Left Atrial Appendage Occlusion Versus Oral Anticoagulation in Atrial Fibrillation : A Decision Analysis.

BACKGROUND: Left atrial appendage occlusion (LAAO) is a potential alternative to oral anticoagulants in selected patients with atrial fibrillation (AF). Compared with anticoagulants, LAAO decreases major bleeding risk, but there is uncertainty regarding the risk for ischemic stroke compared with anticoagulation. OBJECTIVE: To determine the optimal strategy for stroke prevention conditional on a patient's individual risks for ischemic stroke and bleeding. DESIGN: Decision analysis with a Markov model. DATA SOURCES: Evidence from the published literature informed model inputs. TARGET POPULAT

Left atrial appendage thrombus in patients with atrial fibrillation who underwent oral anticoagulation.

BACKGROUND: Electric cardioversion of atrial fibrillation (AF) is associated with an increased risk of embolism, with embolic material existing in the heart cavities. The initiation of oral anticoagulation therapy reduces the risk of thromboembolic events. The aims of this study were to evaluate the prevalence of left atrial appendage (LAA) thrombi in non-valvular AF, to compare vitamin K antagonists (VKAs) and non-vitamin K oral anticoagulants (NOACs) with respect to thrombus prevalence, and to evaluate the rate of LAA thrombus persistence on repeat transesophageal echocardiography (TEE) afte

Left Atrial Appendage Closure in Patients With Intracranial Hemorrhage and Nonvalvular Atrial Fibrillation.

BACKGROUND: Left atrial appendage (LAA) closure is primarily performed in patients who have contraindications to anticoagulants. However, anticoagulants are recommended for the first few weeks after LAA closure to prevent the device related thrombus. Purpose of this study was to evaluate if short term use of anticoagulant is safe and effective after LAA closure in patients with a history of intracranial hemorrhage. METHODS: It is a retrospective observational study done at a single center. Baseline characteristics, perioperative, and postoperative complications of patients with a history of i

Left atrial appendage occlusion in high-risk patients with non-valvular atrial fibrillation.

OBJECTIVE: Percutaneous left atrial appendage (LAA) occlusion has been developed as a viable option for stroke and thromboembolism prevention in patients with non-valvular atrial fibrillation (NVAF) and at high risk for cerebral cardioembolic events. Data on device implantation and long-term follow-up from large cohorts are limited. METHODS: 110 consecutive patients with NVAF and contraindications to oral anticoagulants (OACs) underwent LAA occlusion procedures and achieved a longer than 1 year follow-up. All patients were enrolled in a prospective registry. Procedures were performed using th

Left atrial appendage closure - new choice for no-option patients with atrial fibrillation.

AIM: Remodeling and impaired blood flow in left atrial appendage (LAA), which occurs in patients with atrial fibrillation (AF), may lead to thrombus formation and possible thromboembolic complications. Although there are several pharmacological antithrombotic possibilities, some patients with several co-morbidities and contraindications to such treatment cannot be offered any of them. Therefore LAA closure systems may be an attractive alternative. We present our early experience with two currently available different LAA transcatheter closure systems (Watchman and Amplatzer Cardiac Plug). MET

Percutaneous left atrial appendage occlusion for stroke prevention in atrial fibrillation: an update.

Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia encountered in clinical practice. One of its most devastating complications is the development of thromboembolism leading to fatal or disabling stroke. Oral anticoagulation (OAC, warfarin) is the standard treatment for stroke prevention in patients with AF with an increased stroke risk. However, there are several obstacles to long-term OAC therapy, including the risk of serious bleeding, several drug-drug interactions and the need for frequent blood testing. Although newer oral anticoagulants have been developed, these dr

Unusual left atrial mass: inverted left atrial appendage caused by massive pericardial effusion.

Left atrial appendage inversion is a very rare cause of left atrial mass that usually occur after cardiac surgery. Its echocardiographic appearance often misleads clinicians to the diagnosis of thrombus or cardiac neoplasms. Unfortunately, misdiagnosed patients often undergo unnecessary anticoagulation or surgical treatment. We are presenting a case of left atrial appendage inversion caused by massive pericardial effusion in a patient with no previous history of cardiac surgery together with demonstrative images of transesophageal echocardiography and magnetic resonance imaging.

Histopathological evaluation of left atrial appendage thrombogenesis removed during surgery for atrial fibrillation.

BACKGROUND: In this study, we histopathologically assessed left atrial appendages (LAAs) resected during surgical treatment for atrial fibrillation (AF) to elucidate the mechanism of intra-LAA thrombus formation in valvular AF. METHODS: The clinicopathological study of resected LAA was made on 56 valvular AF cases: 28 with mitral regurgitation (MR), 3 with mitral stenosis, and 25 with mitral stenosis and MR. Pathological findings of thrombi in LAA were compared with clinical features, including history of valvular diseases and embolism, and findings of echocardiography. Results were analyzed

Effect of inotropic stimulation on left atrial appendage function in atrial myopathy of chronic atrial fibrillation.

Atrial fibrillation (AF) leads to remodeling of the left atrium (LA) and left atrial appendage (LAA), resulting in atrial myopathy. Reduced LA and LAA function in chronic AF leads to thrombus formation and spontaneous echo contrast (SEC). The effect of inotropic stimulation on LAA function in patients with chronic AF is unknown. LAA emptying velocity (LAAEV) and maximal LAA area at baseline and after dobutamine were measured by transesophageal echocardiography in 14 subjects in normal sinus rhythm (NSR) and 6 subjects in AF. SEC in the LA was assessed before and after dobutamine. LAAEV increas

Left Atrial Appendage Clip Reentry: The Case for Ablation From the Appendage to Left Pulmonary Veins.

Convergent hybrid atrial fibrillation ablation combines epicardial posterior left atrial ablation with catheter pulmonary vein isolation. Concomitant left atrial appendage AtriClip placement has recently been included to mitigate stroke risk. We describe a 72-year-old woman with long-standing persistent atrial fibrillation in whom atrial flutter with reentry encircling the AtriClip developed 18 months after a successful convergent procedure.

The ABC of sealing following left atrial appendage closure.

BACKGROUND: Peri-device leak (PDL) following left atrial appendage closure (LAAC) can be diagnosed by both transesophageal echocardiography (TEE) and cardiac computed tomography (CCT). Yet, there is a substantial discrepancy in leak quantification. AIMS: We propose a novel ABC classification of LAA sealing for TEE and CCT based on the pattern of intra-device thrombosis (IDT). METHODS: A single-center observational study including patients undergoing LAAC with Watchman FLX or FLX-Pro between 2022 and 2024. Patients underwent CCT and TEE at 14 days, 45 and 90 days after LAAC. Images were analy

Effect of Left Atrial Appendage Closure on Cardiac Function.

Atrial fibrillation (AF) is the most common clinically significant cardiac arrhythmia with a marked age-related increase in prevalence. Current guidelines recommend left atrial appendage closure (LAAC) as an alternative to long-term oral anticoagulation in selected patients at high-risk of both thromboembolic and bleeding events. Although the use of LAAC has increased substantially in recent years, its effects on cardiac function and post-procedural functional recovery remain incompletely understood. Therefore, this study aimed to evaluate changes in periprocedural cardiac function and functio

Giant Right Atrial Appendage Aneurysm Misdiagnosed as Pericardial Effusion.

• RAAA can mimic pericardial effusion on TTE; multimodality imaging can aid diagnosis. • TEE with contrast can distinguish slow swirling flow from thrombus. • CMR and CCT can reveal RV compression and abnormal diastolic contour. • Conservative management is viable for high-risk patients without thrombus.

Current evidence and indications for left atrial appendage closure.

Atrial fibrillation (AF) is the most common arrhythmia worldwide and its prevalence increases with age. The main and most severe complication of AF is ischemic stroke, yet an estimated 50 % of eligible patients cannot tolerate or are contraindicated to receive oral anticoagulation (OAC). In patients with AF, the left atrial appendage (LAA) is the main source of thrombus formation. Percutaneous LAA closure (LAAC) has emerged over the past two decades as a valuable alternative to OAC for reducing the risk of strokes and systemic embolisms in patients with AF who cannot tolerate long-term OAC. The Role of Anticoagulants After Left Atrial Appendage Occlusion.

BACKGROUND: Left atrial appendage occlusion prevents thromboembolic events in atrial fibrillation patients with bleeding risk, but device-related thrombosis (DRT) remains a challenge. CASE SUMMARY: A 78-year-old man with atrial fibrillation (CHA2DS2-VASc score = 3) and prior intracranial hemorrhage underwent successful left atrial appendage occlusion with a 22-mm Amplatzer Amulet. After stepping down from apixaban to aspirin, DRT developed. Thrombi resolved with apixaban but recurred on its discontinuation. ST-Segment Elevation During Percutaneous Left Atrial Appendage Closure.

A 70-year-old patient with paroxysmal atrial fibrillation underwent left atrial appendage closure. The patient experienced transient hypotension during device implantation. The procedure was abandoned because of ST-T-wave changes on electrocardiography and elevated coronary flow velocity on transesophageal echocardiography, which indicated that the device caused coronary artery compression.
