Efficacy and safety-related factors of BTK inhibitors as a bridge to CAR-T therapy in R/R FL.

Although anti-CD19 chimeric antigen receptor (CAR) T cell therapy has achieved satisfactory results in relapsed/refractory (R/R) follicular lymphoma (FL), patients with R/R FL and high-risk disease characteristics, previous hematopoietic stem cell transplantation, bulky disease, and progression of disease within 2 years (POD24) had a low complete response (CR). Twenty-seven patients with R/R FL, later disease stages, higher tumor burden, or higher previous treatment lines who had received Bruton tyrosine kinase (BTK) inhibitors before anti-CD19 CAR T cell therapy, or received BTK inhibitors as

Renal Replacement Therapy for Acute Kidney Injury in Severe Alcohol-Associated Hepatitis as a Bridge to Transplant or Recovery.

BACKGROUND: Acute kidney injury is seen in approximately 30% of patients with severe alcohol-associated hepatitis (AH) and is associated with increased mortality. Controversy exists surrounding initiation of renal replacement therapy (RRT) in these patients, as most are ineligible for early transplantation. AIMS: The primary aim was to identify predictors of survival and identify patients who may benefit from RRT as a bridge to transplant or recovery. METHODS: A retrospective multicenter cohort of adult patients with AH, who received RRT, was developed, including patients from two North Amer

Temporary ventricular assist device implantation by sternotomy-avoiding technique for bridge-to-decision therapy: a comparison with conventional implantation.

OBJECTIVE: Temporary ventricular assist device (VAD) is a commonly used therapeutic option for cardiogenic shock. Patients requiring this treatment are often critical, and clinical outcomes remain unsatisfactory. This study evaluated the feasibility and efficacy of a sternotomy-avoiding technique for temporary VAD implantation to improve patient outcomes. METHODS: Between December 2012 and November 2018, seven patients underwent temporary VAD implantation by sternotomy-avoiding technique (SA group) and eight by median sternotomy technique (MS group). Pre- and intraoperative characteristics, p

Long-term outcomes of stereotactic body radiation therapy in the treatment of hepatocellular cancer as a bridge to transplantation.

Hepatocellular carcinoma (HCC) is potentially curable with hepatic resection or transplantation. Few patients are eligible for resection, and many face a long wait for donor organ availability for liver transplantation. Here we report the safety and efficacy of stereotactic body radiation therapy (SBRT), the explant pathology findings and survival of patients treated with SBRT as a bridge to transplantation for HCC. From April 2005 to August 2010, 10 patients with 11 HCCs were treated with SBRT as a bridge to transplantation. All patients were evaluated by a liver transplant surgeon before rad

Stereotactic body radiation therapy as bridging or downstaging therapy for hepatocellular carcinoma in liver transplant candidates: A systematic review and meta-analysis.

Bridging and downstaging are indications for locoregional therapy (LRT) in hepatocellular carcinoma (HCC) within the context of liver transplantation (LT). Stereotactic body radiation therapy (SBRT) is an emerging LRT modality in these settings. We conducted a systematic review and meta-analysis to assess the efficacy and safety of SBRT as bridging or downstaging therapy. A comprehensive search of PubMed, Embase, and Cochrane Library was performed through July 21, 2025. We included randomized and non-randomized studies evaluating SBRT in LT candidates or potential candidates with HCC, reportin

Perioperative bridging therapy with low molecular weight heparin for patients with inherited thrombophilia and antiphospholipid syndrome on long-term acenokumarol therapy.

The aim of the study is to present our own perioperative bridging therapy with low molecular weight heparin (LMWH) for surgical patients with thrombophilia on long-term acenokumarol therapy [oral anticoagulant (OAC)]. In some European countries, the drug used in secondary antithrombotic prophylaxis is acenokumarol. Forty-two patients with inherited thrombophilia and 21 with antiphospholipid syndrome underwent surgery. All patients were on long-term OAC. This OAC was interrupted 2 days before elective surgery and since that day half of the individual therapeutic dose of LMWH was administered. O

Optimizing Selection of Bridging Therapies Prior to CAR-T Therapy Administration for Multiple Myeloma: Clinical Pearls From an Expert Roundtable.

Autologous chimeric antigen receptor T-cell (CAR-T) therapies have become a key component of the multiple myeloma (MM) treatment landscape. Most patients treated with CAR-T therapies for MM receive bridging therapy to reduce tumor burden prior to CAR-T administration. These treatments are highly individualized, with selection informed by disease characteristics and treatment history, among other factors. While pivotal clinical trials restricted use of bridging therapy to specific regimens, a wide variety of anti-myeloma therapies have been used in real-world settings. Given potential variabili

Bridging the gap to clinical translation: the advanced role of PET molecular imaging in stem cell therapy.

In the past few decades, stem cell therapy has achieved remarkable progress in preclinical and clinical trials, especially for treating major diseases, such as stroke, Parkinson’s disease (PD), myocardial infarction, diabetes, etc (Temple, Cell Stem Cell 30:512-29, 2023). However, the current challenge in clinical translation lies in the difficulty in non-invasively, dynamically, and quantitatively assessing the survival, proliferation, homing, migration, and function of stem cells in vivo. This lack of assessment leads to inconsistent therapeutic effects, unclear mechanisms, and difficulties

Bridging the gap in cell and gene therapy: addressing operational and financial barriers to achieve sustainable access.

Cell and gene therapies (CGTs) represent a major transformation in modern medicine, offering the potential for cure in patients with serious and treatment-refractory diseases. However, despite this promise, real-world delivery remains highly constrained. This white paper synthesizes insights from a multidisciplinary advisory council of 21 healthcare professionals involved in CGT delivery, together with the authors' collective experience, to examine operational and financial barriers from a provider perspective and to identify strategies to support sustainable expansion of CGT programs across d

Emerging Multi-Target Therapies for Type 2 Diabetes: Bridging Drug Innovation and Precision Delivery.

Diabetes is a long-term metabolic disorder characterized by elevated blood glucose levels and is primarily classified into Type 1 Diabetes Mellitus (T1DM) and Type 2 Diabetes Mellitus (T2DM). Conventional drug delivery systems often face limitations such as low bioavailability, inadequate target specificity, and the need for frequent dosing. Drug targeting offers significant advantages in diabetes treatment by enhancing therapeutic efficacy and reducing side effects. This is achieved by binding drug-loaded carriers to specific receptors on insulin-sensitive tissues or pancreatic β-cells, ensur

CAR T‑Cell Therapy as a Bridging Strategy to Second Transplant in Relapsed B‑Cell ALL.

BACKGROUND: A second allogeneic transplant (HSCT2) may offer cure for selected patients with acute lymphoblastic leukemia (ALL) relapsing after first transplant (HSCT1), however, outcomes have historically been poor, largely driven by high incidence of subsequent relapse. Chimeric antigen receptor (CAR) T‑cell therapy induces deep and durable remissions in relapsed/refractory (R/R) ALL, yet its role as a bridge to HSCT2 remains under studied. Consolidation with transplant after CAR-T therapy may enhance post‑HSCT2 disease control and improve outcomes. OBJECTIVES: To compare HSCT2 outcomes acc

Burosumab as bridging therapy in autosomal dominant hypophosphatemic rickets with femoral fracture nonunion and iron deficiency.

Autosomal dominant hypophosphatemic rickets (ADHR) is a rare genetic disorder, characterized by fibroblast growth factor 23 (FGF23)-mediated renal phosphate wasting and impaired bone mineralization. Iron deficiency in ADHR increases FGF23 production and can worsen hypophosphatemia. Evidence to guide optimal management in severe adult cases remains limited. We report the use of burosumab as bridging therapy in a 19-yr-old woman with genetically confirmed ADHR (FGF23 p.Arg176Trp), severe osteomalacia, iron-deficiency anemia, and a low-energy femoral diaphyseal fracture complicated by nonunion de

Gamified Cardiopulmonary Rehabilitation in a Child Undergoing LVAD Bridging Therapy to Heart Transplantation: A Case Report.

This report details the comprehensive, gamified cardiopulmonary rehabilitation of a 6-year-old girl undergoing a complex clinical journey from left ventricular assist device (LVAD) bridge therapy to successful heart transplantation. Given the paucity of standardised protocols for this high-risk population, a structured, multidisciplinary team (MDT)-guided pathway was implemented, adapting interventions to the distinct phases of LVAD support and post-transplant recovery. Key nursing and rehabilitation strategies included initiating very early mobilisation in the cardiac intensive care unit (CIC

Wearable Cardioverter-Defibrillator in Adult Congenital Heart Disease: A Bridge to Decision, Recovery, and Advanced Therapies.

Sudden cardiac death (SCD) remains a major cause of mortality among adults with congenital heart disease (ACHD). Risk stratification for malignant ventricular arrhythmias in this population is challenging because of the heterogeneity of anatomical substrates, lifelong haemodynamic changes, and the limited predictive accuracy of currently risk models. Furthermore, implantation of an implantable cardioverter-defibrillator (ICD) in ACHD patients is frequently complicated by anatomical constraints, limited venous access, and a significant burden of device-related complications, including infection

Hormone replacement therapy in gynecologic cancer survivors: bridging the gap between evidence and clinical practice.

Advances in gynecologic oncology have substantially improved survival, resulting in a rapidly growing population of long-term survivors. However, surgery, chemotherapy, and radiotherapy frequently induce iatrogenic menopause or exacerbate pre-existing menopausal symptoms, leading to impaired quality of life and increased long-term risks, including osteoporosis, cardiovascular disease, and sexual dysfunction. Despite increasing evidence supporting the oncologic safety of hormone replacement therapy in selected patients, its use remains inconsistent because of uncertainty regarding patient selec

Bridging the Gap: A Systematic Review of Modern Hemophilia Therapies and Global Inequities in Clinical Trial Participation.

INTRODUCTION AND OBJECTIVES: Hemophilia management has evolved from conventional factor replacement to non-factor therapies and gene therapy. However, most clinical trials of modern therapies are conducted in high- and upper-middle-income countries, with limited participation from low- and middle-income regions. This disparity reflects broader inequities in access to advanced treatments. This study summarizes the clinical development timeline, regulatory status, methodological approaches, and global distribution of clinical trials on modern hemophilia therapies. METHODS: A PRISMA-guided syste
