Opacification of the posterior capsule caused by residual lens epithelial cells (LEC) is still the most frequent long-term complication of cataract surgery. Beside the opacification of the visual axis with posterior capsule opacification (PCO), resulting in a decrease in visual function, fibrotic changes may also have a mechanical effect on intraocular lens (IOL) position such as axial shift, decentration, tilt and capsule striae. In this article, two types of capsular fibrosis are explored, on the one hand the anterior capsule fibrosis and on the other hand PCO. Results from clinical trials c

Posterior capsular polishing is performed to delay or prevent capsular opacification. To study its effectiveness, a prospective study of 412 patients with bilateral senile cataracts was conducted. Each patient had bilateral extracapsular cataract extraction with laser ridge, posterior chamber lens implantation. The right eye of each patient had posterior capsular polishing using the Tennessee-Freeman polisher. The posterior capsule in the left eye was not polished. All patients above the age of 80 years (22 patients) had bilateral opacification within 16 months postoperatively. For patients le

Posterior-capsule opacification, by far the most common complication of primary cataract surgery, continues to stimulate important work toward understanding its causes, preventing it, and effectively treating it. Of special note here are a report by Koch and Kohnen that a combination of vitrectomy and posterior capsulorhexis is required to inhibit posterior-capsule opacification in pediatric patients; work by Nishi et al. toward the dream of replacing the cataractous lens with a flexible artificial lens, supported by the natural capsular bag; and methods by Tetz et al. and Pande et al. for pre

Cataract is the leading cause of blindness worldwide. It can be treated by surgery, whereby the damaged crystalline lens is replaced by a synthetic lens. Although cataract surgery is highly effective, a relatively common complication named posterior capsular opacification (PCO) leads to secondary loss of vision. PCO is caused by abnormal proliferation and migration of residual lens epithelial cells (LECs) that were not removed during the surgery, which results in interruption to the passage of light. Despite technical improvements to the surgery, this complication has not been eradicated. Effo

We present a case of light-blue posterior capsule opacification (PCO) in a patient who had cerulean cataracts removed 1 year earlier. The color of the PCO was similar to that of the cerulean cataracts prior to extraction. Posterior capsule opacification is a common complication after cataract surgery; however, we could not find a similar case of cerulean posterior capsule opacity in the literature. The findings in this case suggest that the mechanism of the light-blue color formation in the cataract was also present in the lens epithelial cells forming the posterior capsule opacity.

Posterior capsular opacification (PCO) is the main postoperative complication after intraocular lens (IOL) implantation in cataract surgery, because of the proliferation of the residual lens epithelial cells (LECs) in the lens capsule. Drug-eluting IOLs, aimed to develop an in situ drug delivery device, are the promising concept in recent years. As IOLs are optical devices other than implants, the feasibility and applicability remain a challenge for drug-eluting coatings. In this investigation, a centrifugally concentric ring-patterned drug-loaded poly(lactide-co-glycolic acid) (PLGA) coating

Posterior capsular opacification (PCO) is the most common complication after cataract surgery, which is primarily caused by the proliferation of the residual lens epithelial cells (LECs) in the lens capsule. Previous studies have demonstrated that a drug-eluting intraocular lens (IOL), aimed to in situ eliminate LECs, are an effective and promising way to prevent PCO. However, because of the potential toxicities of the antiproliferative drugs to the adjacent tissues, the safety of such drug-eluting IOLs is still a highly important issue to be solved. In this investigation, a facile photodynami

Cataracts are well known to be associated with myotonic dystrophy. Less well known are the phenomena of recurrent posterior capsule opacification and capsulorhexis contracture post cataract surgery. Two cases are described herein of postoperative capsular complications requiring multiple capsulotomies in patients with myotonic dystrophy. It is proposed that a common aetiology may underlie both posterior capsule opacification and capsulorhexis contracture in myotonic dystrophy cases.

PURPOSE: Posterior capsular opacification is the most common surgically related cause of reduced vision after cataract surgery. We studied the effect of capsulorhexis size on the pattern and severity of posterior capsular opacification.
METHODS: In this prospective study 75 patients underwent standardized phacoemulsification with capsulorhexis and in-the-bag placement of a 5.5-mm polymethylmethacrylate intraocular lens implant. The patients were randomly assigned to receive either a small capsulorhexis of 4.5 to 5 mm to lie completely on the intraocular lens optic or a large capsulorhexis of

Opacification of the posterior capsule is the most common post-operative complication of extracapsular cataract surgery. We studied the sequential changes in the posterior capsule of the aphakic cat and monkey after extracapsular surgery using light microscopy, scanning electron microscopy, and transmission electron microscopy. In the cat, but not the monkey, there was proliferation of a pigment-containing membrane arising from the iris root and ciliary body, and extending onto the residual lens capsule. In both animals there was transformation of residual lens epithelial cells to fibroblasts

BACKGROUND: Capsular block syndrome is an uncommon complication that occurs after cataract surgery. It is characterized by capsular distension, anterior intraocular lens displacement, anterior chamber shallowing, and unexpected myopic shifts. We report a case of recurrent fibrotic capsular block syndrome with Elschnig's pearl-type posterior capsule opacification 10 months after neodymium-yttrium-aluminum-garnet (Nd:YAG) laser anterior capsulotomy.
CASE PRESENTATION: A 72-year-old Asian man complained of decreased visual acuity 5 years after undergoing phacoemulsification with posterior chambe

After-cataract can be defined as the retropupillary, non-vitreal opacification of the lens remnants occurring after extracapsular cataract extraction (ECCE) leading to disturbance of transparency and impairment of vision. The synonymous secondary cataract is confusing, since it is also and more frequently used as cataract secondary to ocular diseases (Duke-Elder 1969). The synonymous opacification of the posterior capsule is, in fact, a misnomer, since histological observations revealed that after-cataract is not proven to be the result of changes in the remaining posterior capsule itself, but

From 1984 to 1986, we performed cataract surgery through a 1.5 mm anterior capsule hole in 77 eyes. The hole was enlarged to 6.0 mm by a slit incision, and an intraocular lens was implanted into an almost completely intact capsular bag. Twelve (16%) eyes developed severe postoperative anterior capsule opacification. The opacified central anterior capsule, approximately 5 mm in diameter, was detached by can-opener anterior capsulotomy using a neodymium:YAG laser and fell into the inferior anterior chamber. Inferior corneal endothelial cell loss occurred in nine of the 12 eyes within 20 months a

Posterior capsular opacification (PCO) is the leading complication after cataract surgery, and is mainly induced by the proliferation and migration of residual lens epithelial cells (LECs). Although numerous attempts have been made to reduce the incidence of PCO, this complication remains a critical challenge in postoperative visual recovery. This study aims to report a functionalized intraocular lens (R-IOL) with a region-confined photothermal effect for the active prevention of PCO after implantation. The outer rim of R-IOL (non-optical area) is decorated with a nanoporous gold (NPG) ring, w
