Early Imaging Prediction of Malignant Cerebellar Edema Development in Acute Ischemic Stroke.

BACKGROUND AND PURPOSE: Malignant cerebellar edema (MCE) is a life-threatening complication of acute ischemic stroke that requires timely diagnosis and management. Aim of this study was to identify imaging predictors in initial multiparametric computed tomography (CT), including whole-brain CT perfusion (WB-CTP). METHODS: We consecutively selected all subjects with cerebellar ischemic WB-CTP deficits and follow-up-confirmed cerebellar infarction from an initial cohort of 2635 patients who had undergone multiparametric CT because of suspected stroke. Follow-up imaging was assessed for the pres

Improvised external ventricular drain in neurosurgery: A Nigerian tertiary hospital experience.

BACKGROUND: The most common type of hydrocephalus in developing countries is post infective hydrocephalus. Infected cerebrospinal fluid (CSF) however cannot be shunted for the reason that it will block the chamber of the ventriculo-peritoneal (VP) shunt due to its high protein content. In centers where standard external ventricular drain (EVD) sets are not available, improvised feeding tube can be used. AIM: The main focus of this study is to encourage the use of improvised feeding tube catheters for EVD when standard sets are not available to improve patients' survival. METHODOLOGY: This wa

Supratentorial Dural Venous Channel (SDVC): Imaging Anatomy Evaluated on CT Venography.

OBJECTIVE: A rare anatomical pattern exists in which a cortical vein courses vertically while running parallel to the supratentorial dura mater, enters the dura, and ultimately drains into the transverse sinus (TS). This structure is referred to as the supratentorial dural venous channel (SDVC). In this study, we retrospectively reviewed the imaging characteristics of SDVCs. METHODS: We consecutively included patients with brain tumors, ischemic disease, or vascular malformations who underwent CT venography (CTV) between January 2020 and October 2023. During the study period, CTV was performe

Tentorial venous sinuses: an anatomic study.

OBJECTIVE: Certain neurosurgical procedures require sectioning of the tentorium cerebelli. The presence of venous sinuses within the tentorium makes these procedures difficult. The aim of this study was to investigate the incidence, size, location, configuration, and pattern of venous drainage of these sinuses. METHODS: The tentorium cerebelli was studied in 80 fresh cadavers. After the skull cap and the supratentorial portion of the brain were removed, the tentorium was inspected for the presence of venous sinuses. Their location, size, configuration, and pattern of venous drainage were note

Apollo microcatheter-assisted transvenous embolization for brain arteriovenous malformation with a tortuous stenotic drainage vein and "Caput Medusa" sign: a case report.

INTRODUCTION AND IMPORTANCE: Transvenous embolization (TVE) for brain arteriovenous malformations (AVMs) typically requires two microcatheters or a dual-lumen mini-balloon catheter in the draining vein. A single Apollo microcatheter can replace these devices, offering a novel TVE alternative for AVMs with the "Caput Medusa" sign. This case report describes the technique, as such reports remain limited. CASE PRESENTATION: A 34-year-old male patient suffered intracranial hemorrhage and developed right-sided hemiplegia and aphasia. Angiography revealed a left corpus callosum AVM supplied by the

Drainage of brain extracellular fluid into blood and deep cervical lymph and its immunological significance.

Cerebral extracellular fluids drain from brain to blood across the arachnoid villi and to lymph along certain cranial nerves (primarily olfactory) and spinal nerve root ganglia. Quantification of the connection to lymph in rabbit, cat and sheep, using radiolabelled albumin as a marker of flow, indicates that a minimum of 14 to 47% of protein injected into different regions of brain or cerebrospinal fluid passes through lymph. The magnitude of the outflow to lymph is at variance with the general assumption that the absence of conventional lymphatics from the brain interrupts the afferent arm of

"Floppy brain" in congenital hydrocephalus.

Hydrocephalus is classically considered to be a disorder of altered cerebrospinal fluid (CSF) circulation, leading to the dilation of cerebral ventricles. Here, we report a clinical case of a patient who presented with fetal-onset hydrocephalus with diffusely reduced cortical and white matter volumes resulting from a genetic mutation in L1CAM, a well-known hydrocephalus disease gene involved in neuronal cell adhesion and axon development. After CSF was drained from the ventricle intraoperatively, the patient's cortical mantle collapsed and exhibited a "floppy" appearance on neuroimaging, sugge

A route for cerebrospinal fluid flow through leptomeningeal arterial-venous overlaps enables macromolecule and fluid shunting.

The flow of cerebrospinal fluid (CSF) is important for conveying brain-derived macromolecules for signaling and enabling them to be drained from the brain parenchyma. The glymphatic route is the best-characterized means of this CSF flow; however, it does not permit the movement of larger macromolecules. Here, we identify in mice an alternative route whereby intra-CSF-injected macromolecules can traverse from periarterial to perivenous spaces, with transfer occurring at sites of overlap between leptomeningeal perivascular (arteriovenous) spaces dispersed across the surface of the brain's leptom

Aggregatibacter aphrophilus and Eikenella corrodens: a case of brain abscess.

We report a case of a 26-year-old female who developed a brain abscess that was strongly suspected to be caused by Staphylococcus epidermidis, A. aphrophilus, and E. corrodens species. In general, A. aphrophilus and E. corrodens, members of the HACEK group (Haemophilus spp., Aggregatibacter spp., C. hominis, E. corrodens, and K. kingae), have been associated with the development of endocarditis, meningitis, sinusitis, otitis media, pneumonia, osteomyelitis, peritonitis, and wound infections. Cerebral abscesses are a rare manifestation of these bacteria; only a few cases are described in the li

Chronic Lymphocytic Leukemia Resulting in Hemorrhagic Brain Masses After Sepsis.

Chronic lymphocytic leukemia (CLL) rarely results in central nervous system (CNS) involvement. When CLL does affect the CNS, it typically manifests as leptomeningeal involvement, not commonly causing parenchymal involvement unless having undergone a higher grade transformation. We report a case of a patient with untreated CLL who presented with a large right frontal hemorrhagic mass along with additional bilateral masses after being found unresponsive. He had recently been hospitalized with Staphylococcus aureus sepsis. His neurological examination improved after resection of the largest mass

Traumatic brain edema induced by ventricular puncture. A study by magnetic resonance imaging.

Magnetic resonance imaging demonstrates after ventricular catheterization a focal brain hypersignal corresponding to a parenchymal edema along the drain track. In the course of our daily clinical activity, this hypersignal extension seemed more pronounced when catheterizing the frontal area than the junctional parieto-temporo-occipital parenchyma (or trigonal area). In order to confirm this impression, we prospectively studied ten consecutive patients with normal pressure hydrocephalus in whom both of these brain regions were successively catheterized first by a frontal puncture for intracrani

Cervical lymphadenectomy impairs brain lymphatic clearance and synaptic proteostasis.

Meningeal lymphatic vessels drain brain fluid and waste into the cervical lymph nodes, but how removing these nodes during surgery affects brain health is unknown. Retrospective analysis of cancer patients who underwent neck lymphadenectomy without chemo-radiation therapy found that up to 25% developed mild-to-severe cognitive impairment, and neuroimaging showed accelerated temporal horn enlargement after bilateral surgery. To determine mechanistically how cervical lymphadenectomy contributes to cognitive impairment, mouse and rat models were utilized to phenocopy abrupt disruption of lymphati

Boosting Preclinical Parkinson's Disease Research in Africa.

Parkinson's disease (PD) is the fastest-growing neurodegenerative disease in the world and appears to be an emerging epidemic in Africa, where counteractive measures have become necessary. Previous reports have highlighted the limited epidemiological and clinical PD research in Africa but overlooked the poor preclinical PD research output of the continent. Because preclinical research is a bedrock for translating basic scientific research into clinical practice, a weak preclinical research foundation can hamper advancement in epidemiological and clinical investigations. The sparsity and low im

Cerebrospinal fluid venous fistula: Illustrative case.

BACKGROUND: Spinal cerebrospinal fluid (CSF) venous fistulas are an under-recognized cause for spontaneous intracranial hypotension (SIH) which may result in postural headaches. CASE DESCRIPTION: A 60-year-old-male presented with 6 years of a persistent headache. The initial brain and spine MRIs and conventional CT myelogram (CTM) showed no CSF venous fistula. However, the lateral decubitus dynamic CTM demonstrated a hyperdense paraspinal vein on the right at the T10-11 level consistent with a CSF venous fistula. It was subsequently successfully treated with surgical ligation. Temporary CSF d

Isolated spinal neurocysticercosis : unusual ocular presentation mimicking pseudotumor cerebri.

Spinal intradural cysticercosis is a rare manifestation of neurocysticercosis. We report a unique patient who showed visual symptoms and normal imaging of the brain caused by isolated spinal neurocysticercosis. A 59-year-old male patient was admitted to the emergency unit with a history of severe headache and progressive blurred vision. Brain computed tomographic scanning and magnetic resonance imaging showed normal cerebral anatomy without hydrocephalus. The fundoscopic evaluation by an ophthalmologist showed bilateral papilledema. Perimetry studies revealed visual field defects in both eyes.

Acute spinal cord swelling in a child with Chiari II malformation.

We report on a patient with Chiari II malformation who developed acute cervical myelopathy and brainstem dysfunction following a shunt failure. A brain and spine magnetic resonance image obtained immediately after admission evidenced swelling of the medulla and upper cervical spinal cord. After emergency placement of an external ventricular drain, the swelling receded, the child's respiratory pattern improved and the motor function of his upper and lower extremities progressively returned to its normal condition. The case reported here suggests that in patients with Chiari type II malformation

[Dandy-Walker syndrome].

Two cases of this syndrome occurring in children aged 1.5 and 3 years are reported. In one case, no other abnormalities were present, and the deformity did not manifest itself clinically before the child had fallen from a height. In the other, a cyst of the fourth ventricle coexisted with other abnormalities in the brain which were responsible for neurologic symptoms from the very birth and also, probably, for the atresia of draining orifices.

Cerebrospinal fluid can exit into the skull bone marrow and instruct cranial hematopoiesis in mice with bacterial meningitis.

Interactions between the immune and central nervous systems strongly influence brain health. Although the blood-brain barrier restricts this crosstalk, we now know that meningeal gateways through brain border tissues facilitate intersystem communication. Cerebrospinal fluid (CSF), which interfaces with the glymphatic system and thereby drains the brain's interstitial and perivascular spaces, facilitates outward signaling beyond the blood-brain barrier. In the present study, we report that CSF can exit into the skull bone marrow. Fluorescent tracers injected into the cisterna magna of mice migr
