A comparison of two techniques for digital distraction lengthening in skeletally immature patients.

Since 1987, 16 skeletally immature patients aged 2 to 16 years (mean, 7.9 years) underwent 27 digital lengthening procedures of terminal bones, 20 metacarpals and 7 phalanges, using the distraction callotasis technique. Seven digits were lengthened with 2 fixator half-pins on either side of the osteotomy site (dual half-pin group). Twenty digits, which were too small to accommodate 4 half-pins, were lengthened over a longitudinal intramedullary guidewire with 1 fixator half-pin on either side of the osteotomy site (single half-pin/K-wire group). No bone grafts were needed. The mean preoperativ

[Fractures of the talus].

Peripheral fractures of the talus, such as fractures of the posterior and lateral process, need no special therapy. Larger fragments with dislocation require open reduction and screw fixation. Small flake fractures with stability of the joint can be removed arthroscopically, but larger fractures should be repositioned with K-wires, small screws. Ethipin or fibrin sealant. Dislocated fractures of the head of the talus should be reduced and fixed by screws or K-wires. Fractures of the neck of the talus with anteromedial or lateral dislocation can be treated by closed reduction and external fixat

Comparison of k-wire insertion using oscillatory and unidirectional drilling modes under constant thrust force.

Heat generation during the Kirschner wire (K-wire) insertion process, under either unidirectional or oscillatory drilling mode, places bone at risk of thermal osteonecrosis which can lead to infection. There is a lack of quantitative understanding of the heat generation difference between the two drilling modes and knowledge of optimal thrust force level under each mode is missing. The goal of this study is to investigate the effects of drilling modes and thrust force levels on the bone drilling outcomes. Controlled machine-based constant thrust force K-wire insertion experiments were conducte

A comparative study of fixation techniques for type II fractures of the odontoid process.

Primary screw fixation of a Type II odontoid fracture or non-union is an attractive alternative to posterior atlanto-axial arthrodesis in that normal cervical motion can be maintained. Eight cervical cadaver spines, ranging in age from 17-90 years, were used for study. Type II fractures of the dens were created using an osteotome. Simulated fractures were fixed using one or two 3.5-mm bone screws. After testing each screw fixation technique, the screws were removed and a posterior C1-C2 brooks sublaminar wiring was performed using four 18-gauge wires with wooden blocks to simulate bone graft.

Comparison of two methods in the treatment of congenital pseudarthrosis of clavicle: multicenter experience.

BACKGROUND: Congenital pseudoarthrosis of the clavicle (CPC) is an uncommon entity. Owing to its scarce presentation, treatment of this disorder has not been well established. This study aimed (1) to compare surgical treatment methods that included excision of pseudoarthrosis and iliac crest bone graft and fixate with either the elastic stable intramedullary nail (ESIN) or K-wires or plate and screws, and (2) to assess the clinical outcomes of two different surgical methods. METHODS: A multi-central retrospective study was performed between 2013 and 2017 in four tertiary teaching hospitals. F

[Late results of stapedectomy after 15 years (author's transl)].

Stapedectomy in Otosclerosis in using polyethylene struts or stainless-steel wire protheses results in permanent correction of the postoperative gap. The use of stainless-steel wire protheses however is successful with 94% of the patients regarding the postoperative gap, compared to 89% of the patients in using the polyethylene struts. The postoperative decrement of bone conduction is statistically significantly lower in the stainless-steel wire protheses than in polyethylene struts.

Spinal fixation. Part 1. Principles, basic hardware, and fixation techniques for the cervical spine.

Spinal fixation devices provide stability and restore anatomic alignment in the treatment of fractures, degenerative disease, infection, and tumors and correct congenital deformities such as those seen in scoliosis. The devices provide immediate stability but are not strong enough to withstand prolonged stress and eventually fail, in most cases, if bone fusion does not occur. Bone graft material is often used to promote fusion and to replace bone after resection. Internal fixation is used to maintain position and alignment and to prevent motion as the spine fuses. Plates and rods are attached

The histopathology of the incus after stapedectomy.

Twenty-two temporal bones subjected to stapedectomy were examined by light microscopy for histological changes in the long process of the incus. In 19 cases a stainless steel wire technique had been used and in three a polyethylene tube operation had been performed. Twenty healthy unoperated ears were examined as controls. In all operated ears fibrosis and reduced vascularity of the mucosa was noted. In the polyethylene tube group, 1 ear showed the growth of new bone into the lumen of the tube and 1 showed minor resorption of the long process of the incus. In the stainless steel wire group bon
