Metabolic regulation of leptin production in adipocytes: a role of fatty acid synthesis intermediates.

UNLABELLED: In addition to a signal arising from the physical "stretching" of the adipocytes, metabolic and endocrine regulation of leptin production seems to operate in adipocytes. There is however a paucity of literature examining direct role of fatty acid synthesis in regulating adipocytes leptin production. To clarify the relation between fatty acid synthesis and leptin release in adipocytes, we examined leptin release from primary cultured rat epididymal adipocytes with several substances relevance to de novo fatty acid syntyhesis. Bezafibrate (0.5 or 1.0 mM), known to inhibit acetyl-CoA

[Treatment of hyperlipidemia in obese patients: monotherapy versus bi-therapy].

OBJECTIVES: Severe hyperlipoproteinaemia (increased LDL, light density lipoproteins, and VLDL, very light density lipoproteins) in patients with high body mass index (BMI) is positively associated with the occurrence of coronary heart disease. This condition requires combined drug regimen because high lipid levels frequently remain after monotherapy and diet. The aim of our study was to investigate the efficacy of combined therapy utilizing the following association: HMG CoA reductase inhibitors plus fibrates. METHODS: We examined 50 patients, males, affected by obesity (BMI > 30) and hyperli

Inducers of adipose conversion activate transcription promoted by a peroxisome proliferators response element in 3T3-L1 cells.

Nonmetabolizable fatty acids are shown here to induce adipose conversion of 3T3-L1 preadipocytes as well as to activate transcription of a reporter plasmid promoted by a peroxisome proliferators response element. This dual activity was also observed using the peroxisome proliferator bezafibrate or the differentiation inducer isobutyl methylxanthine. The data suggest a role for a peroxisome proliferators activated receptor (PPAR) in adipose conversion induced by fatty acids, isobutyl methylxanthine, or xenobiotic amphipathic carboxylates.

Biochemical and plasma lipid responses to pemafibrate in patients with primary biliary cholangitis.

AIM: Fibrate addition to ursodeoxycholic acid (UDCA) therapy has been shown to improve both liver biochemistry and long-term prognosis in primary biliary cholangitis (PBC) patients showing an incomplete biochemical response to UDCA alone. We herein describe the clinical outcome of seven cases of PBC that received the new selective peroxisome proliferator-activated receptor α modulator, pemafibrate, in combination with UDCA therapy to investigate the biochemical and plasma lipid responses to the drug. METHODS: Of 124 initially enrolled PBC patients, 12 treated with UDCA alone and seven receivi

Case of primary biliary cirrhosis-autoimmune hepatitis overlap which manifested after delivery.

A 34-year-old woman showed liver dysfunction for the first time at 3 months after delivery. Two years later, she was referred to our department with continued liver dysfunction. She fulfilled the criteria for primary biliary cirrhosis (PBC)-autoimmune hepatitis (AIH) overlap. Liver dysfunction improved after administration of ursodeoxycholic acid and bezafibrate. To the best of our knowledge this represents the second report of PBC-AIH overlap after delivery and we discuss immunological changes during the perinatal period.

Wastewater surveillance for chronic disease drugs in wastewater treatment plants: Mass load, removal, and sewage epidemiology.

As the number of chronic disease patients continues to climb, vast quantities of chronic disease drugs are continuously discharged into the wastewater treatment plants (WWTPs) and then are released to the receiving environment. However, the situations of pollution, removal, and consumption of chronic disease drugs in China were not studied. Here we investigated the mass load and removal efficiency of 14 chronic disease drugs in seven wastewater treatment plants (WWTPs) of Guangdong Province, China, and estimated the proportional usage of chronic disease drugs and the prevalence of chronic dise

Biochemical and Plasma Lipid Responses to Pemafibrate in Patients With Primary Biliary Cholangitis and Dyslipidemia: A Four-Year Analysis.

Background and aims Primary biliary cholangitis (PBC) is a chronic autoimmune liver disease treated with ursodeoxycholic acid (UDCA), though some patients respond inadequately. This study evaluated the mid-term effects of pemafibrate on liver function and lipid profiles in PBC patients with dyslipidemia who were refractory to UDCA alone or with bezafibrate. Methods A retrospective review was conducted on 25 PBC patients (17 female; median age: 71) who began treatment with pemafibrate and UDCA at Shinshu University Hospital or NHI Yodakubo Hospital in 2021. Patients were either given pemafibrat

Treatment of Primary Biliary Cholangitis: First-Line and Second-Line Therapies.

Primary biliary cholangitis (PBC) is an autoimmune disease of the interlobular bile ducts leading to secondary damage of hepatocytes and may progress to cirrhosis and liver failure. The first-line treatment is ursodeoxycholic acid; up to 40% of patients do not have an adequate response and remain at risk of disease progression. Obeticholic acid has been conditionally approved for the treatment of PBC as add-on therapy and bezafibrate has shown similar efficacy in this group of patients. Several new therapies are in development and may further add to the treatment options available to patients

A paradoxical severe decrease in serum HDL-cholesterol after treatment with a fibrate.

There have been a handful of reports in the literature of a paradoxical decrease in serum high density lipoprotein (HDL)-cholesterol in patients on fibrate drugs. The reason for this decline in cardioprotective HDL-cholesterol is not known and may have potential deleterious effects on the patient. This report describes a decrease in serum HDL-cholesterol in a patient on both simvastatin and bezafibrate. This patient also developed abnormal renal function, probably interstitial nephritis. In addition, the literature of fibrate induced serum HDL-cholesterol decline is reviewed and possible mecha

Obeticholic Acid and Fibrates in Primary Biliary Cholangitis: Comparative Effects in a Multicentric Observational Study.

INTRODUCTION: Obeticholic acid (OCA) and fibrates therapy results in biochemical improvement in placebo-controlled trials in patients with primary biliary cholangitis and insufficient response to ursodeoxycholic acid. There is scarce information outside of clinical trials. Therefore, we have assessed the effectiveness and adverse events of these treatments. METHODS: Data from patients included in the ColHai registry treated with OCA, fibrates, or both were recorded during a year, as well as adverse events and treatment discontinuation. RESULTS: Eighty-six patients were treated with OCA, 250

Fibrates in the treatment of cardiovascular risk and atherogenic dyslipidaemia.

PURPOSE: New data have emerged over the last few years about the role of fibrates in treatment of microvascular and macrovascular disease. RECENT FINDINGS: Endpoint studies have been conducted with fibrates in coronary heart disease (CHD) since 1971 and initial results were contradictory. Fibrates later showed benefits in patients with low HDL-C and low LDL-C. The prominence ascribed to the lipid triad of the metabolic syndrome and the increasing prevalence of diabetes has increased the topicality of fibrates given their main action of converting small dense to light buoyant LDL. The Fenofibr

Impact of fibrate therapy on plasma plasminogen activator inhibitor-1: a systematic review and meta-analysis of randomized controlled trials.

OBJECTIVE: The aim of this systematic review was to perform a meta-analysis of randomized controlled trials (RCTs) examining the efficacy of fibrate therapy in reducing plasma concentration or activity of plasminogen activator inhibitor 1 (PAI-1). METHODS: Scopus and MEDLINE databases were searched (up to October 15, 2014) to identify RCTs investigating whether fibrates lower plasma PAI-1 concentration or activity. A random-effects model and the generic inverse variance method were used for quantitative data synthesis. Sensitivity analyses were conducted using the one-study remove approach. R

Comparative effectiveness of peroxisome proliferator-activated receptor agonists as second-line therapies for primary biliary cholangitis: A systematic review and network meta-analysis.

BACKGROUND AND AIMS: Primary biliary cholangitis is a chronic cholestatic liver disease that may progress to cirrhosis and liver failure. For patients with inadequate response or intolerance to ursodeoxycholic acid, peroxisome proliferator-activated receptor (PPAR) agonists have emerged as second-line options. This study assessed the comparative effectiveness and safety of PPAR agonists in primary biliary cholangitis through a systematic review and network meta-analysis. APPROACH AND RESULTS: A systematic review identified randomized controlled trials evaluating PPAR agonists versus ursodeoxy

Effects of Modified Low-Density Lipoproteins and Fenofibrate on an Outer Blood-Retina Barrier Model: Implications for Diabetic Retinopathy.

Purpose: There is a lack of treatment for early diabetic retinopathy (DR), including blood-retina barrier (BRB) breakdown. The robust clinical benefit of fenofibrate in DR provides an opportunity to explore disease mechanisms and therapeutic targets. We have previously found that modified lipoproteins contribute to DR and that fenofibrate protects the inner BRB. We now investigate (1) whether modified lipoproteins elicit outer BRB injury and (2) whether fenofibrate may alleviate such damage. Methods: Human retinal pigment epithelium ARPE-19 cells were cultured in semipermeable transwells to es

[New therapeutic approaches for the treatment of primary biliary cholangitis].

Primary biliary cholangitis (PBC) is a rare disease for which management long consisted of a single treatment: ursodeoxycholic acid. In 2015-2016, this disease regained interest with the first studies on obeticholic acid (FXR agonist) and then on bezafibrate (PPAR agonist). Subsequently, over the past five years, significant progress has been made in the management of PBC. Major studies have helped better predict disease progression and its response to treatment through the development of prognostic tools. Additionally, the therapeutic arsenal has expanded with the arrival of selective PPAR ag

Expression of cFABP and PPAR in trophoblast cells: effect of PPAR ligands on linoleic acid uptake and differentiation.

Throughout gestation, fetal growth depends, in part, on placental transfer of maternal essential fatty acid (EFA) and long-chain polyunsaturated fatty acid. All fatty acid (FA) can cross lipid bilayer by simple diffusion, such as those in the syncytiotrophoblasts, the multinucleated, terminally differentiated trophoblast cells. The trophoblasts differentiation process is accompanied by an increase of human chorionic gonadotropin (hCG) secretion and an inhibition of Human Achaete-Scute Homologue-2 expression (Hash-2). Furthermore, a number of FA-binding proteins (FABPs) have been identified in
