[Need for rheologically active, vasoactive and metabolically active substances in the initial treatment of acute acoustic trauma].

Two rheologically active and 8 vasoactive and metabolically active substances were compared in eight independent studies, some of which were randomised and double blind, on 400 patients who had suffered acute acoustic trauma. The control group was given saline. Spontaneous recovery was excluded as far as possible. The following substances were tested: Dextran 40, hydroxyethyl starch 40/0.5, naftidrofurylhydrogenoxalate, Vinpocetin, betahistine, pentoxifylline, flunaricine, Regeneresen AU 4 and 0.9% saline. All groups showed superior results to the control group in both long-term and short-term

International consensus (ICON) on treatment of Ménière's disease.

OBJECTIVE: To present the international consensus for recommendations for Ménière's disease (MD) treatment. METHODS: Based on a literature review and report of 4 experts from 4 continents, the recommendations have been presented during the 21st IFOS congress in Paris, in June 2017 and are presented in this work. RESULTS: The recommendation is to change the lifestyle, to use the vestibular rehabilitation in the intercritic period and to propose psychotherapy. As a conservative medical treatment of first line, the authors recommend to use diuretics and Betahistine or local pressure therapy. Wh

Longitudinal assessment of endolymphatic hydrops with contrast-enhanced magnetic resonance imaging of the labyrinth.

OBJECTIVE: To visualize the progression of endolymphatic hydrops in a patient with Ménière's disease. PATIENT AND METHODS: A 53-year-old patient with definite Ménière's disease underwent locally enhanced inner ear magnetic resonance imaging (LEIM) for visualizing endolymphatic hydrops. Two years later, a second magnetic resonance imaging was performed for follow-up. RESULTS: The first magnetic resonance image confirmed the presence of endolymphatic hydrops and a treatment with betahistine was begun. Two years later, no more vertigo attacks occurred. The audiometry showed a deterioration of t

Diagnosis and treatment of vertigo and dizziness.

INTRODUCTION: Vertigo is not a separate disease process, but a multisensory and sensorimotor syndrome with various etiologies and pathogeneses. It is among the commonest symptoms presented to doctors, with a lifetime prevalence of around 20% to 30%. Patients have often consulted multiple physicians before a diagnosis is made and therapy initiated. METHODS: Selective literature research and review of the guidelines of the German Neurological Society. RESULTS: A careful history remains the cornerstone of diagnosis. Once the correct diagnosis is made, specific and effective treatments are avail

Management of peripheral vertigo with antihistamines: New options on the horizon.

Vertigo is associated with a wide range of vestibular pathologies. It increasingly affects the elderly, with a high cost to society. Solutions include vestibular suppressants and vestibular rehabilitation, which form the mainstay of therapy. Antihistamines represent the largest class of agents used to combat vestibular vertigo symptoms. Agents targeting the H1 and H3 receptors have been in clinical use for several decades as single agents. Nonetheless, effective management of vertigo proves elusive as many treatments largely address only associated symptoms, and with questionable efficacy. Add

Which medication do I need to manage dizzy patients?

Vertigo and dizziness are not independent disease entities, but instead symptoms of various diseases. Accordingly, a variety of treatment approaches are required. Here we review the most relevant drugs for managing dizziness, vertigo, and nystagmus syndromes. It is important to differentiate symptomatic treatment of nausea and vomiting with, for example, dimenhydrinate and benzodiazepines, and prophylactic treatment of motion sickness with scopolamine from a causal therapy of the underlying disorders. Examples of such causal therapy include aminopyridines for downbeat nystagmus and episodic at

A national survey amongst UK otolaryngologists regarding the treatment of Ménière's disease.

Since Dr Prosper Ménière described the vertiginous syndrome that now bears his name, a large variety of medical and surgical treatments have been introduced. To determine the way in which this condition is currently managed in the United Kingdom, a postal survey amongst consultant otolaryngologists was carried out. It revealed that 52 per cent were actively involved in the treatment of patients with Ménières disease using a wide range of medical and surgical therapies that have little or no evidence base. The survey found that 94 per cent of surgeons prescribe betahistine, 63 per cent diuretic

Cervicogenic Dizziness Associated With Craniocervical Instability: A Case Report.

Cervicogenic dizziness (CGD) is a clinical syndrome characterized by an illusory sensation of motion and disequilibrium secondary to neck pathologies. Reported here is the case of a 40-year-old male presented with neck pain, dizziness, and aural symptoms in the past 12 months. The patient was previously diagnosed with Meniere's disease and treated with a low-sodium diet, betahistine and vestibular rehabilitation for 4 months, but the symptoms had not been relieved. Subsequently he sought out chiropractic evaluation. Radiographic clues illustrated a subtle anterolisthesis of the occiput on the

Menière's disease.

Menière's disease (MD) is a chronic multifactorial disorder of the inner ear characterized by episodic vestibular symptoms associated with sensorineural hearing loss, tinnitus, and aural pressure. Epidemiologic and genomic evidence supports a genetic susceptibility with multiple biochemical pathways involved, including the endocrine system, innate immune response, and autonomic nervous system. Allergens, infectious agents, vascular events, or genetic factors could modify inner-ear homeostasis and trigger MD. The diagnosis of MD is based on clinical criteria and requires the observation of an e

Pressure treatment versus gentamicin for Meniere's disease.

Ménière's disease may be treated in different ways. After obtaining a case history and performing auditory and vestibular tests the diagnosis should be obvious. In terms of treatment, the first steps are to provide the patient with information, institute a low-salt diet and regulate internal medical disorders. The next step is pharmacological treatment using diuretics, betahistine and other drugs. Local pulsated pressure treatment in the ear canal has been used in a placebo-controlled study and showed significant improvement, primarily of vertigo, but also in terms of tinnitus and hearing. Flunarizine. A reappraisal of its pharmacological properties and therapeutic use in neurological disorders.

Flunarizine is a class IV calcium antagonist with a pharmacological profile which suggests its therapeutic potential in a number of neurological and cerebrovascular disorders. It is an effective prophylactic treatment for common or classic migraine in children and adults, and it appears at least as effective as a number of other agents which act by different pharmacological mechanisms, including pizotifen (pizotyline), cinnarizine, methysergide, nimodipine, metoprolol, propranolol, aspirin and cyclandelate. Flunarizine is also effective in reducing the frequency of seizures, when used as an 'a

Lateral Semicircular Canal Occlusion in Patients with Refractory Meniere's Disease.

BACKGROUND: Meniere's disease remains a controversial condition not only in terms of its etiology and diagnosis but also its treatment. Approximately 2%-5% of patients do not improve or reach the control of their vestibular symptoms with any pharmacological therapy. Surgery of lateral semicircular canal (LSCC) occlusion could be an alternative to control the symptoms. METHODS: Twenty consecutive patients with clinical and oto-neurological diagnosis of Meniere's disease were included. Twelve women and 8 men with a mean age of 51 years (range, 29-72) were studied. All affected with unilateral o

Calcium-entry blockers in the treatment of vestibular disorders.

Based upon the results of a double-blind study carried out in a series of 120 patients suffering from vertigo and objective vestibular symptoms, we made the following observations during the treatment of vestibular disorders by means of calcium-entry blockers: Subjective symptoms regress fairly well during treatment, but no better than after betahistine-dihydrochloride (BHC) or thietylperazine therapy (TP). Objective assessment of the therapeutic action of calcium antagonists on vestibular dysfunction is based on the results of the Harmonic Acceleration test, which was carried out by using a c

Pharmacological interventions for prevention of weight gain in people with schizophrenia.

BACKGROUND: Antipsychotic-induced weight gain is an extremely common problem in people with schizophrenia and is associated with increased morbidity and mortality. Adjunctive pharmacological interventions may be necessary to help manage antipsychotic-induced weight gain. This review splits and updates a previous Cochrane Review that focused on both pharmacological and behavioural approaches to this problem. OBJECTIVES: To determine the effectiveness of pharmacological interventions for preventing antipsychotic-induced weight gain in people with schizophrenia. SEARCH METHODS: The Cochrane Sch
