Improving ototoxicity monitoring in patients receiving aminoglycosides using a novel digital approach: a quality improvement project.

Aminoglycoside antibiotics cause ototoxicity for which baseline audiometric testing is recommended but often not done. Barriers to successful implementation include limited availability of sound-booths and audiologists. An ototoxicity monitoring programme (OMP) was implemented using tablet-based audiometry (TBA) by non-audiologists.A quality improvement project conducted over 1 year (19 April 2021 to 18 April 2022), using Plan Do Study Act (PDSA) cycles, monitored the adherence to the OMP using Shoebox Standard Edition application on iPads. Barriers to adoption were identified to determine pot

Clinician Awareness of Stickler Syndromes Among Australian Allied Health Care Professionals.

PURPOSE: Stickler Syndromes are multisystem collagenopathies affecting 1 in 7500-9000 individuals and are associated with craniofacial, ocular, auditory, and musculoskeletal complications. Prophylactic retinopexy treatment reduces the risk of retinal detachment, emphasising the need for early detection and multidisciplinary referral. This study evaluated knowledge and awareness of Stickler Syndromes among allied health professionals and their perceived needs for targeted education to improve multidisciplinary care. METHODS: A cross-sectional survey was undertaken among audiologists, speech pa

Serial auditory-evoked potentials in the diagnosis and monitoring of a child with Landau-Kleffner syndrome.

BACKGROUND: A boy, aged 2 1/2 yr, experienced sudden deterioration of speech and language abilities. He saw multiple medical professionals across 2 yr. By almost 5 yr, his vocabulary diminished from 50 words to 4, and he was referred to our speech and hearing center. PURPOSE: The purpose of this study was to heighten awareness of Landau-Kleffner syndrome (LKS) and emphasize the importance of an objective test battery that includes serial auditory-evoked potentials (AEPs) to audiologists who often are on the front lines of diagnosis and treatment delivery when faced with a child experiencing u

Effectiveness and Safety of Advanced Audiology-Led Triage in Pediatric Otolaryngology Services.

OBJECTIVES: Expansion of the scopes of practice of allied health practitioners has the potential to improve the efficiency and cost-effectiveness of healthcare, given the identified shortages in medical personnel. Despite numerous examples in other allied health disciplines, this has yet to be applied to pediatric Audiology. This study aimed to investigate the effectiveness and safety of using audiologists with advanced training to independently triage children referred to otolaryngology (ORL) services, and compare the subsequent use of specialist resources, and postoperative grommet care to a

Objective Acoustic Quantification of Perceived Voice Tremor Severity.

Purpose This study compared auditory-perceptual measures of presence/absence and severity of vocal tremor to acoustic markers of vocal tremor. The validity (both concurrent and diagnostic) of various acoustic markers of vocal tremor was also assessed. Method Fifty-six midvowel sustained [a:] recordings were selected to yield a representative convenience sample of vocal tremor. After training with 10 synthesized samples, 4 female audiologists rated these samples on "voice tremor severity" on a continuous 10-cm scale. Afterward, 15 randomly selected recordings were presented a 2nd time for intra

Presbycusis and communication.

Impaired hearing, so common among older people, often has disastrous consequences. The infirmity, too frequently concealed or unknown by the patient, is invisible to others. Disability and frustration can be lessened considerably by greater awareness on the part of medical and lay people about various helpful measures, which are currently under-utilized. We offer some practical suggestions: Well-trained audiologists can help with both hearing aids and rehabilitation; groups for the hard of hearing can be supportive. The number and varieties of helpful devices increase steadily. Medical personn

Hearing loss and treatment in young Down's syndrome children.

Twenty-four home-reared Down's syndrome children aged 9-32 months were assessed by trained audiologists on three separate occasions (summer, autumn and spring 1977-1978). Distraction test procedures were used; all failed to pass the screening test and 80-85% registered as having moderate to profound losses. Seventy-five per cent were categorized as having a conductive loss and some improvements were found over the test sessions as a result of surgical and decongestant treatment. Approximately 50% of the infants had been passed on similar screening procedures by the local health services. The r

Management of neurofibromatosis type 2.

Neurofibromatosis type 2 (NF2), an as-yet incurable disease that predisposes patients to multiple intracranial and spinal tumors, requires a team approach to treatment, because of its multisystem nature. Included on the team should be neuro-otologists, neurosurgeons, ophthalmologists, geneticists, audiologists, speech therapists and other rehabilitative personnel, including counselors, psychologists and, occasionally, psychiatrists. The challenge is to arrive at a treatment strategy that preserves useful hearing and quality of life without increasing the risk of complications to the facial ner

The bone-anchored hearing aid compared with conventional hearing aids. Audiologic results and the patients' opinions.

The bone-anchored hearing aid (BAHA) was evaluated in 65 patients by comparing speech recognition and questionnaire results with those obtained with the patients' own previous hearing aids. In the majority of patients who previously used a conventional bone conductor, significantly improved speech recognition scores were found with the BAHA. The majority preferred the BAHA to their previous bone conductors. For the patients who previously used air conduction hearing aids but had to stop because of chronic draining ears, the results were ambiguous. Nevertheless, the BAHA seems to be the best so

Contralateral routing of signals in unilateral hearing impairment. A better method of fitting.

Unilaterally severely hearing impaired persons, who may experience substantial communication problems in situations of high acoustic demands, would in theory benefit from contralateral routing of signals (CROS). However, reports of failure because of too much disturbing noise and distortion have been common. The reason for a low success rate in CROS, though, is an inappropriate fitting procedure, due to lack of understanding of the basic principle of CROS among both patients and audiologists. The precondition for success in CROS is an accurate description of the individual hearing deficit and

Brainstem auditory evoked responses: Objective hearing threshold assessment in Holstein cows.

BACKGROUND: Animal audiology utilizes brainstem auditory evoked responses (BAER) as a non-invasive tool to assess hearing in animals, including Holstein dairy cows. Understanding cows' auditory capabilities is critical for their welfare, especially given their exposure to farm noise. OBJECTIVES: This study provides preliminary normative BAER data for Holstein cows by focusing on absolute and interpeak wave latencies. The objective is to assess the impact of farm noise and expand audiologists' practice scope. METHOD: Ten Holstein cows were tested using monoaural broadband click stimuli with c

Diagnostic accuracy of non-specialist versus specialist health workers in diagnosing hearing loss and ear disease in Malawi.

OBJECTIVE: To determine whether a non-specialist health worker can accurately undertake audiometry and otoscopy, the essential clinical examinations in a survey of hearing loss, instead of a highly skilled specialist (i.e. ENT or audiologist). METHODS: A clinic-based diagnostic accuracy study was conducted in Malawi. Consecutively sampled participants ≥ 18 years had their hearing tested using a validated tablet-based audiometer (hearTest) by an audiologist (gold standard), an audiology officer, a nurse and a community health worker (CHW). Otoscopy for diagnosis of ear pathologies was conducte

Can emergency physicians accurately and reliably assess acute vertigo in the emergency department?

OBJECTIVE: To validate a clinical diagnostic tool, used by emergency physicians (EPs), to diagnose the central cause of patients presenting with vertigo, and to determine interrater reliability of this tool. METHODS: A convenience sample of adult patients presenting to a single academic ED with isolated vertigo (i.e. vertigo without other neurological deficits) was prospectively evaluated with STANDING (SponTAneousNystagmus, Direction, head Impulse test, standiNG) by five trained EPs. The first step focused on the presence of spontaneous nystagmus, the second on the direction of nystagmus, th

Delayed endolymphatic hydrops: a case study.

Delayed endolymphatic hydrops (DEH) is an unusual variation of Ménière's disease characterized by episodic vertigo that develops some time after the onset of a profound, typically unilateral sensorineural hearing loss. This case study describes a 48-year-old male who presented with complaints of episodic vertigo and disequilibrium 15 years following the onset of unilateral sensorineural hearing loss. The patient's history, audiologic findings, and vestibular evaluation led to the diagnosis of DEH. The case highlights the diagnostic and treatment challenges associated with this condition and fo

Comparison of small fenestra and total stapedectomy.

A biostatistical study of the results of 280 consecutive operations for clinical otospongiosis are analyzed over a seven-year period. One hundred forty-one (141) operations were done utilizing the "total stapedectomy" technique, and 139 were done utilizing the "small fenestra" technique. The auditory acuity of the two groups is compared with reference to conductive and sensorineural performance. All the surgery was performed by the authors, the data was collected by a third party and analyzed by a statistician. All audiograms were performed by the same audiologist under the same conditions. A Survey of Current Audiology Practice in Adult Cochlear Implant Programs and Its Impact on Access to Care.

OBJECTIVES: Recent expansion of cochlear implant (CI) indications by Medicare to include patients with better preoperative speech recognition and by the United States Food and Drug Administration to include patients with single-sided deafness and asymmetric hearing loss has increased the number of patients who are candidates for a CI. This expansion has the potential to raise the demand for audiological services related to CI, including preoperative determination of CI candidacy and postoperative programming and evaluation of performance. In this study, we evaluated ways that audiological care

Facial nerve stimulation after successful multichannel cochlear implantation.

Postoperative rehabilitation after cochlear implantation may be difficult and frustrating for both patient and audiologist. The occurrence of facial nerve stimulation with associated twitching and discomfort can interfere with the rehabilitative process and be a cause of great concern to the patient. Such stimulation can occur at the time of initial "tune-up" or can be delayed by months or years. The incidence of postoperative facial nerve stimulation 109 consecutive patients operated by the same surgeon over a period of 7 years is reviewed. Possible causes for immediate and delayed stimulatio

Syllabic stress (trochaic vs. spondaic) and speech thresholds in normal and hearing-impaired children.

Speech thresholds for 6 common two-syllable words on tape with either trochaic (natural) or spondaic stress were collected from children aged 3.5-6.5 yrs, audiometrically grouped as normal-hearing (N:8), conductive (N:8) or sensorineural (N:6). Ss were further grouped as younger and older. No significant differences in mean threshold, nor in time to reach threshold, were observed for either pattern of stress or age, suggesting that audiologists may interchange spondaic with trochaic stress when measuring speech thresholds in young children, with no effect on the mean value or the efficiency of
