Comparison of evoked arousability in breast and formula fed infants.

BACKGROUND: Currently, there is no consistent evidence that breast feeding reduces the risk for sudden infant death syndrome (SIDS). Arousal from sleep is believed to be an important survival mechanism that may be impaired in victims of SIDS. Previously it has been shown that arousability is impaired by the major risk factors for SIDS such as prone sleeping and maternal smoking. AIMS: To establish whether arousability was altered by method of feeding, and whether breast fed infants would have lower arousal thresholds. METHODS: Forty three healthy term infants were studied using daytime polys

Dysregulation of arousal and amygdala-prefrontal systems in paranoid schizophrenia.

OBJECTIVE: The authors investigated impaired differentiation of limbic-prefrontal systems by autonomic arousal in schizophrenia. It was predicted that paranoid patients would be distinguished by a disjunction of hyperarousal but reduced amygdala and medial prefrontal activity relative to both healthy comparison subjects and patients with nonparanoid schizophrenia. METHOD: Pictures depicting facial expressions of fear were presented to 27 schizophrenia patients (13 paranoid, 14 nonparanoid) and 22 matched healthy comparison subjects in an implicit perception task to evoke limbic activity. Simu

The hypocretin neuron system: an arousal system in the human brain.

Hypocretins are recently discovered neuropeptides produced by a small group of posterior hypothalamic neurons which project widely over the neuroaxis. In this study, we note that hypocretin neuron perikarya in the human brain are localized to the perifornical region of the posterior hypothalamus, extending into the lateral hypothalamus. These neurons lightly innervate all areas of cerebral cortex studied in a variable pattern with denser innervation of association cortex than primary motor or sensory cortex. There is a dense innervation of hypothalamus, locus coeruleus, raphe nuclei, midline t

Arousal in patients with gastro-oesophageal reflux and sleep apnoea.

Nocturnal gastro-oesophageal reflux has been observed in patients with obstructive sleep apnoea (OSA). Negative intrathoracic pressure during apnoeas and arousal have been suggested as the underlying mechanisms. In order to evaluate this hypothesis, the coincidence and sequence in time of arousal, apnoea and reflux events were analysed. Fifteen patients with OSA or heavy snoring were studied by means of standard polysomnograpy with parallel recording of 24-h oesophageal pH. Reflux events during the day were present in all patients, five of whom had symptoms of reflux. In three of these and in

[Postoperative, opiate-induced respiratory depression is not dependent on arousal].

INTRODUCTION: Respiratory depression after intravenous anesthesia is supposed to be related to vigilance. This hypothesis could not yet be tested because of the lack of methods measuring continuously important parameters of respiration without altering the patient's vigilance. Pulse Oximetry offers this possibility. The following study was performed to investigate the effects of the Benzodiazepine Antagonist Flumazenil (Anexate) on parameters of vigilance and respiration after Midazolam/Fentanyl anesthesia. METHODS: 40 healthy patients aged 18-65 years who were to undergo arthroscopy were ran

Autonomic markers of arousal during sleep in patients undergoing investigation for obstructive sleep apnoea, their relationship to EEG arousals, respiratory events and subjective sleepiness.

Estimating the degree of sleep fragmentation is an important part of a respiratory sleep study and is conventionally measured using EEG micro arousals or is inferred indirectly from respiratory abnormalities such as apnoeas and desaturations. There is a need for less labour-intensive measures of sleep fragmentation, and transient rises in blood pressure and heart rate may fulfil this role. Forty unselected sleep clinic referrals undergoing investigation for possible obstructive sleep apnoea (OSA) were studied with one night of polysomnography. Three conventional indices of sleep fragmentation

Robust information transfer calculation reveals dynamic networks during sleep arousal.

Understanding the influence of cardiovascular diseases (CVD) on human regulatory functions is a major challenge in modern healthcare. This study explores whether the autonomic response to sleep arousals can serve as a diagnostic marker of the health of physiological networks. We introduce a novel method for the robust calculation of information transfer, based on the optimization of embedding parameters for transfer entropy (TE) calculation. Using this approach, we analyzed the information transfer dynamics of 8,136 sleep arousals from 1,956 subjects from the SHHS follow-up visit. Compared to

Pupil Size Dynamics Predict Momentary Changes in Self-Reported Arousal.

Subjective arousal, or feelings of activation and alertness, can dynamically modulate affect, behavior, and cognition. Momentary changes in subjective arousal have typically been assessed with self-report in humans, which may alter those very feelings. Although researchers have traditionally used pupil size as a physiological index of arousal, links between pupil dynamics and momentary changes in subjective arousal remain unclear. Here, we combined continuous pupillometry with repeated self-report probes of changing arousal as individuals engaged in a modified Monetary Incentive Delay task. Ac

Modulating cortical excitability and cortical arousal by pupil self-regulation.

The brain's arousal state (i.e., central arousal) is regulated by multiple neuromodulatory nuclei in the brainstem and significantly influences high-level cognitive processes. By exploiting the mechanistic connection between the locus coeruleus, a key regulator of central arousal, and pupil dynamics, we recently demonstrated that participants could gain volitional control over arousal-regulating centers including the locus coeruleus using a pupil-based biofeedback approach. Here, we test whether pupil-based biofeedback modulates electrophysiological markers of cortical excitability, cortical a

Pupil-linked arousal, cortical activity, and cognition in Alzheimer's disease.

Arousal dysfunction contributes to impairments seen in Alzheimer's disease. However, the nature and degree of this dysfunction have not been studied in detail. We investigated changes in tonic and phasic arousal using simultaneous pupillometry-EEG, relating these changes to locus coeruleus integrity, a key arousal nucleus. Forty Alzheimer's disease participants and 30 controls underwent neuropsychological testing using the Alzheimer's Disease Assessment Scale-Cognitive Subscale (ADAS-Cog), MRI designed to show contrast in the locus coeruleus as a measure of integrity and simultaneous pupillome

A vertebrate family without a functional Hypocretin/Orexin arousal system.

The Hypocretin/Orexin signaling pathway suppresses sleep and promotes arousal, whereas the loss of Hypocretin/Orexin results in narcolepsy, including the involuntary loss of muscle tone (cataplexy).1 Here, we show that the South Asian fish species Chromobotia macracanthus exhibits a sleep-like state during which individuals stop swimming and rest on their side. Strikingly, we discovered that the Hypocretin/Orexin system is pseudogenized in C. macracanthus, but in contrast to Hypocretin-deficient mammals, C. macracanthus does not suffer from sudden behavioral arrests. Similarly, zebrafish mutat

Transcutaneous Auricular Vagus Nerve Stimulation Facilitates Cortical Arousal and Alertness.

Transcutaneous auricular vagus nerve stimulation (taVNS) is a promising noninvasive technique with potential beneficial effects on human emotion and cognition, including cortical arousal and alertness. However, it remains unclear how taVNS could improve cortical arousal and alertness, which are crucial for consciousness and daily task performance. Here, we aimed to estimate the modulatory effect of taVNS on cortical arousal and alertness and to reveal its underlying neural mechanisms. Sixty subjects were recruited and randomly assigned to either the taVNS group (receiving taVNS for 20 min) or

Intracranial electrical stimulation of corticolimbic sites modulates arousal in humans.

BACKGROUND: Humans routinely shift their sleepiness and wakefulness levels in response to emotional factors. The diversity of emotional factors that modulates sleep-wake levels suggests that the ascending arousal network may be intimately linked with networks that mediate mood. Indeed, while animal studies have identified select limbic structures that play a role in sleep-wake regulation, the breadth of corticolimbic structures that directly modulates arousal in humans remains unknown. OBJECTIVE: We investigated whether select regional activation of the corticolimbic network through direct el

Immortal orexin cell transplants restore motor-arousal synchrony during cataplexy.

Waking behaviors such as sitting or standing require suitable levels of muscle tone. But it is unclear how arousal and motor circuits communicate with one another so that appropriate motor tone occurs during wakefulness. Cataplexy is a peculiar condition in which muscle tone is involuntarily lost during normal periods of wakefulness. Cataplexy therefore provides a unique opportunity for identifying the signaling mechanisms that synchronize motor and arousal behaviors. Cataplexy occurs when hypothalamic orexin neurons are lost in narcolepsy; however, it is unclear if motor-arousal decoupling in

Mother-Daughter Mutual Arousal Escalation and Emotion Regulation in Adolescence.

Adolescence is a period of heightened emotionality, and difficulties with emotion regulation during adolescence are associated with the development of internalizing disorders, especially for girls, who are at elevated risk. Mothers may socialize emotion dysregulation by engaging in frequent interactions with their adolescents that involve mutual increases in arousal. This study tested a model of mother-adolescent mutual arousal escalation in a conflict discussion task in adolescent girls and examined associations between mutual arousal escalation and adolescent emotion regulation. Participants

Quantifying the Arousal Threshold Using Polysomnography in Obstructive Sleep Apnea.

STUDY OBJECTIVES: Precision medicine for obstructive sleep apnea (OSA) requires noninvasive estimates of each patient's pathophysiological "traits." Here, we provide the first automated technique to quantify the respiratory arousal threshold-defined as the level of ventilatory drive triggering arousal from sleep-using diagnostic polysomnographic signals in patients with OSA. METHODS: Ventilatory drive preceding clinically scored arousals was estimated from polysomnographic studies by fitting a respiratory control model (Terrill et al.) to the pattern of ventilation during spontaneous respirat
