Acute Pain Predictors of Remote Postoperative Pain Resolution After Hand Surgery.

INTRODUCTION: Chronic postsurgical pain (CPSP) is a global issue with high prevalence. This study compared acute pain descriptors among patients undergoing carpal tunnel release (CTR) or trigger finger release (TFR). We hypothesized worst pain intensity on postoperative day (POD) 10 would be best to predict the time to pain resolution. METHODS: In this secondary analysis of a negative, randomized, double-blind placebo-controlled trial, adult veterans undergoing CTR or TFR were enrolled January 2012-January 2014, with data analysis February 2020-October 2020. Participants were randomized to re

Acute pain assessment tools: let us move beyond simple pain ratings.

PURPOSE OF REVIEW: This review highlights challenges and current trends in tools used to assess acute pain across the lifespan. RECENT FINDINGS: A plethora of similar assessment tools exist for acute pain, most focused on self-report of pain intensity. Attempts to improve the frequency and visibility of pain assessment by prompting pain as 'the fifth vital sign' resulted in unintended consequences, creating a pressing need for a conceptual shift to multidimensional assessment of acute pain. SUMMARY: Valid and pragmatic assessment of pain is essential for effective pain management. Unidimensi

Acute somatic pain can refer to sites of chronic abdominal pain.

We report a case of a latissimus dorsi muscle strain that presented as a recurrence of chronic abdominal pain. One explanation of the referral of acute pain to a site of chronic pain is the convergence-projection theory, which hypothesizes that pain signals of visceral and somatic origin converge at some point in the sensory pathway. Upon reaching the cortex, these signals are interpreted as coming from the afferents which have previously excited this pathway. In this case an extensive gastrointestinal diagnostic evaluation was pursued unsuccessfully before the latissimus dorsi muscle strain w

Acute Epiploic Appendagitis: An Overlooked Cause of Acute Abdominal Pain.

Acute epiploic appendagitis is a benign condition caused by inflammation of the epiploic appendages that are serosal lined outpouchings of the colon lying adjacent to the tenia coli. This rare condition has non-specific clinical findings and is frequently misdiagnosed as either acute diverticulitis or acute appendicitis. However, unlike other surgical causes of acute abdomen, epiploic appendagitis is a self-limited condition and resolves with conservative management. CT of the abdomen plays a vital role in diagnosing this condition and excluding other causes of acute abdomen. This case report

TRPM2 participates the transformation of acute pain to chronic pain during injury-induced neuropathic pain.

Transient receptor melastatin 2 (TRPM2) is a nonselective Ca2+ -permeable cation channel highly expressed in brain and other tissues. Studies showed that TRPM2 contributed to the induction of inflammatory cytokine and chemokine of immune cells, resulted in neuropathic pain. However, how TRPM2 regulates neuropathic pain is not clear. The sciatic nerve chronic constriction injury (CCI) rat model was used to induce chronic neuropathic pain. The RNA and protein level of TRPM2 was detected with real-time PCR and western blot. SiRNA targeting TRPM2 was used to knockdown the expression of TRPM2. Reac

Paediatric Pain Medicine: Pain Differences, Recognition and Coping Acute Procedural Pain in Paediatric Emergency Room.

Paediatric pain and its assessment and management are challenging for medical professionals, especially in an urgent care environment. Patients in a paediatric emergency room (PER) often undergo painful procedures which are an additional source of distress, anxiety, and pain. Paediatric procedural pain is often underestimated and neglected because of various myths, beliefs, and difficulties in its evaluation and treatment. However, it is very different from other origins of pain as it can be preventable. It is known that neonates and children can feel pain and that it has long-term effects tha

[Patient education for acute low back pain : Contents of pain education in patients with acute non-specific low back pain - a scoping review].

OBJECTIVE: The objective of the review is to map the content of intervention education in people with acute nonspecific lumbar low back pain (LBP) to make it available in a synopsis. BACKGROUND: In the care of people with acute non-specific LBP, education is recommended as an elementary treatment component. However, the proposed content is proving to be heterogenic and the question arises as to how education should be designed in the future. METHOD: A systematic literature search and content analysis of studies with content on pain education was conducted as part of a scoping review. Inclusi

From acute to chronic pain after thoracic surgery: the significance of different components of the acute pain response.

PURPOSE: Acute postoperative pain is a strong predictor for postthoracotomy pain syndrome (PTPS), but the mechanism is unknown. Even though thoracic pain is usually considered the dominating acute pain after thoracic surgery, up to 45% of patients consider shoulder pain to be dominating pain and often this shoulder pain is referred visceral pain. This study aims to examine which components of the acute pain response after thoracic surgery were associated with PTPS and if any signs of a generalized central hypersensitivity could be identified in patients with PTPS. PATIENTS AND METHODS: In a p

Optimizing acute pain management in trauma care: the role, structure and core principles of acute trauma pain services.

Trauma injuries, both accidental and violence-related, account for a significant number of fatalities and non-fatal injuries annually, with pain being a predominant symptom affecting trauma patients. Despite its prevalence, pain is often undertreated in emergency settings, leading to adverse outcomes such as delayed recovery, impaired respiratory function, and increased risk of chronic pain and post-traumatic stress disorder. An Acute Trauma Pain Service (ATPS) is designed to address these issues through a multidisciplinary approach, involving anesthesiologists, pain nurses, and trauma special

Common Biological Modulators of Acute Pain: An Overview Within the AAAPT Project (ACTTION-APS-AAPM Acute Pain Taxonomy).

OBJECTIVE: The ACTTION-APS-AAPM Acute Pain Taxonomy (AAAPT) project relies on the identification of modulators to improve characterization and classification of acute pain conditions. In the frame of the AAAPT effort, this paper presents an overview of common biological modulators of acute pain. METHODS: Nonsystematic overview. RESULTS: Females may experience more acute pain than males, but the clinical significance may be modest. Increasing age is associated with decreasing analgesic requirement and decreasing pain intensity after surgery and with higher risk of acute low back pain. Racial

Acute pain service for postoperative pain in adults: a network meta-analysis.

BACKGROUND: Postoperative pain significantly impacts patients' quality of life and recovery. Although acute pain services (APS) have been implemented in many hospitals worldwide, no study has directly compared the efficacy of different APS subtypes in managing acute postoperative pain. OBJECTIVE: This network meta-analysis aimed to evaluate the effectiveness of various APS models in alleviating postoperative pain in adults undergoing surgery. METHODS: Four English-language databases (PubMed, Web of Science, Embase, and Cochrane Library) and three Chinese-language databases (CNKI, WANFANG, an

Accelerating discovery in pain science: the Acute to Chronic Pain Signatures program.

Chronic pain is a complex and multifaceted disease, with biological causes distributed across tissues and bodily systems and influenced by psychological and social factors. A major aim of pain research is to find better ways to measure signals associated with chronic pain and thus predict, track, and treat pain conditions. Emerging biological measures related to the metabolic, immune, and nervous systems hold promise for identifying the physiology underlying pain and developing new treatments. Developing such measures will require multimodal datasets from large samples in combination with meth

Acute Pain Services and pain-related patient-reported outcomes in Hungarian hospitals.

BACKGROUND: Postoperative pain management is an important part of surgical care, where Acute Pain Service offers added value in terms of patient outcomes and costs. The technology, however, has hardly been adopted in Hungary, with only two hospitals operating Acute Pain Service and whose performance has not been evaluated yet. This research compared pain management outcomes of surgical, orthopedic, and traumatology patients in Hungarian hospitals with and without Acute Pain Service. METHODS: We recruited 348 patients, 120 in the APS group and 228 in the control group, whose experience was sur

A Practical Approach to Acute Postoperative Pain Management in Chronic Pain Patients.

In the United States, more than 100 million people suffer from chronic pain. Among patients presenting for surgery, about one in four have chronic pain. Acute perioperative pain management in this population is challenging because many patients with chronic pain require long-term opioids for the management of this pain, which may result in tolerance, physical dependence, addiction, and opioid-induced hyperalgesia. These challenges are compounded by the ongoing opioid epidemic that has resulted in calls for a reduction in opioid use, with a concurrent increase in the number of patients with chr
