Location of Neurostimulation Affects Short-Term Emotional Wellbeing Outcomes in Parkinson's Disease Patients.

INTRODUCTION: Emotional wellbeing is a major determinant of quality of life (QoL) in Parkinson's disease (PD) patients. Previous studies have found mixed results regarding the effect of subthalamic nucleus (STN) deep brain stimulation (DBS) on depression and anxiety, major contributors to emotional wellbeing. There is evidence that the location of neurostimulation of the STN may explain this variability. However, studies investigating this matter have also yielded inconsistent findings. Therefore, the objective of this study was to determine the effect of STN DBS on emotional wellbeing at 6 mo

Timing of Deep Brain Stimulation and Cognitive Trajectories in GBA1-Associated Parkinson's Disease.

BACKGROUND: Individuals with Parkinson's disease (PD) carrying GBA1 variants often develop advanced motor complications that prompt evaluation for deep brain stimulation (DBS). However, concerns about accelerated cognitive decline after DBS in GBA1 carriers complicate clinical decision making. A critical aspect that is increasingly gaining attention is the time at which DBS is performed. OBJECTIVE: The objective is to determine how the timing of DBS in relation to PD onset impacts cognitive trajectories across different GBA1 variant classes and in individuals without GBA1 variants. METHODS:

Long-term efficacy of deep brain stimulation in Parkinson's disease: over 10-year follow-up and insights into the "DBS honeymoon".

BACKGROUND: Parkinson's disease (PD) is the second most prevalent neurodegenerative disorder globally. Deep brain stimulation (DBS) has become a critical therapeutic option for advanced PD. The efficacy of DBS has been well established for up to 1 or 2 years; however, long-term outcome data for Chinese cohorts are limited, and the duration of the "DBS honeymoon" remains underexplored. OBJECTIVE: This study aimed to evaluate the long-term efficacy (⩾10 years) of subthalamic nucleus (STN)-DBS in patients with PD in Southern China, and to investigate the duration of "DBS honeymoon." DESIGN: Ret

Therapeutic advances in neural regeneration for Huntington's disease.

Huntington's disease is a neurodegenerative disease caused by the expansion mutation of a cytosine-adenine-guanine triplet in the exon 1 of the HTT gene which is responsible for the production of the huntingtin (Htt) protein. In physiological conditions, Htt is involved in many cellular processes such as cell signaling, transcriptional regulation, energy metabolism regulation, DNA maintenance, axonal trafficking, and antiapoptotic activity. When the genetic alteration is present, the production of a mutant version of Htt (mHtt) occurs, which is characterized by a plethora of pathogenic activit

Neurophysiological responses of globus pallidus internus during the auditory oddball task in Parkinson's disease.

Parkinson's disease can be associated with significant cognitive impairment that may lead to dementia. Deep brain stimulation (DBS) of the subthalamic nucleus is an effective therapy for motor symptoms but is associated with cognitive decline. DBS of globus pallidus internus (GPi) poses less risk of cognitive decline so may be the preferred target. A research priority is to identify biomarkers of cognitive decline in this population, but efforts are hampered by a lack of understanding of the role of the different basal ganglia nuclei, such as the globus pallidus, in cognitive processing. Potential Deep Brain Stimulation Targets for the Management of Refractory Hypertension.

Hypertension is the single greatest contributor to human disease and mortality affecting over 75 million people in the United States alone. Hypertension is defined according to the American College of Cardiology as systolic blood pressure (SBP) greater than 120 mm Hg and diastolic blood pressure (DBP) above 80 mm Hg measured on two separate occasions. While there are multiple medication classes available for blood pressure control, fewer than 50% of hypertensive patients maintain appropriate control. In fact, 0.5% of patients are refractory to medical treatment which is defined as uncontrolled

GPi Oscillatory Activity Differentiates Tics from the Resting State, Voluntary Movements, and the Unmedicated Parkinsonian State.

Background: Deep brain stimulation (DBS) is an emerging treatment strategy for severe, medication-refractory Tourette syndrome (TS). Thalamic (Cm-Pf) and pallidal (including globus pallidus interna, GPi) targets have been the most investigated. While the neurophysiological correlates of Parkinson's disease (PD) in the GPi and subthalamic nucleus (STN) are increasingly recognized, these patterns are not well characterized in other disease states. Recent findings indicate that the cross-frequency coupling (CFC) between beta band and high frequency oscillations (HFOs) within the STN in PD patient

Cortico-thalamic tremor circuits and their associations with deep brain stimulation effects in essential tremor.

Essential tremor is one of the most common movement disorders in adults. Deep brain stimulation (DBS) of the ventralis intermediate nucleus of the thalamus and/or the posterior subthalamic area has been shown to provide significant tremor suppression in patients with essential tremor, but with significant inter-patient variability and habituation to the stimulation. Several non-invasive neuromodulation techniques targeting other parts of the CNS, including cerebellar, motor cortex or peripheral nerves, have also been developed for treating essential tremor, but the clinical outcomes remain inc

Directional versus ring-mode deep brain stimulation for Parkinson's disease: protocol of a multi-centre double-blind randomised crossover trial.

BACKGROUND: The effectiveness of Deep Brain Stimulation (DBS) therapy for Parkinson's disease can be limited by side-effects caused by electrical current spillover into structures adjacent to the target area. The objective of the STEEred versus RING-mode DBS for Parkinson's disease (STEERING) study is to investigate if directional DBS for Parkinson's disease results in a better clinical outcome when compared to ring-mode DBS. METHODS: The STEERING study is a prospective multi-centre double-blind randomised crossover trial. Inclusion criteria are Parkinson's disease, subthalamic nucleus DBS in

Deep Brain Stimulation-Withdrawal Syndrome in Parkinson's Disease: Risk Factors and Pathophysiological Hypotheses of a Life-Threatening Emergency.

BACKGROUND AND OBJECTIVES: Subthalamic nucleus deep brain stimulation (DBS) is the most common therapeutic surgical procedure for patients with Parkinson's disease with motor fluctuations, dyskinesia, or tremor. Routine follow-up of patients allows clinicians to anticipate replacement of the DBS battery reaching the end of its life. Patients who experience a sudden stop of the DBS battery experience a rapid worsening of symptoms unresponsive to high dose of levodopa, in a life-threatening phenomenon called "DBS-withdrawal syndrome." In the current context of the COVID-19 pandemic, in which man

Stimulation-Induced Side Effects of Deep Brain Stimulation in the Ventralis Intermedius and Posterior Subthalamic Area for Essential Tremor.

Deep brain stimulation (DBS) targeting the ventralis intermedius (VIM) nucleus of the thalamus and the posterior subthalamic area (PSA) has been shown to be an effective treatment for essential tremor (ET). The aim of this study was to compare the stimulation-induced side effects of DBS targeting the VIM and PSA using a single electrode. Patients with medication-refractory ET who underwent DBS electrode implantation between July 2011 and October 2020 using a surgical technique that simultaneously targets the VIM and PSA with a single electrode were enrolled in this study. A total of 93 patient

STN Versus GPi Deep Brain Stimulation for Action and Rest Tremor in Parkinson's Disease.

OBJECTIVE: To investigate the effects of subthalamic nucleus (STN) and globus pallidus internus (GPi), deep brain stimulation (DBS) on individual action tremor/postural tremor (AT) and rest tremor (RT) in Parkinson's disease (PD). Randomized DBS studies have reported marked benefit in tremor with both GPi and STN and DBS, however, there is a paucity of information available on AT vs RT when separated by the surgical target. METHODS: We retrospectively reviewed the 1-year clinical outcome of PD patients treated with STN and GPi DBS at the University of Florida. We specifically selected patient

Deep brain stimulation improves survival in severe Parkinson's disease.

OBJECTIVES: Levodopa and other dopaminergic treatments have not had the expected effect on survival in Parkinson's disease (PD). Bilateral subthalamic nucleus deep brain stimulation (STN-DBS) has been shown to improve motor function, motor fluctuations, health-related quality of life, and to reduce medication usage and drug-induced dyskinesia in patients with severe PD refractory to medical therapy. Little however, has been described on the impact of STN-DBS on the survival of these patients. We aim in this study to examine the impact of STN-DBS on the survival of patients with severe PD. MET

An in silico cortico-basal ganglia-thalamic testbed for comparing visual and deep brain stimulation in Parkinson's disease.

INTRODUCTION: Deep brain stimulation (DBS) is an established intervention for Parkinson's disease (PD), whereas the network-level effects of prescribed periodic visual input implemented with 10 Hz base pulses remain uncertain. METHODS: We developed a conductance-based cortico-basal ganglia-thalamic (CBT) model comprising 11 Hodgkin-Huxley neuronal populations and a prescribed periodic visual-input unit (RET) to compare 130 Hz subthalamic nucleus (STN) DBS with the archived periodic visual-input condition on the same calibrated PD network. Primary conditions used N = 10 independent runs each.

Genotype-associated outcomes after deep brain stimulation in Parkinson's disease: a systematic review, meta-analysis and epigenetic implications.

INTRODUCTION: Deep brain stimulation (DBS) is an established treatment for advanced Parkinson's disease (PD), but outcomes in genotype-defined PD remain debated. GBA1 variants are associated with faster cognitive decline, while their impact on DBS motor benefit is less clear. STATE OF THE ART: To assess whether DBS motor response differs by genotype in PD by pooling change in Unified Parkinson's Disease Rating Scale III (UPDRS III) (medication-OFF) from baseline to the first postoperative follow-up (T1, ~12 months). Secondary objectives were to summarise cognitive and quality-of-life outcomes

At home adaptive dual target deep brain stimulation in Parkinson's disease with proportional control.

Continuous deep brain stimulation (cDBS) of the subthalamic nucleus (STN) or globus pallidus is an effective treatment for the motor symptoms of Parkinson's disease. The relative benefit of one region over the other is of great interest but cannot usually be compared in the same patient. Simultaneous DBS of both regions may synergistically increase the therapeutic benefit. Continuous DBS is limited by a lack of responsiveness to dynamic, fluctuating symptoms intrinsic to the disease. Adaptive DBS (aDBS) adjusts stimulation in response to biomarkers to improve efficacy, side effects, and effici

Comparison of Dentatorubrothalamic Tractography Methods Based on the Anatomy of the Rubral Wing.

BACKGROUND AND OBJECTIVES: Precise localization of the dentatorubrothalamic (DRT) tract can facilitate anatomic targeting in MRI-guided high-intensity focused ultrasound (HIFU) thalamotomy and thalamic deep brain stimulation for tremor. The anatomic segment of DRT fibers adjacent to the ventral intermediate nucleus of the thalamus (VIM), referred to as the rubral wing (RW), may be directly visualized on the fast gray matter acquisition T1 inversion recovery. We compared reproducibility, lesion overlap, and clinical outcomes when reconstructing the DRT tract using a novel anatomically defined R

Association of polygenic risk score with response to deep brain stimulation in Parkinson's disease.

BACKGROUND: Deep brain stimulation (DBS) is a well-established treatment option for select patients with Parkinson's Disease (PD). However, response to DBS varies, therefore, the ability to predict who will have better outcomes can aid patient selection. Some PD-related monogenic mutations have been reported among factors that influence response to DBS. However, monogenic disease accounts for only a minority of patients with PD. The polygenic risk score (PRS) is an indication of cumulative genetic risk for disease. The PRS in PD has also been correlated with age of onset and symptom progressio
