Unusual EEG patterns in coma, and their evolution.

A 23-year-old chronic asthmatic patient is reported who showed successively four different EEG patterns within a 7 day period of unconsciousness following cardio-respiratory arrest. The initial record was dominated by alpha activity, then beta activity supervened, to be followed by spindle components. Finally an isoelectric tracing occurred before death. A computer file search for all cases of unusual EEG coma patterns was carried out. It failed to reveal any other patient with a similar sequence, but various types of evolution were noted. Such features are seen in only a very small percentage

Clinical Outcomes of Early Rhythm or Rate Control for New Onset Atrial Fibrillation Following Transcatheter Aortic Valve Replacement.

BACKGROUND: New onset atrial fibrillation (NOAF) is a common occurrence after transcatheter aortic valve replacement (TAVR) and portends a poorer prognosis. The optimal strategy for managing NOAF in this population is uncertain. METHODS: This retrospective cohort study utilized deidentified patient data from the TriNetX Research Network. Patients with TAVR and NOAF were stratified into a rhythm control cohort if they were treated with antiarrhythmics, received AF ablation, or underwent cardioversion within 1 year of AF diagnosis. A rate control cohort was similarly defined by the absence of r

[Inversion of the circadian melatonin rhythm in Smith-Magenis syndrome].

Smith-Magenis syndrome (SMS) is a genetic disease ascribed to an interstitial deletion on chromosome 17 (del 17p11); the prevalence is 1/25,000 births. The diagnosis is made on high-resolution karyotype confirmed by FISH. Clinical features include mild dysmorphism, short stature, other malformations (heart, renal, neurologic diseases). Mental retardation is constant; there are major behavioral disturbances and severe sleep disorders. We studied sleep disorders and melatonin secretion in SMS children and we have shown inversion of the circadian rhythm of melatonin, abnormally secreted during th

[Quantitative analysis of EEG basic rhythms in epileptics (author's transl)].

Basic rhythms of EEG were quantitatively analyzed in 92 adult idiopathic epileptic patients and in 57 age-matched control subjects. Epileptic patients have been administered one or both of anticonvulsants, phenobarbital (PB) and diphenyl-hydantoin (DPH). Before the EEG study, blood samples were taken from patients and serum concentration of PB and DPH was examined with UV method. Dose and serum level correlation was statistically significant in both drugs but fairly inferior in DPH. Dominant posterior rhythms in awake subjects were restored in a medical computer. These data were transformed wi

[Anti-arrhythmia therapy of acute heart rhythm disorders in the early period after heart valve prosthesis].

The paper reports the results of a comparative study of the efficiency of antiarrhythmic drugs with different mechanisms of action and quickening electric cardiostimulation (ECS) in 640 patients with acute heart rhythm and conductivity disorders that had developed at early dates after the introduction of valvular prostheses. Beta-adrenoblockers and isoptin were shown to be effective in the control of supraventricular tachysystolic rhythm disorders, membrane-stabilizing agents are indicated in ventricular extrasystole, and ECS combined with trimecaine, in bradyarrhythmias. A study of platelet activation in paroxysmal, persistent and permanent atrial fibrillation.

We hypothesized that the 'residual' thromboembolic risk in therapeutically anticoagulated patients undergoing cardioversion could potentially be related to abnormal haemorheology and platelet activation. To test this hypothesis, we firstly investigated the role of haemorheology and platelet activation in patients with paroxysmal and persistent atrial fibrillation (AF), who were compared with healthy controls and patients with permanent AF. Second, we compared these indices in patients with persistent AF, before and after successful cardioversion. We measured indices of haemorheology (haematocr

Positive Shyness in the Brain: Frontal Electroencephalogram Alpha Asymmetry and Delta-Beta Correlation in Children.

Positive shyness is thought to be an approach-dominant form of shyness, whereas non-positive shyness is thought to be an avoidance-dominant form of shyness. This study examined electrocortical and behavioral correlates of motivation and emotion in relation to these shy subtypes in 67 children (Mage  = 10.41 years, SD = 3.23). Using resting state electroencephalography, findings revealed that positive shy and low shy children had greater relative left frontal alpha asymmetry compared to non-positive shy children, and positive shy children had a higher frontal delta-beta correlation compared to

Avoiding the pitfalls of EEG interpretation in childhood epilepsy.

The accurate interpretation of the electroencephalogram (EEG) of infants and children being evaluated for suspected epilepsy is based on the appreciation of normal and expected age-dependent characteristics, an awareness of the significance of both epileptiform and non-epileptiform activity, and the correlation of epileptiform abnormalities with clinical findings. Avoiding the pitfalls of pediatric EEG interpretation include the recognition of such normal EEG features in wakefulness as posterior slow waves of youth, mu rhythm, and lambda waves. In addition, the understanding of age-dependent c

A circadian rhythm-restricted diet regulates autophagy to improve cognitive function and prolong lifespan.

Diet and circadian rhythms have been found to have a profound impact on health, disease, and aging. Skipping breakfast, eating late, and overeating have adverse effects on the body's metabolism and increase the risk of cardiovascular and metabolic diseases. Disturbance of circadian rhythms has been associated with increased risk of atherosclerosis, Alzheimer's disease, Parkinson's disease, and other diseases. Abnormal deposition of amyloid β (Aβ) and tau proteins in the brain and impaired synaptic function are linked to cognitive dysfunction. A restrictive diet following the circadian rhythm c

The circadian rhythm of angina pectoris.

Analysis of diary card data from 33,999 angina attacks recorded in 1,022 patients with chronic stable angina showed a clear circadian pattern to the frequency of angina that resembles patterns previously described for myocardial infarction and sudden cardiac death, with a peak in the morning. The majority of angina attacks were preceded by physical exertion and the circadian pattern was more pronounced for exertional angina than nonexertional angina. In this study, treatment with beta-blockers did not prevent the morning peak of angina attacks.

Exploring the nexus: Sleep disorders, circadian dysregulation, and Alzheimer's disease.

We reviewed the connections among Alzheimer's disease (AD), sleep deprivation, and circadian rhythm disorders. Evidence is mounting that disrupted sleep and abnormal circadian rhythms are not merely symptoms of AD, but are also involved in accelerating the disease. Amyloid-beta (Aβ) accumulates, a feature of AD, and worsens with sleep deprivation because glymphatic withdrawal is required to clear toxic proteins from the brain. In addition, disturbances in circadian rhythm can contribute to the induction of neuroinflammation and oxidative stress, thereby accelerating neurodegenerative processes

[Triad TMS of Human Motor Cortex].

To study cortical excitability changes induced by external stimulation with a certain rhythm, we developed a new method using motor evoked potentials (MEPs) elicited by transcranial magnetic stimulation (TMS) over the primary motor cortex. In this method, three conditioning TMS with the intensity below the motor threshold are given prior to the supra-threshold test stimulus with the four TMSs were separated by a certain interval (triad-conditioning stimulation: TCS). In healthy volunteers, MEP facilitation was elicited at an interval of 25ms, whereas TCSs with other intervals induced no facili

Cochrane Corner: Perioperative beta-blockers for preventing surgery-related mortality and morbidity.

Randomized controlled trials have yielded conflicting results regarding the impact of beta-blockers on perioperative cardiovascular morbidity and mortality. This Cochrane systematic review assessed the impact of this intervention on mortality and cardiovascular events. Eighty-eight randomized controlled trials with 19 161 participants were included (53 trials on cardiac surgery and 35 trials on non-cardiac surgery). In cardiac surgery perioperative beta-blockers had a protective effect against supraventricular and ventricular arrhythmias but had no significant effect on mortality or on the occ

Beta-blockers or Digoxin for Atrial Fibrillation and Heart Failure?

In patients with atrial fibrillation (AF) and heart failure (HF) with or without systolic dysfunction, either rhythm control or rate control is an acceptable primary therapeutic option. If a rate control strategy is chosen, treatment with a beta-blocker is almost always required to achieve rate control. Adequate ventricular rate control is usually a resting rate of less than 100 beats per minute, but lower resting rates may be appropriate. Non-dihydropyridine calcium channel blockers are often contraindicated when AF is associated with HF with systolic dysfunction. There have been recent debat

Effects of an angiotensin converting enzyme (ACE) inhibitor on plasma endorphin level.

1. In addition to their antihypertensive effect, ACE inhibitors have been reported to increase general well-being, general health and vitality and work performance. The cause of these effects is not known. A possible mechanism may be release of beta-endorphins. 2. In the present study changes in plasma concentration of beta-endorphins on days with ACE inhibitor treatment (n = 12) and on non-treatment control days (n = 12) were compared in 6 patients. 3. Both on control and treatment days the beta-endorphin level fell, by 7.1 and 10.0%, respectively, from 8.00 a.m. to 8.00 p.m., reflecting the

Parietal tACS at beta frequency improves vision in a crowding regime.

Visual crowding is the inability to discriminate objects when presented with nearby flankers and sets a fundamental limit for conscious perception. Beta oscillations in the parietal cortex were found to be associated to crowding, with higher beta amplitude related to better crowding resilience. An open question is whether beta activity directly and selectively modulates crowding. We employed Transcranial Alternating Current Stimulation (tACS) in the beta band (18-Hz), in the alpha band (10-Hz) or in a sham regime, asking whether 18-Hz tACS would selectively improve the perception of crowded st

EEG findings in fetal alcohol syndrome and Down syndrome children.

Results from previous studies evaluating the electroencephalograms (EEGs) of infants born to alcoholic mothers suggest that the neonatal EEG may be a sensitive measure of prenatal ethanol exposure. Few studies, however, have examined EEG records of adolescent children with fetal alcohol syndrome (FAS). The present study investigated the resting EEG recordings of 18 matched triads of FAS, Down syndrome, and normal control subjects. Significant reductions in mean power of the alpha frequencies (7.5-12 Hz) were seen for both clinical groups, however, each syndrome appeared to have distinct EEG sp

[The use of individual EEG peculiarities for increase of neurofeedback efficiency].

An aim of the study was to demonstrate efficiency of neurofeedback in using individual frequency ranges of electroencephalogram (EEG). The sessions of theta/beta decreasing and alpha simulating trainings were carried out in 2 outpatients: one of them with attention deficit disorder (a schoolboy) and another one with functional pain contraction (a professional musician). The neurofeedback with standard frequency did not result in any improvement of psychometric and EEG characteristics of both patients.
