New information on the effects of transcranial direct current stimulation on n-back task performance.

The n-back task is prototypical tool widely used to evaluate working memory (WM) abilities in healthy and clinical populations. Previous studies finding beneficial effects of transcranial direct current stimulation (tDCS) over the dorsal lateral prefrontal cortex (DLPFC) on n-back task performance were limited by the number of n-back "memory loads" utilized and the assessment of performance only immediately following stimulation. Our aims were to investigate both the immediate and lasting effects of six sessions of bilateral tDCS over the DLPFC on n-back performance. We used a 1-, 2-, 3-, and

Real alerts and artifact classification in archived multi-signal vital sign monitoring data: implications for mining big data.

Huge hospital information system databases can be mined for knowledge discovery and decision support, but artifact in stored non-invasive vital sign (VS) high-frequency data streams limits its use. We used machine-learning (ML) algorithms trained on expert-labeled VS data streams to automatically classify VS alerts as real or artifact, thereby "cleaning" such data for future modeling. 634 admissions to a step-down unit had recorded continuous noninvasive VS monitoring data [heart rate (HR), respiratory rate (RR), peripheral arterial oxygen saturation (SpO2) at 1/20 Hz, and noninvasive oscillom

[The sudden infant death syndrome. II. Its prevention by home monitoring].

Sudden infant death syndrome (SIDS) is a very dramatic situation with a very high incidence rate. Children with a high risk of SIDS can be identified by clinical/epidemiological scorings. The preventive usefulness of home monitoring is now a matter of study. Our experience is reported in this article. Thirty-nine infants at high risk were selected, although the monitoring program was finally achieved in only 36 cases. A cardiorespiratory impedance monitor (Centurion, Clinical Data, Boston, USA) was used. The tachycardia alarm was set at 200 b/min, bradycardia at 60 b/min and apnea > 20 seconds

Comparison of Postoperative Continuous Wireless Cardiac Rhythm Monitoring with Traditional Telemetry in Cardiac Surgery Patients: the SMART-TEL Study.

Telemetry monitoring (conventional cardiac monitoring system [CCMS]) is a universal method for postoperative arrhythmia detection; however, the clinical challenge of alarm fatigue, primarily associated with noise or cable disconnections, persists. The introduction of wireless continuous cardiac monitoring (WCCM) represents a potential solution to enhance recording fidelity. Patients were simultaneously outfitted with both a monitoring device considered the standard of care and a novel adhesive wireless patch. A 48-h cardiac monitoring session with the two devices occurred after cardiac surgery

Patients Presenting With Reflux Symptoms - Whom to Test and Whom to Treat?

Gastroesophageal reflux disease (GERD) is a complex condition with diverse clinical presentations, ranging from typical heartburn and regurgitation symptoms to extraesophageal manifestations and alarm symptoms. Determining which patients should be tested first versus those who should receive empirical treatment remains a key clinical challenge. If not recently performed, initial patient testing, commonly involving upper endoscopy, is recommended for patients presenting with alarm or refractory symptoms and for those at high risk for Barrett's esophagus. Additionally, testing should be prioriti

EEG-Based Seizure Prediction Approaches Within Clinically Relevant Pre-seizure Windows Using Scalp EEG Datasets: A Systematic Review.

This systematic review synthesized evidence on EEG-based seizure prediction within clinically relevant pre-seizure windows, focusing on deep learning and machine learning models evaluated on public datasets. The review aimed to summarize predictive performance, including sensitivity, receiver operating characteristic curve findings when reported, and false-alarm outcomes for scalp EEG seizure prediction systems. It also examined methodological heterogeneity across studies, including the preictal window, defined as the period before seizure onset used for prediction; the seizure prediction hori

Limited utility of endoscopy in young patients without alarm features.

OBJECTIVE: Current guidelines recommend using the presence of alarm features to help determine if an upper endoscopy is appropriate as an initial diagnostic tool in young adults with dyspepsia. The study aimed to assess whether young adults without alarm features had clinically significant findings on gastroscopy. METHODS: This was a retrospective study on patients 18-50 years who underwent their first gastroscopy at Landspitali University Hospital 2018-2022. Data was collected on symptoms and endoscopic findings. American Society for Gastrointestinal Endocopy (ASGE) from 2015 were used to de

Differential diagnosis of apneas in preterm infants.

Clinically relevant apneas, which are common in preterm infants, may adversely affect later neuropsychological condition in this group of patients. Pharmacotherapy to stimulate respiratory functions may be unsuccessful. Polygraphic recording may help in the differential diagnosis of these clinically relevant events. Twenty-nine preterm neonates born before 36 weeks of gestational age were examined using polygraphic recording (respiration--two channels, perioral electromyography, oxygen saturation, heart rate, electroencephalography, electrocardiography, electrooculography). The examination was

Comparative evaluation of the Autofield-I, CFA-120, and Fieldmaster Model 101-PR automated perimeters.

Clinical evaluations of the Autofield-I, CFA-120, and Fieldmaster Model 101-PR automated perimeters were performed in comparison with manual kinetic testing on the Goldmann perimeter. All the automated perimeters displayed similar high rates of detecting visual field defects, although their false-alarm (false-positive) rates were considerably different. The principal factors responsible for high-detection and low false-alarm rates are discussed.

24-hour urinary total protein quantitative detection for pregnant women with unit conversion failure: a case report and laboratory administration reflection.

Analogous to medication dosage errors, inaccuracies in measurement units for analyte test results in clinical laboratories can lead to adverse clinical consequences. We report a case of a 35-years-old pregnant woman at 38 weeks of gestation with a history of gestational hypertension. During routine follow-up, she presented with elevated blood pressure, abnormal qualitative urinary protein findings, and an elevated random urinary protein concentration, prompting a request for 24-hour urinary total protein quantification (UTP). The laboratory reported her 24-hour urine total protein excretion as

Pulse oximetry in ventilated preterm newborns: reliability of detection of hyperoxaemia and hypoxaemia, and feasibility of alarm settings.

The aim of our study was to evaluate the reliability of pulse oximetry in detecting both hyper- and hypoxaemic states and to create clinically feasible alarm limits. A total of 792 readings of a pulse oximeter and corresponding values of arterial oxygen tension from 146 (79M, 67F) artificially ventilated preterm newborns with indwelling umbilical artery catheters were compared. Predictive value analysis of pulse oximeter readings related to arterial oxygen tension confirmed the ability of the pulse oximeter to identify both hypoxaemia and hyperoxaemia. However, a clinically feasible and safe r

Actionable Ventricular Tachycardia During In-Hospital ECG Monitoring and Its Impact on Alarm Fatigue.

BACKGROUND: Ventricular tachycardia (V-tach) is the most common lethal arrhythmia, yet 90% of alarms are false and contribute to alarm fatigue. We hypothesize that some true V-tach also causes alarm fatigue because current criteria are too sensitive (i.e., ≥6 beats ≥100 beats/min [bpm]). PURPOSE: This study was designed to determine (1) the proportion of clinically actionable true V-tach events; (2) whether true actionable versus nonactionable V-tach differs in terms of heart rate and/or duration (seconds); and (3) if actionable V-tach is associated with adverse outcomes. METHODS: This was a

A randomised clinical trial of intrapartum fetal monitoring with computer analysis and alerts versus previously available monitoring.

BACKGROUND: Intrapartum fetal hypoxia remains an important cause of death and permanent handicap and in a significant proportion of cases there is evidence of suboptimal care related to fetal surveillance. Cardiotocographic (CTG) monitoring remains the basis of intrapartum surveillance, but its interpretation by healthcare professionals lacks reproducibility and the technology has not been shown to improve clinically important outcomes. The addition of fetal electrocardiogram analysis has increased the potential to avoid adverse outcomes, but CTG interpretation remains its main weakness. A pro

Recognition and management of persistent chylomicronemia: A joint expert clinical consensus by the National Lipid Association and the American Society for Preventive Cardiology.

Extreme hypertriglyceridemia, defined as triglyceride (TG) levels ≥1000 mg/dL, is almost always indicative of chylomicronemia. The current diagnostic approach categorizes individuals with chylomicronemia into familial chylomicronemia syndrome (FCS; prevalence 1-10 per million), caused by the biallelic combination of pathogenic variants that impair the lipolytic action of lipoprotein lipase (LPL), or multifactorial chylomicronemia syndrome (MCS, 1 in 500). A pragmatic framework should emphasize the severity of the phenotype and the risk of complications. Therefore, we endorse the term "persiste

The voiding alert system: a new application in the treatment of incontinence.

The voiding alarm system was developed to control nocturnal enuresis in children more than 5 years of age. Its acceptance by American clinicians, including pediatricians, has been modest at best. This article describes a new application of the voiding alarm system in different patient populations. The concept is that of notifying care attendants of voiding by individuals who are unaware or unable to communicate. Five case examples are presented involving individuals in a long-term care facility with neurogenic bladder, spina bifida, traumatic spinal cord injury, stress incontinence, and organi

Predicting treatment outcome in nocturnal enuresis.

A presenting sample of 127 consecutive referrals to a community based enuresis clinic were evaluated after treatment with baseline behavioural recording and the enuresis alarm. Almost one in five became dry after baseline recording only while 81 of 96 (84%) enuretics who used the alarm achieved the initial dryness criterion. Successful outcome was associated with the absence of adverse environmental factors and psychiatric disorders in the child. A logistic regression procedure enabled a risk score to be created so that successful outcome could be predicted. Psychiatric disorder in the child,

A Remote Surveillance Platform to Monitor General Care Ward Surgical Patients for Acute Physiologic Deterioration.

BACKGROUND: The traditional paradigm of hospital surgical ward care consists of episodic bedside visits by providers with periodic perusals of the patient's electronic health record (EHR). Vital signs and laboratory results are directly pushed to the EHR but not to providers themselves. Results that require intervention may not be recognized for hours. Remote surveillance programs continuously monitor electronic data and provide automatic alerts that can be routed to multidisciplinary providers. Such programs have not been explored in surgical general care wards. METHODS: We performed a quali
