The authors present a case of monoarticular psoriatic arthritis affecting the knee of a woman with no previously identified manifestations of psoriasis. At time of symptom onset, joint aspirates were initially sterile with predominantly neutrophilic leukocytosis; however, repeat aspiration and culture was positive for Candida parapsilosis. Despite appropriate antifungal treatment, symptoms continued to worsen and other large joint manifestations including the contralateral elbow raised suspicions for alternative etiology. Ultimately new onset skin excoriations on the palms suggested psoriatic

OBJECTIVE: We aimed to determine whether calprotectin and α-defensins could discriminate septic from other inflammatory arthritides.
METHODS: Synovial fluids with a predominance of neutrophils from patients with septic arthritis, pseudogout and RA were prospectively collected. Neutrophil-related proteins calprotectin and human neutrophil α-defensins levels were assessed in synovial fluids. Demographic parameters and biomarkers with P-value ⩽0.05 for differentiating septic from non-septic arthritis were included in a multivariable model. Multivariable logistic regression with stepwise selectio

Rheumatoid arthritis treatment has been shown to improve quality of life. There is little data regarding the impact of the disease and treatments on sexual function. The aim of this study was to describe the results of an assessment of sexual activity/sexual satisfaction of rheumatoid arthritis patients, identify the sexual dysfunction features, and assess their association with disease activity/disease activity parameters and other systemic risk factors/comorbidities. Consecutive rheumatoid arthritis patients attending the outpatient rheumatology clinic completed the multidimensional patient-

Rheumatoid arthritis (RA) and juvenile idiopathic arthritis (JIA) are heterogeneous autoimmune diseases characterized by chronic joint inflammation. Methotrexate is used as the gold standard to treat rheumatoid arthritis, yet there are many patients in whom the disease cannot be controlled or who experience unacceptable intolerance. Most of the biologics currently used are effective, but mostly if combined with methotrexate. Long-term possible side effects, such as impaired host defense mechanisms against infection and lymphoma, are distinct disadvantages and a major concern of anticytokine th

There is considerable overlap in the initial clinical and laboratory presentations of acute septic arthritis and Lyme arthritis. The authors present a consecutive case series of 10 children with acute arthritis consistent with septic arthritis who ultimately were diagnosed with Lyme disease. Fifty percent presented with a fever of 38 degrees C or higher. The peripheral white blood cell count and differential were not elevated, but the erythrocyte sedimentation rate and the C-reactive protein were significantly elevated. The joint fluid cell count was elevated, with 91% neutrophils. Seven patie

OBJECTIVE: To test the hypothesis that cystic rheumatoid arthritis (RA), characterized as subchondral cysts as the only radiographic abnormality in hands and feet for 2 years after first abnormal radiograph, is a mild subset of RA.
METHODS: Fifty-four patients with cystic RA were compared with 144 RA controls matched for age, sex, disease duration, and year of first visit. All patients were randomly selected from a database of 1580 patients with RA attending the clinic 1982-88. In 1994, data of 90% of the patients were collected by one investigator, blinded to the study groups. All available

It is reported that most of the causative organisms of suppurative arthritis complicating rheumatoid arthritis (RA) is Staphylococcus aureus and that Streptococcus pneumoniae is rare, representing less than 5% of cases of suppurative arthritis complicating RA. We here report two cases of pneumococcal septic arthritis complicating RA. Both were female, and 68 and 64 years old, respectively. They had active, long-standing RA with destructed changes. Infected joints included both knees (case 1) and right knee (case 2). Pain and loss of motion in the septic joints were prominent. On admission, the

Immunogenetics are supporting the marked heterogeneity of chronic arthritis in children. Thus DRw13-DRw18 and DQw6-DQw18 were associated with persistent pauciarticular disease in children with an early onset of disease. Several studies have shown DPw2 as an additional susceptibility factor in this subgroup. Standardization of diagnostic criteria for juvenile onset spondyloarthropathy and psoriatic arthritis is necessary; various studies are in progress, and although HLA-B27 provides the common marker, this may only apply to a small group of juvenile psoriatics who have spondyloarthropathy. In

An idiopathic, asymmetrical oligoarthritis affecting young adults is prevalent in the tropical regions of Africa, Asia and Melanesia. A serological study was undertaken in 23 consecutive patients with polyarthritis. Each patient was assigned two paired control subjects. A standardized history, physical examination and investigations were performed. Acute and convalescent sera were examined for evidence of recent infection. The presence of HLA-B27 was determined. Twelve (52%) of the 23 patients had more than one of the clinical features of a reactive arthritis. No serological evidence was found

A patient with rheumatoid arthritis treated with corticosteroids developed a septic arthritis of her right knee. She had been bitten on her right leg by her pet cat 2 weeks earlier. Pasteurella multocida, resistant to penicillin, was isolated from the septic joint and from the oropharynx of the cat. The arthritis was successfully treated with ampicillin. No attempt to eradicate the organism from the cat was made because of the expectation of early recolonisation.

Laryngeal arthritis is a rare manifestation of JRA. The difficulty of indirect laryngoscopy in small children has precluded the type of study which has demonstrated this manifestation in up to one quarter of arthritic adults. Recognition of cricoarytenoid arthritis in the patient reported might have prevented the need for tracheostomy, and eventually led to satisfactory reduction of corticosteroid medication and healing of the tracheostomy stoma.

BACKGROUND: Midfoot arthritis affects patient function and can cause notable pain. We describe a distinct pattern of midfoot arthritis in patients without known trauma or underlying medical diagnosis.
METHODS: Patients were identified from clinical practice as having radiographic arthritis involving the medial naviculocuneiform and central tarsometatarsal joints, without a history of trauma. Medical records were reviewed to examine demographic data, bilaterality, treatment, and surgical outcomes (if applicable).
RESULTS: Forty-one patients were identified as having this pattern of midfoot ar

This article focuses on the imaging of 5 discrete entities with a common end result of disability: posttraumatic arthritis, a common form of secondary osteoarthritis that results from a prior insult to the joint; avascular necrosis, a disease of impaired osseous blood flow, leading to cellular death and subsequent osseous collapse; septic arthritis, an infectious process leading to destructive changes within the joint; complex regional pain syndrome, a chronic limb-confined painful condition arising after injury; and cases of cancer mimicking arthritis, in which the initial findings seem to re

BACKGROUND: Despite optimal therapy with disease-modifying antirheumatic drugs, many people with inflammatory arthritis (IA) continue to have persistent pain that may require additional therapy.
OBJECTIVES: To assess the benefits and safety of combination pain therapy for people with IA (rheumatoid arthritis (RA), ankylosing spondylitis (AS), psoriatic arthritis (PsA) and other spondyloarthritis (SpA)). We planned to assess differences in effects between patients on background disease-modifying antirheumatic drug (DMARD) therapy and patients on no background therapy in subgroup analyses.

Rheumatoid arthritis is a systemic autoimmune disease with a multifactorial etiology, resulting from the interaction between genetic susceptibility and environmental factors. These interactions lead to the loss of immune tolerance and the development of autoimmunity directed against citrullinated proteins. The transition to arthritis is accompanied by the maturation of the humoral immune response and the activation of specific T-cell subsets. Early identification of high-risk individuals will enable the development of preventive strategies aimed at altering the disease's natural course.

OBJECTIVES: Gene regulatory microRNAs (miRNAs) have emerged as promising biomarkers and therapeutic targets in autoimmunity pathology. This study examines circulatory miRNAs as cellular biomarkers that can distinguish rheumatoid arthritis (RA) from psoriatic arthritis (PsA) to evaluate the potential implications for disease pathogenesis.
METHODS: RA (n = 48) and PsA (n = 49) patients and healthy controls (HC) (n = 20) were recruited and serums obtained. Multiplex analysis of serum miRNAs was performed using the FirePlex miRNA Immunology-V2 panel (FirePlex Bioworks Inc.). DNA intelligent analy

CASE: A 7-year-old girl presented with a painful effusion in her right knee, fever, and a negative joint aspirate. One week later, she presented again with an effusion in her left elbow, elevated inflammatory markers, and a joint aspirate that raised concerns for septic arthritis. Irrigation and debridement of the elbow were performed, and the cultures returned negative results. Subsequently, the child experienced ongoing migratory polyarthropathy over several months, which was ultimately identified as leukemia. The initiation of chemotherapy resolved her symptoms. Although the orthopaedic lit

Involvement of the temporomandibular joint (TMJ) in patients with juvenile idiopathic arthritis (JIA) has been increasingly reported, affecting up to 87% of cases. This involvement generally occurs after the diagnosis of JIA has been established; however, in the authors' cases, as in a few others documented in the literature, patients presented with isolated TMJ arthritis as the sole joint involvement. The authors performed a narrative literature review on TMJ involvement in JIA and reported 2 cases that presented with isolated TMJ arthritis as the initial manifestation of JIA. Although isolat
