NeonatAIlogy: The GIRISH (Goal, Input, Role, Iterative Refinement, Safety Verification, and Human Accountability) Framework for Structured, Human-Accountable Artificial Intelligence in Neonatal Intensive Care.

Artificial intelligence (AI) chatbots are becoming increasingly accessible at the point of care, including in the neonatal intensive care unit (NICU), yet structured guidance for their safe use remains limited. This technical report reviews AI applications across neonatology and introduces GIRISH (Goal, Input, Role, Iterative refinement, Safety verification, and Human accountability), a practical six-step framework for safe, effective, and human-accountable AI use at the NICU bedside, together with ready-to-adapt prompt templates for eight clinical domains. This is a narrative technical report

[Dyspnea as a presenting symptom in the emergency department - From triage through rational diagnosis to evidence-based patient pathways].

Accounting for 4-8% of all patients in emergency departments, dyspnea is one of the most common presenting symptoms 11 22. Its clinical significance is reflected in the high rate of admission to the intensive care unit (16-18%) 11 and an in-hospital mortality rate of approximately 9% 22. The concurrent presence of multiple organ pathologies often poses a diagnostic challenge 33. This article translates the current evidence from 2025/2026 into action-oriented strategies: from pathophysiology and AI-supported triage to critical airway management and the discussion of specialized dyspnea units 44

Hospital-acquired Infections in the Adult Intensive Care Unit: Epidemiology, Resistance Patterns, and Risk Factors.

INTRODUCTION: Hospital-acquired infections (HAIs) are defined as infections that develop in the patient after being admitted to the hospital. The overall burden of HAIs is known to be higher in developing countries. The most common HAIs include ventilator-associated pneumonia (VAP), catheter-associated urinary tract infection (CAUTI), surgical site infection (SSI), and catheter-related bloodstream infections (CRBSI). OBJECTIVES: Due to limited recent literature, we aimed to evaluate the incidence rates and causative organisms of intensive care unit (ICU)-acquired infections at our tertiary ca

Turning Compliance Into Action: An Augmented Intelligence-Enabled Framework for Advocacy Organizations to Drive Population Health Improvement.

Community Health Needs Assessments (CHNAs), mandated by the Affordable Care Act for tax-exempt hospitals, represent an underutilized yet rich data source for disease-specific advocacy. This commentary proposes a novel framework in which disease advocacy organizations-such as Alzheimer's Los Angeles, the American Heart Association, and the National Alliance on Mental Illness-deploy artificial intelligence (AI) agents to systematically analyze CHNAs, identify gaps in condition-specific care, generate personalized outreach to hospital leadership, and publicly score health systems on their respons

Insight into the Burden of Antimicrobial Resistance among Bacterial Pathogens Isolated from Patients Admitted in ICUs of a Tertiary Care Hospital in India.

Intensive care unit (ICU) patients are prone to develop infections by hospital prevalent organisms. The aim of the study was to determine the bacteriological profiles and their drug resistance pattern among different infections in ICU patients of a tertiary care hospital. The record-based retrospective data of culture reports of the patients admitted to all the ICUs of a tertiary care hospital during the period from January 2020 to May 2022 were analyzed. A total of 3,056 samples were obtained from 2308 patients. The infection rate among ICU patients was found to be 53.40%. Isolates belonged e

Three-level containment model of hospitalized adolescents with borderline pathology: a holistic therapeutic perspective.

Borderline personality disorders account for 50% of adolescent hospitalization cases in psychiatry. The severity and psychopathological complexity of these symptoms indicate the need for inclusive models of understanding. Adopting a holistic approach allows for the consideration of not only the patient's environment, but also their position within that environment and their life history. In this article, a model based on the concept of therapeutic containment at three levels is presented. Global containment refers to the mindset and organization of the institution that provides care, which is

Medical disposals and problem solving: About high blood pressure in Morocco.

In this article, I analyze how in basic health-care facilities in Morocco, general practitioners transform patients' problems into solvable problems, taking into account constraints related to medical standards, financial issues, the organization of the health system, and care. My focus is on hypertension, or high blood pressure. I argue that standards allow the solving of patients' problems through the production of an entity called high blood pressure. However, the 'high blood pressure' enacted is different from the entity defined by standards. Fragments of the latter, borrowed from other co

Critical care of patients with paediatric valvar cardiac disease.

Valvar cardiac disease accounts for a large percentage of heart lesions in the paediatric population. There are a variety of congenital and acquired valvar lesions produced by different mechanisms that produce unique physiologies. With the development of more advanced cardiac interventional procedures and congenital cardiac surgery techniques, most of these lesions can be repaired in a single or multi-stage approach. Given the potential for significant symptomatology before intervention, it is imperative to review the critical care management of paediatric valvar heart disease. The purpose of

The aseptic meningitis syndrome.

The diagnosis and treatment of acute meningitis is a challenge for the primary care physician. Differentiating between bacterial meningitis and aseptic meningitis is not always straightforward. The aseptic meningitis syndrome is usually viral in origin, and enteroviruses account for most cases. The aseptic syndrome also may be caused by unusual bacterial organisms such as Mycobacterium tuberculosis, Leptospira species, Brucella species, Borrelia burgdorferi and others. The classic presentation consists of the acute onset of meningismus, headache, fever, malaise with pleocytosis and normal gluc

Ageing: socio-economic implications for health care in Singapore.

In planning for the health of the population, especially of the aged, demographic variables which interact with social and economic factors must all be considered to assess their impact on the health services. Demographic trends such as the dependency ratio, sex ratio, marital status and family formation would also have a bearing on the care of the elderly at home. Trends in labour force participation and the extent of financial security among the elderly can therefore affect policy implications concerning public subsidy of health care and social services. As health needs are expected to rise

A National Framework for Home Parenteral Nutrition in the United States: Accreditation, Reimbursement, Registry.

PURPOSE OF REVIEW: The United States delivers home parenteral nutrition (HPN) for chronic intestinal failure through a fragmented mix of hospitals, home-infusion companies, and individual clinicians, with no national accreditation pathway and no mandated standards for prescriber expertise or institutional capability. This review proposes a national framework for accountable, outcomes-driven HPN care and a phased path to its implementation. RECENT FINDINGS: Claims-based analyses of approximately 24,000 U.S. adults on parenteral support show that most orders are written by clinicians managing v

[Organizing an active screening campaign for human African trypanosomiasis due to Trypanosoma brucei gambiense].

Organization of an active screening program for human African trypanosomiasis in an outbreak area is subject to strict guidelines that must take into account the size of the population, the specificity and sensitivity of the diagnostic techniques used, and the cost of screening. Numerous parameters can affect the outcome including accessibility of the outbreak area (road conditions, rainy season); awareness of village populations and of local administrative, traditional, and religious personalities; quality of local health-care facilities and personnel; possibility of referring patients to a h

First person account: maintaining mental health in a turbulent world.

The article and the poems that follow are part of the Schizophrenia Bulletin's ongoing First Person Accounts series. We hope that mental health professionals--the Bulletin's primary audience--will take this opportunity to learn about the issues and difficulties confronted by consumers of mental health care. In addition, we hope that these accounts will give patients and families a better sense of not being alone in confronting the problems that can be anticipated by persons with serious emotional difficulties. Although there are a number of books and articles describing the personal experience

Understanding and Addressing Wait Times in Specialized Geriatric Services: Findings From a Multi-Phase Engagement Process.

Persistent wait times in Specialized Geriatric Services increase risks for older adults living with frailty and place additional pressure on primary care. While workforce shortages contribute to delays, variation across programs suggests that access is also influenced by how referral, triage, assessment, follow-up, and care integration are organized. The Regional Geriatric Program of Toronto conducted a multi-phase engagement initiative including a targeted literature scan, stakeholder interviews and surveys, and a regional summit to identify system-level barriers and prioritize opportunities

History and Accomplishments of the North American Clinical Trials Network for Spinal Cord Injury, 2004-2022.

This is a historical account of the origin and accomplishments of the North American Clinical Trials Network (NACTN) for traumatic spinal cord injury (SCI), which was established in 2004 by Christopher Reeve and Robert Grossman. Christopher Reeve was an actor who became quadriplegic and started the Christopher & Dana Reeve Foundation (CDRF), and Robert Grossman was a neurosurgeon experienced in neurotrauma and a university professor in Houston. NACTN has member investigators at university and military centers in North America and has contributed greatly to the improvement of care, primarily ac

Sustainable Healthcare: A Perspective on the Current Landscape, Frameworks, Barriers, and Implementation Model in a Quaternary Hospital System.

Healthcare services account for approximately 4-5% of global greenhouse gas emissions worldwide, and nearly 10% of emissions in the United States are primarily from general hospital operations, surgical and anesthetic care, and pharmaceutical and medical device supply chains. Healthcare services and operations also generate substantial plastic and chemical waste that pollute the environment and disproportionately affect vulnerable communities. Although global and national organizations have introduced decarbonization frameworks, adoption remains hindered by limited leadership engagement, compe

Knowledge Gaps in Urologic Care of Female Spinal Cord Injury Patients.

PURPOSE OF REVIEW: We highlight the substantial gaps in knowledge on urologic care of female spinal cord injury (SCI) patients. RECENT FINDINGS: Males account for approximately 80% of people living with SCI in developed nations. Although there is a robust body of literature in some aspects of urologic care of individuals with SCI, such as treatments for neurogenic detrusor overactivity, there are relatively few studies focusing specifically on females. There are also few studies focusing on other aspects of urologic care of women with SCI such as sexual dysfunction, pelvic organ prolapse, and

Prophylaxis of venous thromboembolism in trauma: What you need to know.

Deep venous thrombosis and pulmonary embolism account for significant morbidity and mortality in trauma patients. Most adult major trauma patients should be initiated on mechanical and chemical prophylaxis at the time of admission, with minimal delays for traumatic brain injury, spinal cord injury, active hemorrhage, and solid organ injury patients. Low molecular weight heparin remains the standard of care with ongoing consideration for optimal dosing strategies. Alternatives including unfractionated heparin, aspirin, and direct oral anticoagulants can be considered in specific patient populat
