Computed tomographic colography and virtual colonoscopy.

CT colography (CTC) is a powerful new approach to imaging the colorectum and a promising screening tool for the detection of colorectal neoplasia. From data generated by a helical CT scan, CTC uses virtual reality technology to produce highly discriminant two- and three-dimensional images that permit a thorough and minimally invasive evaluation of the entire colorectum. A dynamic CTC display technique from the endoluminal perspective, called cf2virtual colonoscopy,cf1 simulates colonoscopy by "flying" through the three-dimensional colon image. CTC offers potential advantages in diagnostic perf

Low-cost, office-based, screening colonoscopy.

OBJECTIVES: Performing full colonoscopy at regular intervals and removing lesions at an early stage might significantly lower the incidence and mortality of colorectal cancer. Such a program must be inexpensive, safe, and time-efficient. METHODS: Screening colonoscopy was performed on 639 patients. For a normal examination, the physician's time is limited to giving the medication for conscious sedation, performing the colonoscopy, and completing a written report form. The total charge for a normal screening colonoscopy is $150. RESULTS: Adenomatous and/or hyperplastic polyps were detected in

Combining Colonoscopy With Fecal Immunochemical Test Can Improve Current Familial Colorectal Cancer Colonoscopy Surveillance: A Modelling Study.

BACKGROUND & AIMS: The authors assessed whether familial colorectal cancer (FCRC) surveillance in individuals without hereditary CRC can be optimized METHODS: The Adenoma and Serrated Pathway to Colorectal Cancer (ASCCA)-FCRC model simulates CRC development in individuals with a family history of CRC at 2-fold and 4-fold increased CRC risk compared with the general population. The authors simulated a strategy without surveillance, the current Dutch guideline (5-yearly colonoscopy between ages 45 and 75 years), and the following 3 sets of alternative strategies: colonoscopy surveillance, survei

Recommendations for follow-up interval after colonoscopy with inadequate bowel preparation in a national colonoscopy quality registry.

BACKGROUND AND AIMS: Endoscopist recommendations regarding a repeat colonoscopy after inadequate bowel cleanliness have not been fully described. Our aim was to evaluate the timing of recommendations for repeat colonoscopy after inadequate bowel preparation using a large, national colonoscopy registry. METHODS: We performed a cross-sectional analysis of all outpatient screening and surveillance colonoscopies among adults ages 50 to 75 reported in the GI Quality Improvement Consortium from 2011 to 2018. The primary outcome was a recommendation to repeat colonoscopy within 1 year. Secondary out

Optimal colonoscopy surveillance interval period for the adenoma patients who had an adequate polypectomy at baseline colonoscopy.

The role of surveillance colonoscopy has long been established: it reduces both the incidence and the mortality of colorectal cancer. We aimed to assess the optimal colonoscopy surveillance interval period for the adenoma patients who underwent an adequate polypectomy at baseline colonoscopy to avoid overuse or underuse of colonoscopy. A retrospective study was carried out on the baseline adenoma patients who had had at least two completed colonoscopy examinations during the years 2000-2013 in the Digestive Endoscopy Center of the First Affiliated Hospital of Kunming Medical University. All th

Taking a pragmatic approach to implement colonoscopy quality at a health system level: Ontario's colonoscopy quality strategy.

OBJECTIVES: To describe Ontario's colonoscopy quality strategy (OCQS), to review outcomes to date, to assess it against the Health Quality Ontario framework, and to identify areas for improvement. METHODS: We provide a descriptive analysis of the OCQS, tracing its development from the launch of the ColonCancerCheck program in 2007 through subsequent initiatives, including funding mechanisms, evidence-based standards, systematic data collection, provider-level performance reporting and feedback. The impact of the OCQS is assessed via temporal trends in colonoscopy quality indicators. RESULTS:

Association Between Index Adenomas and Advanced Findings at Third Colonoscopy: Data From the New Hampshire Colonoscopy Registry.

BACKGROUND & AIMS: The United States Multi-Society Task Force for Colorectal Cancer (CRC) provides recommendations for the second surveillance colonoscopy for patients with adenomas on index exam. However, evidence which informs risk at second surveillance is limited due to lack of data on quality measures and known risk factors. We evaluated the risk for an advanced outcome on second surveillance colonoscopy for patients with index adenomas and a first surveillance colonoscopy without polyps, while accounting for risk factors and adenoma detection rates. METHODS: Our primary outcome was the

Interval post-colonoscopy colorectal cancer following a negative colonoscopy in a fecal immunochemical test-based screening program.

BACKGROUND: In the Dutch colorectal (CRC) screening program, fecal immunochemical test (FIT)-positive individuals are referred for colonoscopy. If no relevant findings are detected at colonoscopy, individuals are reinvited for FIT screening after 10 years. We aimed to assess CRC risk after a negative colonoscopy in FIT-positive individuals. METHODS: In this cross-sectional cohort study, data were extracted from the Dutch national screening information system. Participants with a positive FIT followed by a negative colonoscopy between 2014 and 2018 were included. A negative colonoscopy was def

Development of a Large Colonoscopy-Based Longitudinal Cohort for Integrated Research of Colorectal Cancer: Partners Colonoscopy Cohort.

BACKGROUND AND AIMS: Conventional adenomas (CAs) and serrated polyps (SPs) are precursors to colorectal cancer (CRC). Understanding metachronous cancer risk is poor due to lack of accurate large-volume datasets. We outline the use of natural language processing (NLP) in forming the Partners Colonoscopy Cohort, an integrated longitudinal cohort of patients undergoing colonoscopies. METHODS: We identified endoscopy quality data from endoscopy reports for colonoscopies performed from 2007 to 2018 in a large integrated healthcare system, Mass General Brigham). Impact of Colonoscopy Bowel Preparation Quality on Follow-up Interval Recommendations for Average-risk Patients With Normal Screening Colonoscopies: Data From the New Hampshire Colonoscopy Registry.

BACKGROUND AND AIMS: National guidelines for colonoscopy screening and surveillance assume adequate bowel preparation. We used New Hampshire Colonoscopy Registry (NHCR) data to investigate the influence of bowel preparation quality on endoscopist recommendations for follow-up intervals in average-risk patients following normal screening colonoscopies. METHODS: The analysis included 9170 normal screening colonoscopies performed on average risk individuals aged 50 and above between February 2005 and September 2013. Wide angle colonoscopy with a prototype instrument: impact on miss rates and efficiency as determined by back-to-back colonoscopies.

OBJECTIVE: Polyps are missed during conventional colonoscopy, even with meticulous technique. The aim of this study was to investigate whether a prototype wide angle colonoscope is associated with a reduced miss rate for polyps. METHODS: Two studies were performed. In study 1, a total of 50 patients underwent back-to-back, same-day colonoscopy by a single examiner with the prototype wide angle colonoscope and with a standard colonoscope, with the order of scopes randomized. In study 1, an attempt was made to keep examination time with the two colonoscopes equal. In study 2, a total of 20 pati

Prone or Left for Colonoscopy? A Randomized Controlled Trial of Prone Versus Left-sided Starting Position for Colonoscopy.

BACKGROUND AND AIMS: Data have emerged supporting the right-lateral starting position in reducing time to cecum and improving patient comfort for minimally sedated colonoscopy. We aimed to test whether prone starting position results in similar advantages in procedure time and patient tolerability in comparison to traditional left-sided starting position. MATERIALS AND METHODS: We conducted a randomized controlled trial in which patients were randomized to begin in either the prone or left-lateral (LL) position. A total of 181 adult patients undergoing scheduled colonoscopy were stratified by

Association between index adenomas and advanced neoplasia at third surveillance colonoscopy: Data from the New Hampshire Colonoscopy Registry.

BACKGROUND: Due to a lack of data, the USMTSF does not provide guidance for the 3rd surveillance exam for adenomas detected on index colonoscopy. Our aim was to assess advanced outcome risk at 3rd surveillance in patients with index adenomas. METHODS: We included patients from the New Hampshire Colonoscopy Registry with no genetic syndromes, inflammatory bowel disease, those with first degree relative with CRC < 50 years of age, adequate bowel preparation and no colorectal cancer (CRC) on index, and with three follow-up colonoscopies each >12 months apart. Only patients with normal or low ris

Current roles of colonoscopy in minimally invasive colorectal surgery: Preoperative guidance, intraoperative colonoscopy, and combined endoscopic-laparoscopic surgery.

Colonoscopy is a cornerstone in the detection and diagnosis of colorectal tumors, playing a critical role in both screening and clinical evaluation. More recently, its utility has expanded to therapeutic guidance, particularly with the advent of minimally invasive surgical techniques. Preoperative tattoo marking is commonly used for tumor localization; however, it poses challenges such as intraperitoneal ink scattering and difficulty in defining dissection planes in the lower rectum. To address these limitations, a new technology utilizing a near-infrared fluorescence clip placed preoperativel

Clinical updates in bowel preparation for colonoscopy.

Colonoscopy is essential for colorectal cancer (CRC) screening, but its effectiveness depends on adequate bowel preparation (BP), which remains suboptimal in up to 44 % of cases. Poor BP reduces lesion detection, particularly in the proximal colon, and increases procedure time, repeat exams, and healthcare costs. This narrative review summarizes evidence-based strategies to improve BP, highlighting high- and low-volume polyethylene glycol (PEG) regimens, including newer very-low-volume options with better patient tolerance. Adjunctive agents like simethicone and 5-HT4 agonists benefit select g

Colonoscopy in the diagnosis and management of appendiceal disease.

In this editorial, we comment on the article published in the recent issue of the World Journal of Gastrointestinal Endoscopy. We focused on the understanding of appendiceal disease, and the various options for diagnosis and treatment via endoscopy. Some factors affecting the diagnosis and management of appendiceal diseases are also discussed. The existence of any organ has its natural rationality, and the appendix is such a magical organ. A growing number of experts and scholars have gradually come to a consensus that the appendix is not a useless evolutionary relic. There are many lymphocyte

In vivo multi spectral colonoscopy in mice.

Multi- and hyperspectral endoscopy are possibilities to improve the endoscopic detection of neoplastic lesions in the colon and rectum during colonoscopy. However, most studies in this context are performed on histological samples/biopsies or ex vivo. This leads to the question if previous results can be transferred to an in vivo setting. Therefore, the current study evaluated the usefulness of multispectral endoscopy in identifying neoplastic lesions in the colon. The data set consists of 25 mice with colonic neoplastic lesions and the data analysis is performed by machine learning. Another q
