Predictors and Outcome of Extracorporeal Life Support After Pediatric Heart Transplantation.

BACKGROUND: Extracorporeal life support (ECLS) has proven success after conventional cardiac surgery. Its use after pediatric heart transplantation is less well documented. We reviewed ECLS after pediatric heart transplantation, to understand better predisposing factors, morbidity, and mortality. METHODS: The notes of all patients at Great Ormond Street Hospital undergoing orthotopic heart transplantation from 1999 to 2009 were reviewed (202 transplants; patients aged 0.06 to 17.91 years). Patients were grouped by diagnosis: restrictive cardiomyopathy (n = 17), nonrestrictive cardiomyopathy (

Primary cardiac amyloidosis: A case presentation.

A young man with primary cardiac amyloidosis presented clinically as a case of 'restrictive' cardiomyopathy with an initial diagnosis of constrictive pericarditis, probably of tuberculous origin. Cardiac catheterization showed features compatible with restrictive cardiomyopathy or constrictive pericarditis but pericardial biopsy was negative. Percutaneous right ventricular endomyocardial biopsies established amyloid infiltration of the myocardium, but rectal, tongue and liver biopsies were all negative for amyloidosis. Bence-Jones protein was absent from the urine. Serum electrophoresis, immun

Free-floating biatrial thrombi with concomitant saddle pulmonary embolism.

Free-floating intracardiac thrombi represent a severe yet uncommon condition and mostly coexist with thromboembolism. Notably, mobile right heart thrombus can occur in patients with massive pulmonary embolism. Early diagnosis and management can be life-saving. Spontaneous atrial thrombosis due to restrictive cardiomyopathy has also been rarely reported. We describe here an unusual case of free-floating biatrial thrombi with concomitant saddle pulmonary embolism in a patient with idiopathic restrictive cardiomyopathy and sinus rhythm who had lower extremity deep vein thrombosis following the le

An Update on Pediatric Cardiomyopathy.

PURPOSE OF REVIEW: This review summarizes the clinical characteristics and updated outcomes of primary pediatric cardiomyopathies including dilated (DCM), hypertrophic (HCM), and restrictive cardiomyopathy (RCM), and briefly discusses left ventricular non-compaction (LVNC) and arrhythmogenic cardiomyopathy (ACM), primarily arrythmogenic right ventricular cardiomyopathy (ARVC). RECENT FINDINGS: Pediatric cardiomyopathies are diseases of the heart muscle with an estimated annual incidence of 1.1-1.5 cases per 100,000. They are progressive in nature and are frequently caused by a genetic mutatio

Functional differentiation of various types of cardiomyopathies by echocardiography.

Echocardiography was used to compare the left ventricular function in 35 cases of congestive cardiomyopathy (COCM), 16 cases of obstructive and 28 cases of non-obstructive asymmetric septal hypertrophy, 43 cases of scleroderma heart disease, 21 cases of ischaemic cardiomyopathy (ICM), 2 cases of restrictive cardiomyopathy, and one case of cardiac amyloidosis. COCM was characterized by left ventricular dilatation, decreased contractility and signs of elevated end-diastolic pressure. In asymmetric septal hypertrophy there was a decreased distensibility of the obstructed left ventricle, both in e

[Restrictive myocardiopathy in children. A study of 4 patients].

Restrictive cardiomyopathy is an unusual form of cardiomyopathy during childhood. Only occasional cases or reduced populations have been reported in international journals. The purpose of this report is to document the clinical, echocardiographic and hemodynamic profile and outcome of restrictive cardiomyopathy in a group of four children (two boys and two girls). The mean age at admission was 3.5 +/- 2.7 years (range 1(3/12) to 7(10/12) years). The diagnosis was made by clinical, echocardiographic and hemodynamic criteria. The main symptoms were bronchial disease (two patients), hepatomegaly

[Heterotropic heart transplantation in young patients with elevated pulmonary vascular resistance].

UNLABELLED: Right heart failure due to elevated PVR is one of the major causes of mortality and morbidity after orthotopic heart transplantation. In 5 patients (median age 14 years) with dilated or restrictive cardiomyopathy and important elevation of the PVR, a heterotopic heart transplantation was performed using the technique reported by Yacoub (the donor pulmonary artery was implanted on the recipient right atrium). All the patients presented with at least two of the following parameters: PVR/m2 > 6 U, transpulmonary gradient > 15 mmHg, mean pulmonary pressure > 50 mmHg. One patient with r

Cardiac and skeletal muscle abnormalities in cardiomyopathy: comparison of patients with ventricular tachycardia or congestive heart failure.

Results of cardiac muscle and skeletal muscle biopsies were compared in 22 patients with cardiomyopathy; 11 patients presented with symptoms secondary to ventricular tachycardia (Group 1) and 11 had symptoms of severe congestive heart failure (Group 2). No patient had structural or ischemic cardiac disease. In Group 1 patients, hemodynamic abnormalities were subtle, but invasive study demonstrated dilated cardiomyopathy in two patients and restrictive cardiomyopathy in nine. In Group 2, eight patients had dilated cardiomyopathy and three had restrictive cardiomyopathy. Cardiac biopsy results w

Generation of three TTN knock-out human induced pluripotent stem cell lines using CRISPR/Cas9 system.

TTN mutations are the common genetic cause for various types of cardiomyopathies (e.g., dilated cardiomyopathy, hypertrophic cardiomyopathy, restrictive cardiomyopathy, and arrhythmogenic right ventricular cardiomyopathy) and skeletal myopathies. Here, we generated three TTN knock-out human induced pluripotent stem cell (iPSC) lines using CRISPR/Cas9 system. These cell lines, which exhibit normal karyotype, typical morphology and pluripotency, could provide useful platform for investigating the role of TTN in associated disorders.

Pathology of the cardiomyopathies.

The following article reviews the anatomic and pathologic basis of various cardiomyopathies and illustrates the dilated, hypertrophic, and restrictive types in various echocardiographic planes. Two types of cardiomyopathy are defined: (1) heart muscle disease of unknown cause (idiopathic, primary) and (2) heart muscle disease of known cause or associated with disorders of other organ systems (secondary). Of the cardiomyopathies of unknown cause, the dilated type is the most common. The anatomic diagnosis of hypertrophic cardiomyopathy is based on several morphologic features, the most importan

[Current knowledge of cardiomyopathy].

In 1961, 1964 and 1970, Goodwin proposed a somewhat unwield definition of cardiomyopathies, which was subsequently shortened to "Heart muscle disease of unknown cause" and separated from heart muscle diseases due to conditions involving organs other than the heart: the "specific heart muscle diseases". The cardiomyopathies thus defined were divided into hypertrophic, dilated and restrictive types and their classification was confirmed by the WHO/ISFC Task Force and the report of the WHO Expert Committee on cardiomyopathies in 1984. The cardiomyopathies are now classified by the dominant pathop

Clinical management of amyloid cardiomyopathy.

Clinical heart failure, restrictive cardiomyopathy, and arrhythmias are hallmark features of amyloid cardiomyopathy. In contrast to the advancements in targeted therapies, there is a general lack of evidence-based practice guidelines for clinical management of amyloid cardiomyopathy. In this review, we review the role of routine medical therapy in amyloid cardiomyopathy, from heart failure management to orthostatic hypotension, atrial arrhythmias, thromboembolic complications, and prevention of sudden death. We conclude by discussing approaches to patients with end-stage disease.

How many patients with dilated cardiomyopathy may potentially benefit from cardiac resynchronization therapy?

The clinical and electrocardiographic Marburg Cardiomyopathy database was analyzed to identify potential candidates for cardiac resynchronization therapy (CRT) with biventricular or left ventricular pacing among 566 patients with dilated cardiomyopathy (DCM). All of the following restrictive selection criteria were fulfilled by 38 patients (7%): NYHA functional class > or = 3 (n = 193, 34%), left ventricular ejection fraction (LVEF) < 30% (n = 238, 42%), sinus rhythm (n = 437, 77%), left bundle branch block (LBBB, n = 142, 25%), and QRS duration > or = 150 ms (n = 136, 24%). Heart transplantation in a patient with chloroquine-induced cardiomyopathy.

We present the first report of a patient who underwent heart transplantation (HT) after endomyocardial biopsy (EMB) and revealed chloroquine-induced cardiomyopathy (CIC). This patient, who was treated with chloroquine for 6 years, developed a restrictive cardiomyopathy that progressed to congestive heart failure (CHF) resistant to medical management.

Cardiomyopathy: how far have we come in 25 years, how far yet to go?

Twenty-five years ago clinical investigators began to appreciate that cardiomyopathy is an important and reasonably common form of heart disease. Since then, several functional classifications have been proposed, the specific myocardial diseases have been classified and chronic ischemic ventricular failure has been described. The boundary separating myocarditis from dilated cardiomyopathy remains hazy and, despite intensive research, the causes of dilated cardiomyopathy remain obscure. In particular, we still do not understand the role that may be played by viral infection and alcohol. Myocard

Endomyocardial Fibrosis Associated With Myocardial Infarction.

Endomyocardial fibrosis (EMF) is the most common cause of restrictive cardiomyopathy in Africa and South America. We present a case of a 55-year-old man who experienced an anterior myocardial infarction due to thrombotic occlusion of the left anterior descending artery. Multimodality imaging revealed left ventricular (LV) apical obliteration, apical thrombus, and subendocardial fibrosis, without significant systolic dysfunction, consistent with EMF. EMF should be suspected in patients presenting with heart failure symptoms because of restrictive physiology and/or thrombotic manifestations with

FLNC-Related Cardiomyopathies in an Admixed Population: Expanding the Genotype-Phenotype Spectrum and Clinical Correlates.

BACKGROUND: Pathogenic and likely pathogenic variants in filamin C (FLNC) cause a broad spectrum of cardiomyopathies. However, available genotype-phenotype data are derived predominantly from European and North American populations, with limited representation of admixed populations. METHODS: We characterized carriers of pathogenic/likely pathogenic FLNC variants identified among 2654 unrelated probands undergoing exome sequencing for suspected inherited cardiomyopathy and their relatives identified through cascade screening. Clinical, electrocardiographic, imaging, genetic, and longitudinal

Accurate Classification of Non-ischemic Cardiomyopathy.

PURPOSE OF REVIEW: This article aims to review the accurate classification of non-ischemic cardiomyopathy, including the methods, basis, subtype characteristics, and prognosis, especially the similarities and differences between different classifications. RECENT FINDINGS: Non-ischemic cardiomyopathy refers to a myocardial disease that excludes coronary artery disease or ischemic injury and has a variety of etiologies and high incidence. Recent studies suggest that traditional classification methods based on primary/mixed/acquired or genetic/non-genetic cannot meet the precise needs of contemp

Transcatheter Tricuspid Valve Edge-to-Edge Repair in Cardiac Wild-Type Transthyretin Amyloidosis.

BACKGROUND: Cardiac transthyretin amyloidosis (ATTR-CM) is an infiltrative, restrictive cardiomyopathy leading to heart failure across all stages of the disease. While specific disease-modulating therapies evolve, interventional therapeutic approaches on atrioventricular valve dysfunction have not yet been investigated in this cardiomyopathy and remain subject to personalized strategy. CASE SUMMARY: We present 2 patients with wild-type ATTR-CM who underwent successful transcatheter tricuspid valve edge-to-edge repair (T-TEER) for severe tricuspid regurgitation (TR) 2-4 years after initial dia
