The presence and roles of nurse navigators in acute care hospitals.

OBJECTIVE: A survey of hospital-based nurse executives was conducted to determine the extent, approaches, and outcomes of nurse navigator (NN) programs. BACKGROUND: Nurse navigators are distinct from other recognized healthcare roles. Navigators most commonly focus on a single health condition with the goal of improving the provision of specified health services for an individual patient. METHODS: An 11-question Internet-based survey was e-mailed to 580 nurse executives in Texas. RESULTS: Of the respondents, only 24% implemented any type of NN program. Most of the respondents with navigator

Implications of prosthetic valve implantation: an 8-year follow-up of patients with porcine bioprostheses.

Nurses in cardiovascular critical care settings routinely care for patients with implanted valvular prostheses. The presence of an artificial valve substitutes a new disease state for the preexisting valvular disease. Five hundred nine patients who underwent cardiac valve replacement with porcine bioprostheses and who were followed for a total of 1633 patient-years provide the data base for discussion of long-term survival, functional capacity, and morbidity associated with valvular prostheses. Seventy-two percent of patients survived 5 years after operation. New York Heart Association functio

Survival and determinants of mortality in nursing home residents with young onset dementia.

OBJECTIVES: People with young-onset dementia (YOD), defined as symptom onset before the age of 65, have mortality rates five to eight times higher than those of the general population of similar age. However, survival studies focused on individuals living in the community rather than those residing in nursing homes. This study aimed to estimate survival rates, its determinants, and causes of death in nursing home residents with YOD. DESIGN: Survival data from the BEYOnD (2005-2018) and Care4Youngdem (2016-2021) cohort studies. SETTING: YOD special care units of 20 nursing homes in the Nether

Nursing Home Admission and Initiation of Domiciliary Care Following Infective Endocarditis.

BACKGROUND: Infective endocarditis (IE) may cause debilitating physical and mental changes that can interfere with activities of daily living. Admission to a nursing home and need for domiciliary care following hospitalization for IE represent such relevant outcomes, yet no such data have been reported. METHODS: Using Danish nationwide registries, we identified all patients discharged alive after a first-time IE hospitalization in the period 1996 to 2014. These were matched by age, sex, calendar year, and relevant comorbidities with the background population in a 1:1 ratio. The 1-year rate of

Learning from experience: Revolutionizing vascular care through nursing morbidity and mortality conferences.

BACKGROUND: Traditional morbidity and mortality (M&M) conferences often lack nursing involvement or perspectives despite nurses being frontline caregivers. On a vascular surgery unit, pre-intervention data revealed concerning gaps in nurses' medical knowledge and low confidence in escalating patient concerns to the vascular team. PURPOSE: To implement a structured nursing-led M&M conference to improve nurses' clinical knowledge, care confidence, and interprofessional collaboration in vascular care. METHODS: A structured implementation approach was employed, utilizing a dedicated electronic p

Calcium channel blockers.

Calcium channel blockers are widely used in the treatment of ischemic heart disease, hypertension, and supraventricular tachycardia. The prototype agents, verapamil, nifedipine, and diltiazem, represent three classes of calcium channel blockers, each of which has different pharmacologic effects. Nifedipine and the other dihydropyridines primarily are vasodilators and have no clinical effects on cardiac conduction or contractility. Diltiazem and verapamil also are vasodilators, but they possess, to varying degrees, negative inotropic, chronotropic, and dromotropic effects. Side effects of these

Association of sociodemographic factors and comorbidity with non-receipt of medications for secondary prevention: a cohort study of 12,204 myocardial infarction survivors.

BACKGROUND: Clinical guidelines recommend use of (1) antiplatelet, (2) lipid-lowering, and (3) beta blocker medication, and (4) angiotensin-converting enzyme inhibitor or angiotensin receptor blocker (ACEi/ARB) for secondary prevention following myocardial infarction (MI). This study examines whether sociodemographic factors and comorbidity were associated with receipt of guideline-recommended medication, and whether receipt was associated with all-cause mortality. METHODS: A cohort study was conducted on West of Scotland patients aged 53 years or above who were discharged from hospital alive

[Patterns of detection of mild cognitive impairment in nursing].

Mild cognitive impairment (MCI) is characterized by an acquired cognitive loss that places individuals, mainly older adults, in an intermediate stage between normal cognitive functioning and dementia. This impairment has a high risk of progression to dementia and is suitable for screening, which allows more effective early intervention. Nursing professionals, especially community-based primary care nurses, play an important role in the detection and follow-up of MCI and in interventions for this condition. The first step should be to take a thorough history from both the patient and his or her

Drug utilisation in Dutch nursing homes.

OBJECTIVE: To quantify and evaluate drug utilisation in a sample of Dutch nursing homes. METHODS: A retrospective analysis of computerised medication data of 2355 residents aged 65 years and over from six nursing homes in the Netherlands was performed. For each therapeutic drug group, the number of users was determined. The ten therapeutic groups used most frequently were investigated further. For these, patient characteristics, use of therapeutic subgroups, the average daily dosages and the chronicity of drug use were determined. Chronicity was expressed as the percentage of treatment days d

Community perspectives on cardiovascular disease control in rural Ghana: A qualitative study.

BACKGROUND: Cardiovascular disease (CVD) prevalence is high in Ghana-but awareness, prevention, and treatment is sparse, particularly in rural regions. The nurse-led Community-based Health Planning and Services program offers general preventive and primary care in these areas, but overlooks CVD and its risk factors. METHODS: We conducted in-depth interviews with 30 community members (CM) in rural Navrongo, Ghana to understand their knowledge and beliefs regarding the causes and treatment of CVD and the potential role of community nurses in rendering CVD care. We transcribed audio records, cod

A Pharmacologic Update: New Treatments for Patients with Cardiovascular Disease.

Pharmacologic agents are a key part of the medical armamentarium aimed at reducing the significant morbidity and mortality caused by cardiovascular disease (CVD). In recent years, the landscape of CVD treatment has evolved with the development of new medication classes and the repurposing of existing medications for new indications. This article provides nurses with a pharmacologic update on new and emerging therapies for the treatment of hypertrophic cardiomyopathy, familial hypercholesterolemia, and heart failure. The authors review clinical indications, pharmacology, practical consideration

Sleep and heart failure.

Sleep problems and symptoms of sleep disturbance are very prevalent in patients with heart failure (HF). Numerous contributing factors include sleep-related breathing disorders, increasing age, medications, anxiety and depression, and comorbidities. Thus, the cardiovascular nurse has an important role in the recognition and management of sleep-related problems in persons with HF. This article provides an overview of sleep disturbances in patients with HF, suggests evidence-based strategies for managing the sleep problems, and identifies pertinent areas for future nursing inquiry.

Gallstones and Risk of Coronary Heart Disease: Prospective Analysis of 270 000 Men and Women From 3 US Cohorts and Meta-Analysis.

OBJECTIVE: Gallstone disease has been related to cardiovascular risk factors; however, whether presence of gallstones predicts coronary heart disease (CHD) is not well established. APPROACH AND RESULTS: We followed up 269 142 participants who were free of cancer and cardiovascular disease at baseline from 3 US cohorts: the Nurses' Health Study (112 520 women; 1980-2010), Nurses' Health Study II (112 919 women; 1989-2011), and the Health Professionals Follow-up Study (43 703 men; 1986-2010) and documented 21 265 incident CHD cases. After adjustment for potential confounders, the hazard ratio f

Nursing students' self-reported knowledge of genetics and genetic education.

INTRODUCTION: Nurses need to use genetic information in care; several areas of current need include primary care, public health, cardiovascular, geriatric and oncology nursing. Nursing faculties may be reluctant to add genetics to existing courses, let alone tackle the work of teaching an entire course in genetics. OBJECTIVES: The purpose of this study was to describe the current genetic knowledge of nursing students regarding genetics and genetic education. METHODS: This is a self-administered cross-sectional survey. A total of 162 of 209 nursing students returned a questionnaire. Data from
