A Bayesian mixed treatment comparison of efficacy of biologics and small molecules in early rheumatoid arthritis.

The current paradigm in the management of rheumatoid arthritis (RA) is to treat patients in the early stage of the disease (ERA). Previous meta-analysis-based mixed treatment comparisons (MTCs), aimed to identify the most effective drugs in ERA, are biased by the wide "window" of early definition, ranging from 6 months to 2 years. The aim of this study was to estimate through a Bayesian Network Meta-Analysis which biologics or small molecules are more likely to achieve a 1-year good clinical response in ERA patients with disease duration < 1 year. According to the Preferred Reporting Items for

Discovery of an autoantibody signature for the early diagnosis of knee osteoarthritis: data from the Osteoarthritis Initiative.

OBJECTIVE: To find autoantibodies (AAbs) in serum that could be useful to predict incidence of radiographic knee osteoarthritis (KOA). DESIGN: A Nucleic-acid Programmable Protein Arrays (NAPPA) platform was used to screen AAbs against 2125 human proteins in sera at baseline from participants free of radiographic KOA belonging to the incidence and non-exposed subcohorts of the Osteoarthritis Initiative (OAI) who developed or not, radiographic KOA during a follow-up period of 96 months. NAPPA-ELISA were performed to analyse reactivity against methionine adenosyltransferase two beta (MAT2β) and

Proceedings of the second international molecular pathological epidemiology (MPE) meeting.

Disease classification system increasingly incorporates information on pathogenic mechanisms to predict clinical outcomes and response to therapy and intervention. Technological advancements to interrogate omics (genomics, epigenomics, transcriptomics, proteomics, metabolomics, metagenomics, interactomics, etc.) provide widely open opportunities in population-based research. Molecular pathological epidemiology (MPE) represents integrative science of molecular pathology and epidemiology. This unified paradigm requires multidisciplinary collaboration between pathology, epidemiology, biostatistic

Genetics of ischaemic stroke.

Ischaemic stroke is a heterogeneous multifactorial disorder. Epidemiological data provide substantial evidence for a genetic component to the disease, but the extent of predisposition is unknown. Large progress has been made in single-gene disorders associated with ischaemic stroke. The identification of NOTCH3 mutations in patients with cerebral autosomal dominant arteriopathy with subcortical infarcts and leucoencephalopathy (CADASIL) has led to new insights on lacunar stroke and small-vessel disease. Studies of sickle-cell disease have drawn attention to the importance of modifier genes and

Simulation Extrapolation Method for Cox Regression Model with a Mixture of Berkson and Classical Errors in the Covariates using Calibration Data.

Many biomedical or epidemiological studies often aim to assess the association between the time to an event of interest and some covariates under the Cox proportional hazards model. However, a problem is that the covariate data routinely involve measurement error, which may be of classical type, Berkson type or a combination of both types. The issue of Cox regression with error-prone covariates has been well-discussed in the statistical literature, which has focused mainly on classical error so far. This paper considers Cox regression analysis when some covariates are possibly contaminated wit

Power and sample size determination for group comparison of patient-reported outcomes using polytomous Rasch models.

The analysis of patient-reported outcomes or other psychological traits can be realized using the Rasch measurement model. When the objective of a study is to compare groups of individuals, it is important, before the study, to define a sample size such that the group comparison test will attain a given power. The Raschpower procedure (RP) allows doing so with dichotomous items. The RP is extended to polytomous items. Several computational issues were identified, and adaptations have been proposed. The performance of this new version of RP is assessed using simulations. This adaptation of RP a

Constrained S-estimators for linear mixed effects models with covariance components.

Linear mixed effects (LME) models are increasingly used for analyses of biological and biomedical data. When the multivariate normal assumption is not adequate for an LME model, then a robust estimation approach is preferable to the maximum likelihood one. M-estimators were considered before for robust estimation of the LME models, and recently a constrained S-estimator was proposed. This S-estimator cannot be applied directly to LME models with correlated error terms and vector random effects with correlated dimensions. Therefore, a modification is proposed, which extends application of the c

Intermittent observation of time-dependent explanatory variables: a multistate modelling approach.

Motivated by investigations of factors related to various patient-reported outcome measures in psoriatic arthritis patients, after controlling for the effect of disease activity on these outcomes, we outline an approach for dealing with a rapidly fluctuating explanatory variable in a multistate model. On the basis of a representation of this variable as an ordinal classification, we suggest the use of an expanded multistate model. We examine the bias in estimating effects associated with other variables via simulation for different modelling choices. We present an analysis of a motivating data

Interaction trees with censored survival data.

We propose an interaction tree (IT) procedure to optimize the subgroup analysis in comparative studies that involve censored survival times. The proposed method recursively partitions the data into two subsets that show the greatest interaction with the treatment, which results in a number of objectively defined subgroups: in some of them the treatment effect is prominent while in others the treatment may have a negligible or even negative effect. The resultant tree structure can be used to explore the overall interaction between treatment and other covariates and help identify and describe po

Normative human brain volume growth.

OBJECTIVE While there is a long history of interest in measuring brain growth, as of yet there is no definitive model for normative human brain volume growth. The goal of this study was to analyze a variety of candidate models for such growth and select the model that provides the most statistically applicable fit. The authors sought to optimize clinically applicable growth charts that would facilitate improved treatment and predictive management for conditions such as hydrocephalus. METHODS The Weibull, two-term power law, West ontogenic, and Gompertz models were chosen as potential models. N

Calibration plots for risk prediction models in the presence of competing risks.

A predicted risk of 17% can be called reliable if it can be expected that the event will occur to about 17 of 100 patients who all received a predicted risk of 17%. Statistical models can predict the absolute risk of an event such as cardiovascular death in the presence of competing risks such as death due to other causes. For personalized medicine and patient counseling, it is necessary to check that the model is calibrated in the sense that it provides reliable predictions for all subjects. There are three often encountered practical problems when the aim is to display or test if a risk pred

A marginal mixture model for selecting differentially expressed genes across two types of tissue samples.

Bayesian hierarchical models that characterize the distributions of (transformed) gene profiles have been proven very useful and flexible in selecting differentially expressed genes across different types of tissue samples (e.g. Lo and Gottardo, 2007). However, the marginal mean and variance of these models are assumed to be the same for different gene clusters and for different tissue types. Moreover, it is not easy to determine which of the many competing Bayesian hierarchical models provides the best fit for a specific microarray data set. To address these two issues, we propose a marginal

Standards and Guidance for Transparent Disclosure of Human-Artificial Intelligence Collaboration Content in Medical Research (2026).

The rapid iteration of artificial intelligence (AI) tools is propelling medical research into a new phase characterized by human-AI collaboration. In the course of this process,AI has evolved from an external instrument into a functional component embedded within research workflows. As the collaborative boundary between human researchers and AI becomes increasingly ambiguous, governance challenges have grown increasingly salient.Concurrently, there remains a lack of internationally harmonized standards for the declaration of AI-assisted content, giving rise to notable disclosure gaps and implementa

ComPrEmetidos: A Multidisciplinary Consensus to Transform HIV Prevention in Spain.

Despite progress in antiretroviral therapy and prevention, HIV remains a major public health challenge in Spain. Pre-exposure prophylaxis (PrEP) is a proven and effective prevention strategy, but challenges in access, equity, adherence and persistence limit its population impact. Our aim was to identify current barriers to PrEP adoption in Spain and develop actionable solutions. We conducted a structured consensus process (September 2024-May 2025) with 19 experts from fields including infectious diseases/HIV care, sexual health/sexually transmitted infection services, primary care, emergency m

Epistemic cultures, knowledge types, and disciplinary tensions in population health research: An international interview study.

Population health research involves multiple disciplines including epidemiology, economics, biostatistics, health promotion, and social sciences. Disciplinary differences are widely acknowledged but rarely analysed, leaving the nature and source of interdisciplinary tension underspecified. Drawing on the concept of epistemic cultures and interviews with 45 international researchers sampled from a bibliometric network of health equity scholars, I investigate whether distinct epistemic cultures exist within population health research, and identify specific features underpinning reported interdis

Significant Decline in HCV-Related Mortality From Liver Cirrhosis and Chronic Liver Disease (2015-2022): A Sentinel Center Study.

BACKGROUND: Reducing mortality attributable to hepatitis C virus (HCV) is a key target in the World Health Organization (WHO) HCV elimination strategy. While liver cancer registration in Taiwan includes hepatitis C antibody (anti-HCV) status, no national registry records viral etiologies for deaths from liver cirrhosis and chronic liver disease (LC/CLD). Therefore, this sentinel study aimed to estimate national trends in HCV-related LC/CLD mortality. METHODS: The study period spanned from 2015 to 2022. According to ICD-10 codes K70, K73 and K74, the annual mortality numbers for LC/CLD were ex

ToxicR: A computational platform in R for computational toxicology and dose-response analyses.

The need to analyze the complex relationships observed in high-throughput toxicogenomic and other omic platforms has resulted in an explosion of methodological advances in computational toxicology. However, advancements in the literature often outpace the development of software researchers can implement in their pipelines, and existing software is frequently based on pre-specified workflows built from well-vetted assumptions that may not be optimal for novel research questions. Accordingly, there is a need for a stable platform and open-source codebase attached to a programming language that
