Establishment and cryptic transmission of Zika virus in Brazil and the Americas.

Transmission of Zika virus (ZIKV) in the Americas was first confirmed in May 2015 in northeast Brazil. Brazil has had the highest number of reported ZIKV cases worldwide (more than 200,000 by 24 December 2016) and the most cases associated with microcephaly and other birth defects (2,366 confirmed by 31 December 2016). Since the initial detection of ZIKV in Brazil, more than 45 countries in the Americas have reported local ZIKV transmission, with 24 of these reporting severe ZIKV-associated disease. However, the origin and epidemic history of ZIKV in Brazil and the Americas remain poorly under

Tuberculosis, HIV, and poverty: temporal trends in Brazil, the Americas, and worldwide.

OBJECTIVE: To analyze the temporal trends of the incidence and prevalence of tuberculosis, with and without HIV co-infection, as well as of the associated mortality, in Brazil, the Americas, and worldwide. METHODS: We collected data related to tuberculosis, with and without HIV co-infection, between 1990 and 2010, in Brazil, the Americas, and worldwide. Temporal trends were estimated by linear regression. RESULTS: We identified a trend toward a decrease in tuberculosis prevalence and mortality, and that trend was more pronounced in Brazil and the Americas than worldwide. There was also a tre

Yellow fever virus maintenance in Trinidad and its dispersal throughout the Americas.

Trinidad, like many other American regions, experiences repeated epizootics of yellow fever virus (YFV). However, it is unclear whether these result from in situ evolution (enzootic maintenance) or regular reintroduction of YFV from the South American mainland. To discriminate between these hypotheses, we carried out a Bayesian phylogeographic analysis of over 100 prM/E gene sequences sampled from 8 South American countries. These included newly sequenced isolates from the recent 2008-2009 Trinidad epizootic and isolates derived from mainland countries within the last decade. The results indic

Molecular evidence for the importation of Old World Leishmania into the Americas.

A brief review of work carried out by the laboratory on the systematics of trypanosomatids during the last three years is given. The principal line of research has been on the taxonomy of New World Leishmania and one of the topics studied has been the determination of the autochthonous origin of certain Leishmania species found in the New World. Controversy exists as to whether the etiological agent of American Visceral Leishmaniasis is indigenous. Here, we present evidence from enzyme electrophoresis and schizodeme analysis indicating that L. chagasi has a recent origin and that it is similar

Recommendations for the management of candidemia in neonates in Latin America. Latin America Invasive Mycosis Network.

Candidemia is one of the most frequent opportunistic mycoses worldwide. Limited epidemiological studies in Latin America indicate that incidence rates are higher in this region than in the Northern Hemisphere. Diagnosis is often made late in the infection, affecting the initiation of antifungal therapy. A more scientific approach, based on specific parameters, for diagnosis and management of candidemia in Latin America is warranted. 'Recommendations for the diagnosis and management of candidemia' are a series of manuscripts that have been developed by members of the Latin America Invasive Myco

Zika virus in the Americas: transmission dynamics, clinical lessons, and future risk.

INTRODUCTION: The emergence of Zika virus (ZIKV) in the Americas between 2015 and 2016 transformed a previously neglected arbovirus into a major global public health concern, revealing the consequences of congenital infection and neurological disease while exposing critical weaknesses in surveillance and response capacity. Although epidemic transmission has substantially declined, ZIKV continues to circulate at low levels across several regions of the Americas, and the risk of future resurgence remains uncertain. AREAS COVERED: In this review, we synthesize current evidence on the epidemiolog

Inequalities in injury burden in the Americas, 2000-2021: A systematic analysis.

OBJECTIVE: To assess mortality and disability due to injuries in the Americas, with a focus on subregional and gender disparities, and changes over time. METHODS: Data from the 2021 World Health Organization Global Health Estimates were analyzed, covering all injury causes (ICD-10: V01-Y89) across 35 countries in the Americas from 2000 to 2021. Age-standardized mortality and disability-adjusted life year (DALY) rates were calculated by sex, cause, and subregion. Inequalities were quantified using absolute differences, relative ratios, and the index of disparity (IDisp). RESULTS: In 2021, inj

Lead Poisoning in the Americas: Sources, Regulations, Health Impacts, and Molecular Mechanisms.

Lead poisoning is a significant public health issue, contributing to 0.6% of the global disease burden and disproportionately affecting developing countries. Vulnerable populations, such as children, pregnant women, and low-income communities, remain at high risk, often exposed to lead levels exceeding safe thresholds. While the problem is global, this review focuses specifically on the Americas, regions with diverse regulatory landscapes and persistent environmental lead exposure. Regulatory frameworks vary widely, and the lack of global consensus on acceptable blood lead levels leaves import

Ten Americas: a systematic analysis of life expectancy disparities in the USA.

BACKGROUND: Nearly two decades ago, the Eight Americas study offered a novel lens for examining health inequities in the USA by partitioning the US population into eight groups based on geography, race, urbanicity, income per capita, and homicide rate. That study found gaps of 12·8 years for females and 15·4 years for males in life expectancy in 2001 across these eight groups. In this study, we aimed to update and expand the original Eight Americas study, examining trends in life expectancy from 2000 to 2021 for ten Americas (analogues to the original eight, plus two additional groups comprisi

Evolution and spread of Venezuelan equine encephalitis complex alphavirus in the Americas.

Venezuelan equine encephalitis (VEE) complex alphaviruses are important re-emerging arboviruses that cause life-threatening disease in equids during epizootics as well as spillover human infections. We conducted a comprehensive analysis of VEE complex alphaviruses by sequencing the genomes of 94 strains and performing phylogenetic analyses of 130 isolates using complete open reading frames for the nonstructural and structural polyproteins. Our analyses confirmed purifying selection as a major mechanism influencing the evolution of these viruses as well as a confounding factor in molecular cloc

Risks to the Americas associated with the continued expansion of chikungunya virus.

Chikungunya virus is a mosquito-borne virus that has been responsible for over 2 million human infections during the past decade. This virus, which previously had a geographical range primarily restricted to sub-Saharan Africa, the Indian subcontinent and South East Asia, has recently moved to subtropical latitudes as well as the western hemisphere. This expansion into novel habitats brings unique risks associated with further spread of the virus and the disease it causes.

Future challenges for parasitology: vector control and one health in the Americas.

"One Health" is a term that encapsulates and underscores the inherent interrelatedness of the health of people, animals, and the environment. Vector-borne infections are central in one health. Many arthropod vectors readily feed on humans and other animals, serving as an ideal conduit to move pathogens between a wide spectrum of potential hosts. As ecological niches flux, opportunities arise for vectors to interact with novel species, allowing infectious agents to broaden both geographic and host ranges. Habitat change has been linked to the emergence of novel human and veterinary disease agen
