A new pressure-based device for tip navigation and tip location during central venous catheterization: A prospective clinical study on a cohort of 136 adult patients.

INTRODUCTION: According to current guidelines, tip location of peripherally inserted central catheters (PICCs) should be verified during insertion, preferably using non-invasive methods such as intracavitary ECG (IC-ECG) or echocardiography. An interesting new option is represented by a new pressure-based device, the CatFinder System (CFS), which might be theoretically useful also for tip navigation. METHODS: We planned a single-center, prospective, non-randomized trial on adult patients requiring PICC insertion, using simultaneously CFS and IC-ECG, with the purpose of verifying the applicabi

The SIP protocol update: Eight strategies, incorporating Rapid Peripheral Vein Assessment (RaPeVA), to minimize complications associated with peripherally inserted central catheter insertion.

Insertion of Peripherally Inserted Central Catheters (PICCs) is potentially associated with the risk of immediate/early adverse events, some of them minimal (repeated punctures) and some relevant (accidental arterial puncture or nerve-related injury). Several strategies adopted during the insertion process may minimize the risk of such events, including late complication risks such as infection, venous thrombosis, or catheter dislodgment and/or malposition. This paper describes an update version of the SIP protocol (Safe Insertion of PICCs), an insertion bundle which includes eight effective s

An accidental subclavian artery cannulation: successful catheter removal by percutaneous vascular stenting.

Central venous catheterisation may sometimes be associated with life-threatening complications. Of these, subclavian artery puncture (infraclavicular approach), though seldom, (incidence 1-2%) following accidental arterial cannulation, may lead to arterial occlusion, embolism, pseudoaneurysms, vessel laceration or dissection or fatal hemorrhaging. Such complications may be even more severe in critically ill patients requiring systemic anticoagulation therapy or those with acute coagulation dysfunction. The authors report a case of an accidental cannulation of the subclavian artery with a centr

Complication and Dwell Time of Neonatal Peripheral Venous Catheters with and without Splint: A Descriptive, Correlational, and Prospective Study.

BACKGROUND: The main challenge of using Peripheral Intravenous Catheters (PICs) for neonates is their short dwell time, which requires frequent catheterization. Interventions have been made to increase the length of time intravenous catheters stay in neonates, such as the use of splints. Therefore, this study was conducted with the aim of determining the complications and dwell time of PICs with and without splints in infants admitted to the neonatal intensive care unit. MATERIALS AND METHODS: This descriptive, correlational, and prospective study included all eligible neonates based on the i

Application of Electrocardiogram Localization during Peripherally Inserted Central Catheter Line Insertion into the Persistent Left Superior Vena Cava of Neonates.

BACKGROUND: Most neonates with persistent left superior vena cava (PLSVC) have no clinical symptoms or hemodynamic changes, and this anomaly is only found during cardiac catheterization, pacemaker implantation, or central venous catheterization. Electrocardiogram (ECG) localization is helpful for the application of the peripherally inserted central catheter (PICC) technique in neonates with PLSVC. OBJECTIVE: To explore the characteristic waveforms of the P wave when a PICC under ECG localization is applied in neonates with PLSVC. STUDY DESIGN: The observation and management strategies for th

Development and validation of a nomogram risk prediction model for PICC-related thrombosis in children with hematological malignancies.

Early identification of high-risk patients is crucial for preventing peripherally intravenous central catheter (PICC)-related thrombosis in children with hematological malignancies. This study aims to develop and validate a clinical risk prediction tool for PICC-related thrombosis. A retrospective analysis was conducted on 519 children with hematological malignancies who underwent PICC catheterization at Tongji Hospital between January 2018 and December 2023. Among these children, 98 (18.9%) developed PICC-related thrombosis. A total of 54 potential predictor variables were analyzed, and six i

Invasive hemodynamic monitoring in obstetrics and gynecology. ACOG Technical Bulletin Number 175--December 1992.

Indications for invasive hemodynamic monitoring in obstetrics and gynecology are much the same as in any other area of medicine and surgery. The clinical decision of whether to employ monitoring cannot be made according to absolute criteria but must be made on an individual basis. Obviously, not every patient will benefit from or need invasive hemodynamic monitoring; proper patient selection is important, as is the availability of skilled physicians, nurses, and ancillary support personnel. The information collected at the patient's bedside can be used to both guide and evaluate therapeutic ma

Ultrasound-Guided, Guidewire-Assisted Long Peripheral Catheter in the Great Saphenous Vein for Bilateral Upper Limb Fracture Surgery: A Case Report.

BACKGROUND: Establishing reliable venous access can be particularly challenging in trauma patients with bilateral upper limb fractures, where peripheral cannulation is unfeasible and central venous catheterization may be unnecessary or refused. CASE PRESENTATION: A 51-year-old woman (BMI 38 kg/m2) sustained bilateral elbow fractures requiring staged fixation. Because the patient expressed strong concern and reluctance toward central venous catheterization, and there was no clear indication for it, alternative options were explored. Ultrasound assessment revealed a suitable right great sapheno

Urinary retention in adults: diagnosis and initial management.

Urinary retention is the inability to voluntarily void urine. This condition can be acute or chronic. Causes of urinary retention are numerous and can be classified as obstructive, infectious and inflammatory, pharmacologic, neurologic, or other. The most common cause of urinary retention is benign prostatic hyperplasia. Other common causes include prostatitis, cystitis, urethritis, and vulvovaginitis; receiving medications in the anticholinergic and alphaadrenergic agonist classes; and cortical, spinal, or peripheral nerve lesions. Obstructive causes in women often involve the pelvic organs.

A peripherally inserted central catheter misplacement into lateral thoracic vein: A case report.

INTRODUCTION: Central venous catheter applications and complications are closely related to the tip position. Previous studies have reported some rare cases of catheter misplacement. Here, we report a case of misplacement of a peripherally inserted central catheter into the lateral thoracic vein. CASE REPORT: A 56-year-old cancer patient underwent placement of a peripherally inserted central catheter through the left basilic vein under ultrasound-guided puncture. The catheterisation procedure was uneventful, so the catheter was believed to be in the superior vena cava. However, the post-anter

Intravenous catheter-related infections.

Vascular catheter-related infection is an important cause of mortality and morbidity in hospitalized patients. The mean incidence of catheter-related bloodstream infection in hospitalized pediatric patients is 2.4 episodes per 1,000 days. Totally implantable central venous catheters may be associated with a lower risk of infection. Coagulase-negative staphylococci are the predominant cause and account for about one third of episodes of catheter-related bloodstream infection. The diagnosis of catheter-related bloodstream infection is often difficult because there are frequently no signs of infl

[Virilizing ovarian tumours. Contribution of selective venous catheterization to the diagnosis].

The presence of a virilizing ovarian hilum cell tumours was suspected on account of a rise of plasma testosterone level above 3.5 ng/ml in one patient and a rise of plasma androstenedione level above 3.0 ng/ml in another. The diagnosis was made by selective catheterization of the ovarian and adrenal veins, which showed a high androgen concentration gradient in the vein of the ovary affected as compared with that observed in the adrenal and peripheral veins, and was confirmed by histological examination after ovariectomy. These two cases are contrasted with two other cases with high testosteron

Association Between Early Arterial Catheterization and Prognosis in Patients with Septic Shock: A Retrospective Propensity Score Analysis.

BackgroundArterial catheterization serves as a cornerstone monitoring modality in septic shock management, enabling continuous hemodynamic assessment and serial blood gas analysis. Despite its widespread use, mortality benefits and optimal timing for catheter insertion remains undefined.MethodsAn analysis of clinical data from 6,485 critically ill adult patients, identified as meeting the Sepsis-3 criteria for septic shock, was conducted utilizing the MIMIC-IV database. Through entropy-balanced propensity score matching (PSM, 1:1 ratio) and doubly robust estimation with inverse probability wei

Occult progressive pulmonary arterial occlusion associated with right ventricular hypertension in patients with systemic arteriopathy.

OBJECTIVES: Peripheral pulmonary artery stenosis (PPS) is a heterogeneous disease associated with numerous clinical or syndromic disorders including Williams syndrome. Previous studies focusing on patients with Williams syndrome have documented spontaneous improvement in branch pulmonary artery (PA) stenosis and right ventricular pressure. The goal of this study is to identify angiographic evidence of spontaneous occlusion or loss of segmental PAs in patients with PPS. METHODS: Patients undergoing serial catheterization for PPS at Boston Children's Hospital between 1999 and 2014 were selected
