Foam Splint-The Comfortable Way of Postoperative Immobilization After Surgical Hip Reconstruction in Children-A Randomized Clinical Trial.

Hip joint reconstruction is often necessary for children and adolescents with conditions like developmental dysplasia of the hip (DDH), neurogenic dislocation of the hip (NDH), or Legg-Calvé-Perthes disease (LCPD) when non-surgical treatments are ineffective. Background: Post-operative immobilization after hip reconstruction in children is crucial to promote proper healing and reduce the risk of complications. While spica casting has been the traditional method, it can lead to various issues. Foam splinting has emerged as an alternative approach. This study aimed to compare the effectiveness a

Prosthetic fitting immediately after below-knee amputation.

Frequently surgical amputation of a lower extremity is required when gangrene develops as a result of peripheral vascular disease. This is particularly true in geriatric patients. A below-knee amputation, with refinements in the surgical procedure, and immediate rigid-cast prosthetic fitting are strongly advocated by our group. The progress of two patients treated in this manner is described. Preservation of the knee joint improves the amputee's prognosis for ambulation with a below-knee prosthesis. The rigid-cast dressing on the below-knee amputation reduces edema and postoperative pain, is o

Bilateral congenital vertical talus with severe lower extremity external rotational deformity: treated by reverse Ponseti technique.

Congenital vertical talus, also known as congenital convex pes valgus, is a complex, rare, foot deformity that is resistant to conservative treatment. We report a case of bilateral congenital vertical talus with severe lower extremity external rotational deformity that was treated by means of a method of serial manipulations and casts. Although the initial manipulation and casting before surgical treatment is the current concept of idiopathic congenital vertical talus, a good result can be obtained by a well-organized conservative treatment with the help of patient compliance, so routine surgi

[Ambulatory thromboembolism prevention in traumatology using self-injection of heparin].

In a retrospective and prospective study, 303 patients with fractures of the ankle joint treated surgically with postoperative immobilization in a plaster cast were analysed. After the introduction of thromboembolism prophylaxis with heparin for all outpatients immobilized by plaster casts the incidence of thromboembolic complications decreased significantly, from 4.5% to zero. The prophylaxis was well accepted, about 90% of the subcutaneous injections being administered by the patients themselves or family members. No serious complications were seen. Therefore, this effective prophylaxis of t

The potentially lethal nature of bronchial casts: plastic bronchitis.

The gross and microscopic findings from 2 fatal cases of plastic bronchitis (PB) in children, a rare entity characterized by the formation of large mucous casts in the bronchial tree, are presented. These casts differ from ordinary mucus because of their increased cohesiveness and consistency, resulting in solid structures that model the respiratory airway tree. PB usually presents as a complication of underlying diseases, which determine the prognosis of the afflicted patients. Conditions commonly associated with PB include congenital cardiopathies, lymph vessel malformations, asthma, and sic

Face bow and articulator for planning orthognathic surgery: 2 articulator.

Patients who require orthognathic surgery may have asymmetry of the position of the temporomandibular joints relative to the maxilla, which is impossible to reproduce on the current semiadjustable articulators used for surgical planning. We describe a highly-adjustable spirit level orthognathic face bow that allows records to be made of patients with asymmetrical maxillae. The orthognathic articulator also allows the position of the condylar components of the articulator to be adjusted in three dimensions. The use of the new face bow and articulator made it possible to mount the dental casts o

Hyperalimentation for superior mesenteric-artery (cast) syndrome following correction of spinal deformity.

The cast syndrome, recently called the superior mesenteric-artery syndrome, is a well recognized complication that can occur after a body cast has been applied. We are reporting the cases of three patients who had this syndrome following surgical correction of spinal deformity. The complication failed to resolve with the usual non-operative measures but did resolve with treatment by total parenteral nutrition.

Meningomyelocele with unusual feet deformity combination: A report of a rare case.

A 19-month-old male infant had presented to a paediatric orthopaedic clinic at the age of 6 months with meningomyelocele and bilateral feet deformity. The patient was admitted to the neonatal intensive care unit because of multiple congenital anomalies. These anomalies included meningomyelocele, right clubfoot, left vertical talus, imperforated anus, and an indirect hernia. Radiographs confirmed the diagnosis of right clubfoot and left-sided vertical talus. The course of management was delayed because of late diagnosis. At the age of 9 months, the patient underwent serial casting using the Pon

Dislocations following arthroplasties of the hip. Incidence, prevention, and treatment.

Three methods of avoiding postoperative prosthetic dislocation following total or bipolar hip arthroplasty are described; each approach is recommended for a specific population. The anterior surgical approach was utilized for bipolar hip endoprosthetic arthroplasty in 62 minimal or bed-to-chair ambulators with established flexion contractures about the hip. Two of these patients subsequently experienced a prosthetic dislocation, one of which was secondary to infection. An additional 98 patients who underwent arthroplasty using the posterior approach wore a knee immobilizer postoperatively, wit

A new device for creating and positioning an autogenous cartilage framework during microtia reconstruction.

OBJECTIVE: This paper describes the fabrication and use of a three-dimensional appliance, known as the microtia surgical positioner, to more accurately position and better sculpt the autogenous rib cartilage graft during microtia reconstruction. STUDY DESIGN: The authors introduce a new device design and surgical application. METHODS: An impression and plaster cast were made from the patient's auricular defect. On this cast, the artist then created a wax baseplate and an esthetically pleasing wax sculpture of an ideal ear similar to the patient's normal contralateral ear. The surface contour

Does Strict Adherence to the Ponseti Method Improve Isolated Clubfoot Treatment Outcomes? A Two-institution Review.

BACKGROUND: Despite being recognized as the gold standard in isolated clubfoot treatment, the Ponseti casting method has yielded variable results. Few studies have directly compared common predictors of treatment failure between institutions with high versus low failure rates. QUESTIONS/PURPOSES: We asked: (1) is the provider's rigid adherence to the Ponseti method associated with a lower likelihood of unplanned clubfoot surgery, and (2) at the institution that did not adhere rigidly to Ponseti's principles, are any demographic or treatment-related factors associated with increased likelihood

Correlation of age and severity scores to the number of Ponseti casts in Indian infants with clubfeet.

PURPOSE: World over, age and various severity scores are among the foremost variables studied in relation to the number of casts in clubfoot. We studied the correlation between child's age at presentation, initial Pirani, Dimeglio scores to the number of Ponseti casts in Indian clubfoot children. Further, we matched Pirani versus Dimeglio scores at different severities of deformity to derive a correlation between them. METHODS: We included 90 idiopathic clubfeet in 55 infants and scored them according to Pirani and Dimeglio grading systems. Syndromic, neurological, surgically intervened, atyp

Plastic Bronchitis in Adult and Pediatric Patients: A Review of its Presentation, Diagnosis, and Treatment.

PURPOSE OF STUDY: Plastic Bronchitis (PB) is a rare pulmonary condition characterized by the presence of casts in the trachea or bronchial tree. While there are many individual cases reported in pediatric and adult populations, no thorough reviews of pediatric and adult cases of PB exist in the literature. The purpose of this article is to conduct a comprehensive review of PB regarding presentation, diagnosis, pathophysiology, and treatments. ETIOLOGY: In the pediatric population, PB can be attributed to pediatric cardiothoracic surgeries such as Fontan procedures, infections, inflammatory pr

Long-term follow-up study of patients with a unilateral complete cleft of lip, alveolus, and palate following the Utrecht treatment protocol: Dental arch relationships.

OBJECTIVE: This study sought to evaluate long-term dental arch relationships in adults with a unilateral complete cleft lip and palate (UCLP) treated by the Utrecht protocol and to compare results with the centers from the Eurocleft study. MATERIALS AND METHODS: Retrospective analysis of UCLP patients age 17 or older, treated by two-stage palate closure at the Wilhelmina Children's Hospital, a tertiary center for cleft surgery in Utrecht, the Netherlands. Patients were invited to the clinic for a long-term evaluation. Casts were obtained on the day of follow-up and assessed by the modified Go

Hypospadias--problems of postoperative fistulae formation and a modified 2-stage procedure to reduce these.

A retrospective review of hypospadias repair reveals a high incidence of postoperative fistula nad stenosis. Further studies using Silicone casts have revealed a potential cause for the occurrence of these complications. The close co-operation between urologists and plastic surgeons has resulted in a grea er awareness of the surgical problems and the development of a simplified 2-stage technique which it is hoped will reduce these.

Surgical and nonsurgical treatment of acute ankle sprains.

In the treatment of eversion injuries or tears of the medial structures, exact anatomic restoration is mandatory, and this must be accomplished by either closed reduction or surgery. The treatment of lateral ankle ligament injuries is usually cast immobilization, taping, or a cast-brace. Surgical treatment of lateral ankle ligament sprains is performed only on those highly competitive young individuals who require an almost normally functioning ankle.

A two-step (altered cast) impression technique in the prosthetic rehabilitation of a patient after a maxillectomy: a clinical report.

This clinical report describes the prosthetic rehabilitation complicated by trismus (as a consequence of surgery and radiation therapy) of a patient with an acquired maxillary defect, after surgical removal of the right maxillary sinus tumor. The patient was restored with a partial removable dental prosthesis with a hollow bulb obturator to obtain orofacial rehabilitation and defect obturation. Precise obturation requires an accurate impression of the resection defect. A 2-step impression was performed because of trismus. A primary maxillary impression was made with an irreversible hydrocolloi

Advanced containment methods for the treatment of Perthes disease: Salter plus varus osteotomy and triple pelvic osteotomy.

The goal of intervention in Legg-Calvé-Perthes disease has been to prevent femoral head deformation by containing the head within the acetabulum, using it as a mold for guiding femoral head development. With appropriate proximal femoral morphology, premature arthritis can hopefully be avoided. Both nonsurgical and surgical methods of treatment have evolved over time, from abduction casts and braces to advanced surgical containment methods, which are now the mainstay of treatment. The purpose of this study is to briefly review the evolution of surgical treatment of Legg-Calvé-Perthes disease, a

"Dealer's Choice": Univalve Location Effect on Skin Surface Pressures in Long Leg Casts.

OBJECTIVE: Fiberglass long leg casting is often used to treat specific lower extremity fracture patterns in children. However, cylindrical casting limits swelling, increasing risk of compartment syndrome. To account for edema, casts are frequently univalved, but it remains unclear whether univalve location affects skin surface pressures (SSPs) in long leg casts. We hypothesized that a lateral univalve would decrease anterior SSP, whereas univalve location would not affect posterior SSP in long leg casts. METHODS: A 100-mL saline bag attached to a pressure transducer was placed along the anter
