Evaluation of intracardiac shunts.

This article will review the qualitative and quantitative methods of radionuclide evaluation of intracardiac shunts. Radionuclide angiocardiography of the cardiopulmonary system has proven adequate, with few exceptions, for qualitative detection of left-to-right and right-to-left intracardiac shunts. Numerous authors have reported on the detection of intracardiac shunts using qualitative methods such as rapid sequential visualization of nuclide angiocardiography using the scintillation camera with Polaroid, 35mm or 70mm film. Recent development of videotape storage systems and computers offers

New performed catheter for entry into pulmonary artery in complete transposition of great arteries.

A new performed, semi-rigid, polyethylene catheter, with built-in torque control, has been devised for entry to the pulmonary artery in complete transposition of the great arteries. It has been used 19 times in 17 patients: 18 times the pulmonary artery was entered from the right atrium (via the left atrium and ventricle) in a time between 40 s and 15 min (median 5 min); the patients' ages were 2 days to 6 years (median 8 months) and their weights were 3.1 to 13.3 kg (median 6.9 (kg: in the remaining 1 day-old patient, the procedures was terminated because of atrial flutter. The catheter was i

[Congenital heart disease: diagnostic approaches, surgical indications and operative results].

Our management policies of the main congenital cardiac defects which require emergency surgical intervention in neonate and infancy were reported. Total anomalous pulmonary venous drainage (TAPVD): Recent refinements in two-dimensional (2-D) echocardiography have made it possible to operate for this anomaly using only this modality. When a combination of coarctation is suspected and/or the location of the drainage of the pulmonary vein is obscure, catheterization and angiocardiography are added for detailed diagnosis. It is, however, our policy not to perform right-sided angiocardiography for

Angiographic classification of ventricular septal defects in tetralogy of Fallot.

The ventricular septal defect (VSD) in Fallot's tetralogy (TF) was classified into four types: perimembranous (PM), muscular outlet (MO), doubly committed subarterial (DS), and DS with perimembranous extension. From July 1990 to June 1992, we used angiocardiography to define preoperatively the types of VSD in 30 cases of TF, and correlated them with the operative findings. The angiographic images used to identify the types of VSDs were anteroposterior (AP) plus cranial tilting 20-30 degrees, right anterior oblique 30 degrees plus cranial tilting 30 degrees (elongated RAO view) and true lateral

Retroesophageal right aortic arch.

Right aortic arch with left descending aorta and a retroesophageal aortic segment is an uncommon congenital vascular malformation which may cause symptoms of compression from a vascular "ring." The presence of this malformation may be suspected by the findings on barium swallow and the diagnosis confirmed by angiocardiography. Surgical intervention may be necessary in patients who have severe symptoms.

[Normal position and malposition of the heart (author's transl)].

The main types of heart positions and the malpositions are described as demonstrated on plain films of the thorax. Special attention is drawn to the situs ambi -- guous with the poly -- or asplenia which may be recognized from hypoarterial or epiarterial position of the main bronchi and their equal lengths and courses. This diagnosis is important because of the frequent accompanying congenital cardiac malformations and in respect to the planning of angiocardiography.

Right-to-left patent ductus arteriosus with dysplastic left ventricle in a dog.

A 6-week-old Siberian Husky pup had an unusual group of congenital heart anomalies that included a right-to-left patent ductus arteriosus, a small left ventricular chamber and ascending aorta, and a dysplastic mitral valve that may have been stenotic. Anomalies were diagnosed, using cardiac catheterization, angiocardiography, and blood gas determinations. Findings were confirmed by postmortem examination.

Congenital localized stenosis of the pulmonary veins.

One case of localized stenosis of the extrapulmonary portion of pulmonary veins is reported in a 3 years old child. The chest X-Ray showed a rounded and pulsatile opacification over the right hilum and on angiocardiography the right superior pulmonary vein looked very dilated. 20 other cases of segmental stenoses of pulmonary veins were collected from the literature.

Transesophageal two-dimensional echocardiography in the diagnosis of cor triatriatum in the adult.

Two adult patients with left ventricular inflow obstruction are presented. Conventional two-dimensional echocardiography had failed to yield a definite diagnosis, whereas transesophageal two-dimensional echocardiography clearly documented a membraneous echo structure within the left atrium, diagnostic of cor triatriatum. On the basis of the transesophageal echocardiographic findings, left heart catheterization and angiocardiography were not performed and both patients successfully underwent cardiac surgery.

Pre-operative evaluation with MR in tetralogy of fallot and pulmonary atresia with ventricular septal defect.

PURPOSE: To assess whether MR imaging could replace angiography in preoperative evaluation of patients with tetralogy of Fallot and pulmonary atresia with ventricular septal defect (VSD), especially since the surgical correction was done earlier than was previously the rule. MATERIAL AND METHODS: Fourteen patients with tetralogy of Fallot (n = 10) or pulmonary atresia with VSD (n = 4), mean age 7.5 +/- 4.4 months, were evaluated with angiocardiography and MR before definitive surgical correction. RESULTS: There was good diagnostic agreement between the two modalities when evaluating right ve

Rudimentary dysplastic valvar tissue guarding the tricuspid orifice with dilatation of the right ventricle and a patent outflow tract.

Tricuspid regurgitation due to rudimentary and dysplastic tricuspid valvar tissue associated with a patent right ventricular outflow tract is described in a 10-year-old child presenting with cyanosis since birth. The diagnosis was confirmed by cross-sectional echocardiography, angiocardiography and subsequently by histopathological data. We discuss the differentiation of these findings from Uhl's anomaly and from the congenitally unguarded tricuspid orifice.

[A case of Ebstein disease and its surgical repair with dura-mater prosthesis].

The A.A. present a case of Ebstein's Disease, on a woman 48 years old. After stressing the clinical and laboratorial aspects, as well as hemodynamics and angiocardiography data, they summarize the operation, on which was used a human "dura-mater" valve in tricuspid position. There is no knowledge on literature of previous applications of this kind of valve on Ebstein's Disease. The patient, one year after the operation, is improving quiet well.

[Interventions on the heart and large vessels without prior heart catheterization].

Between May 1983 and May 1987, 161 children of all age groups underwent surgery for congenital cardiac malformations without prior cardiac catheterization and angiocardiography. Their diagnoses were established by clinical investigation, electrocardiogram, chest X-ray and cross-sectional echocardiography only in all cases. The most common surgical procedures were ligation of a patent ductus arteriosus (n = 68), correction of aortic coarctation (n = 28), balloon atrial septostomy (n = 27), and closure of an atrial septal defect (n = 23). To prove the efficiency and accuracy of this method all p

[Ebstein's disease. Study of 5 personal cases].

Ebstein's disease is a frequently cyanotic form of congenital heart disease. The lesions consist of low implantation in the right ventricle of the tricuspid valves, often associated with an atrial septal defect. Our five cases (two men and 3 women) had a variable clinical presentation depending on the degree of the variable clinical presentation depending on the degree of the lesions. A fairly exceptional fact should be emphasised, which is that 3 of our patients brought to term or almost to term 3 pregnancies and gave birth to live children. Ebstein's disease should be suspected when a large

Anomalous origin of the right coronary artery from the main pulmonary artery. A case report.

A fifty-nine-year-old, asymptomatic woman with anomalous origin of the right coronary artery from the main pulmonary artery is presented. The diagnosis was established by catheterization and angiocardiography. The indications for surgery in this condition are discussed, with special reference to asymptomatic patients. In this patient, surgery was not undertaken, because of the absence of signs of impairment of left ventricular perfusion and function. The patient remains asymptomatic after eight years of observation.

Interruption of the aortic arch with associated cardiac anomalies. Survival to adulthood.

Interruption of the aortic arch is a rare and usually lethal cardiac malformation. We report a rare case of a patient with IAA distal to the left subclavian artery associated with double outlet right ventricle, subaortic VSD and patent DA who survived to adulthood. In this patient, the complete diagnosis was made by cardiac catheterization and angiocardiography. We discuss the probable mechanisms, diagnostic problems and therapeutic implications of such long survival.

[Isolated persistent left superior vena cava. Detection of cause during central venous catheterization].

The Authors report the causal discovery of isolated persistent left superior vena cava (IPLSVC) during a central venous incannulation with a Groshong's catheter, in a patient undergoing bone marrow transplant. After a short introduction about the IPLSVC ontogenesis, they stress the need for fluoroscopy during the central venous incannulation. The Authors conclude that the possibility of angiocardiography has been very helpful, in this case, for diagnosis and prognosis.

Ventricular septal defects in two goats.

Two young goats with loud, holosystolic heart murmurs heard best over the left and right heart base were suspected to have interventricular septal defects. Cardiac catheterization with angiocardiography supported the clinical diagnosis in both goats and the ventricular septal defects were confirmed at necropsy. In one goat a ventricular septal defect was associated with a hypoplastic aorta and in the other, an ectopic ureter opened into the vagina.

Posterior deviation of left ventricular outflow tract septal components without ventricular septal defect.

OBJECTIVE: To describe 11 patients with narrowing of the left ventricular outflow tract caused by angular posterior deviation of both the outlet septum and the upper part of trabecular septum, which was diagnosed by cross sectional echocardiography in all and confirmed by angiocardiography in seven. RESULTS: Four patients had a subaortic systolic pressure gradient ranging from 23 to 70 mm Hg by Doppler echocardiography; cardiac catheterisation showed a significant (60 and 104 mm Hg) systolic pressure gradient in two. In four cases aortic regurgitation and two tricuspid pouches were shown by D
