Evaluating pediatric advanced life support in emergency medical services with a performance and safety scoring tool.

INTRODUCTION: Pediatric out-of-hospital cardiac arrests (P-OHCA) are infrequent, have low survival rates, and often have poor neurologic outcomes. Recent evidence indicates that high-performance emergency medical service (EMS) care can improve outcomes. OBJECTIVES: To evaluate Pediatric Advanced Life Support (PALS) guideline performance in the out of hospital setting and introduce an easy-to-use tool that scores guideline compliance and patient safety. METHODS: We observed EMS teams responding to standardized pediatric resuscitation simulations. Teams were dispatched to a mock assisted livin

Challenging the 4- to 5-minute rule: from perimortem cesarean to resuscitative hysterotomy.

Although perimortem delivery has been recorded in the medical literature for millennia, the procedural intent has evolved to the current fetocentric approach, predicating timing of delivery following maternal cardiopulmonary arrest to optimize neonatal outcome. We suggest a call to action to reinforce the concept that if the uterus is palpable at or above the umbilicus, preparations for delivery should be made simultaneous with initiation of maternal resuscitative efforts; if maternal condition is not rapidly reversible, hysterotomy with delivery should be performed regardless of fetal viabili

[Circulatory assist devices in cardiology].

One out of 13 patients with an acute myocardial infarction is endangered of cardiogenic shock. In addition, acute valvular leakage, shunt vitiae, and acute myocarditis can lead to acute myocardial failure. As a therapeutic option, mechanical assist devices offer cardiac support and hemodynamic stabilization under these circumstances. The following minimal-invasive devices are used in cardiology and intensive care medicine: intra-aortic balloon pulsation (IABP), intra-vascular axial screw pumps, extra-corporal centrifugal pumps with and without additional membrane oxygenator. The IABP improves

Gas in coronary artery: A case of fatal decompression sickness evaluated by computed tomography.

A 54-year-old man suffered a leg cramp while diving in the ocean at a depth of 20 meters. He began to surface, with his ascent based on a decompression table. He lost consciousness at the surface and was rescued by a nearby boat. The boat staff judged him to be in cardiac arrest, so they performed chest compressions. When the boat reached port where an ambulance was waiting, emergency medical technicians confirmed that the patient was in cardiac arrest; his initial rhythm was asystole. Treated with basic life support, the patient was then transported to a rendezvous point, where a physician-st

Advanced reperfusion strategies for patients with out-of-hospital cardiac arrest and refractory ventricular fibrillation (ARREST): a phase 2, single centre, open-label, randomised controlled trial.

BACKGROUND: Among patients with out-of-hospital cardiac arrest (OHCA) and ventricular fibrillation, more than half present with refractory ventricular fibrillation unresponsive to initial standard advanced cardiac life support (ACLS) treatment. We did the first randomised clinical trial in the USA of extracorporeal membrane oxygenation (ECMO)-facilitated resuscitation versus standard ACLS treatment in patients with OHCA and refractory ventricular fibrillation. METHODS: For this phase 2, single centre, open-label, adaptive, safety and efficacy randomised clinical trial, we included adults aged

Design and Rationale of the FICAPP Study: Frailty in Cardiac Amyloidosis: Profiling the Phenotype.

BACKGROUND & AIM: Cardiac amyloidosis is an infiltrative cardiomyopathy associated with significant morbidity and mortality. Frailty, a biologic syndrome of decreased physiologic reserve, may be common and clinically significant in this population, yet remains insufficiently characterised. We aim to determine the prevalence and characteristics of frailty among adults with cardiac amyloidosis, and examine its association with functional status, quality of life, and disease severity. METHODS & DESIGN: As a prospective, observational, single-centre cohort study, adults with confirmed cardiac amy

Severe non-hypoxic bradycardia during disconnection from the ventilator during the recovery phase of ARDS.

Four patients with adult respiratory distress syndrome developed sinus bradycardia during weaning procedures with no evidence of hypoxemia. Bradycardia occurred immediately after the patients' endotracheal tubes were disconnected from the ventilator and most of the time resolved after reconnection. However, 3 patients eventually deteriorated, requiring advanced life support and for one of them, cardiac pacing. The precise mechanism of these bradycardic episodes remains unclear but was unrelated to hypoxemia or acid-base disorder.

Accidental "cranial" defibrillation.

A case of accidental shock to the head, caused by a cardiac defibrillator, is presented. The shock resulted in minor burns and symptoms similar to those of postelectro-convulsive shock therapy (ECT). A physician participating in an advanced life support course, and familiar with the equipment, was the victim. An assumption that the demonstration equipment was not real seems to have been the cause of the accident. Suggestions for prevention of further episodes are discussed.

Massive Pulmonary Embolism Mimicking Acute Myocardial Infarction: Successful use of extracorporeal membrane oxygenation support as bridge to diagnosis.

Prolonged cardiac arrest with pulseless electrical activity (PEA) results in death if its aetiology cannot be corrected immediately. We describe the case of a 75-year-old man with chest pain and his electrocardiogram (ECG) revealing ST-segment elevation in leads II, III, and aVf. Inferior wall myocardial infarction was subsequently diagnosed. Before performing emergency coronary angiography, however, a sudden cardiac arrest with PEA developed and the patient was placed on advanced cardiac life support. Oxygenation support for the extracorporeal membrane was initiated approximately 65min after

[Mechanical cardiac assist devices and heart transplantation. Treatment indications and the current situation in Spain].

Despite advances in all areas of cardiology, the incidence of heart failure increases each year. Pharmacologic treatment, cardiac resynchronization therapy, and heart failure surgery have resulted in prolonged survival in these patients and in symptomatic improvement. However, in more advanced phases of the disease, these therapeutic options become less effective. This is when heart transplantation and cardiac assist devices are the only techniques that can lengthen survival and improve quality of life. For the past 25 years, heart transplantation has been the most effective therapeutic option

Extracorporeal membrane oxygenation (ECMO) - in the treatment of severe, life-threatening respiratory failure.

Extracorporeal membrane oxygenation (ECMO) is a technique involving oxygenation of blood and elimination of carbon dioxide in patients with life-threatening, but potentially reversible conditions. Thanks to the modification of extracorporeal circulation used during cardiac surgeries, this technique can be used in intensive care units. Venovenous ECMO is used as a respiratory support, while venoarterial ECMO as a cardiac and/or respiratory support. ECMO does not cure the heart and/or lungs, but it gives the patient a chance to survive a period when these organs are inefficient. In addition, ext

Recent advances in cardiopulmonary-cerebral resuscitation: a review.

Standard external CPR (SECPR) steps A, B, and C can maintain the brain's viability if started immediately, but not after prolonged arrest times. "New CPR" (simultaneous ventilation-compression CPR, SVC-CPR) is not suitable for basic life support, and may not be physiologically superior to optimally performed SECPR. The superiority of interposed abdominal compression CPR (IAC-CPR) over SECPR for basic life support is also uncertain. Open-chest CPR is physiologically superior to all external CPR methods studied thus far. Open-chest CPR should again be taught to physicians, and used more often af

Rationale and specifications for an automatic cardiac arrest-driven alarm and 911 caller ID (ACADA/911).

The rationale and specifications for a permanent non-invasive cardiac monitoring device are described, including both an alarm and a device-initiated call (giving patient residence identification) to the emergency services number 911, if cardiac arrest is detected. This device may be useful for high-risk cardiac patients, who incur an unwitnessed cardiac arrest, by providing a prompt call and alarm for early initiation of bystander CPR, cardiac defibrillation (if necessary), advanced cardiac life-support, and early transfer of resuscitated patients to a hospital cardiac care unit. Unwitnessed

EACTS Expert Consensus Document on protected cardiac surgery: pre-emptive use of temporary mechanical circulatory support in adult cardiac surgery patients at high risk for perioperative low cardiac output syndrome.

Perioperative low cardiac output syndrome (LCOS) remains a significant complication in adult cardiac surgery, contributing to substantial morbidity, prolonged intensive care, and increased mortality. Its incidence is expected to rise further due to the evolving complexity of referred surgical patients, often characterised by advanced age, multiple comorbidities, challenging anatomy, and impaired haemodynamics. Despite advances in pharmacological and perioperative care, outcomes for high-risk patients have not shown significant improvement, prompting interest in temporary mechanical circulatory

Indications and outcomes of extracorporeal life support in trauma patients.

BACKGROUND: The use of extracorporeal life support (ECLS) in the trauma population remains controversial and has been reported only in small cohort studies. Recent ECLS technical advances have increased its use as an advanced critical care option in trauma. Given the degree of resource utilization, costs, and expertise required to provide ECLS support in trauma patients, we sought to perform a multi-institutional study to assess the indications and outcomes of ECLS in trauma. METHODS: A retrospective review of adult (≥16 years) trauma patients receiving ECLS support in the Extracorporeal Life

Assessment of quality of life and cognitive function after out-of-hospital cardiac arrest with successful resuscitation.

This prospective cohort study evaluated the impact of the time-related elements of the "chain of survival" on the quality of life of patients, taking their characteristics into account. Between 1995 and 2002, consecutive, out-of-hospital cardiac arrest patients from Amsterdam and the surrounding areas were included in this study. A total of 227 patients (12%) survived to hospital discharge and 174 were definitive survivors who were available for assessment at 6 months. Quality of life was measured with the 136-item Sickness Impact Profile (SIP); cognitive functioning was assessed through the M

The Safety of Bypass to Percutaneous Coronary Intervention Facility by Basic Life Support Providers in Patients with ST-Elevation Myocardial Infarction in Prehospital Setting.

BACKGROUND: Most patients transferred from a non-percutaneous coronary intervention (PCI) facility for primary PCI do not meet target reperfusion times. Direct transportation of patients with ST-elevation myocardial infarction (STEMI) from the scene by advanced life support (ALS) paramedics has been shown to improve reperfusion times and outcomes. OBJECTIVE: The aim of this study was to determine whether it is safe to bypass the closest hospital and transport by basic life support (BLS) provider to a PCI facility. METHODS: This was a health records review of consecutive patients transported

Presumed futility in paramedic-treated out-of-hospital cardiac arrest: an Utstein style analysis in Tampere, Finland.

AIM: To report prospectively the outcome from prehospital cardiac arrest according to the Utstein template in the city of Tampere, Finland, with special reference to those patients in whom resuscitation was not attempted. MATERIALS AND METHODS: In Tampere (population 203,000), a two-tiered emergency medical service (EMS) system provides first response and basic life support (BLS), supported by advanced life support (ALS) units staffed with nurse-paramedics. We analysed all out-of-hospital cardiac arrests considered for resuscitation during a 12-month period. RESULTS: Of 191 patients with pre

Extracorporeal Membrane Oxygenation-Impella (ECPELLA) in a Patient With Recalcitrant Ventricular Tachycardia Undergoing Ablation.

In advanced heart failure, refractory hemodynamically unstable ventricular tachycardia (VT) can be life-threatening. The utilization of short-term temporary mechanical circulatory support (MCS) has been described. Still, it is limited to the intra-aortic balloon pump (IABP) or Impella 2.5/CP (Abiomed Inc., Danvers, MA, USA) which may only provide up to 1-2.5 L/min of added support. Escalation of MCS therapies should be considered. Referrals to advanced tertiary heart transplant centers should be done early to afford patients the best chance at an optimal outcome, with the option for heart tran
