Accidental occupational injuries to endoscopy personnel in a high-volume endoscopy suite during the last decade: mechanisms, workplace hazards, and proposed remediation.

INTRODUCTION: Scant data exist about accidental occupational injuries in endoscopy suites. This work systematically analyzes injuries, identifies workplace hazards, and proposes hazard remediation to potentially reduce risks. METHODS: A retrospective study was performed on 14 previously unreported injuries among 120 endoscopy suite personnel in a high-volume endoscopy suite during 2000-2010, identified by medical records, employee records, and interviews. Injuries, mechanisms, outcomes, and workplace hazards were analyzed. RESULTS: Seventeen endoscopy personnel suffered occupational accident

Anterior rectal wall perforation from accidental maize stem injury: a rare case and management insight.

INTRODUCTION: Colorectal injuries are rare, with a reported incidence of 1%-3% in trauma cases, predominantly due to penetrating trauma. They may arise from pelvic trauma, ingestion, or insertion of foreign objects, often for sexual gratification, self-treatment, or accidental entry. Such injuries are frequently underreported due to embarrassment or denial, leading to delayed diagnoses. Despite their rarity, prompt diagnosis and aggressive treatment are critical to achieving favorable outcomes. CASE REPORT: A 53-year-old male from Dhankuta, Nepal, presented with anal trauma caused by a maize

What are the computed tomography patterns of head injury following low-velocity accidental head trauma in infants?

BACKGROUND: Differentiating accidental head trauma and abusive head trauma (AHT) may be challenging in infants. OBJECTIVE: To describe computed tomography (CT) scan findings following accidental low-velocity trauma in infants, focusing on features considered to be highly suggestive of AHT, such as rupture of bridging veins. MATERIALS AND METHODS: A single-center, retrospective study (2014-2022) was performed in infants aged 1 month to 12 months. INCLUSION CRITERIA: (i) any kind of minor head trauma leading to an Emergency Department visit (fall from a height of less than 2 m, direct head im

Effectiveness of Fosphenytoin and Levetiracetam to Prevent Posttraumatic Seizures in Young Children with Accidental or Abusive Traumatic Brain Injury.

BACKGROUND: Seizures occur frequently in pediatric patients with traumatic brain injury (TBI), particularly abusive head trauma (AHT). There are limited data on the effectiveness of fosphenytoin and levetiracetam to prevent posttraumatic seizures. METHODS: We performed a retrospective single center cohort study of children < 3 years old admitted with mild [Glasgow Coma Scale (GCS) score 13-15], moderate (GCS 9-12), and severe (GCS 3-8) TBI at a level I trauma center from 2011 to 2021. Antiseizure medications were used at the discretion of the treating physicians. Nonparametric tests were used

Primary normocephalic pancraniosynostosis detected incidentally after an accidental head injury: a case report and review of the literature.

OBJECTIVE: Majority of multi-suture craniosynostosis are related to single-gene disorders or chromosomal abnormalities. Children with craniosynostosis usually present at an early age due to the presence of an abnormal head shape, with the exception of a unique entity termed primary normocephalic pancraniosynostosis. The objective of this article is to describe an unusual case of primary normocephalic pancraniosynostosis, detected incidentally following an accidental head injury. A comprehensive review of the literature will also be included. To the best of our knowledge, only eight cases of pr

A two-center retrospective review of the hematologic evaluation and laboratory abnormalities in suspected victims of non-accidental injury.

Investigation for bleeding disorders in the context of suspected non-accidental injury (NAI) is inconsistent. We reviewed the hematologic evaluation of children who presented with symptoms of bleeding and/or bruising suspicious for NAI to determine the frequency of hematologic tests, abnormal hematologic laboratory results, and hematologic diagnoses. A retrospective cohort study design was employed at two freestanding academic children's hospitals. ICD-9 codes for NAI were used to identify 427 evaluable patients. Medical records were queried for the details of clinical and laboratory evaluatio

Isolated cerebral cortical tears in children: aetiology, characterisation and differentiation from non-accidental head injury.

A wide spectrum of intracranial injuries has been described as complicating difficult birth, particularly following instrumental delivery. We describe five children in whom isolated cortical tears were observed on MRI. Four cases were characterised by a difficult instrumental delivery. None of the children developed long-term neurological sequelae. As far as we are aware, isolated cerebral cortical tears have not been reported previously although recognition of this injury pattern is important because of its possible misinterpretation as a marker of a non-accidental head injury. Other differen

Bone pathology mimicking non-accidental injury in a child - Bewildered by bones, case report and review of the literature.

INTRODUCTION AND IMPORTANCE: Physical abuse of children is criminal conduct in the purview of medico-legal sciences and the confirmative diagnosis of 'child abuse' is imperative for further legal proceedings. Clinicians play a pivotal role in the protection of children by recognizing and reporting such cases and treating the victims of abuse. PRESENTATION OF CASE: To the best of our knowledge, we present the first case report in literature where osteofibrous dysplasia resulted in a pathological fracture in a 10-month-old South-Asian child which resembled the picture of child abuse. Continuous dynamic mapping to avoid accidental injury of the facial nerve during surgery for large vestibular schwannomas.

In vestibular schwannoma (VS) surgery postoperative facial nerve (CN VII) palsy is reducing quality of life. Recently, we have introduced a surgical suction device for continuous dynamic mapping to provide feedback during tumor resection without switching to a separate stimulation probe. The objective was to evaluate the reliability of this method to avoid CN VII injury. Continuous mapping for CN VII was performed in large VS (08/2014 to 11/2017) additionally to standard neurophysiological techniques. A surgical suction-and-mapping probe was used for surgical dissection and continuous monopola

Patterns of injury seen in deaths from accidental falls down a staircase: a study from South Delhi.

The present study is a retrospective analysis of 92 cases of death from an accidental fall down a staircase, which were autopsied at the All India Institute of Medical Sciences, New Delhi, India, during a ten-year period from 1996 to 2005. The cases were analysed with regard to age, sex, location, cause of death and pattern of injury. Males (62, 67.39%) outnumbered females (30, 32.61%). The commonest age group involved was 31-40 years (28.26%) followed by 21-30 years (18.47%) and 51-60 years (14.13%). The lowest number of cases (4.34%) was in the 11-20 years age group. All deaths were accident

Development of chronic pain following severe accidental injury. Results of a 3-year follow-up study.

OBJECTIVE: Motor vehicle accidents (MVA) and work-related injuries are two of the more common causes of chronic pain. Nevertheless, there is little evidence on predicting factors regarding the development of chronic pain following physical injury. METHODS: The present study investigated temporal associations between accident-related factors, psychological factors [symptoms of posttraumatic stress disorder (PTSD), anxiety, depression, coping], and the development of chronic pain in a sample of individuals who had sustained severe accidental injuries (N=90). Assessments were performed within 1

BACKGROUND: Ophthalmologic complications of endoscopic sinus surgery are rare, consequences may be serious. PATIENTS: Two patients are presented who suffered lesions of the internal rectus muscles during endoscopic ethmoidectomy. The clinical picture of this rare complication resembles that of a paralysis of the medial rectus muscle. THERAPY AND COURSE: In one patient the very thinned medial rectus muscle was tucked in a first surgical step. In the second step the external rectus muscle was recessed. Fusion in primary gaze was possible. In the second patient surgery was planned, he died howe

COVID-19-Relevant Insights Into the Elevated Risk of Accidental Injuries in Survivors of SARS and Their Relatives in Taiwan: Retrospective Cohort Study.

BACKGROUND: The 2003 outbreak of severe acute respiratory syndrome (SARS), caused by a novel coronavirus, heavily impacted Taiwan's health care system, triggering clinical crises and lasting effects among affected individuals and families. The first case in Taiwan was identified on February 25, 2003, and the final case was reported on June 15, 2003. During the epidemic, 346 people were diagnosed with SARS, leading to 37 deaths. Outbreaks also occurred in China, Singapore, and Toronto (Canada), showing the vulnerability of global health systems to new zoonotic diseases. Clinically, SARS causes

Do paediatric patients with suspected non-accidental burn injuries have worse clinical outcomes? A 5-year retrospective review from a UK burns centre.

BACKGROUND: Burns are a very common reason for attendance and admission of children to hospital. Non-accidental injury (NAI) and child protection concerns (CP) are crucially important in paediatric burns presentations - prevalence in the UK is reported between 5 % and 14 %. This range reflects challenges in identifying suspicious injuries, as well as the underlying geographic and socioeconomic variations in practice. AIMS: This study examines the demographics, management, and outcomes of paediatric burns admissions with NAI or CP concerns in a UK burns centre. METHODS: Retrospective analysis

Acupuncture is associated with lower accidental injury risk among patients with Parkinson's disease: a nationwide retrospective cohort study.

BACKGROUND: Patients with Parkinson's disease (PD) have a high risk of accidental injuries because motor and non-motor symptoms can impair gait, balance, cognition, and autonomic function. Acupuncture has been reported to improve gait and balance, but its association with long-term injury prevention in PD remains unclear. METHODS: We conducted a nationwide retrospective cohort study using Taiwan's National Health Insurance Research Database. Patients newly diagnosed with PD between 2001 and 2012 were followed through 2018. Propensity score matching (PSM) was used to improve comparability betw

Break in Corneal Shine due to Lime: Successful Restoration of Cornea after Accidental Chemical Injury - A Case Report.

Chemical burns are potentially blinding injuries and represent true ophthalmic emergencies that demand immediate evaluation and prompt initiation of treatment. Among these, alkali burns are more common than acid burns. Such injuries can cause widespread damage to the ocular surface epithelium, cornea, anterior segment structures, and limbal stem cells, often leading to permanent visual impairment in one or both eyes. We report a case of a 64-year-old female who developed diminution of vision following accidental chemical trauma with lime (calcium hydroxide). Clinical evaluation revealed deeply

Prevalence and Correlates of Poor Safety Awareness and Accidental Injury in ASD, ADHD, ASD + ADHD, and Neurotypical Youth Samples.

The purpose of the present study is to compare risk and predictors of poor safety awareness and accidental injuries in ASD, ADHD, and neurotypical samples. Neurodivergent groups (ADHD-I n = 309; ADHD-C n = 747; ASD-only n = 328; ASD + ADHD n = 1,108) were 2-17 years old. The neurotypical group (n = 186) was 6-12 years of age. Maternal ratings on the Pediatric Behavior Scale examined safety awareness, accidental injury, and psychological problems. Children with ASD + ADHD had significantly poorer safety awareness and accidental injury ratings than all other groups. Predictors of poor safety awa

[A case of hepatic portal venous gas and upper gastrointestinal mucosal injury due to accidental ingestion of hydrogen peroxide].

A 40-year-old woman was admitted to our hospital by ambulance due to accidental ingestion of 100ml of 35% hydrogen peroxide. Although the patient suffered from frequent vomiting, abdominal distension, and abdominal pain, signs of peritonitis were not observed. An abdominal computed tomography examination demonstrated obvious gas images in the gastric wall and intrahepatic portal veins. Upper gastrointestinal endoscopy revealed mucosal redness, swelling, and erosion from the lower part of the esophagus to the duodenum. Portal venous gas and upper gastrointestinal mucosal injury due to accidenta

Non-Accidental Trauma Associated with Withdrawal of Life-Sustaining Medical Treatment in Severe Pediatric Traumatic Brain Injury.

INTRODUCTION: In highly developed countries, as many as 16 percent of children are physically abused each year. Traumatic brain injury (TBI) is the most common injury in non-accidental trauma (NAT) and is responsible for 80 percent of fatal NAT cases, with most deaths occurring in children younger than three years old. Cases of abusers who refuse withdrawal of life-sustaining medical treatment (LSMT) to avoid criminal charges have previously been reported. Therefore, we hypothesized that NAT is associated with a lower risk for withdrawal LSMT in pediatric TBI. METHODS: The pediatric Trauma Qu
