developing (TD) individuals. We identified aberrant behavior profiles (ABPs) that differentially track symptom dimensions across ASD and TD groups, and between minimally verbal ASD (MV-ASD) and fluently verbal ASD (FV-ASD). METHODS: We pooled three NIMH Data Archive studies with CASI-5 severities (12 domains) and Aberrant B

Distinct serum endocannabinoid profiles in treatment-naïve Han Chinese children with ADHD: a case-control pilot study.

OBJECTIVE: Although peripheral endocannabinoid system (ECS) signatures have been implicated in ADHD among European and American populations, evidence from Asian pediatric cohorts remains scarce. This study quantified serum concentrations of four major endocannabinoids-anandamide (AEA), 2-arachidonoylglycerol (2-AG), oleoylethanolamide (OEA), and palmitoylethanolamide (PEA)-in treatment-naïve Han Chinese children with ADHD and examined their associations with symptom severity as measured by the SNAP-IV scale. METHODS: This cross-sectional study enrolled 22 children with ADHD (aged 6-12 years,

Correction of Reward Processing Deficits in Youth with Disruptive Behavior and Trauma Exposure: A Pilot Study of Neural Responses to Fluoxetine.

OBJECTIVE: Youths with disruptive behavior disorders (DBDs) and a history of trauma exposure often exhibit deficits in neural mechanisms related to reward anticipation and assessment. This preliminary investigation examines the potential of a serotonergic agent (fluoxetine) to modulate neural activity in reward-related pathways for this population. METHODS: Three participant groups were: (i) youth with DBDs and trauma exposure who received fluoxetine treatment for 8 weeks (n = 12); (ii) a matched group of youth with DBDs and trauma exposure who received routine regular follow-up in an outpati

Systematic Review and Meta-Analysis of the Effectiveness of Teacher Delivered Interventions for Externalizing Behaviors.

UNLABELLED: Teachers contribute to the process of identifying and referring students for mental health services, however, relatively little is known about how they make those decisions and how decision-making differs across school contexts. This study used a vignette-based method to investigate individual and school contextual factors associated with the likelihood that teachers identify and refer students for mental health services. Teachers were recruited from public middle and high schools across the U.S. using a stratified random sampling strategy. Teachers (N = 462) responded to vignettes

Penile strangulation by a metallic nut in an 8-year-old male: A rare case report of urological emergency.

INTRODUCTION AND IMPORTANCE: Penile strangulation due to metallic objects in pediatric age groups is a rare urological emergency that warrant emergency management. Removal of metallic ring from strangulated penis poses a great challenge to urologist as it may require industrial gadgets. Multidisciplinary management involving psychiatrist is required. To the best of our knowledge, this is the first reported novel case of penile strangulation by a metallic nut in children in Tanzania. Furthermore, this case report shows that Industrial Manual Scissor metal (IMSMC) cutter offers a safe way of rem

Behavioural and emotional disorders in childhood: A brief overview for paediatricians.

Mental health problems in children and adolescents include several types of emotional and behavioural disorders, including disruptive, depression, anxiety and pervasive developmental (autism) disorders, characterized as either internalizing or externalizing problems. Disruptive behavioural problems such as temper tantrums, attention deficit hyperactivity disorder, oppositional, defiant or conduct disorders are the commonest behavioural problems in preschool and school age children. The routine Paediatric clinic or Family Medicine/General Practitioner surgery presents with several desirable cha

Strengthening Positive Parenting in the Context of Intimate Partner Abuse.

Intimate partner violence has been associated with long-term negative effects on mothers' mental health status, use of positive parenting practices, and their children's externalizing behavioral difficulties. Especially strong is the association between concurrent experiences of partner violence and maternal parenting. In this case study, we examined the impact of behavioral parent training on parenting and outcomes for a 6-year-old Latino diagnosed with oppositional defiant disorder (ODD). The family was participating in a larger study examining the implementation and cultural adaptations of

Behavior and emotion modulation deficits in preschoolers at risk for bipolar disorder.

BACKGROUND: Bipolar disorder (BD) is highly familial, but studies have yet to examine preschoolers at risk for BD using standardized, developmentally appropriate clinical assessment tools. We used such methods to test whether preschoolers at familial risk for BD have more observed difficulty modulating emotions and behaviors than do low-risk preschoolers. Identification of emotional and behavioral difficulties in at-risk preschoolers is crucial for developing new approaches for early intervention and prevention of BD. METHODS: Using the standardized disruptive behavior diagnostic observation

Parent- and self-reported dimensions of oppositionality in youth: construct validity, concurrent validity, and the prediction of criminal outcomes in adulthood.

BACKGROUND: Different dimensions of oppositional defiant disorder (ODD) have been found as valid predictors of further mental health problems and antisocial behaviors in youth. The present study aimed at testing the construct, concurrent, and predictive validity of ODD dimensions derived from parent- and self-report measures. METHOD: Confirmatory factor analyses were performed to test a three-dimensional model (ODD-irritability, ODD-headstrong, and ODD-hurtful) and a two-dimensional model (ODD-irritability, ODD-headstrong/hurtful) based on items of the Child Behavior Checklist (CBCL) and the

Impact of maternal depression across the first 6 years of life on the child's mental health, social engagement, and empathy: The moderating role of oxytocin.

OBJECTIVE: Maternal depression across the postbirth period has long-term negative consequences for infant development. Little is known of the neurobiological underpinnings, but they could involve oxytocin, a neuropeptide that is dysfunctional in depression and is implicated in birth and parenting. METHOD: The authors recruited a community cohort of women with high or low depression scores 2 days after childbirth and measured depression again at 6 and 9 months. When the child was 6, the authors evaluated the families of 46 chronically depressed mothers and 103 mothers reporting no depression s

The adolescent outcome of hyperactive children diagnosed by research criteria--III. Mother-child interactions, family conflicts and maternal psychopathology.

The present study reports the results of a prospective, 8-year follow-up study of 100 hyperactive and 60 normal children followed from childhood into adolescence. Ratings of child behavior problems and family conflicts as well as direct observations of mother-child interactions were taken in childhood and again at adolescent follow-up. At outcome, hyperactives continued to have more conduct and learning problems and to be more hyperactive, inattentive, and impulsive than controls. Hyperactives were also rated by their mothers as having more numerous and intense family conflicts than the normal

The Effects of Child Mental Health on Juvenile Criminal Justice Contact and Victimization.

BACKGROUND: There is extensive evidence on the associations between mental disorders such as attention deficit hyperactivity disorder (ADHD), conduct disorder (CD), oppositional/defiant disorder (ODD), and anxiety/depression and delinquency among youths. However, research has largely overlooked the potential confounding from comorbidity of these mental disorders as well as unobserved familial heterogeneity. AIMS OF THE STUDY: This study aims to estimate the causal effects of mental health in childhood (age 4-12) on delinquency in adolescence (age 13-18) by adjusting for comorbid mental disord
