Post-Concussion Changes in the Functional Brain Connectome Relative to Pre-Injury Baseline: A Prospective Observational Study.

There is growing concern about the long-term health consequences of concussion, stemming from its high incidence and evidence of post-injury sequelae. This has raised critical questions about clinical assessments of concussion recovery, and whether brain function has fully recovered at medical clearance. The major knowledge gap is only partly addressed by conventional cross-sectional neuroimaging studies, due to a lack of pre-injury baseline imaging. To address this gap, 187 university-level athletes had resting-state functional magnetic resonance imaging collected at pre-season baseline. Of t

Investigating the Association of Concussion and Contact Sport Exposure History With Brain Microstructure Using Quantitative Susceptibility Mapping.

A growing body of evidence suggests that repeated concussions and exposure to repetitive head impacts may be associated with subtle abnormalities in neurological health. Prior studies have demonstrated associations of prior concussion and repetitive head impacts with the brain's microstructure, typically using diffusion magnetic resonance imaging, though the direction of these relationships has varied across groups. Quantitative susceptibility mapping is a quantitative extension of susceptibility weighted imaging that is sensitive to pathophysiology associated with neurotrauma and thus represe

Altered longitudinal structural connectome in paediatric mild traumatic brain injury: an Advancing Concussion Assessment in Paediatrics study.

Advanced diffusion-weighted imaging techniques have increased understanding of the neuropathology of paediatric mild traumatic brain injury (i.e. concussion). Most studies have examined discrete white-matter pathways, which may not capture the characteristically subtle, diffuse and heterogenous effects of paediatric concussion on brain microstructure. This study compared the structural connectome of children with concussion to those with mild orthopaedic injury to determine whether network metrics and their trajectories across time post-injury differentiate paediatric concussion from mild trau

Cerebral Blood Flow Predicts Recovery in Children with Persistent Post-Concussion Symptoms after Mild Traumatic Brain Injury.

Persistent post-concussion symptoms (PPCS) following pediatric mild traumatic brain injury (mTBI) are associated with differential changes in cerebral blood flow (CBF). Given its potential as a therapeutic target, we examined CBF changes during recovery in children with PPCS. We hypothesized that CBF would decrease and that such decreases would mirror clinical recovery. In a prospective cohort study, 61 children and adolescents (mean age 14 [standard deviation = 2.6] years; 41% male) with PPCS were imaged with three-dimensional (3D) pseudo-continuous arterial spin-labelled (pCASL) magnetic res

The Sport Concussion Assessment Tool (SCAT2) for evaluating civilian mild traumatic brain injury. A pilot normative study.

Self-report measures, particularly symptom inventories, are critical tools for identifying patients with persistent post-concussion symptoms and their follow-up. Unlike in military or sports-related assessment, in general civilian settings pre-injury levels of concussion-like symptoms are lacking. Normative data are available in adolescent and college populations, but no reference data exist to guide clinical adult explorations. The purpose of this study was to use the second edition of the Sport Concussion Assessment Tool (SCAT2) to profile a cohort of 60 healthy community volunteers who had

Brain photobiomodulation therapy for acute sport-related concussion: a randomized, single-blinded, pilot clinical trial (RECOVERY study).

BACKGROUND: The current standard of care for concussion relies on symptom management; there are almost no disease-modifying treatments. The RECOVERY study investigated the safety of RGn550 (REGEnLIFE), a novel neurostimulation device combining brain photobiomodulation with a static magnetic field, in sportspeople with acute concussion and described concussion-related parameters in patients receiving this therapy. METHODS: This prospective, randomized (central randomization, 1:1 ratio), single-blind, monocentric, pilot study compared two pulse frequencies (5 Hz and 10 Hz). Adult sportspeople w

Oral Contraceptives, Concussion History, and Contact-Sport Exposure: Multimodal MRI and Brain Injury Biomarkers in Female Athletes.

Neuroimaging and blood-based biomarkers are increasingly utilized to identify long-term effects of concussion and contact-sport exposure (CSE), but female athletes are underrepresented in this work, despite potential sex-related differences. Early research indicates that widely used oral contraceptives (OCs) influence brain structure and function and concussion-related biomarkers. This study investigated whether current oral contraceptive (OC) use moderates associations of concussion history and CSE with magnetic resonance imaging (MRI) measures and brain injury biomarkers in female athletes.

Hindi Adaptation and Psychometric Validation of Rivermead Post-Concussion Symptoms Questionnaire on Patients with Traumatic Brain Injury.

BACKGROUND: The Rivermead Post-Concussion Symptoms Questionnaire (RPCSQ) is a widely used tool for assessing the pattern and severity of post-head injury symptoms, and has been translated and validated in various languages. However, a Hindi version of this questionnaire is not yet available. Therefore, the present study aimed to adapt the RPCSQ into Hindi and examine its factorial structure along with its psychometric properties. METHOD: The original English version of RPCSQ was adapted into Hindi by using the cross-cultural adaptation process. The final adapted Hindi version of RPCSQ (RPCSQ-

Assessing blood oxygen level-dependent signal variability as a biomarker of brain injury in sport-related concussion.

Mild traumatic brain injury is a complex neurological disorder of significant concern among athletes who play contact sports. Athletes who sustain sport-related concussion typically undergo physical examination and neurocognitive evaluation to determine injury severity and return-to-play status. However, traumatic disruption to neurometabolic processes can occur with minimal detectable anatomic pathology or neurocognitive alteration, increasing the risk that athletes may be cleared for return-to-play during a vulnerable period and receive a repetitive injury. This underscores the need for sens

Relations between concussion symptoms and depression among patients with mild traumatic brain injury: A moderated mediation model.

INTRODUCTION: Concussion symptoms following a traumatic accident are both common and known to adversely affect mental health and recovery in patients with traumatic brain injury. Depression, highly prevalent among patients with traumatic brain injury, is also associated with the important factors of sleep quality and resilience. However, the mediator and moderator roles of depression following concussion in patients with traumatic brain injury have been underexplored. The aims of this study were to investigate the mediating role of sleep quality in the relation between concussion symptoms and

Reducing Brain Injury Misconceptions and Willingness to Risk Concussion with a Three-Week Introductory-level Neuroscience Course.

Misconceptions of brain injury are common and persistent in the general public (Ralph and Derbyshire, 2013). Moreover, undergraduate students are in an age range where they are at high risk of concussion and traumatic brain injury, but often lack knowledge of the symptoms, severity, recovery, and varied impacts of brain injury on cognition. Introductory-level undergraduate neuroscience courses have the potential to reach a broad audience and improve students' knowledge of the brain. It is also important to know, however, if neuroscience courses can combat common misconceptions and impact real-

Lack of association between four biomarkers and persistent post-concussion symptoms after a mild traumatic brain injury.

Approximately 15 % of individuals who sustained a mild Traumatic Brain Injury (TBI) develop persistent post-concussion symptoms (PPCS). We hypothesized that blood biomarkers drawn in the Emergency Department (ED) could help predict PPCS. The main objective of this project was to measure the association between four biomarkers and PPCS at 90 days post mild TBI. We conducted a prospective cohort study in seven Canadian EDs. Patients aged ≥ 14 years presenting to the ED within 24 h of a mild TBI who were discharged were eligible. Clinical data and blood samples were collected in the ED, and a sta

Complicated versus uncomplicated mild traumatic brain injuries: A comparison of psychological, cognitive, and post-concussion symptom outcomes.

INTRODUCTION: A complicated mild traumatic brain injury (MTBI) is defined as mild by all clinical severity indicators but is complicated due to a traumatic intracranial abnormality visible on neuroimaging. Researchers have reported mixed findings regarding whether neuropsychological and functional outcomes following complicated MTBI are worse than, or similar to, outcomes following uncomplicated MTBI. This study examined patients referred from a Taiwanese emergency department to a neurosurgical outpatient clinic. Participants with complicated MTBI, uncomplicated MTBI, and those who did not und

Baseline vs. cross-sectional MRI of concussion: distinct brain patterns in white matter and cerebral blood flow.

Neuroimaging has been used to describe the pathophysiology of sport-related concussion during early injury, with effects that may persist beyond medical clearance to return-to-play (RTP). However, studies are typically cross-sectional, comparing groups of concussed and uninjured athletes. It is important to determine whether these findings are consistent with longitudinal change at the individual level, relative to their own pre-injury baseline. A cohort of N = 123 university-level athletes were scanned with magnetic resonance imaging (MRI). Of this group, N = 12 acquired a concussion and were

A Projectile Concussive Impact Model Produces Neuroinflammation in Both Mild and Moderate-Severe Traumatic Brain Injury.

Traumatic brain injury (TBI) is a major cause of death and disability and is experienced by nearly 3 million people annually as a result of falls, vehicular accidents, or from being struck by or against an object. While TBIs can range in severity, the majority of injuries are considered to be mild. However, TBI of any severity has the potential to have long-lasting neurological effects, including headaches, cognitive/memory impairments, mood dysfunction, and fatigue as a result of neural damage and neuroinflammation. Here, we modified a projectile concussive impact (PCI) model of TBI to delive

Persistent post-concussive syndrome in children after mild traumatic brain injury is prevalent and vastly underdiagnosed.

Data on epidemiology and prognosticators of persistent post-concussion syndrome (PPCS) after mild traumatic brain injury (mTBI) in the pediatric population is scarce. The aim of this study was to evaluate the prevalence of PPCS in children after mTBI and to identify clinical variables in children who are at high risk for developing PPCS. A multicenter, retrospective matched cohort in which PPCS symptoms were evaluated in children 8-15-year-old, 6-60 months after being admitted to the emergency department because of mTBI. The control group included children admitted to the emergency department
