Regional Nerve Blocks for Middle Ear Surgery: A Systematic Review and Meta-Analysis of Randomized Controlled Trials With GRADE Assessment.

OBJECTIVE: Middle ear surgery is frequently associated with significant postoperative pain, nausea, and vomiting. Such issues have prompted clinicians to adopt regional nerve blocks, such as the superficial cervical plexus block (SCPB) and the great auricular nerve block (GANB). Our aim was to evaluate the efficacy of regional nerve blocks in adult patients undergoing different middle ear surgeries. DATABASES REVIEWED: PubMed, the Cochrane Central Database of Controlled Trials and Systematic Reviews (CDSR), Scopus, Web of Science, and Google Scholar. METHODS: Eligible randomized controlled t

Effects of regional anesthesia techniques on local anesthetic plasma levels and complications in carotid surgery: a randomized controlled pilot trial.

BACKGROUND: The ultrasound guided intermediate cervical plexus block with perivascular infiltration of the internal carotid artery (PVB) is a new technique for regional anesthesia in carotid endarterectomy (CEA). We conducted a pilot study investigating the effects of deep cervical block (DCB), intermediate cervical block alone (ICB) and PVB on perioperative complications in patients undergoing elective CEA. We hypothesized, that the ropivacaine plasma concentration is higher in patients receiving DCB compared to PVB and ICB. METHODS: In a randomized controlled pilot study thirty patients sch

Anesthetic Management of Tracheal Resection and Anastomosis for Postintubation Tracheal Stenosis Involving a Sequential Airway Strategy.

A postintubation tracheal stenotic patient who requires surgical resection and anastomosis is a challenging case for an anesthesiologist, requiring meticulous planning during surgical correction due to a shared airway. This case report is about the anesthetic management of a 23-year-old male who developed severe tracheal stenosis following prolonged mechanical ventilation after decompressive craniectomy and presented with severe dyspnea, wheeze, and stridor five weeks later. Tracheal resection and anastomosis were the ultimate solution for this patient. For that, a sequential airway strategy w

Interscalene perineural catheter placement using an ultrasound-guided posterior approach.

BACKGROUND AND OBJECTIVES: The posterior approach to the brachial plexus--or cervical paravertebral block--has advantages over the anterolateral interscalene approach, but concerns regarding "blind" needle placement near the neuraxis have limited the acceptance of this useful technique. We present a technique to place an interscalene perineural catheter that potentially decreases neuraxial involvement with the use of ultrasound guidance. METHODS: A 55-year-old man scheduled for total shoulder arthroplasty underwent placement of an interscalene perineural catheter. The posterior approach was s

Oral Methadone for Patients With Neuropathic Pain Due to Neoplastic Brachial Plexopathy.

Background: The brachial plexus nerves originate from the cervical (C5-C8) and first thoracic (T1) spinal nerves, and innervate muscles and skin of the chest, shoulder, arm and hand. Brachial plexus injuries can occur as a result of shoulder trauma and inflammation. Malignant tumors can also cause neoplastic brachial plexopathy (NBP), and refractory neuropathic pain is the most common symptom of NBP. Methadone is a synthetic opioid with high efficacy as an opioid-receptor agonist, and its inhibitory effects on N-methyl-D-aspartate (NMDA) may play a role in pain relief. However, there is a need

A Randomized Comparison Between Interscalene and Small-Volume Supraclavicular Blocks for Arthroscopic Shoulder Surgery.

BACKGROUND AND OBJECTIVES: This randomized trial compared ultrasound (US)-guided interscalene block (ISB) and small-volume supraclavicular block (SCB) for arthroscopic shoulder surgery. We hypothesized that SCB would provide equivalent analgesia to ISB 30 minutes after surgery without the risk of hemidiaphragmatic paralysis (HDP). METHODS: All patients received an US-guided intermediate cervical plexus block. In the ISB group, US-guided ISB was performed with 20 mL of levobupivacaine 0.5% and epinephrine 5 μg/mL. In the SCB group, US-guided SCB was carried out using 20 mL of the same local an

Proper management of the rotational vertebral artery occlusion secondary to spondylosis.

A 66-year-old man with cervical spondylosis noticed severe vertigo when turning his head to the right. He underwent subclavian arteriography elsewhere, which showed a block of the contrast medium in the right vertebral artery (VA) at the C5/6 level when the patient turned his head to the right. After referral to our institute, however, postcontrast CT scan revealed an attenuated shadow of the venous plexus around the right VA at the C3/4 level. Repeated selective angiography with rotation of the head after visualization of the entire VA verified the level of obstruction to be at C3/4. Resectio

Invasive Procedures to the Neck and Chest and Hemidiaphragmatic Paralysis: Are They Always Causally Associated?

Three cases of hemidiaphragmatic paralysis are reported. One case was associated with an interscalene brachial plexus block, another with the insertion of an implantable cardioverter defibrillator, and a third case had undergone a coronary artery bypass grafting operation. In only one of these cases, there was a causal association, while in the other two, it was determined that the paralysis was coincidental. A change in the diaphragmatic position on the chest X-ray needs to be demonstrated, preferably also confirmed by ultrasound or a fluoroscopic sniff test, to show that the paralysis was a

Great Auricular Neuralgia: Case Series.

BACKGROUND/OBJECTIVE: The great auricular nerve (GAN) arises from C2-C3 and provides innervation over the skin in the pre-auricular region, jaw angle, posteroinferior pinna, and mastoid. Although damage to the GAN has been reported following trauma or procedures nearby this nerve course, neuralgia of this nerve is uncommon with knowledge based on a handful of case reports in literature. The objective of this study is to describe the presentation, treatment, and outcome of 13 cases of GAN neuralgia. METHODS: Case series. Retrospecive review of charts from 1994 to 2018 with diagnoses: "auricula

A Central Role for Sympathetic Nerves in Herpes Stromal Keratitis in Mice.

PURPOSE: Herpes simplex virus type 1 (HSV-1) is a neurotrophic virus that can cause herpes stromal keratitis (HSK), a severe corneal inflammation that can lead to corneal scarring and blindness. This study identified neurologic changes that occur in HSV-1-infected corneas and related them to HSV-1-induced immunopathology. METHODS: Corneas of BALB/c and C57BL/6 mice were infected with HSV-1 strains that induce HSK. Changes in sensory nerves were identified by immunofluorescence staining of sensory and sympathetic nerves for substance P (SP) and tyrosine hydroxylase (TH), respectively, and conf

Ultrasound-Guided Interscalene Catheter Complicated by Persistent Phrenic Nerve Palsy.

A 76-year-old male presented for reverse total shoulder arthroplasty (TSA) in the beach chair position. A preoperative interscalene nerve catheter was placed under direct ultrasound-guidance utilizing a posterior in-plane approach. On POD 2, the catheter was removed. Three weeks postoperatively, the patient reported worsening dyspnea with a subsequent chest X-ray demonstrating an elevated right hemidiaphragm. Pulmonary function testing revealed worsening deficit from presurgical values consistent with phrenic nerve palsy. The patient decided to continue conservative management and declined fur

Anatomical variations in the emergence of the cutaneous nerves from the nerve point in the neck and identification of the landmarks to locate the nerve point with its clinical implications: a cadaveric study on South Indian human foetuses.

OBJECTIVE: The cutaneous nerves from the cervical plexuses are anaesthetized by using local anaesthetics for pain relief or when minor surgical operations are performed. Knowing the variations in these nerves is important for anaestheticists to administer an effective anaesthesia to a particular nerve. So, the aim of this study was to look for the variations in the emerging patterns of the cervical cutaneous nerves in the neck and to locate the nerve point in the neck by using the superficial landmarks. MATERIALS AND METHODS: The neck was dissected in 16 foetal cadavers (total 32). The foetus

Enhancing anterior shoulder analgesia with a modified PECS II block during shoulder arthroscopy under regional anesthesia alone: a translational anatomical and clinical feasibility study.

BACKGROUND: Regional anesthesia (RA) alone offers important perioperative advantages for shoulder arthroscopy, but conventional interscalene brachial (ISB) and superficial cervical plexus (SCP) blocks may insufficiently cover the anterior shoulder region-especially during biceps, rotator interval, or anterior capsular procedures. This gap likely reflects sensory input from intercostobrachial and pectoral nerves, which are not reliably anesthetized with standard techniques. We therefore hypothesized that a cranially directed modified pectoserratus plane block (PECS II) (second-third intercostal
