BACKGROUND: We describe the preliminary clinical results of a new operative technique for MPFL reconstruction using a strip of quadriceps tendon (QT).
PATIENTS: 17 patients (7 male, 10 female; mean age 21.5 years ± 3.9) have been operated on with this technique. All patients were evaluated clinically, radiologically and with subjective questionnaires (Tegner-, Lysholm-, Kujala Score) pre-operatively and post-operatively at 6 and 12 months (m).
SURGICAL TECHNIQUE: A 10 to 12 mm wide, 3mm thick and 8 to 10 cm long strip from the central aspect of quadriceps tendon is harvested subcutaneously.

BACKGROUND: The medial patellofemoral ligament (MPFL) is the primary restraint to extreme lateral displacement and is typically disrupted with an acute lateral patellar dislocation. Patients who fail a comprehensive nonoperative program and experience recurrent lateral patellar instability episodes are candidates for surgical treatment. Current surgical procedures include a variety of proximal realignment techniques, including repair or reconstruction of the MPFL along with distal realignment of the tibial tubercle when indicated.
PURPOSE: The objective of this study was to review the clinica

BACKGROUND: Patella alta is common in children with cerebral palsy (CP), often resulting from quadriceps spasticity and tendon elongation. In typically developing children, patella alta is linked to trochlear dysplasia and instability. Whether these associations exist in CP, and how they vary with age and functional ability, has not been established.
PURPOSE: We sought to determine whether patella alta is associated with alterations in trochlear morphology in ambulatory children with CP and to evaluate the influence of age and functional severity.
METHODS: We conducted a retrospective study

BACKGROUND: Dancers sustain lower limb musculoskeletal injuries at rates exceeding most other athletic populations, yet few rehabilitation protocols account for the constraints specific to dancer populations - concern over altered body aesthetics from hypertrophy, narrow recovery windows imposed by performance schedules, and poor tolerance of heavy loading in early post-surgical phases. Blood flow restriction (BFR) training produces muscular adaptations at loads as low as 20-30% of one-repetition maximum, a profile that could address each of these constraints. This scoping review mapped the av

BACKGROUND AND PURPOSE: Patellofemoral pain (PFP) is a common condition in women and is associated with high recurrence rates. Kinematic alterations are among the factors that may persist even after muscle strengthening programs. Given the multifactorial nature of PFP, non-mechanical factors, such as kinesiophobia, may be related to the movement patterns adopted by this population. This study aimed to investigate the associations between kinesiophobia, pain-related factors, and lower limb kinematics during uphill walking and treadmill running in women with PFP.
METHODS: This was a cross-secti

The tibial tubercle osteotomy (TTO) is a versatile surgical approach for patellofemoral joint conditions, with nonunion, fracture, and recurrent instability as common indications for revision. This case series describes the management and outcomes of revision TTOs performed for these complications. A retrospective review of 20 patients undergoing revision TTO was conducted. Patients were included if they underwent revision for nonunion, delayed union, or fracture after primary TTO. Data collected included patient demographics, primary and revision TTO details, concurrent procedures, biological

OBJECTIVE: The efficacy of secondary patellar resurfacing (SPR) in alleviating anterior knee pain (AKP) following total knee arthroplasty (TKA) remains uncertain. The purpose of this study was to assess the suitability of SPR using single photon emission computed tomography/computed tomography (SPECT/CT) in conjunction with triple-phase bone scan (TPBS).
METHODS: We performed a retrospective analysis on a prospectively-enrolled cohort of patients suffering from AKP in the context of TKA. In this cohort, we identified a subset of 17 patients (involving 18 knees), who had unexplained AKP and we

INTRODUCTION AND PURPOSE: Ensuring proper femoral component alignment post-Total Knee Arthroplasty (TKA) is crucial for normal patellofemoral (PF) kinematics. However, the customary 3° external rotation relative to the Posterior Condylar Axis (PC Axis) may not universally apply, and the expected final femoral component rotation remains unclear in functionally aligned knees. This study examines the relation between the Transepicondylar Axis (TEA) and PC axis, known as Posterior Condylar Angle (PCA) in Indian patients along with factors influencing PCA, and the feasibility of reproducing patient

The purpose of this study was to compare radiologic, morphometric, and clinical outcomes between kinematically aligned (KA) and mechanically aligned (MA) total knee arthroplasty (TKA) in Korean patients. Overall, 168 patients who underwent primary TKA were retrospectively reviewed, and propensity matching (age, sex, and body mass index) was performed as 1:3 ration (KA TKAs [n = 42]: MA TKAs [n = 126]). Joint-line orientation angle (JLOA), coronal and axial alignments of implants, hip-knee-ankle (HKA) angle, and patellar tilt angle were assessed using full-length standing radiograph, axial comp

BACKGROUND: The purpose of this study was to examine the effect of debridement (CL-Deb) versus observation (CL-noDeb) of unstable chondral lesions on knee pain 5 years after arthroscopic partial meniscectomy (APM) in patients enrolled in the Chondral Lesions And Meniscus Procedures (ChAMP) Trial. Secondarily, other knee symptoms, function, general health, and the rate of additional surgery on the affected knee were examined.
METHODS: Patients aged ≥30 years who had an unstable Outerbridge grade-II, III, or IV chondral lesion when undergoing APM were randomly allocated to the CL-Deb (n = 98) o

AIMS: Although knee osteoarthritis (OA) is diagnosed and monitored radiologically, actual full-thickness cartilage loss (FTCL) has rarely been correlated with radiological classification. This study aims to analyze which classification system correlates best with FTCL and to assess their reliability.
METHODS: A prospective study of 300 consecutive patients undergoing unilateral total knee arthroplasty (TKA) for OA (mean age 69 years (44 to 91; standard deviation (SD) 9.5), 178 (59%) female). Two blinded examiners independently graded preoperative radiographs using five common systems: Kellgre

PURPOSE: The descending geniculate artery's branching pattern includes periosteal vessels supplying the cartilage-bearing trochlea of the medial patellofemoral joint. Previous cadaveric studies described anatomic similarities between the greater curvature of the proximal scaphoid and the convex surface of the medial femoral trochlea (MFT). We describe the technique and report our first 16 consecutive cases of vascularized osteocartilaginous arthroplasty for chronic scaphoid proximal pole nonunions using the MFT, with a minimum of 6 months of follow-up.
METHODS: Chart reviews of 16 consecutive

BACKGROUND: The prevalence of knee osteoarthritis is traditionally based on radiographic findings, but magnetic resonance imaging is now being used to provide better visualization of bone, cartilage, and soft tissues as well as the patellar compartment. The goal of this study was to estimate the prevalences of knee features defined on magnetic resonance imaging in a population and to relate these abnormalities to knee osteoarthritis severity scores based on radiographic findings, physical functioning, and reported knee pain in middle-aged women.
METHODS: Magnetic resonance images of the knee

PURPOSE: Unicompartmental knee arthroplasty (UKA) for isolated lateral compartment osteoarthritis (OA) is less common, technically demanding and is potentially associated with an increased revision risk relative to total knee arthroplasty. This review aimed to assess survivorship, complications and patient-reported outcome measures (PROMs) of robotic-assisted (RA) lateral UKA.
METHODS: A systematic review and meta-analysis were undertaken and registered with PROSPERO and followed Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines. Data search was performed

A select subset of patients can present with anterior knee pain in the setting of normal patellar tracking, no significant cartilage damage, and the presence of a bone marrow lesion (BML) involving the patella on advanced imaging. One novel treatment option for this condition is patellar subchondroplasty, where calcium phosphate is injected into the subchondral bone under fluoroscopic guidance. The purpose of this study is to report preliminary outcomes of patients who have undergone subchondroplasty of the patella. The surgical log of the senior author was retrospectively reviewed to identify
