Ecological momentary assessment (EMA) is a useful tool for assessing peoples' experiences, behaviors, and environments in everyday life. EMA is considered a standard for self-reporting, because its short reporting periods and multiple assessments in everyday life offers reduced recall bias and increased ecological validity. In this study we tested how well within-person EMA measurements of levels of physical activity (prompt-level data) match objective reports of activity from accelerometry. We looked at two kinds of validity: duration of EMA recall (5 m; 120 m) and type of physical activity r

BACKGROUND: Tremor is one of the cardinal motor symptoms of Parkinson's disease (PD). Its severity can be quantified with clinical rating scales and neurophysiology. The pathophysiology of PD tremor involves activity in the cerebello-thalamo-cortical circuit (CTC), which can be quantified with concurrent accelerometry-functional magnetic resonance imaging (fMRI). This opens possibilities for fMRI-informed precision medicine, but it remains unclear if tremor-related fMRI activity is a reliable biomarker at the individual level.
OBJECTIVES: To assess the test-retest reliability of three PD trem

Patients with Duchenne muscular dystrophy (DMD) develop skeletal muscle weakness and cardiomyopathy. Validated skeletal muscle outcome measures are limited to ambulatory patients, but most DMD patients in cardiac trials are non-ambulatory. New objective functional assessments are needed. This study's objective was to assess the correlation and longitudinal change of two measures: quantitative muscle testing (QMT) and accelerometry. Patients with DMD were prospectively enrolled and underwent QMT and wore wrist and ankle accelerometers for seven days at baseline, 1-, and 2-years. QMT measures we

Duchenne muscular dystrophy (DMD) is characterized by muscle deterioration and progressive weakness. As a result, patients with DMD have significant cardiopulmonary morbidity and mortality that worsens with age and loss of ambulation. Since most validated muscle assessments require ambulation, new functional measures of DMD progression are needed. Despite several evaluation methods available for monitoring disease progression, the relationship between these measures is unknown. We sought to assess the correlation between imaging metrics obtained from cardiac magnetic resonance imaging (CMR) an

BACKGROUND: Duchenne muscular dystrophy (DMD) is associated with progressive muscle weakness and respiratory decline. To date, studies have focused on respiratory decline and loss of ambulation as a metric of loss of skeletal muscle strength. However, new functional measures can assess skeletal muscle disease regardless of ambulatory status. The relationship between these tests and concurrent lung function is currently unexplored.
OBJECTIVE: To assess the correlation between spirometry measurements and functional muscle assessments such as accelerometry and quantitative muscle testing (QMT).

BACKGROUND: Activity pattern features measured by continuous accelerometry are strongly associated with frailty, making accelerometry a promising, scalable screening tool for frailty. However, adherence to typical 5-7 day wear protocols decreases with age, reducing feasibility. Whether accelerometry measures collected over shorter wear periods predict frailty decline is unknown. In cross-section, we have shown that frailty is most strongly differentiated by morning activity patterns.
OBJECTIVE: The objective of this study was to determine whether two morning activity measures, mean hourly act

OBJECTIVE: The purpose of this study was to compare physical activity (PA) in a group of patients with psoriatic arthritis (PsA) versus healthy controls and to determine whether the mobility of these patients is affected by disease activity.
METHODS: A group of 52 patients with PsA and 53 controls were included in this case-control study. PA was assessed by accelerometry in both groups and additionally with the International Physical Activity Questionnaire (IPAQ) in patients with PsA. Multiple regression analysis was used to compare PA between groups and to determine the relationship between

BACKGROUND CONTEXT: Lumbar spinal stenosis (LSS) and knee osteoarthritis (OA) are 2 of the leading causes of disability worldwide. In order to provide disease-specific prescriptions for physical activity, there is a clear need to better understand physical activity in daily life (performance) in these populations.
PURPOSE: To discover performance phenotypes for LSS and OA by applying novel analytical methods to accelerometry data. Specific objectives include the following: (1) to identify characteristic features (phenotypes) of free-living physical activity unique to individuals with LSS and

BACKGROUND: Generation of functional tremor relies on the structures involved in the control of voluntary movements. The clinical diagnosis is based on the presence of "positive signs", which are expression of cognitive and motor distractibility and reflect functional tremor dependence on explicit motor control. In patients who manifest less distractibility, habitual (implicit) control may be of greater significance. Habitual behaviours are inflexible and difficult to eradicate.
OBJECTIVES: To investigate if motor and cognitive distractibility predicts response to treatment with repetitive tr

BACKGROUND: Sleep problems are both symptoms of and modifiable risk factors for many psychiatric disorders. Wrist-worn accelerometers enable objective measurement of sleep at scale. Here, we aimed to examine the association of accelerometer-derived sleep measures with psychiatric diagnoses and polygenic risk scores in a large community-based cohort.
METHODS AND FINDINGS: In this post hoc cross-sectional analysis of the UK Biobank cohort, 10 interpretable sleep measures-bedtime, wake-up time, sleep duration, wake after sleep onset, sleep efficiency, number of awakenings, duration of longest sl

BACKGROUND AND OBJECTIVES: Glycogen storage disease type III (GSD-III) is a genetic metabolic disorder affecting liver and muscle, frequently leading to walking difficulties in adults. This study aimed to quantify walking performance and its progression in GSD-III, identify relevant outcome measures for future clinical trials, and determine predictors of gait decline.
METHODS: Participants enrolled in the French GSD-III registry underwent an annual assessment of their 6-min walk distance (6MWD) combined with 3D trunk accelerometry. Muscle strength for flexion and extension of the knee and ank

BACKGROUND: Treatment for essential tremor (ET) and cortical myoclonus (CM) differs. As their clinical distinction can be difficult, with large inter- and intra-observer variability, there is a need for additional diagnostic tools.
OBJECTIVES: We aim to develop a machine learning based approach using accelerometry data to support clinicians in their diagnosis of ET versus CM.
METHODS: We recorded the upper body movements of 19 ET and 19 CM patients performing 21 tasks using eight accelerometry sensors. The phenotype was classified by applying an explainable machine learning method called gen

BACKGROUND: The ability of the Essential Tremor (ET) Rating Assessment Scale (TETRAS) to detect changes in tremor severity is unknown.
METHODS: Fifteen adult ET patients received a single oral ethanol dose calculated to reach 0.05 g/dL breath alcohol content (brAC). Effects were investigated independently with accelerometry and TETRAS.
RESULTS: Accelerometry data were log-transformed and a cumulative score logACC(R+L) was calculated. Correlation between logACC(R+L) and TETRAS was significant. TETRAS and accelerometry showed a significant effect of time point using repeated-measures analysis

INTRODUCTION: This study examined the potential of a device agnostic approach for predicting physical activity from consumer wearable accelerometry compared with a research-grade accelerometry.
METHODS: Seventy-five 5- to 12-year-olds (58% male, 63% White) participated in a 60-min protocol. Children wore wrist-placed consumer wearables (Apple Watch Series 7 and Garmin Vivoactive 4) and a research-grade device (ActiGraph GT9X) concurrently with an indirect calorimeter (COSMED K5). Activity intensities (i.e., inactive, light, moderate-to-vigorous physical activity) were estimated via indirect c

OBJECTIVE: To evaluate the clinical usefulness of a non-invasive accelerometric device to diagnose GER in preterm babies.
STUDY DESIGN: An accelerometer was taped over the sub-xiphoid process in 110 preterm (GA 29.6 ± 3.3 wk) infants (133 studies). Low frequency, sub-audible signals were captured via digital recording (sampling rate 200 Hz), then re-sampled (rate = 60 Hz) to create a spectrogram (focused range 0-30 Hz). Mean amplitude in the focused range was calculated.
RESULTS: Of 85 studies with simultaneous pH-metry and accelerometry, 18 had concurrent positive and 23 had concurrent nega

We performed a pilot, double-blind, placebo-controlled, randomized trial to evaluate the efficacy and tolerability of pregabalin (PGB, Lyrica), an antiepileptic agent, in treating essential tremor (ET). Twenty two patients with ET were randomly assigned to receive PGB or placebo. PGB was initiated at 50 mg/day and was escalated by 75 mg/day every 4 days to a maximum dose of 600 mg/day. Patients were evaluated by accelerometry and the Fahn-Tolosa-Marin (FTM) rating scale. There was a significant reduction in tremor amplitude in the PGB group compared with the placebo group, as measured by accel

BACKGROUND: Ambulatory measurements of trunk accelerations can provide valuable information on the amount and quality of daily-life activities and contribute to the identification of individuals at risk of falls. We compared associations between retrospective and prospective falls with potential risk factors as measured by daily-life accelerometry. In addition, we investigated predictive value of these parameters for 6-month prospective falls.
METHODS: One week of trunk accelerometry (DynaPort MoveMonitor) was obtained in 169 older adults (mean age 75). The amount of daily activity and qualit

A clinical rating scale which measured the severity of tremor in 20 patients (12 with essential tremor and 8 with "dystonic" tremor) was assessed at specific anatomical sites for both inter and intra-rater reliability using four raters. The scores obtained with the scale were compared with the results of upper limb accelerometry, an activity of daily living self-questionnaire and estimates of the tremor induced impairment in writing and drawing specimens. The results show that, for the purposes of routine assessment and therapeutic trials, a clinical rating scale can produce reliable results w
