Important emerging bacterial zoonotic infections affecting the immunocompromised.

The immunocompromised are at particular risk for infection with zoonotic diseases. Persons can be temporarily immunocompromised due to pregnancy or developmental stage (i.e. infants); longer-term or permanent states of immunosuppression can occur as a result of immunosuppressive treatment following cancer or organ transplant, or from infectious diseases, such as AIDS. The focus of this review article is on emerging bacterial zoonotic diseases that are of particular concern among the immunocompromised. Factors that affect disease emergence can include factors such as human demographics and beha

Severe bacterial infections of the skin: uncommon presentations.

Although most bacterial infections of the skin prove to be minor in nature, a few such dermatologic entities are significant, to the point of even being fatal. Their course can be extremely rapid and can lead to dreadful complications. The mortality rate is usually up to 30% to 50% and depends upon the type of infection, underlying disease, and immune status. Patients suffering them usually need to be hospitalized, sometimes in intensive care or burn units. They should be treated systemically with appropriate antimicrobial therapy plus aggressive supportive care. The two life-threatening skin

[Endocranial bacterial infections originating in the otorhinolaryngologic area].

The study reviewed 60 patients with various endocranial infections complicated by previous ORL infections. The mean age of patients was 41 years. Significantly more patients were males (70%). The majority of patients (55%) had previous chronic otitis media, 25% had tonsillopharyngitis and 20% had sinusitis. The majority of patients (68%) presented with meningitis. In addition to meningitis, 13% of the affected patients manifested signs of cerebritis. Cerebritis was found in 13% of patients. Three simultaneous complications were seen in two patients: meningitis, cerebritis and subdural empyema.

Sexual and enteric bacterial infections eliciting reactive arthritis.

In this review synonymous definitions of reactive arthritis are discussed first. Major clinical symptoms, their infectious etiology and epidemiology define post-dysenteric and post-veneral forms of reactive arthritis. Classical (smear, culture, biochemistry, antigen detection) and molecular (DNA and RNA detections) techniques are used in the routine microbial diagnosis that is retrospective in the majority of cases. In the pathomechanism of this disorder, HLA-B27 antigen positivity of patients is a frequent risk factor. Molecular mimicry between microbial and self-antigens, abnormal antigen pr

Skin microflora and bacterial infections of the skin.

The skin is a milieu for controlled bacterial growth. Skin supports the growth of commensal bacteria, which protect the host from pathogenic bacteria. Environmental and local factors, host immunity, and organism adherence and virulence are intricately related to cutaneous infection. Resident gram-positive bacteria include Staphylococcus, Micrococcus, and Corynebacterium sp. Staphylococcus aureus and Streptococcus pyogenes are notoriously pathogenic in the skin. In order for bacteria to be pathogenic, they must be able to adhere to, grow on, and invade the host. Bacteria possess numerous virule

Bacterial infections of the small intestine and colon.

Bacterial infections of the small and large intestine are widespread and continue to be topics of active research. Surveys document the importance of diarrheal disease in many settings. Major breakthroughs in the understanding of pathogenic mechanisms (especially the interactions of bacteria and intestinal cells) continue, particularly with respect to shigella, salmonella, Yersinia species, and enteropathogenic Escherichia coli. Pathogenic mechanisms of other bacteria, such as campylobacter and entero-aggregative E. coli, are not well defined. Vaccines for cholera and typhoid fever are availab

Actinomycosis and other bronchopulmonary infections with bacterial granules.

Pulmonary infections with formation of bacterial granules are rare. We reviewed the clinical and pathologic data from 18 cases diagnosed using surgical specimens in our department during the last 10 years. Three clinicopathologic forms were observed: endobronchial infections complicating tuberculous sequelae or bronchiectases (n = 7), tumor-like lesions (n = 8), and diffuse pneumonia (n = 3). The two latter forms contrasted with the former by a male predominance, association with general debilitating conditions and inflammatory syndrome, and pathologically by smaller granules often located in

Pathogenesis of bacterial infections of the respiratory tract.

The main function of the lung is gas exchange, which means that the respiratory mucosa is constantly exposed to noxious substances and potentially infective agents inhaled from the environment. Despite this, in health the number of bacteria in the upper respiratory tract (nose and pernasal tissue) is low and the lower (bronchopulmonary) respiratory tract is sterile. However, respiratory tract infections are common in humans, either due to malfunction of a complex array of defence mechanisms or because of the virulence of a particular microorganism. The pathogenesis of these infections is discu

[The etiology of bacterial respiratory infections in adulthood].

Upper and lower respiratory tract infections are the most frequently reported pathologies both in children and in adults. In particular, the most common clinical pictures are otitis media, principally chronic, and acute and chronic sinusitis. The microbiology of otitis media, which has been investigated thoroughly in the last year, confirmed a bacterial origin in 90% of the cases (while the incidence of the viral form is low). The etiologic agents responsible for chronic infections are Gram-negative bacteria, staphylococci and anaerobic bacteria, while in the acute forms, which are rare in adu

[The efficacy of enterosorption in bacterial intestinal infections].

The article studies clinic manifestations after the prescription of polyphepanum, SKN-1P (based on recovered carbon) and enterodesum in 35 patients with acute dysentery, 104 patients with acute gastroenterocolitis, and 123 patients with typhoid. The first two drugs turned out to be highly effective for treatment of these infections. Enterosorption produced an insignificant positive effect in cases of typhoid. An early prescription of enterosorbents in enhanced doses could rapidly stop the basic proliferation of acute intestinal diseases. It is reasonable to start enterosorption even at the pre

Bacterial infections in human immunodeficiency virus-infected children.

A retrospective review of 71 children infected with human immunodeficiency virus cared for over a 3.5-year period revealed that 44 of 71 (63%) required a bacterial culture and 27 of 71 (37%) had bacteriologically documented infection. There were 125 episodes in 27 patients. Pneumonia (24 of 125 (19%)), upper respiratory tract syndromes (23 of 125 (19%)), urinary tract infection (24 of 125 (19%)) and wound infection (12 of 125 (10%)) were the most common syndromes identified. Bacteremic infections occurred in 35 of 125 (28%), and in 17 of 125 (14%) no other primary source could be identified. P

[The effectiveness of ciprofloxacin in bacterial skin infections].

Ciprofloxacin in a low dosage of 250 mg twice a day was applied in the treatment of bacterial infections of the skin. All cultured staphylococci, streptococci, and pseudomonas proved to be highly sensitive to ciprofloxacin. The cases of pyoderma showed quick healing, but there were relapses of Lyme borrelia within 6 weeks. As the titers of IgM and IgG steadily increased during the following months, additional therapy was necessary. The tolerance of ciprofloxacin was generally very good.

Ascaris lumbricoides as a vehicle of bacterial infections.

Investigations were conducted on microflora on the body surface and internal organs of adult Ascaris lumbricoides. The investigations involved examination of wet faecal preparations of over 100 pupils using light microscope. The body surface of the adult Ascaris worms was cultured on selective media. Adult worms were dissected and different parts of the worms' gut were cultured for isolation of micro-organisms. The results of the bacteriological examination of the body surface of A. lumbricoides yielded many genera of bacterial organisms. The results from internal organs of dissected adult Asc

Bacterial and candidal cutaneous infections in the neonate.

The skin and oral cavity of the neonate are colonized by a variety of organisms during the first few days of life. Some of these organisms constitute normal flora, but others are true pathogens or are capable of pathogenicity when host or environmental factors are altered in their favor. Cutaneous bacterial and yeast infections that afflict the newborn are discussed in terms of clinical presentation, laboratory evaluation, and appropriate management.

Oral vaccination against enteric bacterial infections: an overview.

The present situation and the future prospects for the use of oral vaccines against the major enteric diseases typhoid fever, shigellosis and cholera are discussed in this paper. No significant protection could be demonstrated for oral inactivated whole-cell vaccines. In contrast, an oral live vaccine based on the attenuated Salmonella typhi strain Ty 21a was highly efficacious in volunteer challenge studies and in a controlled field trial. Two attenuated strains are presently being tested in volunteer studies as candidate vaccines against shigellosis; one uses S. typhi Ty 21a and the other Es

Bacterial infections of the CNS in neutropenic patients.

In neutropenic patients, fever and mental status changes are frequently the only overt clinical manifestations of bacterial infections of the CNS. Prominent headache and meningeal signs are exceptional. CNS infections may occur even in patients receiving large doses of broad-spectrum antibiotics. CSF culture and Gram's stain are required to establish or exclude the diagnosis and are often positive, even in patients receiving antibiotics for other indications. The CSF cell counts and chemistries are helpful if abnormal, but, when normal, provide no assurance that infection is not present. The C
