Drug-coated balloon angioplasty after directional atherectomy improves outcome in restenotic femoropopliteal arteries.

BACKGROUND: Restenosis remains an unresolved problem despite different treatment modalities and new stent technology in femoropopliteal arteries. No standard therapy has proven to provide acceptable outcome data for this entity. Directional atherectomy alone did not result in satisfactory long-term patency rates. The outcome might be improved in conjunction with drug-coated balloon angioplasty. METHODS: In this retrospective study, restenotic lesions of the femoropopliteal arteries were treated with directed atherectomy in 89 lesions of consecutive patients (58% male; mean age, 69 ± 11 years)

LASER endovascular atherectomy with secondary stenting of technically challenging calcified celiac trunk stenosis.

Endovascular therapy for chronic mesenteric ischemia is a proven, feasible alternative approach to open surgical repair with significantly lower morbidity and mortality. A clinical dilemma is encountered when technically challenging lesions are encountered during the procedure. We present here an 86 year-old woman who had intractable abdominal pain with postprandial exacerbation. The patient had successful endovascular light amplification by stimulated emission of radiation atherectomy of a highly calcified celiac truck, showing the feasibility of this technique.

[Rotational atherectomy in the drug-eluting stent era: a recent single-center experience].

INTRODUCTION: Percutaneous coronary intervention (PCI) of heavily calcified lesions is a challenge for the interventional cardiologist and is associated with a high rate of restenosis and target lesion revascularization (TLR). Adequate lesion preparation by rotational atherectomy followed by drug-eluting stent implantation has shown favorable results. OBJECTIVE: To report the recent experience of our center with rotational atherectomy (RA) of complex and heavily calcified coronary lesions. METHODS: We retrospectively analyzed consecutive patients who underwent PCI with RA in our center betwe

Treatment of lower extremity vascular disease: the Diamondback 360 degrees Orbital Atherectomy System.

Atherosclerotic disease affecting the arteries of the lower extremities becomes symptomatic with the onset of pain either at rest or with ambulation. The disease, commonly referred to as peripheral arterial disease (PAD), affects nearly 30 million individuals in Europe and North America alone. The predominant factor responsible for the impaired functional capacity and quality of life in the presence of lower-extremity PAD is inadequate blood flow due to atherosclerotic plaque build-up within the arteries. Without successful lifestyle risk modifications or judicious pharmacotherapy, individuals

Impact of restenosis after optimal directional coronary atherectomy on regional left ventricular function.

To assess the effect of optimal directional coronary atherectomy (DCA) on restenosis and left ventricular (LV) function, 95 patients who underwent DCA and adjunctive balloon angioplasty for de novo lesions were prospectively followed for 6 months. Absolute and relative coronary lumen measurements were analyzed with online quantitative coronary angiography. LV volumes, ejection fraction, and segmental wall motion were measured off-line according to the radial method for LV cineangiograms acquired in a right anterior oblique projection. Target vessels were the left anterior descending artery in

Restenosis after directional coronary atherectomy. Effects of luminal diameter and deep wall excision.

BACKGROUND: Deep wall excision during directional atherectomy has been reported in one study to increase the risk of subsequent restenosis. On the other hand, we have observed that the probability of late (6-month) restenosis is reduced by maximizing postprocedure luminal diameter. Although such maximal luminal enlargement by directional atherectomy has not increased procedural complications in our experience, it might well increase the incidence of subintimal (deep wall component) recovery. We performed this study to evaluate the relative influences of luminal enlargement and deep wall compon

Directional atherectomy. New approaches for treatment of obstructive coronary and peripheral vascular disease.

Atherectomy is defined as the controlled removal of atherosclerotic tissue from vessel walls. The directional atherectomy catheter consists of a cup-shaped cutter within a housing unit and a small balloon. It was developed to perform transluminal atherectomy for the treatment of atherosclerotic vascular disease. This new procedure was performed on 195 lesions in peripheral arteries of lower limbs in 134 procedures, and 52 lesions in coronary arteries in 50 procedures. A successful angiographic outcome was obtained in 89% of procedures and 90% of lesions in peripheral experience, and 60% of pro

Successful Cutting and Retrieval of an Unraveled Embolization Coil With Rotational Atherectomy: A Case Report.

Coil unraveling is a rare but potentially serious complication during transcatheter coil embolization. Once the braided coil has unraveled and loosened into wire filaments, percutaneous retrieval becomes extremely challenging. Although various percutaneous techniques have been described for retrieving migrated or unraveled coils, surgical removal or leaving the coil in situ may ultimately be required. We describe the first case of successful cutting and retrieval of an unraveled coil during coil embolization for a coronary pulmonary artery fistula (CPAF) using rotational atherectomy (RA). This

Retrieval of stuck burr by pullback orbital atherectomy with RESCUE technique: a case report.

BACKGROUND: Rotational atherectomy (RA) burr entrapment is an uncommon but potentially life-threatening complication. Additional percutaneous bailout is necessary when conventional percutaneous bailout strategies are unsuccessful in patients with prohibitive surgical risk. CASE SUMMARY: A 91-year-old man with a history of coronary artery bypass graft surgery presented with unstable angina caused by severely calcified diffuse stenosis of the right coronary artery. During lesion preparation, an RA burr became entrapped after crossing a heavily calcified segment. Multiple conventional percutaneo

Safety and Efficacy of a Novel Rotational Atherectomy System in Coronary Calcifications: The CORECT Trial.

BACKGROUND: Calcified coronary lesions present challenges during percutaneous coronary intervention, often impeding successful stent delivery and expansion. Rotational atherectomy (RA) facilitates plaque modification in such cases. However, clinical performance of a novel RA system remains unclear. OBJECTIVES: This noninferiority study evaluated the safety and efficacy of the novel system. METHODS: This trial included 2 prospective phases, a first-in-human (FIH) study and a pivotal randomized controlled trial. In the FIH study, 15 patients underwent percutaneous coronary intervention with th

Racial Differences in the Trends, Clinical Characteristics and Outcomes of Patients Undergoing Coronary Atherectomy.

Racial disparities persist in cardiovascular care, but little is known about differences in outcomes following coronary atherectomy by race or ethnicity. This study examines in-hospital outcomes and utilization trends of coronary atherectomy among Non-Hispanic Black (NHB) and Non-Hispanic White (NHW) patients using data from the National Inpatient Sample database (2016-2021). NHB patients were younger and exhibited a higher prevalence of comorbidities such as diabetes, hypertension, and chronic kidney disease compared with NHW patients. Despite this, multivariate analyses revealed that NHB pat

Mechanism of Stent Failure in Patients With Eruptive Calcified Nodule Treated With Rotational Atherectomy.

BACKGROUND: Percutaneous coronary intervention (PCI) for lesions with eruptive calcified nodules (CNs) is associated with worse outcomes compared with that for other calcified lesions. We aimed to clarify the relationship between eruptive CNs at index PCI, optical coherence tomography (OCT) findings at the 8-month follow-up, and clinical outcomes using serial OCT. METHODS: This retrospective observational study used data from a prospective, single-centre registry. We conducted consecutive PCI for calcified lesions requiring rotational atherectomy (RA) with OCT guidance. We categorized 51 pati

Injury of the superficial femoral artery and posterior tibial artery associated with an atherectomy device.

ObjectivesRotational atherectomy can offer a viable treatment for occlusive peripheral artery disease; maintaining the minimal invasiveness of an endovascular procedure, while allowing for a more complete lesion debridement compared with balloon angioplasty. This case report outlines a complication of guidewire entrapment associated with rotational atherectomy in the superficial femoral artery (SFA).MethodsA 57-year-old male underwent an atherectomy with Rotorex for left lower limb foot pain. During the procedure, the guidewire was suctioned into the atherectomy device, preventing any further

Comparison of Coronary Intravascular Lithotripsy and Rotational Atherectomy in the Modification of Severely Calcified Stenoses.

Debulking techniques are often necessary for successful lesion preparation in percutaneous coronary intervention. The aim of this study was to compare plaque modification of severely calcified lesions by coronary intravascular lithotripsy (IVL) with that of rotational atherectomy (RA) using optical coherence tomography (OCT). ROTA.shock was a 1:1 randomized, prospective, double-arm, multicenter noninferiority trial designed to compare final minimal stent area after IVL with RA for lesion preparation in percutaneous coronary interventional treatment of severely calcified lesions. The "Skipping Rope" Technique: A Novel Strategy for Distal Burr Entrapment During Rotational Atherectomy.

We describe the "skipping rope technique" for managing burr entrapment during rotational atherectomy. Sequential counterclockwise rotational force is applied to the drive shaft, generating alternative torque dynamics. This maneuver provides a safe method to disengage a stuck rotaburr, offering operators an additional strategy for this challenging complication.

Incidence of periprocedural myocardial injury and clinical outcomes after rotational atherectomy vs. intravascular lithotripsy.

BACKGROUND: Intravascular lithotripsy (IVL) and rotational atherectomy (RA) are established plaque-modifying strategies for heavily calcified coronary lesions. Despite their different mechanisms, the incidence of significant periprocedural myocardial injury (PMI) and its clinical implications remain inadequately studied. This study compared the incidence of significant PMI and clinical outcomes following PCI with IVL versus RA. METHODS: We retrospectively analyzed 422 lesions (RA: 332; IVL: 90) in 398 patients who underwent PCI for moderately or severely calcified lesions between November 201

Association of Lesion Complexity and Diabetes Mellitus with Target Lesion Failure after Femoropopliteal Atherectomy.

BACKGROUND: Diabetes mellitus (DM) is associated with adverse outcomes after femoropopliteal endovascular interventions, but diabetic patients often present with greater anatomical complexity and impaired distal outflow, potentially confounding its relationship with target lesion failure (TLF). METHODS: We retrospectively reviewed 123 consecutive patients who underwent femoropopliteal atherectomy for symptomatic peripheral artery disease between January 2017 and August 2025. All patients received drug-coated balloon angioplasty after atherectomy, and no stents were implanted. The primary end
