We noted periodic alternating nystagmus (PAN) in two patients (mother and daughter) who were diagnosed as familial congenital cerebellar ataxia. The mother was 51-year-old and her PAN had about 4 minutes cycle. During eye closure there was a deviation of arms and trunk parallel to the direction of the slow phase of nystagmus. Vestibular stimulation during rotatory and caloric tests disclosed enhanced vestibulo-ocular reflexes, changes of direction and prolonged cycle of PAN. The patient had no visual suppression during caloric stimulation. The daughter was 14-year-old and although she had no s

The combined galvanic test (CGT) was developed in our department for simultaneous recording of galvanic eye movement test (GEMT) and galvanic body sway test (GBST). Twelve cases of vestibular neuronitis were studied first with the caloric test, and then with the CGT. The findings of the caloric test corresponded to those of the GEMT, but did not always correspond to those of the GBST. Thus we suggest that the GBST reflects an abnormality in a system different from that reflected by the caloric test and GEMT. It is likely that the caloric response mainly originates from the semicircular canal s

BACKGROUND: To diagnose acoustic neuromas (ANs), the auditory brainstem response test and the caloric test have been used in addition to magnetic resonance imaging. The auditory brainstem response and the caloric tests mainly reflect functions of the auditory pathway, ie, the cochlear nerve and the superior vestibular nerve, respectively. Because the vestibular evoked myogenic potential (VEMP) has been thought to originate in the inferior vestibular nerve, we hypothesized that the VEMP could provide different information from the auditory brainstem response and the caloric test and that it cou

Fifteen panic disorder patients with prominent dizziness underwent audiologic, caloric, and vestibuloocular reflex activity testing and were compared with normal controls. There were no abnormalities detected on audiologic and caloric tests. Compared with normal controls, panickers with dizziness showed a greater discrepancy between eye and head movements on vestibulo-ocular reflex activity in the dark. Panickers with prominent dizziness did not differ from a second control group of panickers with severe heart palpitations on a number of psychological tests. The results did not support the hyp

OBJECTIVE: Motion sickness is poorly understood, although it has been recognized for years as debilitating. Vestibular function is required for motion sickness to occur, but motion sickness can also be brought on without body motion. The aim of this study was to see if there was a correlation between caloric response and motion sickness susceptibility.
DESIGN: One experiment was a prospective study carried out on 200 patients. A second prospective study was carried out on 121 patients.
SETTING: Patients referred to our tertiary/quaternary care dizziness clinic.
METHODS: In experiment 1, cal

UNLABELLED: Pusher syndrome is an interesting disorder of balance in patients with encephalic lesions characterized by the peculiar behavior of actively pushing away from the non-hemiparetic side and resisting against passive correction, with a tendency to fall toward the paralyzed side. The role of vestibular system on the pushing behavior is not clear.
AIM: To evaluate horizontal semicircular canal function in patients with Pusher syndrome, using caloric and rotation tests.
STUDY DESIGN: Observational prospective.
MATERIAL AND METHOD: We evaluated 9 inpatients with stroke and Pusher syndr

Multiple sclerosis is characterized by the presence of multiple plaques within the central nervous system, manifesting as remission and exacerbation of neurologic dysfunction over variable time courses. We present the case of a 20-year-old woman. Before treatment, her auditory brain stem response (ABR) test revealed bilateral prolongation. A caloric test showed canal paresis of the right ear and a normal response on the left. A vestibular evoked myogenic potential (VEMP) test displayed an absent response in the right ear and a delayed response in the left. A magnetic resonance imaging (MRI) sc

The diagnostic value of routine clinical tests for predicting the presence of an acoustic neuroma (AN) was assessed retrospectively in a group of 391 patients having magnetic resonance imaging (MRI) scans because of a suspected retrolabyrinthine lesion. An AN was found by MRI only in 9% of these patients. The positive predictive value of unilateral sensorineural hearing loss in a pure-tone audiogram (PTA) and unilaterally impaired caloric response was 9% and 12%, respectively. In contrast, AN was found in 36% of patients with pathological brainstem evoked response audiometry (BE-RA). A normal

Results of electronystagmographic (ENG) position and bithermal caloric tests from patients with abnormally enlarged summating potentials (SPs) were compared with results from a unilateral weakness (UW)-matched ENG-comparison group and with a group of patients with unilateral cochlear deficits and dizziness but without enlarged SPs. The patients with enlarged SPs had significantly higher incidence of caloric UW on the involved side than did the patients with cochlear deficits but without enlarged SPs. Also, the patients with SP enlargement had a significantly higher incidence of directional pre

OBJECTIVES: Recent studies suggest that hearing loss is associated with balance dysfunction and an increased risk of falls. The purpose of this study was to investigate whether adults with unilateral hearing loss (UHL) have an underlying vestibular dysfunction and whether vestibular testing explains balance function and fall risk.
STUDY DESIGN: Cross-sectional.
SETTING: Urban otology practice and a human motion laboratory.
PATIENTS: We recruited 41 healthy controls (mean age, 52 years; range, 22-78 years) and 29 adults with stable UHL (mean age, 53 years; range, 18-80 years) who denied symp

Cochlear and vestibular functions of 30 patients suffering from Méniere's disease have been analyzed through audiometric and electronystagmographic studies. Auditive thresholds in low, medium and high frequencies have been correlated with the results of the caloric and pendular tests. Statistical analysis has demonstrated that there is no significant relationship between caloric and pendular reflectivity and hearing loss in the mentioned frequencies. In conclusion, in Méniere's disease hearing losses and vestibular alterations follow a different pathologic pattern.

OBJECTIVE: Combined use of vestibular evoked myogenic potential (VEMP) and caloric response testing has enabled us to examine the function of the inferior and superior vestibular nerves separately. Although results of VEMP testing and caloric response testing have been reported for many diseases, a clinical entity showing abnormal VEMP responses but normal caloric test responses has rarely been reported. The aim of the study was to investigate clinical features of diseases showing abnormal VEMP responses with normal caloric test responses.
STUDY DESIGN: Retrospective.

UNLABELLED: Response inversion during warm air stimulation is the most controversial finding seen in caloric tests of individuals with tympanic membrane perforation. In such cases, very few studies explore the possible interferences found in the caloric test, bringing about controversies in the interpretation of test results.
AIM: This paper aimed at analyzing warm air stimulation effects in individuals with tympanic membrane perforation in comparison with normal healthy controls.
MATERIALS AND METHODS: Prospective, non-randomized study in which 48 individuals without vestibular complaints w

This study presents the neuro-otological findings of 523 patients attending a tertiary vestibular clinic with migraine- and nonmigraine-related dizziness. Subjects were categorized into one of 4 groups, definite migrainous vertigo, probable migrainous vertigo, vestibular disorder coexisting with migraine and nonmigraine-related dizziness. No notable relationship was found between the numbers of abnormal findings between the groups for the majority of the neuro-otological tests. However, there was a significant trend in emetic response to caloric testing. The definite migrainous vertigo group w

It is well known that inverted caloric nystagmus is seen during air caloric testing in cases of chronic otitis media. The mechanism of inversion and its clinical significance are discussed here. Temperature changes in the tympanic cavity and external ear canal were measured with a microthermister and a digital tester in seventeen ears with tympanic membrane perforation, during bithermal air caloric testing. The tympanic cavity mucosa was cooled by hot stimulation because of the evaporation of heat. When the perforation was closed or humidified air was used, the tympanic cavity mucosa was not c

OBJECTIVES: To determine the clinical significance of an isolated directional preponderance (DP) on bithermal caloric testing. An isolated caloric DP was defined as a DP, calculated according to the standard Jongkees formula, of > or = 40%, with a spontaneous nystagmus (SN) in darkness of < or = 2 degrees/s and a canal paresis (unilateral weakness) of < or = 25%.
STUDY DESIGN: A retrospective analysis of all 15,542 bithermal caloric tests performed in the authors' department in the previous 10 years to identify all tests with an isolated DP of > or = 40%. This was followed by a review of the
