Acute brachial plexus neuritis: an uncommon cause of shoulder pain.

Patients with acute brachial plexus neuritis are often misdiagnosed as having cervical radiculopathy. Acute brachial plexus neuritis is an uncommon disorder characterized by severe shoulder and upper arm pain followed by marked upper arm weakness. The temporal profile of pain preceding weakness is important in establishing a prompt diagnosis and differentiating acute brachial plexus neuritis from cervical radiculopathy. Magnetic resonance imaging of the shoulder and upper arm musculature may reveal denervation within days, allowing prompt diagnosis. Electromyography, conducted three to four we

Structure of the brachial plexus root and adjacent regions displayed by ultrasound imaging.

Brachial plexuses of 110 healthy volunteers were examined using high resolution color Doppler ultrasound. Ultrasonic characteristics and anatomic variation in the intervertebral foramen, interscalene, supraclavicular and infraclavicular, as well as the axillary brachial plexus were investigated. Results confirmed that the normal brachial plexus on cross section exhibited round or elliptic hypoechoic texture. Longitudinal section imaging showed many parallel linear hypo-moderate echoes, with hypo-echo. The transverse processes of the seventh cervical vertebra, the scalene space, the subclavian

Origin and distribution of the brachial plexus of the Van cats.

Knowing the structure and variations of the plexus brachialis is important in neck and shoulder surgery. The knowledge of the brachial plexus reduces the injury rate of the nerves in surgical interventions to the axillary region. The major nerve trunks of the thoracic limb were the suprascapular, subscapular, axillary, radial, musculocutaneous, median and ulnar nerves. In Van cats, the brachial plexus was formed by the ventral branches of the spinal nerves, C6-C7-C8 and T1. The 7th cervical nerve was quite thick compared to the others. The subscapular nerve was the thinnest (on the right side,

[Two post-traumatic "Gilliatt-Sumner hands" from decompensation of thoracic outlet syndrome].

The clinical picture of hand atrophy related to a cervical rib has been well described in 1970 by Gilliatt and Sumner. These authors reported a series of nine patients whose motor status was stabilized following decompressive surgery of the brachial plexus. We report two young patients decompensating a predisposed thoracic outlet (rudimentary cervical rib), following a scapular or cervical trauma. After several years of neck and arm painful complaints, the two patients progressively developed hand atrophy. One patient had been operated both at cervical (double discectomy and disc prosthesis) a

Infantile myofibromatosis: a nontraumatic cause of neonatal brachial plexus palsy.

Most injuries to the neonatal brachial plexus occur acutely at birth, and are iatrogenic in origin. However, when weakness is accompanied by atrophy, nontraumatic etiologies should be considered. The differential diagnosis of chronic congenital brachial plexopathy includes cervical bone malformations, humeral osteomyelitis, varicella, and compression from various types of infantile tumors. An illustrative male infant delivered at 37 weeks of gestation with wasted musculature of the left upper arm, ipsilateral Horner's syndrome, and a hemidiaphragm is presented. On further examination, this pat

Sonographic Nerve Tracking in the Cervical Region: A Pictorial Essay and Video Demonstration.

Imaging of the nerves in the cervical region is more complicated than those of the extremities. Although high-resolution ultrasound enables the depiction of peripheral nerves' morphology and their associations with the adjacent soft tissues, precise identification of the nerves in the neck is still challenging. Familiarization with the cervical nerve tracking techniques can help interventional physiatrists explore/treat relevant entrapment syndromes, so does guiding proper electrode placement during nerve conduction studies. The present article integrates serial ultrasound images and videos to

Brachial plexus injury due to compression: an alternate mechanism of injury: case report and review of the literature.

We present a case of a brachial plexus injury due to compression of the nerves from a traumatic hematoma, with no associated bone or vascular injury. The paralysis in this case was not evident for 48 hours after the initial injury, implying that the brachial plexus was not damaged directly. Electromyograms documenting brachial plexopathy were obtained. The mechanism of injury in this case is different from the usual mechanisms of injury in brachial plexus trauma. The majority of brachial plexus injuries are associated with multisystem trauma. The mechanism of injury to the brachial plexus is e

Symptoms and signs of irritation of the brachial plexus in whiplash injuries.

We investigated the incidence of evidence of irritation of the brachial plexus in 119 patients with whiplash injuries sustained in road-traffic accidents. We compared the symptoms, physical signs, autonomic status, psychological status and findings from radiographs of the cervical spine using examination charts and a modified Cornell Medical Index Health questionnaire, in patients in two distinct groups: those with irritation of the brachial plexus and those without. There were 45 patients (37.8%) in the first group. The ratio of women to men was significantly higher in patients with irritatio

Neck dissection injury of a brachial plexus anatomical variant.

We describe an unusual brachial plexus anatomical variant that renders it vulnerable to injury in radical or modified neck dissection. It is represented by a looping redundant course of the most cephalad cervical division contribution to the brachial plexus. Unless recognized during development of the inferior surgical plane by blunt dissection techniques, injury to this portion of the brachial plexus may occur. An awareness of this infrequent variation in brachial plexus anatomy should be maintained.

Evaluation of a Patient with Klumpke's Palsy. A Case Report.

Klumpke's palsy is a rare form of paralysis involving the muscles of the forearm and hand, resulting from a brachial plexus injury in which the eighth cervical (C8) and first thoracic (Th1) nerves are injured either before or after they have joined to form the lower trunk. We report a case of a 45 years-old woman with post-traumatic left cervical-brachial pain afflicted by obstetric brachial plexus palsy on the right side. Magnetic Resonance Imaging (MRI) examination of the cervical spine revealed a meningeal stretch in the left side at C7-Th1 level, responsible for pain, and also multiple int

Traumatic cervical syringomyelia related to birth injury.

A rare case of cervical syringomyelia related to breech delivery is reported. The initial diagnosis was bilateral brachial plexus palsy due to birth injury, which was revealed by magnetic resonance imaging (MRI) to be traumatic syringomyelia. The usefulness of MRI in the early diagnosis of cervical cord birth injury, especially in differentiating between brachial plexus palsy due to birth injury and spinal cord trauma due to birth injury in infancy, is emphasized.

[Shoulder surgery using a plexus block: for or against. Against].

Brachial plexus block is an alternative and elegant procedure for shoulder surgery. Three main concerns have to be considered when this technique is planed. Firstly, large amounts of local anaesthetic solution are necessary to achieve complete blockade of the region especially when the deeper tissues of the shoulder are concerned by the procedure. Secondly interscalenic cervical plexus block produces homolateral phrenic paralysis and may impair respiratory function in patients at risk or when supplemental IV drugs are administered. Thirdly, when nerve lesions of different mechanisms including

Reliable Initial Trauma CT Findings of Supraclavicular Brachial Plexus Injury in Patients Sustaining Blunt Injuries.

BACKGROUND AND PURPOSE: Traumatic brachial plexus injuries are uncommon but can be debilitating. Early diagnosis is critical. Most patients undergo CT after trauma. We sought to identify correlative CT findings of supraclavicular brachial plexus injuries to discern who may require further evaluation with MR imaging and to measure multireviewer performance for their interpretations. MATERIALS AND METHODS: We identified all MR imaging examinations of the brachial plexus from our institution from January 2010 to January 2021 and included those performed for trauma. We excluded patients with pene

Traumatic Spinal Epidural Hematoma Associated with Cervical Nerve Root Avulsion without Vertebral Fractures: Case Report.

Traumatic spinal epidural hematoma (TSEH) is a rare condition that may cause acute spinal cord compression and lead to irreversible neurological impairment. TSEH not only compresses the cord, but it can also worsen cervical nerve root avulsion. To our knowledge, only five cases of combined TSEH and cervical nerve root avulsion have been reported in the literature. We present the case of a 42-year-old woman who suffered a motorcycle accident. On admission, she presented with mild traumatic brain injury and cervical spine and right shoulder trauma. A physical examination revealed numbness and fl

Diagnosis and surgery of brachial plexus injuries.

The surgical outcome of traumatic injuries of the brachial plexus (BP) depends on the following parameters: 1) accurate preoperative diagnosis of cervical root avulsion; 2) time interval between injury and surgery; 3) delicate handling of the nerve tissue; and 4) postoperative physiologic training. This report is based on a 15-year experience in brachial plexus surgery and is supported on the grounds of two major studies. In a prospective study, the authors controlled for the reliability of preoperative radiologic diagnosis by myelo-CT and MRI scans for 40 patients, to evaluate the integrity o

Unexpected neuroimaging abnormalities in patients with apparent C8 radiculopathy: broadening the clinical spectrum.

INTRODUCTION: C8-root impingement by C7/T1 lesions on neuroimaging studies is not consistently observed in C8 radiculopathy. We hypothesized that C7 or T1 root lesions (with a pre- or postfixed plexus) or cervical myelopathy might explain some "C8 radiculopathies" without C8 root compression. METHODS: Retrospective analysis of cervical neuroimaging in 31 consecutive patients with EMG-confirmed C8 radiculopathy. RESULTS: Five patients (16%) had C8-root compression at C7/T1. Of those without C8-root compression, 5 (16%) had C7-root compression at C6/7, one (3%) had T1-root compression at T1/T2

[Cervical cancer metastasis to the lumbosacral spinal root: a case report].

A 67-year-old woman underwent total hysterectomy for cervical cancer followed by chemotherapy, achieving partial remission. Post-treatment, lymph node metastases to the left obturator region remained decreased in size compared with pre-treatment. Nine months prior to the last chemotherapy, she developed abnormal sensations and muscle weakness in her left lower limb. Nerve conduction studies revealed axonal damage in the left tibial, peroneal, and sural nerves. Pelvic CT and lumbar MRI showed continuous contrast enhancement and enlargement of the left L5, S1, and S2 nerve roots originating from

[The skin and mucosa of the head and neck area...What was the innervation again?].

The skin innervation of the head and neck area is largely provided by the trigeminal nerve and the cervical plexus. The separation between the two areas is approximately on the line crown-jaw-chin. The face is provided by the trigeminal nerve (nervus V), while the area below it up to the shoulders is provided by the cervical plexus. The innervation from the oral mucosa to the esophagus is provided by the trigeminal nerve, glossopharyngeus and vagus. This segmental distribution allows for a clear organization of the sensory input, which is clinically important in diagnosing disorders.
