BACKGROUND: Mitral valve surgery in octogenarians is a clinical challenge due to the impact of inherent, age-related comorbidities. Within the context of an aging population, the number of mitral surgery candidates over 80 has been gradually increasing. We sought to evaluate our institutional experience with mitral valve surgery in octogenarians to identify factors that may prove useful during the process of clinical decision-making.
METHODS: We retrospectively reviewed our institutional database for all patients over 80 years of age who underwent mitral valve surgery in our department from O

BACKGROUND: The development of computational methodologies to support clinical decision-making is of vital importance to reduce morbidity and mortality rates. Specifically, prescriptive analytic is a promising area to support decision-making in the monitoring, treatment and prevention of diseases. These aspects remain a challenge for medical professionals and health authorities.
MATERIALS AND METHODS: In this study, we propose a methodology for the development of prescriptive models to support decision-making in clinical settings. The prescriptive model requires a predictive model to build th

BACKGROUND: Diagnosis and management of patients presenting with acute dyspnea is one of the major challenges for physicians in emergency department (ED). A correct diagnosis is frequently delayed and difficult to ascertain, and clinical uncertainty is common, explaining the need for rapid diagnosis and a management plan. The primary aim of our study is to assess a diagnostic strategy using multiorgan point of care ultrasonography (USG) to differentiate patients presenting with acute dyspnea to ED into different diagnostic categories for timely management in a resource-limited setting.

Assessment of decision-making capacity (DMC) can be difficult in acquired brain injury (ABI) particularly with the syndrome of organic personality disorder (OPD) (the "frontal lobe syndrome"). Clinical neuroscience may help but there are challenges translating its constructs to the decision-making abilities considered relevant by law and ethics. An in-depth interview study of DMC in OPD was undertaken. Six patients were purposefully sampled and rich interview data were acquired for scrutiny using interpretative phenomenological analysis. Interview data revealed that awareness of deficit and th

Given the importance and the burden of pathological gambling (PG) and its effect on gamblers and their relatives, better comprehension of the phenomena involved in its development and persistence is needed. The present paper describes the previous 5 years of studies on decision making and magnetic resonance imaging in people with PG. Twenty-six studies (456 participants with PG) were included. Perseveration in monetary tasks was associated with PG despite intact executive planning abilities. PG was linked with impairment in decision making under uncertainty. Hyperactivation of the reward pathw

PURPOSE: Residents must learn which infants require a lumbar puncture (LP), a clinical decision-making skill (CDMS) difficult to evaluate because of considerable practice variation. The authors created an assessment model of the CDMS to determine when an LP is indicated, taking practice variation into account. The objective was to detect whether script concordance testing (SCT) could measure CDMS competency among residents for performing infant LPs.
METHOD: In 2011, using a modified Delphi technique, an expert panel of 14 attending physicians constructed 15 case vignettes (each with 2 to 4 SC

To investigate the importance of fiberoptic bronchoscopy with transbronchial biopsy (TBB) in clinical decision making, we reviewed 242 consecutive TBB procedures with respect to clinical presentation, roentgenographic pattern of disease, bronchoscopy result, and patient outcome. The specific yield of TBB was greatest in patients with multilobar disease and suspected sarcoidosis, metastatic neoplasm, or opportunistic infection. The value of a nonspecific TBB was estimated from its influence in changing clinically-derived suspicions of disease. Using pretest probabilities based on prebronchoscop

In this large multi-center cohort of children evaluated for Lyme disease in a Lyme-endemic emergency department, we assessed the diagnostic accuracy of clinician suspicion and subsequent clinical decision-making for children presenting with monoarthritis. Among 1,582 children with monoarthritis evaluated for Lyme disease, 623 (39%) had Lyme arthritis and 32 (2%) had septic arthritis. Overall, 313 (20%) had an invasive joint procedure (arthrocentesis or arthroscopy), 194 (12%) received parenteral antibiotics, and 376 (24%) were hospitalized. Clinician suspicion had moderate discriminative abili

Oroantral fistula (OAF) is a persistent pathological communication between the oral cavity and the maxillary sinus, often arising as a complication of dental extractions, trauma, or surgical procedures involving the maxilla. Despite various treatment options, a standardized, algorithmic approach remains lacking. To develop and present a comprehensive, evidence-based clinical decision-making algorithm for the diagnosis and management of OAF based on defect size, chronicity, sinus involvement, and infection status. A narrative review and synthesis of the literature were conducted, evaluating dia

INTRODUCTION: Acute mesenteric ischemia (AMI) is a surgical emergency with a high mortality and morbidity rate. Prompt diagnosis and early surgical management are the cornerstones of management. In certain patients, however, treatment is futile and early palliative care, lessens patient, and family distress. The aim of this study was to investigate factors, focusing on patient comorbidity, that may predict futility of treatment and hence, guide clinicians in their decision making.
PATIENTS AND METHODS: Consecutive adult patients with AMI diagnosed on imaging were reviewed for demographics, co

Europace : European pacing, arrhythmias, and cardiac electrophysiology : journal of the working groups on cardiac pacing, arrhythmias, and cardiac cellular electrophysiology of the European Society of Cardiology2018eng
Recurring questions when dealing with arrhythmias in athletes are about the cause of the arrhythmia and, more importantly, about the eligibility of the athlete to continue sports activities. In essence, the relation between sports and arrhythmias can be understood along three lines: sports as arrhythmia trigger on top of an underlying problem, sports as arrhythmic substrate promotor, or sports as substrate inducer. Often, there is no sharp divider line between these entities. The athlete's heart, a heart that adapts so magically to cope with the demands of exercise, harbours many structural an

PURPOSE: To investigate the impact of a postgraduate training module on optometrists' clinical decision-making in relation to the diagnosis and management of primary open-angle glaucoma.
METHODS: A group of United Kingdom community optometrists (n = 53) were assessed immediately before and again 3 months after completing a 3-day didactic postgraduate university module on the diagnosis and management of glaucoma. A smaller control cohort (n = 20), who did not receive the intervention, was recruited and completed the same assessments on two occasions, separated by approximately 3 months. The as

STUDY DESIGN: Case report.
BACKGROUND: Coccydynia is a painful condition of the sacrococcygeal region, with symptoms associated with sitting and rising from a seated position. There is no gold standard for diagnosis of this condition; however, coccyx mobility assessment, pain provocation testing, and imaging have been proposed as reasonable diagnostic approaches. Once correctly diagnosed, treatment options for coccydynia include conservative management and surgical excision. The purpose of this report is to describe the different but successful clinical management strategies of 2 patients wit

Clopidogrel (a widely used second-generation thienopyridine) therapy is associated with an unpredictable pharmacodynamic response whereby approximately 1 in 3 patients will have a high on-treatment platelet reactivity to adenosine diphosphate. High on-treatment platelet reactivity is an established risk factor for ischemic event occurrence in patients undergoing percutaneous coronary intervention. Platelet function testing may have a role in monitoring therapeutic efficacy when clopidogrel is the chosen agent and in safety when more potent drugs are used, especially in patients with high bleed

Sound clinical decision making (CDM) is one critical factor in the delivery of quality health care. The purpose of this cross-sectional study was to examine therapists' autonomy in CDM, to describe therapists' clinical reasons influencing CDM, and to describe the influence of autonomy on actual clinical practice. A survey was distributed to 754 hand therapists and descriptive statistics was performed. Of the 754 surveys, 191 were returned (response rate 25.3%). Autonomy in CDM was perceived to be low by most therapists. Greatest autonomy was seen in setting the frequency of rehabilitation sess

A 75-year-old woman was admitted because of fever of unknown origin (FUO). In the year before the current admission she developed myalgias and was treated for polymyalgia rheumatica with low-dose prednisone. Her complaints persisted and prednisone was discontinued. Five months before the present admission she developed fever (37.7-38.9 degrees C), malaise, fatigue and occipital headache. Laboratory tests showed an elevated erythrocyte sedimentation rate (98 mm in the first hour) and a severe hypochromic, slightly microcytic, anaemia. Although a recent temporal artery biopsy was negative, a sec
