Colistin and rifampicin in the treatment of multidrug-resistant Acinetobacter baumannii infections.

OBJECTIVES: The increased incidence of nosocomial infections by multidrug-resistant organisms has motivated the re-introduction of colistin in combination with other antimicrobials in the treatment of infections. We describe the clinical and microbiological outcomes of patients infected with multidrug-resistant Acinetobacter baumannii who were treated with a combination of colistin and rifampicin. PATIENTS AND METHODS: Critically ill patients with pneumonia and bacteraemia caused by A. baumannii resistant to all antibiotics except colistin in medical and surgical intensive care units were enr

Resistance to disinfectants in epidemiologically defined clinical isolates of Acinetobacter baumannii.

Decreased susceptibility to biocides may contribute to epidemic spread of Acinetobacter baumannii in the hospital. This study was conducted to evaluate the susceptibility of different clinical A. baumannii strains to disinfectants. Twenty A. baumannii strains were examined, ten of which were outbreak-related and ten that were sporadic. Clinical isolates were selected on the basis of demonstrating a unique pulsed-field gel electrophoresis pattern. The in-vitro activities of propanol, combination of 1-propanol, 2-propanol and mecetronium ethylsulphate, polyvinylpyrrolidone (PVP)-iodine, triclosa

Multidrug-resistant Acinetobacter baumannii: Molecular insights, clinical challenges, and therapeutic approaches.

Acinetobacter baumannii has emerged as an agent of potentially life-threatening nosocomial infections, particularly among immunocompromised patients. Its ability to rapidly acquire resistance genes has made traditional antibiotic therapies progressively ineffective. The spread of bacterial contamination in hospital facilities increased due to lack of awareness among healthcare workers and improper management/handling of infectious samples. The persistence of pathogen in the hospital environment is increased with its ability to form biofilms, quorum sensing, and virulence factors. The infection

Metabolic rewiring of bacterial subpopulations governs polymyxin responses in Acinetobacter baumannii.

Polymyxins are often last-resort antibiotics against high-priority Gram-negative pathogens, particularly Acinetobacter baumannii. However, most mechanistic studies to date have overlooked the heterogeneity within bacterial populations during polymyxin treatment. Using time-lapse imaging and propidium iodide (PI) staining, we observed that a subset of PI-positive (PI+) cells, which are traditionally considered non-viable, were capable of regrowth. This unexpected scenario promoted further investigation into the distinct metabolic responses of PI+ or PI-negative (PI-) subpopulations following po

Comprehensive identification of sequence types belonging to Acinetobacter baumannii clonal complexes.

Acinetobacter baumannii is a major nosocomial pathogen, with multiply antibiotic-resistant (MAR) isolates primarily belonging to two globally disseminated clonal complexes, Global Clone 1 (GC1) and Global Clone 2 (GC2). A. baumannii has two MLST schemes: the Pasteur and the Oxford scheme. However, the Pasteur scheme identifies clones more simply. Under the Pasteur MLST scheme, these clones centre on sequence types 1 (ST1) and 2 (ST2), respectively, but both encompass related single-locus variants (SLVs) and double-locus variants (DLVs) and even some triple-locus variants (TLVs). Despite their

Emergence of pmrB mutation-mediated colistin resistance in clinical Acinetobacter baumannii.

OBJECTIVES: While colistin-resistant Acinetobacter baumannii continues to be discovered, this study aimed to investigate the molecular mechanism of colistin resistance in two clinical Acinetobacter baumannii strains (coli-S and coli-R) isolated from a single patient. METHODS: Antimicrobial drug susceptibility was assessed by broth microdilution method. We monitored growth curves, and analyzed biofilm formation using confocal laser scanning microscopy. Combined with MLST, next generation and third-generation sequencing techniques, the genomic differences of the strains were compared, and expre

A regulator of amino acid catabolism controls Acinetobacter baumannii gut colonization.

Asymptomatic gut colonization increases the risk of clinical infection and transmission by the multidrug-resistant pathogen Acinetobacter baumannii. Ornithine utilization was shown to be critical for A. baumannii competition with the resident microbiota to persist in gut colonization, but the regulatory mechanisms and cues are unknown. Here, we identify a transcriptional regulator, AstR, that specifically activates the expression of the A. baumannii ornithine utilization operon astNOP. Phylogenetic analysis suggests that AstR was co-opted from the Acinetobacter arginine utilization ast(G)CADBE

Therapeutic significance of surface-exposed virulence factors in Acinetobacter baumannii pathogenicity.

Acinetobacter baumannii is a rapidly evolving opportunistic pathogen that has developed strategies to resist multiple antimicrobials and evade the host immune system. Its global emergence as a multidrug-resistant (MDR) pathogen has severely limited treatment options and increased mortality in hospital settings. Virulence factors associated with the cell envelope and outer membrane vesicles play central roles in bacterial survival and pathogenicity. In this context, this review aims to provide a concise overview of the virulence apparatus present on the surface of A. baumannii and evaluate its

Potential of Cannabidiol (CBD) to overcome extensively drug-resistant Acinetobacter baumannii.

Extensively drug-resistant (XDR) Acinetobacter baumannii poses a serious clinical challenge due to its resistance to nearly all available antibiotics, including carbapenems and colistin. Cannabidiol (CBD), a non-psychoactive phytochemical from Cannabis sativa L., has recently shown promising antimicrobial activity. This study evaluates the antibacterial and anti-biofilm effects of CBD against XDR A. baumannii isolates and explores its mechanism of action and potential as an adjunct therapeutic agent. Twenty-six A. baumannii isolates collected from ICU medical devices were identified using MALD

The critical human pathogen Acinetobacter baumannii exhibits light-regulated circadian rhythms.

Acinetobacter baumannii is recognized as the paradigm of multidrug-resistant superbug, topping the WHO priority list of critical human pathogens. Interestingly, it senses and responds to blue light, which modulates global aspects of its physiology, including pathogenicity. We hypothesized that light could serve as a signal to synchronize the bacterial physiology to the host's behavior and/or to the environment. At environmental temperatures, light regulation is mainly governed by the photoreceptor BlsA. In this work, we identified the existence of daily rhythms in blsA promoter activity displa

Synergistic Antibacterial Activity of Amorolfine Combined with Colistin Against Acinetobacter baumannii.

Emerging resistance to colistin in Acinetobacter baumannii is concerning because of the limited therapeutic options for this important clinical pathogen. Given the shortage of new antibiotics, one strategy that has been proven to be therapeutically effective is to overcome antibiotic-resistant pathogens by combining existing antibiotics with another antibiotic or non-antibiotic. This study was designed to investigate the potential synergistic antibacterial activity of amorolfine, a morpholine antifungal drug, in combination with colistin against A. baumannii. In this work, antibiotic susceptib

Acinetobacter baumannii Complex Infections: New Treatment Options in the Antibiotic Pipeline.

Acinetobacter baumannii complex (ABC) can result in a panoply of severe syndromes, including pneumonia and septic shock. Options available for treating infections caused by ABC and, more importantly, by carbapenem-resistant ABC (CRAB) are limited because of the increasing prevalence of antimicrobial resistance. Furthermore, many older agents, such as polymyxin and colistin, have limited lung penetration and are associated with significant toxicities. These factors underscore the urgent need for new paradigms to address ABC and CRAB. Two agents, cefiderocol and sulbactam-durlobactam, are now av

Comprehensive molecular epidemiology of Acinetobacter baumannii from diverse sources in Nigeria.

BACKGROUND: Acinetobacter baumannii, a Gram-negative bacterium, is a public health threat due to its role in nosocomial infections and increasing antibiotic resistance. In Nigeria, data on the molecular epidemiology of A. baumannii is scarce. This study investigates the genetic diversity and the presence of antimicrobial resistance determinants and virulence-related genes in whole-genome sequencing data of 189 Nigerian A. baumannii isolates deposited in public repositories. Genotypes were determined in-silico by multilocus sequence typing (MLST) and core genome MLST (cgMLST). Further, antimicr

Carbapenem-resistant Acinetobacter baumannii carrier detection: a simple and efficient protocol.

UNLABELLED: Timely detection of carbapenem-resistant Acinetobacter baumannii (CRAB) carriers is essential to direct infection control measures. In this work, we aimed to develop a practical protocol to detect CRAB from screening samples. To choose a selective medium that detects CRAB with high sensitivity and specificity, 111 A. baumannii clinical isolates were inoculated on three types of agar: mSuperCARBA (SC), CHROMagar Acinetobacter (CaA), and modified CHROMagar Acinetobacter (mCaA) containing 4.5 mg/mL meropenem. SC was non-selective, CaA was the most sensitive (100%), but only moderately

Copper induced augmentation of antibiotic resistance in Acinetobacter baumannii MCC 3114.

Increasing antibiotic resistance among the common nosocomial pathogen i.e. Acinetobacter baumannii poses life threat to the health care workers as well as to the society. The dissemination of antibiotic resistance in this pathogen at an alarming rate could be not only due to the overuse of antibiotics but also due to the stress caused by exposure of bacterium to several environmental contaminants in their niches. In the present study, effect of copper stress on augmentation in the antibiotic resistance of A. baumannii MCC 3114 against three clinically used antibiotics was investigated along wi

Preventative treatment with Fluorothiazinon suppressed Acinetobacter baumannii-associated septicemia in mice.

The high prevalence of multidrug-resistant Acinetobacter baumannii has emerged as a serious problem in the treatment of nosocomial infections in the past three decades. Recently, we developed a new small-molecule inhibitor belonging to a class of 2,4-disubstituted-4H-[1,3,4]-thiadiazine-5-ones, Fluorothiazinon (FT, previously called CL-55). FT effectively suppressed the T3SS of Chlamydia spp., Pseudomonas aeruginosa, and Salmonella sp. without affecting bacterial growth in vitro. In this study, we describe that prophylactic use of FT for 4 days prior to challenge with resistant clinical isolat

Potential genes associated with survival of Acinetobacter baumannii under ciprofloxacin stress.

Acinetobacter baumannii is an opportunistic pathogen that has acquired resistance to all available drugs. The rise in multi-drug resistance in A. baumannii has been exacerbated by its ability to tolerate antibiotics due to the persister cells, which are phenotypic variants of normal cells that can survive various stress conditions, resulting in chronicity of infection. In the present study we observed that A. baumannii formed persister cells against lethal concentration of ciprofloxacin in exponential phase. The transcriptome of A. baumannii was analyzed after exposure to high concentration of
