Anorexia nervosa is a syndrome with multifactorial etiology in which several genetic, biologic, psychological and social factors are involved. Patients affected by anorexia nervosa (AN) may develop multiple endocrine abnormalities, e.g. amenorrhea, hypothalamus-pituitary-adrenal axis hyperactivity, low T3 syndrome and peculiar changes of somatotroph axis function. These endocrine abnormalities are also found after prolonged starvation and may represent an adaptive response developed in order to save energy and proteins. It is still a matter of debate whether these endocrine changes are etiolog

BACKGROUND: Chronic rhinosinusitis with nasal polyps (CRSwNP) is characterised by inflammatory mucosa and polyp formation in the paranasal sinuses. The study's primary objective was to evaluate the outcomes of postoperative oral corticosteroid (OCS) in treating patients with bilateral CRSwNP. The secondary objective was to determine whether preoperative serum IgE levels (sIgE)and/or blood eosinophil count (BEC) correlate with postoperative outcomes following OCS use.
METHODS: Patients with bilateral CRSwNP (n=236) who underwent endoscopic sinus surgery (ESS) were randomly assigned to receive

Anorexia nervosa is a severe and complex disorder with incompletely known vulnerability factors. It is generally recognized that anorexia nervosa is a familial disorder, but the majority of twin studies have shown that the concordance rate for monozygotic twins is higher (on average 44%) than for dizygotic twins (on average 12.5%). This difference in concordance rates shows that genetic factors, more than common familial environment, may explain why the 'anorexia nervosa' phenotype runs in families. In order to estimate the heritability in the broad sense of anorexia nervosa according to publi

We report a 19-year-old woman with the associated disorders of generalized pruritus, hypertrichosis and anorexia nervosa, all of which had been present for approximately 4 months. Psychotherapy induced both weight gain and resolution of the pruritus. We propose that anorexia-associated pruritus be considered one of the important cutaneous signs in anorexia nervosa.

Some patients seen for the treatment of an eating disorder do not fulfill criteria for either of the two established disorders (Anorexia Nervosa and Bulimia Nervosa). This large number of patients currently can only be diagnosed as eating disorder not otherwise specified-EDNOS-(in DSM-III-R) or as atypical disorder (in ICD-19). The authors study some proposal of definition and classification of the Eating Disorders chapter for EDNOS and atypical disorders would be far smaller than they are now. Problems associated with the inclusion of weight concern as a necessary and defining criterion (the

The records of 100 patients with anorexia nervosa were reviewed. Neurologic complications were present in 47 patients. Neuromuscular abnormalities were most common and were present in 45% of patients. Generalized muscle weakness was detected in 43% of patients and peripheral neuropathies in 13%. Less common neurologic complications included headaches (6%), seizures (5%), syncope in the absence of orthostatic hypotension (4%), diplopia (4%), and movement disorders (2%). Neurologic problems due to chronic deficiency diseases were rare; only one patient had symptoms directly attributable to a vit

No definitive therapy exists for anorexia nervosa (AN) or bulimia nervosa (BN). Nevertheless, biologic and psychologic research into these disorders has increased over the last decade. We examine the various drugs available for treatment. Advances in pharmacotherapy for AN have been modest and have reflected efforts either to stimulate hunger and weight gain or to control complications of the starvation process. Food remains the "drug" of choice. Antidepressants have been found to be beneficial in the treatment of BN. The meaning of this in the context of a relation between BN and mood disorde

Menstrual dysfunction is common in adolescents who are involved in intensive athletic activity or who are limiting their nutritional intake excessively. The mechanism for hypothalamic amenorrhea in athletes and dieters is not yet fully understood. Other causes of menstrual dysfunction due to pregnancy, central lesions, hormone imbalance, or ovarian failure should be excluded in the athlete with amenorrhea. Amenorrheic patients who have sufficient estrogen effect on their endometrium to have withdrawal bleeding following exposure to progestins should be cycled with progestins on a regular basis

Preclinical inpatient therapy and postclinical prognostic factors concerning 113 cases were empirically scrutinized and hierarchically classified in accordance with their importance, especially in light of the relative weight of somatic, psychologic, or social variables. The criterion is the state of health at the time of follow-up, after 6.4 years. The results thus obtained are the basis of therapy recommendations.

The main objective of the study was to evaluate the endocrinological picture of anorexia. The sample consisted of 23 anorectic patients (20 females, 3 males) with a control group of 10 normal females and 5 normal males. All participants underwent a work-up which included testing for hypothalamic, hypophyseal, thyroidal, adrenal, gonadal functioning and glucose metabolism. Our results revealed a reduced urinary output and low serum levels of gonadotropins with different responses to LHRH correlating with the stage of the illness. We found reduced urinary estrogens and elevated testosterone leve

Twenty-nine anorectic female patients were subdivided in three groups: group I when primary amenorrhoea; group II when secondary amenorrhoea together with a weight (for their height) equal or above the 10th percentile of the ratio of total water over body weight (TW/B Wt); group III when secondary amenorrhoea was associated with a weight below this 10th percentile. Despite very low mean oestradiol levels (respectively 22, 36 and 32 pg/ml), prolactin secretion remained normal both in basal conditions as well as after 200 micrograms TRH iv. For group III patients, a delay in the peak prolactin r

After a brief survey on etiopathogenesis, clinic, paraclinic and differential diagnosis of the disease, the authors describe a patient, who at the age of 18 developed Anorexia nervosa, with a progressive body weight reduction from 70 to 30 kg. After the consultation with a psychiatrist the patient was admitted to a psychoneurological dispensary for treatment. Attention is drawn to the diagnostic difficulties and the internist was underlined, has to think and diagnose that, even though rare disease.

Anorexia nervosa (AN), a psychosomatic disease mainly affecting young women, is characterized by conscious starvation, periods of excessive carbohydrate intake and often deliberate vomiting. Medical history, dental examination, and saliva tests of 39 patients aged 14 to 42 years, having suffered from AN for periods of 1 to 20 years, showed dental caries, due to excessive carbohydrate consumption, in all subjects, often in a rampant form. In patients with a history of intense vomiting (27 cases) severe lingual-occlusal erosion (perimylolysis) was nearly always present. Buccal erosion, mainly du

Follow-up data on 26 patients hospitalized during adolescence for anorexia nervosa are presented. The mean length of follow-up was 4.9 years. There were no mortalities. Only one patient at the time of follow-up was still possibly anorexic. However, a substantial number of the patients did have other eating difficulties. Most striking was the clinical psychopathology present at follow-up, particularly the incidence of affective disorder. A large number of patients manifested depressive symptomatology in both the premorbid and the postmorbid states, as well as at the time of follow-up. A family

The historical roots of anorexia nervosa are closely linked to religious ascetic traditions. During the Middle Ages, numerous "fasting saints," including Catherine of Siena and Saint Margaret of Hungary, regarded prolonged fasting as a path to spiritual purification, self-mastery, and union with God. The denial of bodily needs symbolized holiness, moral purity, and transcendence. In modern secular societies, however, these meanings have not disappeared but have been transformed. Several scholars argue that the contemporary cult of thinness functions as a quasi-religious ideology in which weigh

Congenital duodenal web (CDW) is a rare cause of intestinal obstruction in adults, making diagnosis challenging. We present the case of a 26-year-old woman with chronic epigastric symptoms who recently developed vomiting and significant weight loss. Prolonged compensatory duodenal dilation obscured typical radiological signs and delayed preoperative identification. The diagnosis was confirmed intraoperatively and managed surgically. This case highlights the need to consider CDW in adults with unexplained proximal gastrointestinal obstruction.

More than any other mental illness, the course, prognosis, and therapy of anorexia nervosa are shaped by the physical changes associated with being underweight. This article provides an overview of the endocrine changes associated with malnutrition and underweight. This overview serves as a basis for understanding the other articles in this special issue, which deal with the health risks associated with being underweight. In this context, the differences between underweight in anorexia nervosa and in constitutional thinness are of particular importance in assessing the impact of intentional we
