Ethical Dilemmas in End-of-Life Care: A Psychological and Anesthetic Perspective.

End-of-life care presents some of the most complex and emotionally challenging ethical dilemmas in modern medicine. For anesthetists and clinical psychologists, these dilemmas intersect at the critical juncture where physiological support meets psychological suffering. This editorial advances the novel argument that the psychological distress inherent to the dying process constitutes a specific clinical entity (existential suffering), which requires interdisciplinary management distinct from standard symptom control. It explores the major ethical challenges encountered in end-of-life care, inc

Anesthetic implications of postpolio syndrome: new concerns for an old disease.

Poliomyelitis was pandemic in the United States and much of the world in the first half of the 20th century. The uses of polio vaccines have essentially eradicated the disease in the United States today. But poliovirus infection survivors who had experienced a paralytic attack can see a return of some symptoms, which is a syndrome called postpolio syndrome (PPS). The anesthetist must preoperatively assess reported amounts of patient physical activity and patient age, which can indicate the amount of muscle degeneration that may have already occurred. Patients with PPS demonstrate altered respi

Interventional radiology in the airway in children.

Interventional procedures in the airway can be performed in interventional radiology suites or the operating room, by radiologists or other specialists. The most common therapeutic interventions carried out by radiologists are balloon dilatation, stenting, and the treatment of certain airway fistulas. These operations can be very challenging for anesthetists in terms of planning, airway management, the identification and treatment of procedural complications and postoperative care. In particular, a multidisciplinary approach to decision-making and planning is important to obtain the best resul

A Novel Device for Training and Evaluating Ultrasound-Guided Procedures in Anesthesia.

To improve dynamic needle-tip visualization, nurse anesthesiology students were evaluated during simulated ultrasound-guided procedures. This quasiexperimental study utilized two computed tomography 3D printed models. Thirty-two students performed each procedure twice, once without and once with needle guidance. Measures focused on accuracy and procedural performance to determine the impact that guidance versus no guidance had on attempts. Students evaluated their experiences, self-confidence, feasibility, and usability using needle guidance technology. Needle guidance improved the distance to

Improving Anesthesia Providers' Needle Cricothyrotomy Success With Ultrasound-Guidance: A Cadave Quality Improvement Project.

Difficult and failed airway management remains a significant cause of anesthesia-related morbidity and mortality. Failed airway management guidelines include performing a cricothyrotomy as a final step. Correct identification of the cricothyroid membrane (CTM) is essential for safe and accurate cricothyrotomy execution. Ten certified registered nurse anesthetists were assessed for ultrasound-guided (USG) needle cricothyrotomy competency following an online and hands-on education session using a human cadaver and then assessed 60 days later, without additional education or preparation. Both kno

Mitochondrial diseases and anesthesia: a literature review of current opinions.

This course explores the considerations that the anesthesia provider has to be aware of, when caring for a patient with a mitochondrial myopathy. Even though these disorders are rare, these patients may also need surgical care, requiring that the anesthesia provider be informed of the best anesthesia options to consider. A narrative review of documented cases and their outcomes is used to generate a resource of current opinions in the anesthetic care of this patient population.

Nurse anesthesia students' perceptions of the anesthesia patient simulator: a qualitative study.

The purpose of this study was to describe the perceptions of nurse anesthesia students (NAS) who used a MedSim simulator (MedSim USA, Inc, Ft Lauderdale, Fla) as part of their educational training. A convenience sample of 12 NAS in their first year of clinical training was researched. The researcher analyzed data qualitatively from observations made during 4 different sessions. Session 1 introduced the students to the simulator. Session 2 involved each student performing an anesthetic induction. A minor incident such as hypotension, hypertension, bradycardia or tachycardia occurred in session

The Mayo block: an efficacious block for hallux and first metatarsal surgery.

The Mayo block is an extremely efficacious regional anesthetic technique used to provide anesthesia of the forefoot. Patients requiring surgical correction of hallux deformity, bunionectomy, and first metatarsal surgery may benefit from this technique. The Mayo block is a field block that anesthetizes the specific nerves of the forefoot that innervate the surgical field. This technique requires less local anesthesia than that required by direct local anesthetic infiltration and does not distort the surgical tissue planes. The Mayo block is effective. This technique has been used at one militar

Perioperative Management of Elderly patients (PriME): recommendations from an Italian intersociety consensus.

BACKGROUND: Surgical outcomes in geriatric patients may be complicated by factors such as multiple comorbidities, low functional performance, frailty, reduced homeostatic capacity, and cognitive impairment. An integrated multidisciplinary approach to management is, therefore, essential in this population, but at present, the use of such an approach is uncommon. The Perioperative Management of Elderly patients (PriME) project has been established to address this issue. AIMS: To develop evidence-based recommendations for the integrated care of geriatric surgical patients. METHODS: A 14-member

Anticoagulation Management in CEPOD and Semi-elective Tracheostomy: A Multidisciplinary Survey.

BACKGROUND: Tracheostomy is a frequently performed airway procedure in both emergency (CEPOD) and semielective settings. Within our institution, perioperative anticoagulation management varies between clinicians, highlighting the absence of a standardized local protocol and the limited availability of procedure-specific guidance in the literature. METHODS: A cross-sectional survey was conducted among ENT surgeons and anesthetists at Mater Dei Hospital to explore current approaches to anticoagulation management in CEPOD and semielective tracheostomy. Data were collected on clinician experience

Radiation dose reduction during venous access port implantation: the importance of upgrading equipment and radiation-protection training.

BACKGROUND: Implantable central venous port systems are widely used in oncology. We upgraded our fluoroscopy machines, and all anesthetists completed two training courses focusing on the risks of ionizing radiation for patients and health workers. AIMS: This study aimed to evaluate the impact of upgrading the machines and the radiation-protection training on ionizing radiation exposure during venous port system implantation. METHODS: We retrospectively analyzed consecutive venous port implantations between 2019 and 2022. The older fluoroscopy machines were replaced by two new machines. A fir

A bench study comparing between scalpel-bougie technique and cannula-to-Melker technique in emergency cricothyroidotomy in a porcine model.

BACKGROUND: The ideal emergency cricothyroidotomy technique remains a topic of ongoing debate. This study aimed to compare the cannula-to-Melker technique with the scalpel-bougie technique and determine whether yearly training in cricothyroidotomy techniques is sufficient for skill retention. METHODS: We conducted an observational crossover bench study to compare the cannula-to-Melker with the scalpel-bougie technique in a porcine tracheal model. Twenty-eight anesthetists participated. The primary outcome was time taken for device insertion. Secondary outcomes were first-pass success rate, in

Giant Occipital Encephalocele in an Infant: A Surgical Challenge.

Giant occipital encephalocele is an uncommon condition. The sheer size of the lesion poses a challenge to the surgeon and the anesthetist. Here, we present a 4-month-old girl child presenting with a progressively increasing swelling in the occipital region since birth. The child after evaluation underwent repair of the encephalocele. The purpose of this report was to highlight the difficulties faced during surgery while operating on such a huge occipital encephalocele.

[Perioperative onset of sarcoidosis : Rare differential diagnosis of a difficult airway].

Sarcoidosis is a rare multisystemic chronic inflammatory condition. Typically, there is a big discrepancy between the patient's subjectively perceived symptoms and the presence of clinical signs. Due to the high number of potential sites of onset (e. g. larynx, lungs, heart, liver, kidneys) the affected organs may show functional restrictions that are relevant for decisions related to anesthesia. Therefore, an anesthetists working in a clinical setting should be familiar with the special needs of this patient collective. This article describes the onset of sarcoidosis in a case of an unexpecte

Suspected recurrent anaphylaxis in different forms during general anesthesia.

We report on a patient who presented with recurrent severe shock during general anesthesia. The patient was a man scheduled for lung surgery whose first attack was a coronary spasm, which was followed by a second shock with severe bronchospasm and hypotension 4 weeks later. An elevated serum tryptase concentration was observed, and subsequent skin testing revealed negative reactions to some drugs administered in this case. This case serves to alert anesthetists to the possibility of some different forms of allergy and highlights the importance of rigorous investigation of all the reagents and

Forensic aspects of ocular injury.

A case of homicidal stabbing resulting in bilateral penetrating ocular injuries is described. The case is noteworthy in that it highlights an unusual mechanism of death in homicidal stabbing. Disturbances in heart rhythm including asystole can be ascribed to the so-called oculocardiac or trigeminocardiac reflex. Although this phenomenon is well known to ophthalmologists, neurosurgeons, and anesthetists, it is much less familiar to forensic pathologists. This is a potential mechanism of death worthy of consideration in cases of sudden unexpected death occurring in the context of facial injury.

AANA Journal Course-Trauma-Informed Care Considerations for Anesthesia Providers.

Posttraumatic stress disorder (PTSD) is a prevalent, underrecognized condition among adults undergoing surgical procedures requiring anesthesia that poses significant risks throughout the perioperative period. Elements inherent to anesthesia care (e.g., loss of control, anesthetic induction, and emergence) may trigger trauma re-experiencing, resulting in heightened anxiety, dissociation, altered pain perception, and an increased incidence of emergence delirium. Despite evidence linking PTSD to adverse perioperative outcomes, current anesthesia protocols lack standardized screening measures and

Comparative Evaluation of Tube First Versus Video Laryngoscope First Techniques in a Normal and Simulated Difficult Airway Model: A Randomized Controlled Trial.

In patients with difficult airways, there can be difficulty with advancing the endotracheal tube into the airway even with a good view of the glottis using video laryngoscopy. The purpose of this study was to determine if the time required to intubate an airway and the number of gaze changes by the laryngoscopist could be decreased by using a novel video laryngoscope technique. Sixteen experienced Certified Registered Nurse Anesthetists were recruited to intubate a manikin with a normal or difficult airway using both the laryngoscope first technique and a new endotracheal tube first technique

Cryoneurolysis for the Treatment of Sensory Nerve Pain.

Acute and chronic pain account for high costs both societally and economically, estimated in the United States to be $635 billion. Untreated or undertreated acute pain is associated with substantial morbidity and may become chronic pain. Surgical patients are at an increased risk of long-term opioid use or addiction. Overdose from drugs of abuse, including opioids, is the leading cause of death due to injury in the United States. Traditional pain management strategies for acute and chronic pain have focused on opioid medications, which are often associated with severe side effects. Cryoneuroly
