MR angiography facilitates the differentiation of aneurysmal from unicameral bone cysts.

OBJECTIVES: The objective of this work was to evaluate the incremental value of MR angiography over plain radiographs and MRI for the differentiation of aneurysmal bone cysts (ABCs) from unicameral bone cysts (UBCs). METHODS: Thirty-six juvenile patients with histologically secured primary ABCs or UBCs were included in this retrospective study. Two radiologists assessed all obtained images in a blinded fashion using a catalog of previously suggested imaging findings. A second readout with supplementary MR angiography images was performed after 8 weeks to prevent observer recall bias. Diagnost

Multidisciplinary management of spinal aneurysmal bone cysts: A single-center experience.

OBJECTIVE: The management of spinal aneurysmal bone cysts (ABCs) is complex and often requires multimodality therapy, including surgical intervention to stabilize the axial skeleton, and avoid neurologic injury or death. With en bloc resection, ABCs have a recurrence rate of 12%, which increases to >50% with subtotal resection. The use of doxycycline sclerotherapy has been reported to reduce the recurrence rate of non-spinal ABCs to 5% at >24 month follow-up. We retrospectively reviewed our institutional results for sodium tetradecyl sulfate (STS)/doxycycline sclerotherapy and surgical interve

Surgical treatment of active or aggressive aneurysmal bone cysts in children.

The aim of this study was to evaluate functional and radiological results, tumor control, and complications of the surgical treatment of aneurysmal bone cysts (ABCs) in children by extended curettage using a mechanical burr and cauterization, grafting, and internal fixation in specific locations. Sixty-four children [38 males, 26 females, median age=10 years (range, 5-18 years)] with active or aggressive ABCs of long and flat bones were subjected to a median follow-up of 66 months (range, 28-130 months) following surgical treatment. The pathological fracture rate was 72%. Surgical procedures i

Long-term follow-up of Ethibloc injection in aneurysmal bone cysts.

The aim of this study is to assess the long-term results of Ethibloc injection in aneurysmal bone cysts (ABC). Thirty-three patients with ABC were treated with computed tomography-guided percutaneous injection of Ethibloc into the cyst cavity. Twenty-two patients had Ethibloc injection as primary treatment and 11 patients had presented to us with recurrence after previous procedures including steroid injection, bone marrow injection, curettage bone grafting and various other surgical procedures. The mean follow-up was 54 (22-90) months. Symptoms were relieved in all patients. Two patients were

Surgical treatment and recurrence rate of aneurysmal bone cysts in children.

High recurrence rates have been reported after surgical treatment of aneurysmal bone cysts, ranging from 10-59%, and greater (five of seven) in children younger than 10 years. No previous study has focused specifically on recurrence as a function of age in a large pediatric study. The purpose of the current study was to review the outcome of current surgical techniques in the treatment of primary aneurysmal bone cysts in children and to determine if recurrence rates were higher in younger children, as proposed by some authors. This study included 45 children with primary aneurysmal bone cysts

Intracortical and subperiosteal aneurysmal bone cysts: a report of three cases.

Aneurysmal bone cyst of the long bones in a purely intracortical or subperiosteal location is unusual. Three such cases are reported, and the radiographic and pathologic differential diagnoses are discussed. Those subperiosteal or intracortical aneurysmal bone cysts with radiographic features similar to the intramedullary variety should suggest the same diagnosis. However, the radiographic features may be less specific, so that a diagnosis of aneurysmal bone cyst must be entertained when considering a subperiosteal or intracortical lytic lesion.

Alternatives in the management of aneurysmal bone cysts of the mandible.

Aneurysmal bone cyst (ABC) is an unusual expansile lesion of bone which may arise within the mandible on rare occasions. This lesion may arise with very few signs or symptoms. Surgical treatment consists of complete excision or curettage. The recurrence rate may be high in lesions which are incompletely excised. Three cases of aneurysmal bone cysts in children will be presented, demonstrating the wide variation of clinical presentation. One case required an extensive mandibular resection, necessitating the first reported microvascular mandible reconstruction for this entity in a pediatric pati

Quantitative analysis of the plain radiographic appearance of aneurysmal bone cysts.

RATIONAL AND OBJECTIVES: To quantitate radiographic features that distinguish the plain radiographic appearance of aneurysmal bone cyst (ABC) from other solitary lesions of bone. MATERIALS AND METHODS: Seven hundred nine cases of focal bone lesions, including 32 ABCs were analyzed according to demographic, anatomic, and plain radiographic features. Vector analysis of groups of features was performed to determine those that are most sensitive and specific for the appearance of ABC in contrast with other lesions in the data base. RESULTS: Aneurysmal bone cysts most consistently are medullary-b

Spontaneous healing of aneurysmal bone cysts. A report of three cases.

We report three cases of spontaneous healing of aneurysmal bone cysts (ABC). In one case histological material was obtained after resection of the already ossified expansile mass discovered as a lytic lesion seven months previously. In the two other patients, spontaneous ossification of a radiologically presumed ABC in the lytic and expansile phase was observed after nine and seven months respectively. The healed lesions have remained stable at 12, 32, and 36 months respectively. These findings suggest that when the diagnosis can be made with confidence, and the lesion is in a location and at

Multiple soft tissue aneurysmal cysts: An occurrence after resection of primary aneurysmal bone cyst of fibula.

We report a case of multiple extraosseous aneurysmal cysts occurring in the muscle and subcutaneous plane of postero-lateral aspects of the upper right leg. They appeared about 15 months after resection of aneurysmal bone cyst of the upper end of the fibula. They varied in size from 2 cm to 5 cm. Radiologically they were well-defined lesions with central septate areas surrounded by a rim of calcification. Histologically they showed central cystic spaces separated by septa consisting of fibroblasts, osteoclast type of giant cells and reactive woven bone. Thus they showed histological similarity

Doxycycline Sclerotherapy of Mandibular Aneurysmal Bone Cysts: A Brief Clinical Study.

Aneurysmal bone cysts (ABCs) are benign bone tumors typically affecting children. Mandibular ABCs can be difficult to treat surgically, given their sensitive anatomic location and functional and cosmetic impacts. This report presents 3 pediatric patients with mandibular ABCs successfully treated with image-guided percutaneous doxycycline sclerotherapy. The first 2 patients presented with pain and swelling, whereas the third was diagnosed incidentally. Sclerotherapy was the sole treatment for cases 1 and 2, whereas case 3 had sclerotherapy after recurrence following prior surgeries. In all 3 pa

Radiologic and pathologic correlation of aneurysmal bone cysts at unusual sites.

Aneurysmal bone cyst (ABC) is a benign solitary lesion. It usually occurs in the long bones but uncommonly in the calcaneum, mandible, femur and clavicle. Its frequency of occurrence in skull and mandible is 4%, clavicle and ribs is 5%, femur is 13% and foot is 3%. Only a few cases of ABC of these unusual sites have been reported till date. Here we report four cases of ABC at unusual sites namely the calcaneum, mandible, femur and clavicle diagnosed primarily on the basis of different imaging modalities and later confirmed on histopathology.

Is sclerotherapy better than intralesional excision for treating aneurysmal bone cysts?

BACKGROUND: Minimally invasive approaches such as sclerotherapy have been introduced to treat aneurysmal bone cysts. Sclerotherapy has been associated with reasonable healing rates during the past two decades. However, it is unclear whether sclerotherapy compares with the more traditional extended curettage and bone grafting. QUESTIONS/PURPOSES: We therefore compared the healing rates and functional scores in patients having percutaneous repetitive sclerotherapy using polidocanol (Group 1) with those with intralesional excision (extended curettage with a high-speed burr) and bone grafting (Gr

Injectable form of calcium sulphate as treatment of aneurysmal bone cysts.

BACKGROUND: Aneurysmal bone cysts (ABC) are a rare condition in adolescents and teenagers but may result in pain, fracture and growth abnormalities. The gold standard of open curettage carries the risk of surgical complications and still a local recurrence rate of 20-30%. Percutaneous treatment of ABC have rarely been reported and a poor response the usual outcome. This study investigated a new technique of percutaneous aspiration and injection of ABC using an aqueous solution of calcium sulphate. METHODS: A radiological diagnosis of a bone cyst was made in 15 consecutive patients and patholo

Aneurysmal bone cysts recur at juxtaphyseal locations in skeletally immature patients.

Aneurysmal bone cysts are associated with a high rate of recurrence. Many aneurysmal bone cysts arise near open physes or articular cartilage in skeletally immature patients. Fear of damaging these structures could cause surgeons to curette the tumors less aggressively. We hypothesized location of an aneurysmal bone cyst in a periarticular or juxtaphyseal location would increase the risk of recurrence. We retrospectively studied 53 patients with aneurysmal bone cysts treated between 1989 and 2004. All patients had primary disease, and all patients underwent curettage of the lesion. Ten patient

Aneurysmal bone cysts in the cranial vault and base of skull.

Three cases of aneurysmal bone cyst of the skull are reported. The localization in the base of the skull (two cases) is extremely rare and simulates a space-occupying intracerebral lesion. The clinical and microscopic findings, and their significance for the differential diagnosis from malignant brain tumors, are described.

Giant-cell tumors and aneurysmal bone cysts of ribs in childhood.

Cystic or solid lesions of ribs containing giant cells are rarely seen in children. Three cases are presented which are uncommon with regard to the age of the patients (5-14 yr old) as well as the site. These lesions were diagnosed late and exclusively by X-ray. Radiographically it was impossible to distinguish between a giant cell tumor and an aneurysmal bone cyst. Although the distinction of aneurysmal bone cysts and giant cell tumors sensu strictiori is justified from a clinical point of view, the two may be variants of the same pathologic growth process, i.e. growth of angiogribous tissue
