Revisiting the L-Dopa Response as a Predictor of Motor Outcomes After Deep Brain Stimulation in Parkinson's Disease.

Objective: To investigate the correlation between preoperative response to the L-dopa challenge test and efficacy of deep brain stimulation (DBS) on motor function in Parkinson's disease (PD). Methods: We retrospectively reviewed the data of 38 patients with idiopathic PD who underwent DBS surgery with a median follow-up duration of 7 months. Twenty underwent bilateral globus pallidus interna (GPi) DBS, and 18 underwent bilateral subthalamic nucleus (STN) DBS. The Movement Disorder Society Unified Parkinson Disease Rating Scale-Motor Part (MDS UPDRS-III) was assessed before surgery and at the

Success and Failure With the Antidyskinetic Effect of Globus Pallidus Internus Deep Brain Stimulation: A Sweet Spot and Connectivity Analysis in the CSP-468 Cohort.

OBJECTIVES: Deep brain stimulation (DBS) of the globus pallidus internus (GPi) can reduce levodopa-induced dyskinesias, yet GPi targeting sometimes fails or worsens them, suggesting distinct prokinetic/antikinetic circuit mechanisms. Using the broad globus pallidus (GP) sampling from the Veteran Affairs Cooperative Study Program 468 (CSP-468) trial, we sought to identify stimulation zones and connectivity maps that predict dyskinesia outcomes in patients treated with GPi-DBS. MATERIALS AND METHODS: Data were drawn from the multicenter, randomized CSP-468 study which compared subthalamic nucle

Association between choroid plexus susceptibility features and motor outcomes following deep brain stimulation in Parkinson's disease.

OBJECTIVE: Deep brain stimulation (DBS) of the subthalamic nucleus (STN) is an effective therapy for advanced Parkinson's disease (PD), but postoperative motor outcomes show substantial interindividual variability. Recent evidence suggests choroid plexus (ChP) dysfunction in PD pathophysiology. This study aimed to investigate the association between preoperative ChP MRI characteristics and postoperative motor outcomes following STN-DBS in PD patients. METHODS: Forty-one PD patients who underwent bilateral STN-DBS were included for final analysis. Motor outcomes were assessed using the Movemen

Clinical Efficacy and Dosing of Vibrotactile Coordinated Reset Stimulation in Motor and Non-motor Symptoms of Parkinson's Disease: A Study Protocol.

Enhanced neuronal synchronization of the subthalamic nucleus (STN) is commonly found in PD patients and corresponds to decreased motor ability. Coordinated reset (CR) was developed to decouple synchronized states causing long lasting desynchronization of neural networks. Vibrotactile CR stimulation (vCR) was developed as non-invasive therapeutic that delivers gentle vibrations to the fingertips. A previous study has shown that vCR can desynchronize abnormal brain rhythms within the sensorimotor cortex of PD patients, corresponding to sustained motor relief after 3 months of daily treatment. A Review of Cognitive Outcomes Across Movement Disorder Patients Undergoing Deep Brain Stimulation.

Introduction: Although the benefit in motor symptoms for well-selected patients with deep brain stimulation (DBS) has been established, cognitive declines associated with DBS can produce suboptimal clinical responses. Small decrements in cognition can lead to profound effects on quality of life. The growth of indications, the expansion of surgical targets, the increasing complexity of devices, and recent changes in stimulation paradigms have all collectively drawn attention to the need for re-evaluation of DBS related cognitive outcomes. Methods: To address the impact of cognitive changes foll

Subthalamic stimulation in Parkinson's disease: restoring the balance of motivated behaviours.

Addictions to dopaminergic drugs or to pleasant behaviours are frequent and potentially devastating neuropsychiatric disorders observed in Parkinson's disease. They encompass impulse control disorders, punding and dopamine dysregulation syndrome. A relationship with dopaminergic treatment is strongly suggested. Subthalamic stimulation improves motor complications and allows for drastic reductions in medication. This treatment might, therefore, be considered for patients with behavioural addictions, when attempts to reduce dopaminergic medication have failed. However, conflicting data have repo

Rewiring reward in addiction: a systematic review of human evidence for deep brain stimulation and lesion approaches in substance use disorders.

OBJECTIVE: Neurosurgical modulation of reward circuitry has been explored for severe, treatment-refractory substance use disorders (SUDs), yet the human literature remains fragmented across targets, modalities, and outcomes, limiting target selection and future trial design. We systematically reviewed human clinical evidence for deep brain stimulation (DBS) and lesion approaches targeting reward network nodes for SUDs, and summarized convergent preclinical findings to identify where the strongest clinical signal exists and to provide a framework for mechanism-informed future trials. METHODS:

Interventional MRI-guided deep brain stimulation in pediatric dystonia: first experience with the ClearPoint system.

OBJECT: The placement of deep brain stimulation (DBS) leads in adults is traditionally performed using physiological confirmation of lead location in the awake patient. Most children are unable to tolerate awake surgery, which poses a challenge for intraoperative confirmation of lead location. The authors have developed an interventional MRI (iMRI)-guided procedure to allow for real-time anatomical imaging, with the goal of achieving very accurate lead placement in patients who are under general anesthesia. METHODS: Six pediatric patients with primary dystonia were prospectively enrolled. Pat

Effects of early vs. late initiation of levodopa treatment in hemiparkinsonian rats.

We investigated the effect of early vs. late initiation of levodopa treatment on dyskinetic movements, rotational behavior and molecular markers in hemiparkinsonian rats. Male Sprague-Dawley rats received a unilateral 6-hydroxydopamine (6-OHDA) administration in the nigrostriatal pathway. Rats were divided into three groups treated with: (i) levodopa (6 mg/kg) twice daily for 22 days starting at 4 weeks after 6-OHDA (Early group); (ii) levodopa at the same dose, regimen and duration but starting at 12 weeks after 6-OHDA (Late group), and (iii) saline starting at 4 weeks after 6-OHDA and contin

The connections of the dopaminergic system with the striatum in rats and primates: an analysis with respect to the functional and compartmental organization of the striatum.

This Commentary compares the connections of the dopaminergic system with the striatum in rats and primates with respect to two levels of striatal organization: a tripartite functional (motor, associative and limbic) subdivision and a compartmental (patch/striosome-matrix) subdivision. The topography of other basal ganglia projections to the dopaminergic system with respect to their tripartite functional subdivision is also reviewed. This examination indicates that, in rats and primates, the following observations can be made. (1) The limbic striatum reciprocates its dopaminergic input and in a

[An 81-year-old woman with progressive motor disturbance, extrapyramidal features, dementia, and oculomotor palsy].

We report an 81-year-old woman who presented with motor disturbance in her right hand which was followed by parkinsonism, dementia, and supranuclear gaze palsy. She was well until her age of 73 (1989) when she had an onset of difficulty in using her right hand; she did not have weakness. She also developed small step gait. These symptoms had progressively become worse. She was admitted to our hospital in July of 1992 when she was 75 years old. On admission, she was alert and oriented, but she showed some difficulty in recent memory. She did not have aphasia or ideomotor apraxia, but she showed

Deep brain stimulation for dystonia.

The few controlled studies that have been carried out have shown that bilateral internal globus pallidum stimulation is a safe and long-term effective treatment for hyperkinetic disorders. However, most recent published data on deep brain stimulation (DBS) for dystonia, applied to different targets and patients, are still mainly from uncontrolled case reports (especially for secondary dystonia). This precludes clear determination of the efficacy of this procedure and the choice of the 'good' target for the 'good' patient. We performed a literature analysis on DBS for dystonia according to the

Deep brain stimulation under general anaesthesia for Parkinson's disease: A retrospective evaluation of perioperative events and anaesthetic management after switching away from conscious sedation.

BACKGROUND: Deep brain stimulation has become the standard of care for refractory Parkinson's disease. Neuroimaging advances have led to image-based targeting of the subthalamic nucleus under general anaesthesia (GA), an approach that renders unnecessary microelectrode recording and stimulation tests under local anaesthesia and conscious sedation (CS). We aimed to compare procedures and incidents related to each anaesthetic approach. METHODS: Retrospective descriptive comparison of deep brain stimulation under CS versus G. We collected patient and procedure data (e.g., comorbidities, difficul

Plasma pTau217 and Neurofilament Light Chain as Differentiative Markers for Progressive Supranuclear Palsy From Parkinson Disease.

BACKGROUND AND OBJECTIVES: Progressive supranuclear palsy (PSP) is a 4-repeat tauopathy neurodegenerative disorder that presents with parkinsonism features and is often indistinguishable from Parkinson disease (PD) in the early stages. The potential of plasma levels of phosphorylated tau (pTau) to differentiate PSP from PD remains unclear. The aim of this study was to identify noninvasive, cost-effective alternatives to tau PET imaging for differentiating PSP from PD. METHODS: We measured plasma biomarkers, including pTau species (pTau217, pTau181, and pTau231), neurofilament light chain (NfL

Botulinum Toxin for the Management of Parkinson's Disease: A Systematic Review.

Parkinson's disease (PD) is a terminal, debilitating neurodegenerative disorder typically affecting individuals over 60. It is associated with various conditions that drastically affect the patient's quality of life (QoL). Although there is no cure for PD, its symptoms can be significantly improved and even resolved through different treatments. Mainstay treatments for PD include levodopa combined with carbidopa, dopamine agonists, and even deep brain stimulation (DBS) of the subthalamic nucleus. New treatment methods have emerged, such as botulinum toxin (BoNT), which further improve symptoms

Subcortical Intermittent Theta-Burst Stimulation (iTBS) Increases Theta-Power in Dorsolateral Prefrontal Cortex (DLPFC).

INTRODUCTION: Cognitive symptoms from Parkinson's disease cause severe disability and significantly limit quality of life. Little is known about mechanisms of cognitive impairment in PD, although aberrant oscillatory activity in basal ganglia-thalamo-prefrontal cortical circuits likely plays an important role. While continuous high-frequency deep brain stimulation (DBS) improves motor symptoms, it is generally ineffective for cognitive symptoms. Although we lack robust treatment options for these symptoms, recent studies with transcranial magnetic stimulation (TMS), applying intermittent theta

Effects of implantation of a deep brain stimulation device on patient weight in Parkinson's disease and essential tremor.

OBJECTIVE: Deep brain stimulation (DBS) is a well-established therapy for treating neurological movement disorders. Some patients who have received DBS therapy have noticed significant weight gain. Further investigation into correlations between patient characteristics and weight gain following DBS device implantation, which the authors here have done, will provide physicians with useful clinical information. METHODS: The authors performed a retrospective study of patients with Parkinson's disease (PD) and essential tremor (ET) who had received DBS therapy in the period from 2012 to 2017. Pat

Globus Pallidus Internus (GPi) Deep Brain Stimulation for Parkinson's Disease: Expert Review and Commentary.

INTRODUCTION: The globus pallidus internus (GPi) region has evolved as a potential target for deep brain stimulation (DBS) in Parkinson's disease (PD). DBS of the GPi (GPi DBS) is an established, safe and effective method for addressing many of the motor symptoms associated with advanced PD. It is important that clinicians fully understand this target when considering GPi DBS for individual patients. METHODS: The literature on GPi DBS in PD has been comprehensively reviewed, including the anatomy, physiology and potential pitfalls that may be encountered during surgical targeting and post-ope

Long-term outcomes following deep brain stimulation for Parkinson's disease.

OBJECTIVE: Deep brain stimulation (DBS) is an effective treatment for several movement disorders, including Parkinson's disease (PD). While this treatment has been available for decades, studies on long-term patient outcomes have been limited. Here, the authors examined survival and long-term outcomes of PD patients treated with DBS. METHODS: The authors conducted a retrospective analysis using medical records of their patients to identify the first 400 consecutive patients who underwent DBS implantation at their institution from 1999 to 2007. The medical record was used to obtain baseline de
