[Debridement, antimicrobial therapy, and implant retention for acute periprosthetic infections].

BACKGROUND: The DAIR procedure (debridement, antibiotic therapy, and implant retention) represents a treatment option for acute periprosthetic joint infections (PJI). Compared to revision arthroplasty, it is technically less invasive and, under appropriate indications, can preserve a well-fixed endoprosthesis. However, treatment success depends on numerous patient-, pathogen-, and procedure-related factors. METHODS: Based on a comprehensive review of current international studies and guidelines, we systematically analyzed the key factors influencing the success of DAIR. In addition, clinical

Advance Directives in Portugal: A Qualitative Survey.

(1) Background: Advance directives (ADs) in Portugal have been legalized since 2012. What has been observed over time, from the few studies carried out, is that despite the positive attitudes in the population, there is a low level of adherence to ADs. To try to understand the reasons for these data, the current study aimed to explore and describe the experiences of the Portuguese population regarding AD. (2) Methods: For this exploratory and descriptive qualitative study, the researchers conducted open (unstructured) interviews with a convenience sample aged over 18 years until data saturatio

Exogenous ketone esters as a potential therapeutic for treatment of sarcopenic obesity.

Identifying effective treatment(s) for sarcopenia and sarcopenic obesity is of paramount importance as the global population advances in age and obesity continues to be a worldwide concern. Evidence has shown that a ketogenic diet can be beneficial for the preservation of muscle quality and function in older adults, but long-term adherence is low due in part to the high-fat (≥80%), very low carbohydrate (<5%) composition of the diet. When provided in adequate amounts, exogenous ketone esters (KEs) can increase circulating ketones to concentrations that exceed those observed during prolonged fa

Staged Hybrid Techniques With Straightforward Bypass Surgery Followed by Flow Diverter Deployment for Complex Recurrent Middle Cerebral Artery Aneurysms.

BACKGROUND: Recurrent complex middle cerebral artery (MCA) aneurysms after combined clipping and endovascular surgery are challenging, and if conventional techniques are adapted, advanced surgical, endovascular, and a combination of both techniques are often required. For such complex aneurysms, safe and effective straightforward techniques for all neurovascular surgeons are warranted. We describe the details of staged hybrid techniques with straightforward bypass surgery followed by flow diverter deployment in a patient with complex MCA aneurysm. ILLUSTRATIVE CASE: A 69-year-old woman presen

Attitudes among dermatologists regarding actinic keratosis treatment options.

Actinic keratosis (AK) is considered a precancerous lesion that can develop into invasive squamous cell carcinoma. Its prevalence is increasing, and it is estimated that it affects between 1% and 44% of the adult population worldwide. Advanced age, fair skin phototypes, and cumulative sun exposure are the main risk factors for AK. Therapies for AK consists of lesion-directed treatment (i.e., cryotherapy, curettage, electrocoagulation, and laser therapy) or field therapy [i.e., photodynamic therapy (PDT), 5-fluorouracil (5-FU), diclofenac sodium (DIC), imiquimod (IMQ), and ingenol mebutate (Ing

Advances in host-directed therapies for tuberculosis.

INTRODUCTION: Tuberculosis (TB) treatment is lengthy and complex, requiring patients to adhere to multidrug regimens, which may be difficult to tolerate. Importantly, traditional antimicrobial therapy does not prevent infection-associated lung damage. Adjunctive host-directed therapies (HDTs) could address the issues associated with standard antitubercular therapy. We highlight recent advances in pulmonary TB HDT clinical trials and discuss the molecular pathways targeted, focusing on approaches that future research can build on to develop new TB regimens that may include HDTs. Fabry Disease: An Updated Perspective and Review of Treatment and Therapies.

Fabry disease (FD) is a progressive, multisystemic, X-linked lysosomal storage disorder that when untreated leads to decreased quality of life, chronic pain, and end organ disease; however, advances in therapy have significantly altered its original natural history. This paper briefly reviews the pathophysiology and clinical features of FD before focusing on current and future treatments. Enzyme replacement therapy (ERT) and pharmacologic chaperone therapy are established treatments with long-term data showing disease stabilization and improved outcomes when the therapies are initiated prior t

Dual-Targeted Antiemetic Strategy: A Review of Granisetron-Scopolamine Transdermal Patch for PONV and CINV Management.

Post-Operative Nausea and Vomiting (PONV) and Chemotherapy-Induced Nausea and Vomiting (CINV) are common clinical concerns that adversely affect patient recovery and quality of life. Existing pharmacological interventions, including single-agent and combination therapies, often exhibit limitations in fully controlling symptoms and may lead to undesirable side effects. This review examines the pharmacological rationale, clinical implications, and potential benefits of combining granisetron, a selective serotonin (5-HT3) receptor antagonist, with scopolamine, a muscarinic (M1) receptor antagonis

Therapeutic horizons in metabolic dysfunction-associated steatohepatitis.

Metabolic dysfunction-associated steatohepatitis (MASH), the progressive inflammatory form of MASLD, is now a leading cause of chronic liver disease worldwide. Driven by obesity and type 2 diabetes, MASH significantly increases the risk of cirrhosis, hepatocellular carcinoma, and liver failure. While public health interventions remain essential, therapeutic strategies targeting metabolic dysfunction, inflammation, and fibrosis are urgently needed. This Review focuses on pharmacological treatments in advanced development, including incretin-based therapies (GLP-1, dual, and triple agonists), me

Natural Products and Exercise Synergy: Unraveling Molecular Mechanisms to Enhance Brain Function in Alzheimer's Disease.

Alzheimer's disease (AD) as well as other memory disorders significantly impair cognitive and behavioral functions, posing a growing global health burden. Research increasingly supports the synergistic effects of natural products-bioactive compounds derived from plant-based diets-and regular physical exercise in promoting brain health. Natural products exhibit neuroprotective effects by modulating oxidative stress, reducing neuroinflammation, and enhancing synaptic plasticity, while exercise complements these benefits by improving neurogenesis, cerebral blood flow, and mitochondrial function.

In vitro modeling of endometriosis and endometriotic microenvironment - Challenges and recent advances.

Endometriosis is a chronic condition with high prevalence in reproductive age women, defined as the growth of endometrial tissue outside the uterine cavity, most commonly on the pelvic peritoneum. The ectopic endometrial lesions exist in a unique microenvironment created by the interaction of epithelial, stromal, endothelial, glandular, and immune cell components, dominated by inflammatory, angiogenic, and endocrine signals. Current research is directed at understanding the complex microenvironment of the lesions and its relationship with different endometriosis stages, phenotypes, and disease

Leveraging smart technologies to improve the management of diabetic foot ulcers and extend ulcer-free days in remission.

The prevalent and long neglected diabetic foot ulcer (DFU) and its related complications rank among the most debilitating and costly sequelae of diabetes. Management of the DFU is multifaceted and requires constant monitoring from patients, caregivers, and healthcare providers. The alarmingly high rates of recurrence of ulcerations in the diabetic foot requires a change in our approach to care and to the vernacular in the medical literature. Our efforts should be directed not only on healing of open wounds, but also on maximizing ulcer-free days for the patient in diabetic foot remission. The

Prevention and management of treatment failure to new oral hepatitis C drugs.

INTRODUCTION: Chronic hepatitis C virus (HCV) infection has become a curable disease. Sustained virologic response rates above 90% have been achieved with recommended direct-acting antiviral (DAA) combinations in most registration trials. However, outcomes in real-world patients are lower. In patients experiencing DAA failure, resistance-associated variants (RAVs) are almost universally selected. At this time it is unclear when and how to re-treat hepatitis C in patients with prior DAA failure. AREAS COVERED: The rate of DAA failure and predictors of lack of treatment response using distinct

Psychiatric treatment considerations with direct acting antivirals in hepatitis C.

BACKGROUND: Despite recent advances in hepatitis C (HCV) treatment, specifically the addition of direct acting antivirals (DAAs), pegylated interferon-alpha remains the backbone of HCV therapy. Therefore, the impact of DAAs on the management of co-morbid psychiatric illness and neuropsychiatric sequalae remains an ongoing concern during HCV therapy. This paper provides a review of the neuropsychiatric adverse effects of DAAs and drug-drug interactions (DDIs) between DAAs and psychiatric medications. METHODS: We conducted a Pubmed search using relevant search terms and hand searched reference

Emerging Targeted Therapies in Asthma: From Upstream Epithelial Biology to Novel Biologic and Small-Molecule Strategies.

Biologic and targeted small-molecule therapies have revolutionized the management of severe asthma by specifically targeting key inflammatory pathways. These advances have led to significant reductions in exacerbations, improved lung function, and decreased dependence on corticosteroids. However, a substantial number of patients still experience breakthrough exacerbations and inadequate disease control, indicating that there are residual inflammatory mechanisms beyond the classical type-2 pathways. Current biologics that target immunoglobulin E, interleukin-5, interleukin-4 receptor alpha, and
