Postpyloric Balloon Occlusion to Increase Technical Success during Pediatric Percutaneous Gastrostomy/Gastrojejunostomy Tube Placement.

Gastric distension through insufflation is a key step in creating a safe percutaneous window during gastrostomy/gastrojejunostomy (G/GJ) placement; however, poor or incomplete gastric distention can occur, despite the use of glucagon, and lead to rapid egress of air from the stomach into the duodenum. This report describes the adjunctive technique using postpyloric balloon occlusion in 29 patients to maximize gastric insufflation during G/GJ tube placement after failure of conventional methods. Balloon occlusion was successful in salvaging 23 of 29 (79.3%) of G/GJ tube placements without any c

Resuscitative endovascular balloon occlusion of the aorta for thoracic trauma: A translational swine study.

UNLABELLED: Noncompressible torso hemorrhage in trauma is particularly lethal. Resuscitative endovascular balloon occlusion of the aorta (REBOA) has the potential to stabilize these patients, but currently is contraindicated for major thoracic bleeding. The goal of this study was to evaluate the effect of REBOA on the hemodynamic and metabolic profile as well as its effect on early survival in a porcine model of thoracic hemorrhage and shock. METHODS: Forty-eight male Yorkshire swine (60-80 kg) underwent 30% hemorrhage and were randomized to three thoracic injuries, with and without zone 1 RE

Resuscitative endovascular balloon occlusion of the aorta (REBOA) during cardiopulmonary resuscitation: A pilot study.

AIM: Resuscitative endovascular balloon occlusion of the aorta (REBOA) during cardiopulmonary resuscitation (CPR) increases coronary and cerebral perfusion pressure, which might improve neurologically intact survival after refractory cardiac arrest. We investigated the feasibility of REBOA during CPR in the emergency department. METHODS: Patients in refractory cardiac arrest not qualifying for extracorporeal CPR were included in this pilot study. An introducer sheath was placed by ultrasound-guided puncture of the femoral artery, and a REBOA catheter was advanced to the thoracic aorta in 15 p

Vascular balloon occlusion and planned caesarean hysterectomy for morbidly adherent placenta: A systematic review.

BACKGROUND: Morbidly adherent placenta is potentially life-threatening, often requiring technically difficult surgery and large blood loss. Use of intravascular balloon occlusion with or without hysterectomy to reduce blood loss is increasing despite associated morbidity and lack of evidence of efficacy. AIMS: To evaluate if prophylactic use of vascular balloon occlusion at the time of planned caesarean hysterectomy for antenatally diagnosed morbidly adherent placenta reduces blood loss and transfusion requirements, and determine rate of associated complications. MATERIALS AND METHODS: A sys

Endovascular recanalisation with drug coated balloon for chronic symptomatic middle cerebral artery total occlusion.

The optimal treatment for patients with chronic symptomatic middle cerebral artery (MCA) total occlusion is not well established. In addition to medical therapy, vessel recanalisation with stenting has shown much promise, especially for patients with recurrent ischemic symptoms. Nevertheless, the incidence of symptomatic in-stent restenosis (ISR) is high, and is associated with an unfavorable prognosis. Drug coated balloons (DCBs) have been proven to be effective in treating and preventing ISR. However, the feasibility of DCBs for de novo intracranial atherothrombotic stenosis has not been pre

External aortic cross-clamping and endoaortic balloon occlusion in minimally invasive mitral valve surgery.

BACKGROUND: Minimally invasive cardiac surgery has increasingly been used for patients with valvular pathology. Two techniques of aortic occlusion are utilized with this technique: transthoracic aortic clamp (TTC) and endoaortic balloon occlusion (EAO). Both possibilities present peculiar advantages and limitations whose current evidence is based on few observational studies. We performed an analysis with the primary objective to evaluate outcomes and the incidence of major complications of these two techniques. METHODS: The data of 258 patients who underwent minimally invasive mitral valve s

Stent collapse caused by balloon occlusion and aspiration system in saphenous vein graft intervention.

We present the case of an 82-year-old male patient with unstable angina pectoris referred for percutaneous coronary intervention in the saphenous vein graft (SVG) to left circumflex coronary artery. An everolimus-eluting cobalt chrome stent was successfully deployed, using an embolic protection device with balloon occlusion and aspiration system. During the procedure of aspiration thrombectomy, the deployed stent unexpectedly collapsed. We re-dilated the collapsed stent using the same distal protection and aspiration system. However, the stent collapsed again during the aspiration procedure. A

Intra-aortic balloon occlusion catheter for treating hemorrhagic shock after massive duodenal ulcer bleeding.

Clamping the descending aorta by emergency thoracotomy is a well-known effective procedure to stop bleeding from lesions under the diaphragm. We successfully treated a case of cardiopulmonary arrest resulting from a massive duodenal ulcer hemorrhage using an intraaortic balloon occlusion (IABO) catheter instead of the conventional technique. Our experience suggests that IABO catheters can be used to treat patients with hemorrhagic shock regardless of the presence of cardiopulmonary arrest. This can be a life-saving procedure, which prevents ischemic brain injury. This article describes the adv

[Perioperative management of life-threatening intra-abdominal bleeding with intra-aortic balloon occlusion catheter].

Intra-thoracic aortic clamping using an intra-aortic balloon occlusion catheter (IABOC) is employed for patients with life-threatening intra-abdominal and/or extra-abdominal bleeding in spite of massive transfusion. For perioperative management, we inserted an IABOC preoperatively into a 59-year-old man with life-threatening intra-abdominal bleeding from an abscess formed around his traumatically injured pancreas. We could perform a safe operation in which bleeding was controlled by intermittently occluding the IABOC and the patient was thus prevented from developing into severe hemorrhagic sh

"Interventional angiography." A possible role in intraoperative balloon occlusion catheterization of lacerated pelvic arteries.

The use of interventional angiography has been limited in the past to the angiography suite. Significant technical difficulties have been associated with transfer of the unstable patient from the operating room for this procedure. However, current radiographic sophistication now exists to allow rapid intraoperative digital subtraction angiography of uncompromised quality to be performed. An animal model using these techniques is presented.

Coaxial Method of PTA Balloon and Aspiration catheter using the delivery wire of a stent retriever for tandem occlusion: CoMBAt tandem occlusion.

PurposeTandem occlusion, in which the intracranial artery is obstructed along with proximal carotid occlusive or stenotic lesions, occurs in 10-15% of patients with large vessel occlusion. Our study introduces a novel technique for tandem occlusion treatment that involves the simultaneous coaxial deployment of a percutaneous transluminal angioplasty balloon and aspiration catheters with the delivery wire of a stent retriever. After percutaneous transluminal angioplasty of the carotid lesion with a thin balloon catheter, the aspiration catheter was advanced over the balloon to eliminate the led

Total resuscitative endovascular balloon occlusion of the aorta causes inflammatory activation and organ damage within 30 minutes of occlusion in normovolemic pigs.

BACKGROUND: Resuscitative endovascular balloon occlusion of the aorta (REBOA) causes physiological, metabolic, end-organ and inflammatory changes that need to be addressed for better management of severely injured patients. The aim of this study was to investigate occlusion time-dependent metabolic, end-organ and inflammatory effects of total REBOA in Zone I in a normovolemic animal model. METHODS: Twenty-four pigs (25-35 kg) were randomized to total occlusion REBOA in Zone I for either 15, 30, 60 min (REBOA15, REBOA30, and REBOA60, respectively) or to a control group, followed by 3-h reperfu

Prophylactic resuscitative endovascular balloon occlusion in the management of placenta percreta: a case report.

INTRODUCTION: Few reports exist on the prophylactic use of a resuscitative endovascular balloon occlusion of the aorta (REBOA) catheter and its application for prolonged occlusion times exceeding 60 minutes during cesarean hysterectomy in placenta accreta spectrum disorders. CASE: In the case of suspected placenta percreta, the prophylactic placement of a REBOA catheter before cesarean hysterectomy provided prompt life-saving post-arrest resuscitation and 166 minutes of occlusion time without any REBOA-related complications. CONCLUSION: Prophylactic REBOA catheter placement is a groundbreaki

Internal Jugular Vein Avulsion Controlled With Intraoperative Foley Balloon Occlusion Following Blunt Neck Trauma.

Internal jugular vein injuries caused by blunt trauma are rare. Here, we describe the case of a severe internal jugular vein injury high in neck zone II and immediately lateral to the hypoglossal nerve, making surgical management extremely challenging. A Foley balloon inserted into the cranial end of the internal jugular vein allowed for effective hemorrhage control while the vein could be suture ligated. Balloon tamponade was helpful in a field with distorted and blood-stained anatomy. It allowed critical time to carefully dissect around the hypoglossal nerve and avoid the sequalae of its inj

Balloon protection of the mesenteric-renal vessels during embolectomy of acute paravisceral aortic occlusion.

Acute thrombosis of the aorta is an uncommon, but potentially devastating, vascular emergency. The absence of pathognomonic signs or symptoms makes the diagnosis challenging and, often, time consuming. No optimal treatment has been established, with greater challenge present when the thrombus involves the mesenteric-renal vessels. In this case report, we present an innovative, hybrid treatment of a nearly occluded paravisceral aorta with endovascular aortic embolectomy through a surgically accessed right femoral artery. We protected the superior mesenteric, renal, and left common iliac arterie
