Attachment disturbances in young children. II: Indiscriminate behavior and institutional care.

OBJECTIVE: To assess convergence among three different measures of indiscriminate behavior and to assess the relationship of indiscriminate behavior to having an attachment figure and to aggressive behavior among young children living in a Romanian institution. METHOD: Caregivers in the institution were interviewed with semistructured interviews regarding the behavior of 61 children with special emphasis on indiscriminate behavior. The study was conducted in Bucharest, Romania (1999). RESULTS: Substantial convergence among measures of indiscriminate behavior was demonstrated. Indiscriminate

Chronic community violence: what is happening to our children?

Chronic violence is a growing problem in our society today as evidenced, among other factors, by the ever-increasing murder rate in many of our large urban centers in the United States. Emphasis has begun to be placed on chronic violence, causes that may contribute to it, and the impact of this violence on cities and the country at large. While concern has been expressed, we still have not addressed adequately, nor do we fully understand, the effects on the children who must grow up in environments where they are repeatedly being exposed to significant levels of violence.

A study of associations among attachment patterns, maltreatment, and behavior problem in institutionalized children in Japan.

The present study investigates the relationships among children's history of maltreatment, attachment patterns, and behavior problems in Japanese institutionalized children. Twenty-nine children (12 boys and 17 girls) from three different institutions in the Kinki area (Western part of Japan) participated in this study. Their average age was 6. 41-years (ranging from 4 to 10). Thirteen of the children (44.8%) had history of maltreatment before they were institutionalized. Children's attachment was assessed by the Attachment Doll Play Assessment (George & Solomon, 1990, 1996, 2000). The child's

Chronic Tic Disorders in Youth: Clinical Phenotypes and Response to Pharmacological Treatment with Aripiprazole.

BACKGROUND/OBJECTIVES: Tic disorders are neurodevelopmental conditions often associated with comorbidities like attention deficit hyperactivity disorder (ADHD) and autism spectrum disorder (ASD). Our aims were: (a) in a sample of youth with tic disorders to explore the clinical and psychopathological characteristics of different phenotypes based on the presence of comorbid ADHD and/or ASD and gender; (b) in a subgroup of patients treated with Aripiprazole, to evaluate symptoms variation over time and to identify potential predictors of response. METHODS: A total of 95 subjects with tic disord

Disinhibited social engagement and reactive attachment behaviours in children assessed for Fetal Alcohol Spectrum Disorder (FASD): Distinguishing effects of maltreatment from inhibitory control difficulties.

The developmental origins of reactive attachment disorder (RAD) and disinhibited social engagement disorder (DSED) continue to be poorly understood. This cross-sectional study reports on the rates of RAD and DSED behaviours in a clinical sample of children presenting for Fetal Alcohol Spectrum Disorder (FASD) assessment (N = 123). It examines whether RAD and DSED behaviours, assessed by diagnostic interview with carers, are better explained by maltreatment (timing/type) or neurodevelopmental inhibitory-control deficits, assessed using the BRIEF-2, adjusting for age, sex and variables related t

The impact of maltreatment on children's psychosocial adjustment: a review of the research.

While there is wide consensus that child victims of maltreatment are at risk for future social and psychological maladjustment, conspicuously few controlled empirical studies which examine this problem exist. The relative inattention to the psychological sequelae of child maltreatment is unfortunate since observations suggest that exposure to physical abuse and/or neglect has serious consequences for the child's present and long-term adjustment. The present paper reviews the most current empirical research on the psychosocial adjustment of maltreated children.

Can inattention/overactivity be an institutional deprivation syndrome?

Elevated rates of attention deficit and overactivity have been noted previously in samples of institution-reared children. This study examined the hypothesis that inattention/overactivity(I/O) might constitute a specific deprivation syndrome. One hundred and sixty five children adopted at varying ages (e.g., 0-42 months of age) into the UK following severe early deprivation were compared with 52 within-UK adoptees who did not suffer deprivation. The children were rated by teachers and parents on levels of I/O, conduct difficulties, and emotional difficulties using the Revised Rutter Scales. Da

Changes in Socio-Emotional and Behavioral Functioning After Attending a Camp for Children with Tourette Syndrome: A Preliminary Investigation.

Children and adolescents with Tourette syndrome are at risk for social, emotional, and behavioral difficulties that may negatively influence feelings of self-competence and attitudes toward having this diagnosis. Attending a camp for children with Tourette syndrome may facilitate improvements in overall socio-emotional and behavioral functioning and self-perception. The current study obtained data from 37 campers (76 % male, 24 % female) and 47 caregivers of campers to investigate changes in children's emotional and behavioral functioning, self-perception (i.e., social and general), attitudes

Acquired Capillary Hemangioma of the Palpebral Conjunctiva in a Child with Vernal Keratoconjunctivitis: A Case Report and Pathogenetic Hypothesis.

INTRODUCTION: Acquired capillary hemangioma of the conjunctiva is a rare benign vascular tumor, particularly in the pediatric population and when arising from the palpebral conjunctiva. Its clinical appearance often overlaps with reactive vascular lesions such as pyogenic granuloma, making accurate diagnosis challenging. Vernal keratoconjunctivitis (VKC) is a chronic allergic inflammatory disorder characterized by a pro-angiogenic microenvironment, which may contribute to vascular proliferation. CASE PRESENTATION: An 8-year-old boy with a known history of VKC presented with a 1-year history o

Pathogens of the Human Central Nervous System- the Major Bacterial Pathogens.

Numerous pathogen infections can initiate potentially fatal neurological inflammation, including encephalitis, meningitis, meningoencephalitis, myelitis, and other inflammations affecting the central nervous system. Several bacterial, viral, fungal, and protozoan parasite pathogens can initiate such neurological disorders. The original infections can be transmitted between humans by aerosols, respiratory droplets, ingestion, or physical contact. Some pathogens can also be transmitted from mother to child. Or the infections can be transmitted to humans by non-human vectors, by means of bites fr

Adjustment disorders in adolescents and adults.

The face and descriptive validity of the diagnosis of adjustment disorders is examined in a sample of 402 adolescents and adults who received a DSM-II diagnosis of "transient situational disturbance" over a four-year period. The study suggests that the redefinition of the disorder in DSM-II should have substantially better face and descriptive validity. This redefinition recognizes that both the stressors that precipitate the disorder and the disorder itself may often by quite chronic, and that the stressor need not be overwhelming. Among adolescents in this study, 59% of the stressors had bee

Unraveling middle childhood attachment-related behavior sequences using a micro-coding approach.

Attachment theory states that children learn to trust in their parent's availability and support if they repeatedly experience that their parents respond sensitively to their needs during distress. Attachment is thus developed and shaped by day-to-day interactions, while at the same time, each interaction is a momentary expression of the attachment relation. How attachment-related behaviors of mother and child follow upon each other during interactions in middle childhood, and how these sequences differ in function of attachment quality, has hardly been studied up to now. To fill this gap, we

Clinical characteristics, impairment, and psychiatric morbidity in 102 youth with misophonia.

BACKGROUND: There is little information on the clinical presentation, functional impact, and psychiatric characteristics of misophonia in youth, an increasingly recognized syndrome characterized by high emotional reactivity to certain sounds and associated visual stimuli. METHOD: One-hundred-two youth (8-17 years-old) with misophonia and their parents were recruited and compared with 94 youth with anxiety disorders. Participants completed validated assessments of misophonia severity, quality of life, as well as psychiatric symptoms and diagnoses. RESULTS: The most common misophonia triggers

Rages--what are they and who has them?

OBJECTIVE: The purpose of this study was to examine rages and define their associated clinical and diagnostic conditions systematically. Children's severe anger outbursts, sometimes called "rages," have been associated with many disorders, including mania, "severe mood dysregulation," and oppositional defiant/conduct disorder. Although reactive aggression has been studied extensively, there are almost no data on this important and disabling clinical phenomenon. METHOD: A total of 130 different 5-12 year olds were hospitalized over 151 consecutive admissions were evaluated diagnostically with

The unique contribution of blushing to the development of social anxiety disorder symptoms: results from a longitudinal study.

BACKGROUND: Self-conscious emotional reactivity and its physiological marker - blushing has been proposed to be an etiological mechanism of social anxiety disorder (SAD), but so far, untested in longitudinal designs. This study tested, for the first time, whether self-conscious emotional reactivity (indexed as physiological blushing) contributes to the development of SAD symptoms over and above social behavioral inhibition (BI), which has been identified as the strongest predictor of SAD development in early childhood. METHODS: One hundred fifteen children (45% boys) and their mothers and fat

Parental History of Major Depressive Disorder Moderates the Relation Between Neighborhood Disadvantage and Reward Responsiveness in Children.

In this study, we examined associations between census-derived indices of neighborhood disadvantage and children's reward outcome processing and whether these relations would be stronger among children already at risk for alterations in reward processing due to having a parental history of major depressive disorder (MDD) compared to children of never depressed parents. Participants were 224 children aged 7-11 years old and their parent. Parents were required to either have a history of MDD or no lifetime history of any depressive disorder. To measure reward outcome processing, we focused on th

Attachment and behavior problems in middle childhood as reported by adult and child informants.

The predictive relation between attachment and mother, teacher, and self-reported psychopathology was examined for a diverse socioeconomic status French Canadian sample of 96 children. Attachment classifications were assigned on the basis of reunion behavior with mother when the children were approximately 6 years old, and child problem behavior was assessed 2 years later using the Child Behavior Checklist (mother report), the Social Behavior Questionaire (teacher report), and the Dominic Questionnaire (child self-report). Results indicated that both insecure/ambivalent and insecure/controllin

The differential effects of PTSD, MDD, and dissociation on CRP in trauma-exposed women.

OBJECTIVE: C-reactive protein (CRP), a marker of systemic inflammation, has been associated with psychiatric disorders including major depressive disorder (MDD) and post-traumatic stress disorder (PTSD). Some research suggests that exposure to trauma can trigger increased activity in the inflammatory system. Dissociation is associated with chronic trauma exposure and may be an important factor in understanding the risk for psychiatric outcomes associated with inflammation. The main objective of the current study was to understand how CRP was related to trauma, dissociation, PTSD and MDD in a s
