Internal carotid artery dissection in a community. Rochester, Minnesota, 1987-1992.

BACKGROUND AND PURPOSE: Cervical internal carotid artery dissections are diagnosed with an increasing frequency, but reliable epidemiologic data are not available. The aim of this study was to determine the incidence rate of spontaneous cervical internal carotid artery dissection in a defined population. METHODS: Using the medical record linkage system used for epidemiologic studies in Rochester, Minn, all patients diagnosed with spontaneous cervical ICA dissection for 1987 through 1992 were identified. RESULTS: A total of 10 patients with spontaneous cervical internal carotid artery dissect

Multiple cranial neuropathy as a feature of internal carotid artery dissection.

The traditional presentation of spontaneous internal carotid artery dissection includes ipsilateral hemicranial headache, oculosympathetic paresis, and contralateral focal cerebral ischemic deficits. However, we describe two cases with multiple cranial nerve involvement ipsilateral to the dissection as the principal feature. The first patient, a 36-year-old man, had involvement of the 9th, 10th, 11th, and 12th cranial nerves. The second case was a 53-year-old man with abnormalities of the 5th, 7th, 9th, 10th, and 12th cranial nerves. In both, magnetic resonance imaging revealed a ring-like are

[Spontaneous dissection of the cervical internal carotid artery. Diagnosis by MRI].

A case of spontaneous dissection of the cervical internal carotid artery documented by angiography and MRI is reported. Angiography showed an irregular stenosis of the upper part of the cervical internal carotid artery extending up to the base of the skull, suggestive of spontaneous dissection. The diagnosis was confirmed beyond doubt by MRI which showed a sub-intimal collection of blood with a typical hyperintense appearance.

Non-invasive diagnosis of internal carotid artery dissections.

Arteriography is thought to be mandatory for the diagnosis of internal carotid artery (ICA) dissection. With the introduction of transcranial Doppler sonography (TCD) and magnetic resonance imaging (MRI), however, this is no longer the case. In 13 consecutive patients with ICA dissections the diagnosis was made by means of non-invasive tests including extracranial and transcranial Doppler sonography, contrast enhanced computed tomography (ceCT), and, in five patients, MRI. Intra-arterial digital subtraction angiography used as the gold standard in all cases was confirmative. Extracranial and t

Patterns of brain infarctions in internal carotid artery dissections.

In 15 patients with internal carotid artery (ICA) dissections, patterns of brain infarctions visible on CT were categorized according to a pathogenetically oriented classification system. This differentiated ischemic brain damage due to low flow from thromboembolic infarctions. Simultaneously, cerebral hemodynamic reserve was evaluated by means of both CO2-dependent vasomotor reactivity and HMPAO- and 99mTc-RBC-SPECT. Six out of 11 patients with ischemic infarctions had the territorial type of brain lesion, suggesting distal embolism. Five patients revealed the pattern of hemodynamically induc

Traumatic and spontaneous extracranial internal carotid artery dissections.

Seventy patients with spontaneous and 21 with traumatic extracranial internal carotid artery dissections were studied clinically and angiographically with mean follow-ups of 64 (spontaneous group) and 40 months (traumatic group). Sixty percent of the patients in the spontaneous group and 71% in the traumatic group also had follow-up angiograms. In traumatic dissections aneurysms were common, significantly fewer aneurysms resolved or became smaller and fewer stenoses resolved or improved, whereas more stenoses progressed to occlusion. Traumatic dissections were more likely to leave the patients

Spontaneous dissecting aneurysms of the cervical internal carotid artery.

Spontaneous dissecting aneurysm of the cervical internal carotid artery is a rare condition. Three cases are described, all in the younger age group, and the clinical features and characteristic radiological findings are discussed. One case demonstrated concomitant unruptured saccular intracranial aneurysms, an even more uncommon association, and the implications of this association for the pathogenesis of the dissecting aneurysm are considered. Attention is also drawn to the fact that marked clinical and angiographic improvement may occur spontaneously, indicating that conservative management

Dissecting aneurysm of the cavernous portion of the internal carotid artery.

A case of dissecting aneurysm involving the cavernous portion of the internal carotid artery, a site not previously described, is presented. The dissection was presumed due to arteriosclerosis and was complicated by a carotid-cavernous sinus fistula.

Mid-term outcomes of endovascular treatment for internal carotid artery dissection.

BACKGROUND: This study aims to assess the feasibility and safety of endovascular treatment of spontaneous internal carotid artery dissections (ICAD) using a proximal embolic protection device (EPD). METHODS: From January 2012 to December 2023 all consecutive patients admitted for ICAD who underwent carotid artery stenting using MoMa Ultra (Medtronic, Minneapolis, MN, USA) proximal EPD were included in this single-center study and were retrospectively analyzed. The presence of neurological symptoms was the only indication for treatment. Early outcomes included technical success, perioperative

An Unusual Cause of Internal Carotid Artery Dissection: Mobile Phone Misuse.

The use of mobile phones has become an indispensable part of our lives, especially due to widespread use of the internet. We report the case of a 38-year-old male patient who developed internal carotid artery dissection after talking on the phone between her left shoulder and ear by laterally flexing the neck for 20 minutes. In addition to many positive effects of technology that facilitate the daily life, the development of neurological deficits may be observed with widespread use of mobile phones. Misuse of mobile phone should be considered in patients with carotid artery dissection.

[Internal carotid artery dissection in the neurologist's practice - a case study].

UNLABELLED: Dissection of the interior carotid artery is rare in the general population. It can however be a potentially life-threatening condition. In the group of patients below 45 years of age, it constitutes a fairly common cause of cerebral stroke. A CASE REPORT: The study describes the case of a patient with the right interior carotid artery dissection, sustained most probably in the course of work. The patient was admitted to hospital for a severe headache of a few days' duration accompanied by Horner's syndrome on the right side. Promptly undertaken diagnostic procedures allowed for i

Internal Carotid Artery Dissection Presenting as Partial Horner's Syndrome and Vertigo.

Distinguishing between central and peripheral causes of vertigo can be challenging not only in an acute setting but also in chronic settings. A thorough review of systems and physical exam can assist providers in differentiating central versus peripheral etiologies and the need for urgent imaging. In this case, a 47-year-old man presented with vertigo, right-sided hearing loss, right-sided headache, and right-sided facial pain that began 4 weeks before while the patient was on a cruise ship. His physical exam findings were notable for anisocoria with right pupil 3.5 and left pupil 4.5, mild pt

[Intracerebral hemorrhage in the late period of internal carotid artery dissection].

Cervical artery dissection is the common cause of ischemic stroke in young and middle-age patients. According to our previous studies, dissection is related to arterial wall dysplastic changes, which in their turn are due to mitochondrial cytopathy. The authors describe three male patients who at the age of 53, 25 and 35 years underwent internal artery (ICA) dissection with occlusion of its lumen and subsequent recanalization in one of them. In 3.5 months, 13.5 years and 3 years respectively, patients developed intracerebral hemorrhage (IСH), which was not related to arterial hypertension, cer
