Recognition of facial emotions among maltreated children with high rates of post-traumatic stress disorder.

OBJECTIVE: The purpose of this study is to examine processing of facial emotions in a sample of maltreated children showing high rates of post-traumatic stress disorder (PTSD). Maltreatment during childhood has been associated independently with both atypical processing of emotion and the development of PTSD. However, research has provided little evidence indicating how high rates of PTSD might relate to maltreated children's processing of emotions. METHOD: Participants' reaction time and labeling of emotions were measured using a morphed facial emotion identification task. Participants inclu

Public health system and psychiatry in the treatment of 'dangerous' young offenders in Brazil.

We describe the Experimental Health Unit, a special forensic mental health facility in Brazil, created by court order and administered by the São Paulo Department of Health. It was designed for young offenders receiving compulsory inpatient treatment for severe personality disorders. All nine patients admitted to date came from Foundation CASA (a socio-educational centre of assistance for adolescents, the juvenile correctional centres managed by the São Paulo state Department of Justice). The court decision is questionable, relying on a new interpretation of the Child and Adolescent Statute an

Brief cognitive behavioural therapy with male adolescent offenders in open custody or on probation: an evaluation of management of anger.

A 6-wk. cognitive behavioural program administered to 24 adolescent male offenders assigned to open custody and 5 to probation showed no mean differences on the Test of Nonverbal Intelligence, the Minnesota Multiphasic Personality Inventory, and the Coopersmith Self-esteem Inventory. Significant correlations were found for the personality measures with pretreatment and posttreatment scores on the Buss-Durkee Hostility Inventory.

"You have to take this medication, but then you get punished for taking it:" lack of agency, choice, and fear of medications to treat opioid use disorder across the perinatal period.

INTRODUCTION: Medications to treat opioid use disorder (MOUD) during pregnancy and in the postpartum period remain underutilized. A need exists to enhance our understanding of modifiable factors, facilitators, and barriers to MOUD utilization and adherence in the perinatal period to improve maternal and child outcomes. METHODS: The study conducted semi-structured qualitative interviews with recently pregnant people with opioid use disorder (OUD) to explore experiences as a pregnant and/or parenting person with OUD, perceptions of enabling factors and barriers to treatment utilization, incenti

Impact of family-friendly prison policies on health, justice and child protection outcomes for incarcerated mothers and their dependent children: a cohort study protocol.

INTRODUCTION: Female imprisonment has numerous health and social sequelae for both women prisoners and their children. Examples of comprehensive family-friendly prison policies that seek to improve the health and social functioning of women prisoners and their children exist but have not been evaluated. This study will determine the impact of exposure to a family-friendly prison environment on health, child protection and justice outcomes for incarcerated mothers and their dependent children. METHODS AND ANALYSIS: A longitudinal retrospective cohort design will be used to compare outcomes for

Mother-grandmother coparenting relationships in families with incarcerated mothers: a pilot investigation.

Using new methods designed to assess coparenting between incarcerated mothers of preschool-aged children and the maternal grandmothers caring for the children during their absence, we examined relationships between coparenting quality during the mother's jail stay and both concurrent child behavior problems and later coparenting interactions following mothers' release and community reentry. Forty mother-grandmother dyads participated in joint coparenting discussions during the incarceration, with a smaller subset completing a parallel activity at home 1 month postrelease. Both women also parti

Characteristics of child abuse in immigrant Korean families and correlates of placement decisions.

OBJECTIVE: This study examined the characteristics and patterns of child abuse among immigrant Korean families in Los Angeles and critical variables that contribute to the type of placement made by the child protective services (CPS) system. METHOD: Data were obtained from reviewing and analyzing 170 active Korean case files maintained by the Asian Pacific Unit (APU) of the Los Angeles County Department of Children and Family Services (LAC-DCFS) during July through September, 2001. Logistic regression analysis was employed to examine the odds of children being kept in or removed from the home

The favoured child?

This case conference concerns a child who has been in care following a diagnosis of emotional abuse and a serious incident of physical abuse. She wants to return home again, and her parents, who had previously scapegoated her, now blame the family's previous ills on her sister instead. The Children Act 1989 gives considerable weight to the child's wishes, but what if the child returns home and is re-abused? In this case conference a child psychiatrist, a philosopher and a lawyer discuss the issues of clinicians' responsibilities, moral luck, and child care law.

A descriptive study of abuse and neglect in out-of-home-placement.

Selected characteristics of 290 reports of physical abuse, sexual abuse and neglect in foster homes, group homes, residential treatment centers, and institutions are described. At each type of setting physical abuse reports were most common, and neglect reports were least common. Quantitative and qualitative methods demonstrate that a significant percentage of confirmed reports are of a serious nature. Injuries occurred most frequently because of physical abuse while sexual abuse reports were most likely to be confirmed. Prior allegations of abuse or neglect regarding the perpetrator were indi

Factors associated with caregiver stability in permanent placements: a classification tree approach.

OBJECTIVE: Identify individual and environmental variables associated with caregiver stability and instability for children in diverse permanent placement types (i.e., reunification, adoption, and long-term foster care/guardianship with relatives or non-relatives), following 5 or more months in out-of-home care prior to age 4 due to substantiated maltreatment. METHODS: Participants were 285 children from the Southwestern site of Longitudinal Studies of Child Abuse and Neglect (LONGSCAN). Caregiver instability was defined as a change in primary caregiver between ages 6 and 8 years. Classificat

Child abuse and domestic violence in families of children seen for suspected sexual abuse.

We surveyed families of children seen in a sexual abuse evaluation clinic regarding domestic violence in the child's home and physical and sexual abuse during childhood among the mothers. Domestic violence occurred in 54% (216/402) of the children's homes; 28% (111/392) of mothers reported childhood physical abuse, and 42% (167/395) reported sexual abuse. There were no differences in the rates of domestic violence (chi 2 = 0.42, 2 df, p = 0.8), maternal physical abuse (chi 2 = 2.40, 2 df, p = 0.3), or maternal sexual abuse (chi 2 = 2.11, 2 df, p = 0.3) based on whether our current, patient was

Munchausen by proxy syndrome mimicking childhood-onset systemic lupus erythematosus.

Childhood-onset systemic lupus erythematosus (cSLE) is a chronic inflammatory multisystem autoimmune disease that requires multiple differential diagnoses. Munchausen by proxy syndrome (MBPS) is a form of child abuse, where a caregiver intentionally creates a medical history and induces or fabricates signs or disease in a patient. To our knowledge, there is no case report of MBPS mimicking cSLE diagnosis. We reported herein a 9-year-old male patient, with a history of multiple hospitalizations due to seizures with altered levels of consciousness. The mother reported malar rash, photosensitivit

Utility or futility? Toward an operational definition of addiction 'recovery'.

The 'recovery' construct has received growing scrutiny over the past 20 years as individuals and organizations have tried to define what 'recovery' is, or should contain, as something distinct from remission. Yet, despite goals to improve definitional clarity and utility, confusion has persisted hindering attainment of objective epidemiological recovery prevalence rates, as well as its utility as a helpful decision tool in important legal and social determinations (e.g. child custody). We argue that the basis for confusion lies in four major areas: 1. Whether we are talking about an objective

Does the forensic physician have a role beyond injury documentation and specimen collection in Australia? A personal view.

Clinical forensic medicine (CFM), as a single discipline, encompasses a number of areas of medico-legal practice including injury interpretation, management of sexual and physical assault cases (both adult and child; alleged victim and offender), mental health issues, traffic medicine, custodial medicine and toxicology. The cases are usually alive but in some jurisdictions the forensic practitioner also engages in death investigation with some undertaking autopsies. During the last 20-30 years, the discipline has fragmented with areas being hived off to other medical specialist disciplines and

Structural stigma in state legislation.

OBJECTIVE: This article discusses examples of structural stigma that results from state governments' enactment of laws that diminish the opportunities of people with mental illness. METHODS: To examine current trends in structural stigma, the authors identified and coded all relevant bills introduced in 2002 in the 50 states. Bills were categorized in terms of their effect on liberties, protection from discrimination, and privacy. The terms used to describe the targets of bills were examined: persons with "mental illness" or persons who are "incompetent" or "disabled" because of mental illnes

Statistical definition of relapse: case of family drug court.

At any point in time, a patient's return to drug use can be seen either as a temporary event or as a return to persistent use. There is no formal standard for distinguishing persistent drug use from an occasional relapse. This lack of standardization persists although the consequences of either interpretation can be life altering. In a drug court or regulatory situation, for example, misinterpreting relapse as return to drug use could lead to incarceration, loss of child custody, or loss of employment. A clinician who mistakes a client's relapse for persistent drug use may fail to adjust treat

Clinical applications of hair testing for drugs of abuse--the Canadian experience.

During the last 2 decades there has been a substantial increase in illicit drug consumption in North America. It has been repeatedly shown that the personal history of drug use is far from being accurate. Fearing legal consequences and embarrassment of admitted illicit substance use, most users tend to deny or, to under-report illicit drug consumption. These facts have stressed an urgent need for a biological marker which does not lose its sensitivity within a few days after the end of exposure and which may yield a cumulative reflection of long term exposure to illicit drugs. Hair analysis ha

Screening for drugs of abuse (II): Cannabinoids, lysergic acid diethylamide, buprenorphine, methadone, barbiturates, benzodiazepines and other drugs.

Requirements for the provision of an efficient and reliable service for drugs of abuse screening in urine have been summarized in Part I of this review. The requirements included rapid turn-around times, good communications between requesting clinicians and the laboratory, and participation in quality assessment schemes. In addition, the need for checking/confirmation of positive results obtained for preliminary screening methods was stressed. This aspect of the service has assumed even greater importance with widespread use of dip-stick technology and the increasing number of reasons for whic

Extreme Control and Instability: Insight Into Head Injury From Intimate Partner Violence.

AIM: The aim of this study was to understand the social context of the lives of women who experienced a head injury from intimate partner violence. BACKGROUND: Sixty percent to 92% of survivors of intimate partner violence receive head trauma during the abuse. Little research exists regarding the episodes of abuse when women receive a head injury, or the reasons women might not seek medical care for the head injury or the abuse. METHOD: Twenty-one interviews from nine women who self-reported passing out from being hit in the head were analyzed using thematic analysis. FINDINGS: Themes of ex
