Clevidipine for controlled hypotension during spinal surgery in adolescents.

BACKGROUND: Clevidipine is an ultrashort-acting, intravenous calcium channel antagonist. Metabolism by blood and tissue esterases results in a half-life of 1 to 2 minutes. We present preliminary experience with this novel agent to provide controlled hypotension (CH) in a cohort of adolescents undergoing spinal surgery. METHODS: The records of patients ≤18 years of age who received clevidipine for CH were retrospectively reviewed. Demographic data included age, weight, sex, and comorbid disease processes. Information regarding clevidipine included the initial infusion rate, time to achieve the

Closed-circuit xenon delivery using a standard anesthesia workstation.

BACKGROUND: Xenon (Xe) is an anesthetic with minimal side effects, now also showing promise as a neuroprotectant both in vitro and in vivo. Although scarce and expensive, Xe is insoluble and patient uptake is low, making closed circuits the optimum delivery method. Although the future of Xe anesthesia is uncertain, effective neuroprotection is highly desirable even if moderately expensive. A factor limiting Xe research in all these fields may be the perceived need to purchase special Xe anesthesia workstations that are expensive and difficult to service. We investigated the practicality of 1)

Endovascular treatment of brain aneurysms under conscious sedation: a systematic review of procedural feasibility and safety.

Over the last decades, minimally invasive techniques have revolutionized the endovascular treatment (EVT) of brain aneurysms. In parallel, the development of conscious sedation (CS), a potentially less harmful anesthetic protocol than general anesthesia (GA), has led to the course optimization of surgeries, patient outcomes, and healthcare costs. Nevertheless, the feasibility and safety of EVT of brain aneurysms under CS have yet to be assessed thoroughly. Herein, we systematically reviewed the medical literature about this procedure. In accordance with the PRISMA guidelines, four databases (P

Side Effect Rates of Opioids in Equianalgesic Doses via Intravenous Patient-Controlled Analgesia: A Systematic Review and Network Meta-analysis.

BACKGROUND: Side effects of opioids used for the treatment of acute pain frequently limit their analgesic quality. Many studies have compared opioid side effects in patient-controlled analgesia (PCA), but it remains unclear whether there are specific side effect profiles that can be exploited when choosing an opioid for a patient. In this review, we wanted to determine the risk ratios (RRs) for the most common side effects when using different opioids for intravenous PCA in equianalgesic doses and rank the substances accordingly. METHODS: A search of MEDLINE, EMBASE, the Cochrane Library (CEN

A specialized post anaesthetic care unit improves fast-track management in cardiac surgery: a prospective randomized trial.

INTRODUCTION: Fast-track treatment in cardiac surgery has become the global standard of care. We compared the efficacy and safety of a specialised post-anaesthetic care unit (PACU) to a conventional intensive care unit (ICU) in achieving defined fast-track end-points in adult patients after elective cardiac surgery. METHODS: In a prospective, single blinded, randomized study, 200 adult patients undergoing elective cardiac surgery (coronary artery bypass graft (CABG), valve surgery or combined CABG and valve surgery), were selected to receive their postoperative treatment either in the ICU (n

Ectopic ACTH syndrome revealed as severe hypokalemia and persistent hypertension during the perioperative period: a case report.

Both severe hypokalemia and persistent hypertension are clinical symptoms of hyperaldosteronism. Hyperaldosteronism may occur as a primary or secondary syndrome. Excess ACTH produced ectopically by tumors may induce hyperaldosteronism through the mineralocorticoid activity of glucocorticoids that are upregulated by ACTH. Licorice, with the active ingredient glycyrrhiza, is also a well-known inducer of hyperaldosteronism under specific conditions. In this report, we describe a case of severe hypokalemia caused by ectopic ACTH syndrome (EAS) elicited by an intrathoracic carcinoid tumor, which ha

Minor increase of endtidal CO2 during sevoflurane-induced malignant hyperthermia.

Malignant hyperthermia (MH) in a pediatric patient during sevoflurane anesthesia with only a minor rise of endtidal CO(2) is described. MH was considered because of increased rectal temperature. The outcome was favorable after the initiation of a few simple measures (increased minute ventilation, cessation of sevoflurane). As recommended by the regional MH Reference Center, dantrolene was not used. In vitro contracture tests were performed on muscle biopsies from both parents. A strong contracture in response to halothane confirmed the father's MH susceptibility, thereby according a high proba

Efficacy of serratus anterior plane block on postoperative nausea and vomiting: a meta-analysis of randomized controlled trial.

BACKGROUND: Postoperative nausea and vomiting (PONV) still often occurs after surgery, which adversely affects recovery and patient satisfaction. Serratus anterior plane block (SAPB) has been proposed as a regional anesthesia technique that may reduce PONV indirectly by improving postoperative analgesia and decreasing perioperative opioid requirements. METHODS: A systematic review and meta-analysis of the randomized controlled trial (RCT) was conducted to evaluate the impact of SAPB on PONV. From its establishment to April 20, 2026, the central register of PubMed, Embase, CNKI and Cochrane co

[Combined transgluteal ischial and femoral nerve block: retrospective data on 65 risk patients with leg amputation].

For amputations of the lower limbs the combined blockade of the sciatic nerve (via the transgluteal approach) and the femoral nerve (via the inguinal route) may be a suitable alternative to general or neuroaxial anaesthetic methods. In highly comorbid, high risk patients this catheter-linked regional anaesthesia combines the advantages of avoiding controlled ventilation with decreased cardiovascular depression, as conveyed by the use of general anaesthesia or neuraxial techniques. Furthermore, improved postoperative analgesia may be achieved for several days. In our retrospective study, 65 pat

Combination of dexmedetomidine and esketamine for postoperative nausea and vomiting in patients undergoing laparoscopic surgery: study protocol for a prospective, randomized, controlled trial.

BACKGROUND: Laparoscopic surgery has gained widespread acceptance due to its advantages over traditional open procedures, including less postoperative pain and faster recovery. However, postoperative nausea and vomiting (PONV) remains a prevalent complication, affecting up to 80% of high-risk patients and significantly reducing patient satisfaction and recovery quality. Although opioids are frequently used for intraoperative analgesia, their side effects, particularly PONV, highlight the need for opioid-sparing strategies. Dexmedetomidine and esketamine are two agents with analgesic and anesth
