Bacterial Biofilms on Extracorporeal Membrane Oxygenation Catheters.

Despite the advantages of extracorporeal membrane oxygenation (ECMO), secondary catheter infection remains a major concern during ECMO support. In this study, to clarify the mechanism of ECMO catheter-related infection, we evaluated the impact of infection on biofilm formation on the surfaces of ECMO catheters, and we investigated clinical factors associated with biofilm formation. Catheters used for ECMO were prospectively collected aseptically from the femoral vein, internal jugular vein, and femoral artery of 81 patients with acute cardiorespiratory failure between January 2015 and October

[Efficacy and Safety of Indwelling Pleural Catheters].

In symptomatic malignant pleural effusions, mostly in a palliative situation, therapeutic procedures should be chosen to improve dyspnoea and the concomitant impairment of quality of life. Indwelling pleural catheters (IPC) have played an increasing role in recent years. The efficacy and safety of this method have not been adequately clarified under real-life clinical conditions. 94 patients, in whom IPC had been implanted because of a clinical indication, were analysed retrospectively with respect to efficacy and safety, together with patient characteristics, peri- and postinterventional comp

Alternatives to Indwelling Catheters Cause Unintended Complications.

To reduce the risk of catheter-associated urinary tract infection (CAUTI), limiting use of indwelling catheters is encouraged with alternative collection methods and early removal. Adverse effects associated with such practices have not been described. We also determined if CAUTI preventative measures increase the risk of catheter-related complications. We hypothesized that there are complications associated with early removal of indwelling catheters. We described complications associated with indwelling catheterization and intermittent catheterization, and compared complication rates before a

Haemodialysis catheters in the intensive care unit.

Ten to 15% of critically ill patients need renal replacement therapy (RRT) for severe acute kidney injury. The dialysis catheter is critical for RRT quality and efficiency. Catheters have several properties that must be optimized to promote RRT success. The distal tip has to be located in a high blood flow location, which means central venous territory. Therefore, catheters are mostly inserted into the right internal jugular vein or in femoral veins. External diameter should vary from 12 to 16 Fr in order to ensure adequate blood flow inside the catheter. Lumen shapes are theoretically designe

Can silver alloy catheters reduce infection rates?

BACKGROUND: Catheter associated urinary tract infections remain a persistent challenge in healthcare practice. AIM: To establish whether silver alloy urinary catheters reduce catheter-associated urinary tract infections compared with standard silicone or latex urinary catheters in short-term hospitalised adult patients. METHOD: Analysis of eight studies using the Critical Appraisal Skills for Practitioners tool. RESULTS: A consistent pattern emerged that supported the effectiveness of silver alloy urinary catheters over uncoated catheters to reduce infections in adult patients. CONCLUSION:

Technique for insertion of intraventricular baclofen catheters.

Intraventricular baclofen (IVB) infusion has been used recently to treat secondary dystonia. Techniques of catheter implantation have not been thoroughly described. The object of this study was to describe a technique developed in the past 6 years to implant intraventricular catheters for baclofen infusion. Thirty-one patients underwent endoscopic placement of intraventricular catheters for IVB, primarily patients whose anatomy made implantation of intrathecal catheters difficult or inappropriate. Using the technique described, catheters were successfully inserted into the desired ventricular

Inflation of Foley catheters for postnasal packing.

BACKGROUND: Foley catheters are commonly used to control posterior epistaxis in most ENT departments. There are very few studies of the ideal substance for inflating Foley catheters for posterior epistaxis. This in vitro study compared the effects of water, saline and air in delivering effective tamponade. METHODS: Three groups of five Foley catheters each were filled with 10 ml of water, saline or air. Forty-eight hours later, the residual volume was aspirated and measured. RESULTS: At 48 hours, the following residual volumes were aspirated: air, 0 ml; saline, 8.5 ml; and water, 8.7 ml. CO

A hydrophilic technology for intermittent urinary catheters.

A biocompatible and highly lubricous hydrophilic coating has been developed for intermittent urinary catheters. The coating has additives incorporated into it, which reduce the drying out of the catheter when it is inserted into the urethra, and antimicrobial agents can be incorporated into it to minimise the trauma associated with catheterisation.

Minimizing the complications associated with migrating catheters.

This article seeks to explore the clinical practice of urinary catheter fixation. Traditionally, this area of practice has been neglected and nurses are familiar with tension lesions and dermal problems associated with inappropriate or incorrect urinary catheter fixation. A novel solution to this problem is a catheter fixation device. This device secures the catheter safely, making clinical practice safer and the experience of catheterization more tolerable for the patient. An example of a urinary catheter fixation device available in the UK is Bard's StatLock.

Insertion, care and management of suprapubic catheters.

A suprapubic catheter may be used to drain the bladder of urine as an alternative to an indwelling urethral catheter. It can be inserted as a temporary or permanent measure in patients with urinary dysfunction or if initial urethral catheter insertion or recatheterisation is problematic. This article discusses insertion of suprapubic catheters, management and care, including changing suprapubic catheters, drainage systems and the importance of patient education.

Care of urinary catheters and drainage systems.

A question about the reuse of drainable urinary catheter night drainage bags in a patient's home promoted the South East Thames continence advisory group to examine the evidence for the management of urinary catheter drainage systems. The objective of this review was to establish if there is evidence available to support clinical practice and provide recommendations to staff and patients to ensure best practice.

Using bladder instillations to manage indwelling catheters.

Indwelling urinary catheters are commonly used in both acute and primary care settings but patients often experience problems with blockage of the catheter as a result of encrustation. Bladder instillations of differing solutions are used in an attempt to prevent and treat this problem of encrustation. This article looks at some of the issues surrounding the use and methods of these solutions. Based on an audit performed by a group of continence clinical nurse specialists in five areas of the UK, results of a questionnaire sent to acute and primary care nursing staff in 2003 are presented and

[Frequency and colonization rate of intravascular catheters].

Clinical signs are not sufficiently reliable for establishing diagnosis of intravascular catheter-related infection. Therefore, microbiological confirmation, based on the culture of the catheter tip after its removal, is necessary in diagnosing the infection. The aim of this study was to determine the frequency and the degree of microbial colonization of intravascular catheters (IVK), and the risk for the onset of sepsis, by using qualitative, semiquantitative (roll plate) and quantitative (vortexing) catheter culture techniques. During the period April 2001-December 2002, 289 intravascular ca

The clinical use of intrauterine pressure catheters.

We question the need for an intrauterine pressure catheter (IUPC) to monitor uterine contractions during the induction or augmentation of labor with oxytocin, or to verify adequate uterine contractions to support the diagnosis of arrest of labor. There are appropriately conducted clinical trials demonstrating that uterine contraction frequency in spontaneous, induced and augmented labors obtained with the use of external tocography is sufficient for the clinical management of labor. The value of an IUPC is further diluted by the inherent variability of pressure recordings with its use and the

Laparoscopic management of malfunctioning peritoneal dialysis catheters.

BACKGROUND: Continuous ambulatory peritoneal dialysis (CAPD) is an established alternative method to hemodialysis for treating end-stage renal disease patients. However, this method is associated with a significant number of complications, such as catheter malposition, omental wrapping, and infection. The purpose of this study was to determine the efficacy of laparoscopy in the treatment of malfunctioning CAPD catheters. METHODS: Between November 1994 and June 1999, a total of 16 patients with CAPD underwent laparoscopy for the evaluation and management of CAPD catheter dysfunction. Two troca

Groshong catheters: technical aspects and clinical features.

After thousands of Groshong catheter applications over more than twenty years, the cause of blood withdrawal drawbacks is not yet completely understood. This phenomenon - which is not typical of closed-tip catheters since it is also common in open-tip catheters - in the Groshong catheter is attributed to the valve (ball-valve effect). The aim of this work is to understand the relationship between causes and effects in order to provide clinicians with practical solutions. Out of the 16 catheters examined, we observed no clear correlation among electron microscope ultrastructural analysis, valve
