Digital templating and preoperative deformity analysis with standard imaging software.

Analysis of deformity and subsequent correction are the basis for many orthopaedic surgical procedures. In advanced cases of joint degeneration, arthroplasty may be the only available treatment option. Until recently, these analyses and preoperative surgical plans have been performed using standard radiographs, tracing paper, and/or plastic overlays. Numerous customized, commercially available, computer-based preoperative planning software programs have been introduced. The purposes of this study were to describe (1) the techniques used in deformity analysis and preoperative surgical planning

Fresh osteochondral allografts: results in the patellofemoral joint.

UNLABELLED: Twenty knees in 18 patients were treated (mean age, 42 years; range, 19-64 years) with fresh osteochondral allografting limited to the patellofemoral joint. The knees were analyzed retrospectively to determine the rate of successful outcomes. The trochlea and patella were treated in 12 patients and the patella was treated in eight patients. There were 11 women and seven men. The primary outcome measures were revision allografting, arthrodesis, or arthroplasty, and clinical scoring using a modified Merle D'Aubigné-Postel 18-point scale. Radiographs were available for 12 knees. There

Serological bone markers and joint damage in early polyarthritis.

OBJECTIVE: To investigate the relationship between osteocalcin (OC), a marker of bone formation, and the recently developed serum marker of bone resorption, beta-C-telopeptide (beta-CTx), and radiographic damage in patients with early oligo- and polyarthritis. METHODS: Patients with peripheral arthritis of > or = 2 joints and < 2 years of symptom duration were studied. The OC and beta-CTx concentrations at baseline were correlated with disease activity and radiographic damage at baseline, and with radiographic progressive disease after 2 years (delta Sharp/van der Heijde score > or = 5). Suture anchor repair of ulnar-sided triangular fibrocartilage complex tears.

Traditional open repair of traumatic triangular fibrocartilage complex (TFCC) tears requires a relatively extensive exposure, and arthroscopic repair, though conceptually simple, can be technically demanding. We describe a mini-open suture anchor technique that, while minimally invasive, is easier to perform than previously described open or arthroscopic techniques. Results achieved using this technique in eight cases compare favourably with those reported for other techniques.

Arthroscopic debridement for the treatment of Kienbock's disease.

The treatment results of seven patients (age range, 37 to 74 years, male:female ratio, 1:6) with Kienbock's disease (Lichtman stage IIIA-IIIB) who underwent arthroscopic debridement of the necrotic lunate bone and degenerative intrinsic ligaments were studied retrospectively. Osteoarthritic changes of the articular cartilage were documented in all cases. Partial (n = 2) and complete (n = 5) ruptures of the lunotriquetral and scapholunate ligaments, local synovitis (n = 5), and loose fragments (n = 6) occurred most commonly in the radiocarpal joint. All patients were available for an average of

More than Just Type 1 or Type 2: Radiologically and Anatomically Refined Lunate Classification Correlating Ulnar Carpal Alignment and Hamate-Lunate Osteoarthrosis.

Background: Hamate-lunate impingement or osteoarthritis can be a cause of ulnar-sided wrist pain. In the literature, the lunate has commonly been classified according to the configuration of its distal articular surface into type 1 and type 2, as described by Viegas. A type 1 lunate possesses only a distal articular surface for the capitate, while a type 2 lunate shows an additional medial facet articulating directly with the hamate. Type 2 lunates have been identified as a risk factor for ulnar-sided wrist pain and the development of osteoarthritis in the midcarpal wrist. However, this does n

Factors influencing outcomes of nonsurgical treatment for baseball players with SLAP lesion.

BACKGROUND/OBJECTIVE: Superior labrum anterior posterior (SLAP) lesion of the shoulder joint occurs in throwing athletes as a results of overuse, such as from repetitive baseball pitching. Nonsurgical treatments are usually the first-line therapy for symptomatic SLAP lesion. However, some patients cannot obtain satisfactory improvement of their symptoms, leading to dysfunction of the shoulder and diminished pitching performance. The purpose of this study was to analyze factors that influence outcomes of nonsurgical treatments for SLAP lesion. METHODS: Forty-five baseball players with SLAP les

Assessment of adult hip dysplasia and the outcome of surgical treatment.

Hip dysplasia and hip joint deformities in general are recognized as possible precursors of osteoarthritic development. Early and correct identification of hip dysplasia is important in order to offer timely joint preserving treatment. In the contemporary literature, several controversies exist, and some of these were the focus of this doctoral thesis. Categorized into subjects, the major findings and their possible importance are listed below. DIAGNOSTIC ASSESSMENT OF HIP DYSPLASIA: A multi-observer study quantified the variability of different methods for diagnostic assessment of hip dysplas

Can Radiographs Predict the Use of Modular Stems in Developmental Dysplasia of the Hip?

BACKGROUND: Abnormal anatomy frequently results in the use of a modular stem in patients undergoing primary total hip arthroplasty (THA) for developmental dysplasia of the hip (DDH). However, because these stems are not always available in the operating room, it would be helpful if standard radiographic views could be analyzed in such a way that patients whose femoral anatomy might call for stem modularity could be anticipated before surgery. To our knowledge, no such parameters have been defined. QUESTIONS/PURPOSES: In the senior author's practice, we used femoral neck anteversion of more th

The Definition and Measurement of Axial Psoriatic Arthritis.

This review seeks to update the state of the art of axial psoriatic arthritis (axPsA). The definition and assessment of axPsA can be problematic because no agreement and no definitive data on this topic have been published, resulting in uncertainty as to the best approach to deal with these patients. A few recent scientific reports show new data on the possible coincidence of diffuse idiopathic skeletal hyperostosis and axPsA, as well as on the radiological assessment as measured with the validated instruments for axPsA. Moreover, the role of magnetic resonance imaging has also been evaluated

Contemporary imaging of the temporomandibular joint.

This article discusses imaging techniques for visualization of the temporomandibular joint. Conventional plain film modalities are discussed briefly, with an emphasis on the more contemporary modalities, such as CT with cone-beam technology, MRI, and nuclear imaging, including single-photon emission computed tomography, and positron emission tomography. Indications, advantages, and limitations are discussed. As advancements in this area continue, our understanding of this complex joint and its pathology will follow, which will lead to more defined imaging indications and ultimately, to improve

Methicillin-resistant Staphylococcus aureus-induced septic arthritis after anterior cruciate ligament reconstruction.

We encountered a case of methicillin-resistant Staphylococcus aureus (MRSA)-induced septic arthritis after anterior cruciate ligament (ACL) reconstruction. A 20-year-old woman underwent left double-bundle ACL reconstruction by use of the hamstring tendon 18 months after her injury. She had a fever higher than 39 degrees C, and swelling of the left knee developed on day 5 after surgery. The white blood cell count was 19,500/microL, and the C-reactive protein level was 16.9 mg/dL. Arthroscopic debridement with joint lavage was performed on day 7, followed by intermittent articular irrigation. Be

Clinical, laboratory and immunogenetic aspects of arthritis associated with chronic lymphocytic thyroiditis.

OBJECTIVE: Synovial fluid effusions and/or arthritis may be found in patients with chronic lymphocytic thyroiditis (CLT), even in the absence of hypothyroidism. Sometimes these arthropathies can be attributed to the rheumatic diseases frequently associated with CLT, but in some instances the arthritis seems to be independent of any of the diseases known to cause arthritis, and therefore remains unclassified. This study was carried out in an attempt to characterize the type and outcome of arthritis associated with CLT. METHODS: We performed a prospective study with a follow-up of 6.42 years (r
