Regulatory measures for mitigating physical and mental health impacts in aerospace environment: A systematic review.

Long-term spaceflight poses significant challenges to astronauts' physical and mental health, resulting in physiological issues such as osteoporosis, muscle atrophy, and cardiovascular dysfunction, as well as psychological problems like depression, anxiety, social withdrawal, and cognitive decline. As the duration of space missions continues to increase, the above challenges cannot be ignored. Therefore, identifying effective regulatory measures is essential. This article provides a concise review of the latest domestic and international research on strategies to mitigate physiological and psy

Midodrine prescribed to improve recurrent post-spaceflight orthostatic hypotension.

Many astronauts exhibit post-spaceflight orthostatic hypotension due to inadequate norepinephrine release when in an upright posture. We hypothesized that an alpha1-adrenergic agonist, midodrine, would be an effective countermeasure. A female astronaut, who had problems with postflight orthostatic hypotension after a previous flight, consumed 10 mg midodrine after a subsequent flight, prior to her tilt test. Hemodynamic variables were compared between the two flights. Midodrine prevented severe falls in stroke volume, cardiac output and systolic pressure, and severe increases in heart rate wit

Neuroplasticity changes during space flight.

Neuroplasticity refers to the ability of neurons to alter some functional property in response to alterations in input. Most of the inputs received by the brain and thus the neurons are coming from the overall sensory system. The lack of gravity during space flight or even the reduction of gravity during the planned Mars missions are and will change these inputs. The often observed "loop swimming" of some aquatic species is under discussion to be based on sensory input changes as well as the observed motion sickness of astronauts and cosmonauts. Several reports are published regarding these ch

Evaluation of deep space exploration risks and mitigations against radiation and microgravity.

Ionizing radiation and microgravity are two considerable health risks encountered during deep space exploration. Both have deleterious effects on the human body. On one hand, weightlessness is known to induce a weakening of the immune system, delayed wound healing and musculoskeletal, cardiovascular, and sensorimotor deconditioning. On the other hand, radiation exposure can lead to long-term health effects such as cancer and cataracts as well as have an adverse effect on the central nervous and cardiovascular systems. Ionizing radiation originates from three main sources in space: galactic cos

Intracranial Effects of Microgravity: A Prospective Longitudinal MRI Study.

Background Astronauts on long-duration spaceflight missions may develop changes in ocular structure and function, which can persist for years after the return to normal gravity. Chronic exposure to elevated intracranial pressure during spaceflight is hypothesized to be a contributing factor, however, the etiologic causes remain unknown. Purpose To investigate the intracranial effects of microgravity by measuring combined changes in intracranial volumetric parameters, pituitary morphologic structure, and aqueductal cerebrospinal fluid (CSF) hydrodynamics relative to spaceflight and to establish

Reactivation of Latent Epstein-Barr Virus: A Comparison after Exposure to Gamma, Proton, Carbon, and Iron Radiation.

Among the many stressors astronauts are exposed to during spaceflight, cosmic radiation may lead to various serious health effects. Specifically, space radiation may contribute to decreased immunity, which has been documented in astronauts during short- and long-duration missions, as evidenced by several changes in cellular immunity and plasma cytokine levels. Reactivation of latent herpes viruses, either directly from radiation of latently infected cells and/or from perturbation of the immune system, may result in disease in astronauts. Epstein‒Barr virus (EBV) is one of the eight human herpe

Evaluation of the left ventricle contractility during and after 6 months on the ISS using a speckle tracking modality.

PURPOSE: Cardiac contractility can be evaluated during ultrasound evaluations using speckle tracking to map the motion of myocardium during the cardiac cycle. The purpose of the current study was to use this method to evaluate cardiac contractility in astronauts before, during, and after 6 months of spaceflight on the International Space Station. METHODS: Speckle tracking data were recorded from ten astronauts (1F) preflight (PRE), on flight day 150 (FD150) and approximately 4 days after returning to Earth (R4). A 2D apical view video of the left ventricle was captured at each time point for

Effects of Spaceflight Stressors on Brain Volume, Microstructure, and Intracranial Fluid Distribution.

Astronauts are exposed to elevated CO2 levels onboard the International Space Station. Here, we investigated structural brain changes in 11 participants following 30-days of head-down tilt bed rest (HDBR) combined with 0.5% ambient CO2 (HDBR + CO2) as a spaceflight analog. We contrasted brain changes observed in the HDBR + CO2 group with those of a previous HDBR sample not exposed to elevated CO2. Both groups exhibited a global upward shift of the brain and concomitant intracranial free water (FW) redistribution. Greater gray matter changes were seen in the HDBR + CO2 group in some regions. Characteristics of blood plasma proteome changes associated with the hemorrhagic purpura of cosmonauts on the first day after long-term space missions.

The interest in the role of the gravitational factor during landing after long-term space flights (SF) leads to the search for various innovative approaches to assessing the compliance of external changes observed by clinicians. The results of special research methods such as Omics technologies that may reflect physiological responses to the conditions created during landing are of great interest. Our purpose is to compare the blood plasma proteome changes associated with the trauma and endothelial dysfunction processes prior to launch and on the day of landing, as well as the groups of cosmon

The role of nutrition in space exploration: Implications for sensorimotor, cognition, behavior and the cerebral changes due to the exposure to radiation, altered gravity, and isolation/confinement hazards of spaceflight.

Multi-year crewed space exploration missions are now on the horizon; therefore, it is important that we understand and mitigate the physiological effects of spaceflight. The spaceflight hazards-radiation, isolation, confinement, and altered gravity-have the potential to contribute to neuroinflammation and produce long-term cognitive and behavioral effects-while the fifth hazard, distance from earth, limits capabilities to mitigate these risks. Accumulated evidence suggests that nutrition has an important role in optimizing cognition and reducing the risk of neurodegenerative diseases caused by

Spaceflight associated dry eye syndrome (SADES): Radiation, stressors, and ocular surface health.

Crewed spaceflight missions require careful scrutinization of the health risks including alterations to the tear film lipid layer in astronauts. We review the current literature and prior published work on tear film lipid layer biophysics and secondary spaceflight-associated dry eye syndrome (SADES). We define the term spaceflight-associated dry eye syndrome to describe the collection of ocular surface signs and symptoms experienced by astronauts during spaceflight. Our review covers the ocular surface and lipidomics in the spaceflight environment. From our literature review, we extrapolate bi

Spatial and Temporal Changes in Choroid Morphology Associated With Long-Duration Spaceflight.

PURPOSE: Amid efforts to understand spaceflight associated neuro-ocular syndrome (SANS), uncovering the role of the choroid in its etiology is challenged by the accuracy of image segmentation. The present study extended deep learning-based choroid quantification from optical coherence tomography (OCT) to the characterization of pulsatile and topological changes in the macular plane and investigated changes in response to prolonged microgravity exposure. METHODS: We analyzed OCT macular videos and volumes acquired from astronauts before, during, and after long-duration spaceflight. Deep learni

Can the Biophilia Hypothesis Be Applied to Long-Duration Human Space Flight? A Mini-Review.

The International Space Station (ISS) has around 3-5 crew members on-board at all times, and they normally stay on the ISS for about 5-7months in duration. Since March 2020, 170 long-duration space missions have occurred on the ISS. Thus, long-duration space missions are an integral part of space exploration and will only continue to expand in duration as missions to the Moon and Mars are on the horizon. However, long-duration space missions present several challenges to human crew members. Most of these challenges have been associated with physiological adaptation to microgravity, including m

Changes in Vertebral Bone Density and Paraspinal Muscle Morphology Following Spaceflight and 1 Year Readaptation on Earth.

Astronauts have an increased risk of back pain and disc herniation upon returning to Earth. Thus, it is imperative to understand the effects of spaceflight and readaptation to gravity on the musculoskeletal tissues of the spine. Here we investigated whether ~6 months of spaceflight led to regional differences in bone loss within the vertebral body. Additionally, we evaluated the relationships between vertebral bone density and paraspinal muscle morphology before flight, after flight, and after readaptation on Earth. We measured vertebral trabecular bone mineral density (Tb.BMD), paraspinal mus

Human vagal baroreflex mechanisms in space.

Although astronauts' cardiovascular function is normal while they are in space, many have altered haemodynamic responses to standing after they return to Earth, including inordinate tachycardia, orthostatic hypotension, and uncommonly, syncope. Simulated microgravity impairs vagal baroreceptor-cardiac reflex function and causes orthostatic hypotension. Actual microgravity, however, has been shown to either increase, or not change vagal baroreflex gain. In this study, we tested the null hypothesis that spaceflight does not impair human baroreflex mechanisms. We studied 11 American and two Germa

Development of a new device for manipulating frozen mouse 2-cell embryos on the International Space Station.

Whether mammalian embryos develop normally under microgravity remains to be determined. However, embryos are too small to be handled by inexperienced astronauts who orbit Earth on the International Space Station (ISS). Here we describe the development of a new device that allows astronauts to thaw and culture frozen mouse 2-cell embryos on the ISS without directly contacting the embryos. First, we developed several new devices using a hollow fiber tube that allows thawing embryo without practice and observations of embryonic development. The recovery rate of embryos was over 90%, and its devel

Physiological Alterations in Relation to Space Flight: The Role of Nutrition.

Astronauts exhibit several pathophysiological changes due to a variety of stressors related to the space environment, including microgravity, space radiation, isolation, and confinement. Space motion sickness, bone and muscle mass loss, cardiovascular deconditioning and neuro-ocular syndrome are some of the spaceflight-induced effects on human health. Optimal nutrition is of the utmost importance, and-in combination with other measures, such as physical activity and pharmacological treatment-has a key role in mitigating many of the above conditions, including bone and muscle mass loss. Corneal Edema and the Endothelium in Spaceflight.

INTRODUCTION: With future manned missions that extend beyond low Earth orbit, it would be wise to anticipate all risks to astronaut health, including those relevant to ophthalmology and the ocular surface. Corneal edema has been documented among mice experiments conducted onboard the Space Transportation System mission, STS-133, owing to increased stress response gene expression. METHODS: A targeted, relevant search of the literature on topics relating to ocular surface and spaceflight was conducted with scholarly databases PubMed, Web of Science, and Embase from inception to July 2024.
