Language specificity of lexical-phonological therapy in bilingual aphasia: A clinical and electrophysiological study.

Based on findings for overlapping representations of bilingual people's first (L1) and second (L2) languages, unilingual therapies of bilingual aphasia have been proposed to benefit the untrained language. However, the generalisation patterns of intra- and cross-language and phonological therapy and their neural bases remain unclear. We tested whether the effects of an intensive lexical-phonological training (LPT) in L2 transferred to L1 word production in a Persian-French bilingual stroke patient with Broca's aphasia. Language performance was assessed using the Bilingual Aphasia Test, a 144-i

An enhanced protocol for constraint-induced aphasia therapy II: a case series.

PURPOSE: The initial version of Constraint-Induced Aphasia Therapy (CIAT I) consisted of a single exercise. This study sought to evaluate the feasibility for future trials of an expanded and restructured protocol designed to increase the efficacy of CIAT I. METHOD: The subjects were 4 native English speakers with chronic stroke who exhibited characteristics of moderate Broca's aphasia. Treatment was carried out for 3.5 hr/day for 15 consecutive weekdays. It consisted of 3 components: (a) intensive training by a behavioral method termed shaping using a number of expressive language exercises i

Cerebral glucose metabolism as a predictor of recovery from aphasia in ischemic stroke.

OBJECTIVE: The recovery of language function seen in poststroke aphasia is significantly related to the severity of the initial neurologic deficit. However, a great deal of variability still remains in the improvement that is actually achieved. To detect predictors that will help in explaining this variability, positron emission tomography (PET) and fluorodeoxyglucose F 18 (18-F-fluorodeoxyglucose) were used and the results were analyzed by stepwise regression. DESIGN: Using PET imaging after injection of fluorodeoxyglucose F 18, the regional changes in glucose metabolism in 26 patients at a

Changes in the corpus callosum during the recovery of aphasia: A case report.

RATIONALE: The corpus callosum, which is the most important fiber pathway linking the bilateral hemispheres, plays a key role in information access, as well as the functional coordination and reorganization between the bilateral hemispheres. However, whether the corpus callosum will undergo structural changes during the recovery of aphasia is still unclear. In the current study, a Chinese aphasic patient with stroke was reported to develop changes in the corpus callosum after speech therapy. PATIENT CONCERNS: A 33-year-old right-handed male patient had aphasia only without limb paralysis at 1

Syndromes in developmental dysphasia and adult aphasia.

We have attempted to draw some parallels between syndromes of adult acquired aphasia and of childhood developmental dysphasia. There appear to be two syndromes that are almost exact duplicates in the adults and the children: (a) pure word deafness and verbal auditory agnosia, and (b) aphemia and verbal dyspraxia. Two other syndromes seem to have rather close but not exact counterparts: Broca's aphasia and the phonologic-syntactic deficit syndrome, and transcortical sensory aphasia and the semantic-pragmatic deficit syndrome. There are two dysphasic syndromes, the phonologic production deficit

Body image and comprehension of body part names.

The relation between awareness of body topology and auditory comprehension of body part names was studied in 22 aphasic subjects. Two nonverbal tasks--human figure drawing and placement of individual body parts in relation to a drawn face--were compared with two auditory tests of body part comprehension. The two nonverbal and the two verbal tasks were closely correlated with each other, but there was no relation involving either of the verbal tests with either of the nonverbal tests. Selection errors in the auditory comprehension tasks were predominantly semantically based and equally distribu

[Localization of lesions in aphasia: clinical-CT scan correlations (Part II)].

On the basis of the characteristic symptoms or the result of a speech examination, 127 right-handed cases with various types of aphasia were subdivided into two groups within each aphasic syndrome. Using a microcomputer, the locus and extent of the lesions, as demonstrated by computed tomography for each group were superimposed onto standardized matrices. The relationship between the focus and the extent of the lesions and the various symptoms was investigated. Broca aphasics: More than 80% of the group with obvious anarthric components had lesions of the third frontal gyrus involving Broca's

Intelligence and left hemisphere disease. The role of aphasia, apraxia and size of lesion.

The Raven Progressive Matrices and four subtests of the Wechsler-Bellevue Performance Scale were given to 173 left hemisphere patients subdivided according to presence/absence, type (fluent/non-fluent) and severity (moderate/severe) of aphasia. Constructive and ideomotor apraxia scores and CT scan data of each subject entered the statistical analysis. Factors significant in producing a low score on Progressive Matrices and Wechsler-Bellevue were presence of aphasia and constructive apraxia. Site and size of lesion per se failed to account for the intelligence scores. The relationship between a

Effects of different frequencies of repetitive transcranial magnetic stimulation in stroke patients with non-fluent aphasia: a randomized, sham-controlled study.

OBJECTIVE: The purpose of this study was to compare the efficacy of repetitive transcranial magnetic stimulation (rTMS) applied at different frequencies to the contra-lesional hemisphere to optimize the treatment of post-stroke non-fluent aphasia. METHOD: Patients with post-stroke non-fluent aphasia were divided randomly into four groups: a high-frequency rTMS (HF-rTMS) group (10 Hz), a low-frequency rTMS (LF-rTMS) group (1 Hz), a sham stimulation group, and a control group. All groups received the standard treatment (consisting of drug therapy, conventional physical exercises, and speech tra

Language recovery after epilepsy surgery of the Broca's area.

Epilepsy surgery is indicated in select patients with drug-resistant focal epilepsy. Seizure freedom or significant reduction of seizure burden without risking new neurological deficits is the expected goal of epilepsy surgery. Typically, when the seizure onset zone overlaps with eloquent cortex, patients are excluded from surgery. We present a patient with drug-resistant frontal lobe epilepsy who underwent successful surgery with resection of Broca's area, primarily involving the pars triangularis (BA 45). We report transient expressive aphasia followed by recovery of speech. This case provid

Nonlanguage disorders of speech reflect complex neurologic apparatus.

The mute or nearly mute patient who is alert and has good understanding of speech and a right hemiparesis could have Broca's aphasia, akinesia of speech (transcortical motor aphasia), or aphemia. The patient who has Broca's aphasia does not write well, and his speech does not improve greatly with repetition. The speech of a patient with akinesia of speech improves with repetition. The aphemic patient writes normally, but his speech does not improve with repetition. The mute patient whose eyes are open but who is poorly responsive and moves little or not at all could be an akinetic mute (with e

Where language meets meaningful action: a combined behavior and lesion analysis of aphasia and apraxia.

It is debated how language and praxis are co-represented in the left hemisphere (LH). As voxel-based lesion-symptom mapping in LH stroke patients with aphasia and/or apraxia may contribute to this debate, we here investigated the relationship between language and praxis deficits at the behavioral and lesion levels in 50 sub-acute stroke patients. We hypothesized that language and (meaningful) action are linked via semantic processing in Broca's region. Behaviorally, half of the patients suffered from co-morbid aphasia and apraxia. While 24% (n = 12) of all patients exhibited aphasia without ap

Idiosyncratic strategies in sentence production: a case report.

In their case study of a patient with selective apraxia of phonation, Marshall, Gandour, and Windsor (1988, Brain and Language, 35, 313-339) reported that many of his utterances were ill-formed syntactically. In this note, a detailed syntactic analysis of his speech is presented. Although the patient's syntax is deviant, it is seen to result from the application of certain consistent, identifiable, compensatory strategies. Moreover, it is shown that the particular, abnormal syntactic structures are internally consistent with his phonatory apraxia coupled with the normal rules of English prosod

[Cerebral lateralization in two cases of crossed dextral aphasia with right-hemisphere arteriovenous malformation].

This paper reports two cases of crossed dextral aphasia. The first patient was a 60-year-old right-handed male with no family history of sinistrality. He experienced sudden onset of left hemiplegia and loss of consciousness. A CT scan showed high-density area in the right fronto-parietal region. An angiography revealed an arteriovenous malformation (AVM) in the right parietal lobe. It was fed by a branch of the middle cerebral artery and drained through a cortical vein. Neuropsychological examination one week after the surgery showed severe defects of all language moderalities. He was alert an

Functional connectivity to the premotor cortex maps onto longitudinal brain neurodegeneration in progressive apraxia of speech.

Primary progressive apraxia of speech (PPAOS) is a neurodegenerative motor speech disorder affecting the ability to produce speech. If agrammatic aphasia is present, it can be referred to as the non-fluent/agrammatic variant of primary progressive aphasia (nfvPPA). We investigated whether resting-state functional MRI (rs-fMRI) connectivity from disease "epicenters" correlated with longitudinal gray matter atrophy and hypometabolism in nfvPPA and PPAOS. Eighteen nfvPPA and 23 PPAOS patients underwent clinical assessment, structural MRI, rs-fMRI, and [18F] fluorodeoxyglucose (FDG)-PET at baselin

Moyamoya disease and arteriovenous fistula of the epiaortic vessels.

The association of Moyamoya disease (MD) with other cerebrovascular disease has been reported in literature but not the occurrence of MD and arteriovenous fistula of the epiaortic vessels. A 61-year old Italian woman was admitted to our Department because of the sudden onset of Broca's aphasia. At general examination she presented a murmur above the right clavicle. Brain magnetic resonance imaging (MRI) showed an ischemic lesion involving left frontal and temporal lobes. Cerebral digital subtraction angiography (DSA) showed moyamoya vessels and an arteriovenous fistula between vertebral artery

Progressive agrammatic aphasia without apraxia of speech as a distinct syndrome.

Agrammatic aphasia affects grammatical language production and can result from a neurodegenerative disease. Although it typically presents with concomitant apraxia of speech, this is not always the case. Little is known about the clinical course and imaging features of patients that present with agrammatism in the absence of apraxia of speech, which we will refer to as progressive agrammatic aphasia. We aimed to make a detailed description of the longitudinal clinical, linguistic, and neuroimaging features of a cohort of 11 patients with progressive agrammatic aphasia to provide a complete pic

Walter Moxon and his thoughts about language and the brain.

Walter Moxon wrote a well-timed paper in 1866 on aphemia with accompanying right sided hemiplegia. Like many other investigators of this apparent lopsidedness of the articulatory system for human speech. Moxon had to reconcile left hemisphere specialization for this function with the overriding law of symmetry, which for a large sector of the scientific community was a sine qua non of the anatomy and function of high level animal cognition. His reasoning was essentially that since the right dominant hand (and hence the left hemisphere) in some sense led overall bilateral limb movement patterns
